Given the rapidly growing area of molecular genetic laboratory testing, we sought to assess changing issues over a 2-year period of time pertaining to the availability of testing for GJB2 mutations associated with non-syndromic hearing loss. Laboratory assessments carried out by telephone interviews with directors or other key personnel revealed variations among laboratories in informed consent practices, evaluation of test requests for appropriateness, and the reporting of results. From 1999 to 2000, referral patterns shifted as did sources for reimbursement, policies regarding evaluation of incoming test requests, and reporting procedures. We propose that these results reflect changes occurring as a result of a new test moving from the research to the clinical phase.  相似文献

Sixty-nine farmer's lung patients and 28 normal controls from four countries (Finland, Switzerland, Canada and the United States) were investigated for antibody levels against 13 antigens commonly used for the screening panel for hypersensitivity pneumonitis. Of these antigens, eight were from the Medical College of Wisconsin (United States) and five were from the University of Kuopio (Finland). IgG antibodies against these antigens were studied in 97 sera using a sensitive biotin-avidin-linked enzyme immunoassay. The results indicate that the mean antibody titer against Micropolyspora faeni was highest in the United States (U.S.) followed by Finland. Both Finnish and U.S. antigens reacted almost identically against various groups of patients, although the degree of reactivity varied considerably. Higher antibody levels against Thermoactinomyces vulgaris were detected in Finnish patients than patients from other countries while patients from all four countries showed elevated levels of antibodies against T. candidus. This study demonstrates that antigens from identical species, irrespective of geographic origin, reacted similarly. However, variability between antigens of the same species was still considerably significant. Since the microbiological flora of moldy hay varies widely in different regions, the microbial species associated with the disease at a given geographical area has to be determined before selecting antigens for serological studies. The antigens currently used in various laboratories are crude preparations and need to be purified and standardized for dependable results. Until such antigens are available, all antigenic preparations used in the immunological evaluation of patients should be immunochemically characterized for their reproducibility and reliability although the ultimate goal should be to obtain standardized pure antigens for dependable immunodiagnosis of farmer's lung.  相似文献

Timely release and communication of critical test results may have significant impact on medical decisions and subsequent patient outcomes. Laboratories therefore have an important responsibility and contribution to patient safety. Certification, accreditation and regulatory bodies also require that laboratories follow procedures to ensure patient safety, but there is limited guidance on best practices. In Australasia, no specific requirements exist in this area and critical result reporting practices have been demonstrated to be heterogeneous worldwide.Recognising the need for agreed standards and critical limits, the AACB started a quality initiative to harmonise critical result management throughout Australasia. The first step toward harmonisation is to understand current laboratory practices. Fifty eight Australasian laboratories responded to a survey and 36 laboratories shared their critical limits. Findings from this survey are compared to international practices reviewed in various surveys conducted elsewhere. For the successful operation of a critical result management system, critical tests and critical limits must be defined in collaboration with clinicians. Reporting procedures must include how critical results are identified; who can report and who can receive critical results; what is an acceptable timeframe within which results must be delivered or, if reporting fails, what escalation procedures should follow; what communication channels or systems should be used; what should be recorded and how; and how critical result procedures should be maintained and evaluated to assess impact on outcomes.In this paper we review the literature of current standards and recommendations for critical result management. Key elements of critical result reporting are discussed in view of the findings of various national surveys on existing laboratory practices, including data from our own survey in Australasia. Best practice recommendations are made that laboratories are expected to follow in order to provide high quality and safe service to patients.  相似文献

Molecular genetic techniques have entered many areas of clinical practice. Public expectations from this technology are understandably high. To maintain confidence in this technology, laboratories must implement the highest standards of quality assurance (QA). External quality assessment (EQA) is recognized as an essential component of QA. The United Kingdom National External Quality Assessment Service (UKNEQAS) for Molecular Genetics, first set up in 1991, is currently the longest provider of EQA to molecular genetic testing laboratories in the UK, The Netherlands, and Ireland. Errors in the scheme are sporadic events. However, evidence from this and other EQA schemes suggests that a residual error rate persists, which should be taken into account in clinical practice. This EQA scheme has evolved from the respective scientific bodies of the constituent countries and retains a strong emphasis on collective peer review. It is essential that the steps taken to ensure quality in this rapidly expanding field are clear and transparent to participants and public alike. We describe the procedures developed and the governance imposed to monitor and improve analytical and reporting standards in participant laboratories and we compare our experiences with those of equivalent EQA services in the United States.  相似文献

The herbarium specimens amassed by Paul A. Fryxell while working at the U.S. Department of Agriculture, College Station, Texas, were moved in October 1993 to The New York Botanical Garden and integrated into the Herbarium in 1994. The collection is particularly rich in specimens of the Malvaceae and other families of the Malvales, the areas of Fryxell’s taxonomic specialty. Also included is the cotton reference herbarium of the former Cotton Branch of the Agricultural Research Service, U.S. Department of Agriculture, which consists of voucher specimens of germplasm introductions of cotton (Gossypium) from expeditions and from test plots in the United States and Mexico. A total of 19,918 specimens, including 167 type specimens, have been incorporated into The New York Botanical Garden Herbarium.  相似文献

Five university centres in Canada and the United States have collaborated in a study of a test for carcinoembryonic antigen, with the Montreal General Hospital laboratory acting as the reference laboratory, examining duplicate halves of specimens from patients examined in local laboratories.Between June 1971 and April 1972, 503 patients were admitted to the study, 146 with cancer of the colon or rectum. Discrimination between colo-rectal cancer and “other” diagnoses was good at 2.5 ng./ml. or more, such a result being obtained in 66% of the patients with colo-rectal cancer in the local laboratories and 62% in the reference laboratory compared with 39% and 33% respectively of patients with “other” diagnoses. Patients with Duke''s C lesions of the colon or rectum were more likely to have raised CEA levels than patients with more localized lesions. CEA was also more commonly found in patients with other cancers and in patients with liver disease than in patients with other (benign) diagnoses.It is concluded that the test can be reproduced in other laboratories and that it or another simplified technique could become an important diagnostic aid for colo-rectal cancer.  相似文献

Data from the World Health Organization, U.S. Vital Statistics, the Historical Abstracts of the United States and the census of the United States were used to compare current world and states data on cancer death rate and age with historical data that shows the changing percentage of elderly. Current annual death rates from malignant neoplastic disease among the 73 countries for which data exist are highly correlated with the percent of the population ≥ 60 yrs of age and a virtually identical relation exists presently among the 50 states of the U.S. The current world and states data provide a remarkably accurate snapshot of U.S. cancer death rates over the past century during which time the U.S. elderly population increased substantially. Data on U.S. lung cancer death rates since 1940 were similarly examined and these historical numbers also show good agreement with current values among countries that today span the same proportion of elderly that the U.S. had over this period. The agreement of current and historical CA death rate and age contrasts sharply with the picture that emerges when cardiovascular death rates and death rate from all causes are examined. The geographic and time invariance seen in the cancer data seems incosistent with identical exposure to carcinogens and suggests that biological aging is an important intrinsic factor in cancer.  相似文献

Measurement of C-reactive protein (CRP), a marker of inflammation, in dried blood spots has been increasingly incorporated into community-based social surveys internationally. Although the dried blood spot-based CRP assay protocol has been validated in the United States, it remains unclear whether laboratories in other less-developed countries can generate CRP results of similar quality. We therefore conducted external quality monitoring for dried blood spot-based CRP measurement for the Indonesia Family Life Survey and the Longitudinal Aging Study in India. Our results show that dried blood spot-based CRP results in these two countries have excellent and consistent correlations with serum-based values and dried blood spot-based results from the reference laboratory in the United States. Even though the results from duplicate samples may have fluctuations in absolute values over time, the relative order of C-reactive protein levels remains similar, and the estimates are reasonably precise for population-based studies that investigate the association between socioeconomic factors and health.  相似文献

Thirty U.S. Trypanosoma cruzi stocks isolated mainly from wild mammals were characterized by multilocus enzyme electrophoresis at 22 genetic loci and random amplification of polymorphic DNA for 10 primers. Two main phylogenetic clusters, separated by large genetic distances, were discriminated by both methods, corresponding, respectively, to the formerly described zymodemes I and III. Two stocks isolated from indigenous human cases were identified as zymodeme I. Genetic diversity of the U.S. T. cruzi isolates was considerable, comparable to that scored in similarly sized samples from South America. These results favor the hypothesis that T. cruzi U.S. stocks were not imported at a historical time and are indigenous to the native fauna of the United States. The population structure of these stocks appeared to be basically clonal, as previously reported in South America, and no evidence of hybrid genotypes was found in the United States.  相似文献

The United States is not only the world's largest economy, but it is also one of the world's largest consumers of natural resources. The country, which is inhabited by some 5% of the world's population, uses roughly one‐fifth of the global primary energy supply and 15% of all extracted materials. This article explores long‐term trends and patterns of material use in the United States. Based on a material flow account (MFA) that is fully consistent with current standards of economy‐wide MFAs and covers domestic extraction, imports, and exports of materials for a 135‐year period, we investigated the evolution of the U.S. industrial metabolism. This process was characterized by an 18‐fold increase in material consumption, a multiplication of material use per capita, and a shift from renewable biomass toward mineral and fossil resources. In spite of considerable improvements in material intensity, no dematerialization has happened so far; in contrast to other high‐income countries, material use has not stabilized since the 1970s, but has continued to grow. This article compares patterns and trends of material use in the United States with those in Japan and the United Kingdom and discusses the factors underlying the disproportionately high level of U.S. per capita resource consumption.  相似文献

The incidence for AIDS per hundred thousand is several times higher in the United States than in the rest of the First World. Earlier work by Thompson (1984, 1989a, b, 1990, 1998) indicated that a relatively small proportion of gay males frequenting the bathhouses in the United States, drove AIDS over the epidemiological threshold in the U.S. It is shown that the rate of growth of AIDS is essentially the same for the United States and other First-World countries. An argument is advanced, based on WHO AIDS data, to the effect that it is contact with the pool of infectives in the United States that drives the epidemic in other First-World Countries.  相似文献

Sampling for livestock-associated Staphylococcus aureus (LA-SA) in the United States is haphazard. The diversity of LA-S. aureus in the U.S. appears to be greater than in other countries. Here I provide unvarnished advice for young biomedical scientists on the difficulties that lie ahead and on how to find the right laboratories for training in the skills that you will need to succeed. 碳税和碳关税作为一种价格调整机制,长期而言会对经济系统中各主体产生较强的约束力。通过GTAP-E模型的模拟分析,得出以下结论:在目前的发展阶段,碳关税并非有效的低碳经济发展政策,尤其是美国对中国征收碳关税对降低世界碳排放量的影响有限;美国针对中国采取的碳关税政策将在一定程度上对我国出口贸易产生负向影响,尤其是对中国的高碳产业影响较大;当中美采取相同的碳税政策时,碳税的征收对中国GDP造成较大的负面影响,且中国自主采取节能减排方案不足以应对美国碳关税的负面效应;而当两者采取差异化政策时,中国主动采取节能减排行动可以有效应对美国碳关税的威胁。  相似文献

The lesser grain borer (Rhyzopertha dominica (F.)) is an important pest of stored grain in many parts of the world (Paleartic, Ethiopian, Oriental, Australian, Neotropical, and Neartic regions) with the ability to fly long distances. These insects have been shown to be resistant to organophosphorus insecticides in several studies. Polyacrylamide gel electrophoresis was used to assess the genetic variability within and among eight Brazilian and seven United States populations of R. dominica and to determine how insecticide resistance may be spreading within both countries. Significant variation in allele frequency among populations was observed at all six polymorphic enzyme loci that were examined. The Brazilian and U.S. populations were genetically differentiated from one another; populations within the U.S. and those within Brazil were also differentiated from one another. The mean genetic similarity among the seven U.S. populations collected in a small region in northeast Kansas was smaller than that among eight Brazilian populations collected in a relatively large geographical area. These results are consistent with the resistance ratios to chlorpyriphos-methyl in R. dominica populations from Brazil and the U.S. and the information available concerning patterns of flight activity in this insect.  相似文献

Sponsored by the National Science Foundation and the U.S. Department of Agriculture, a wheat genome sequencing workshop was held November 10-11, 2003, in Washington, DC. It brought together 63 scientists of diverse research interests and institutions, including 45 from the United States and 18 from a dozen foreign countries (see list of participants at http://www.ksu.edu/igrow). The objectives of the workshop were to discuss the status of wheat genomics, obtain feedback from ongoing genome sequencing projects, and develop strategies for sequencing the wheat genome. The purpose of this report is to convey the information discussed at the workshop and provide the basis for an ongoing dialogue, bringing forth comments and suggestions from the genetics community.  相似文献

Background

The virulence, antimicrobial resistance, and prevalence of S. aureus underscores the need for up-to-date and extensive insights regarding antimicrobial susceptibility trends. One approach to meet this need is analysis of clinical laboratory – based surveillance data.

Methods

Data from The Surveillance Network-USA (TSN), an electronic surveillance network that collects microbiology data from 300 clinical microbiology laboratories across the United States, were used as the source for analysis that included prevalence of S. aureus in clinical specimens, MRSA and multi-drug resistance phenotype rates and trends according to patient location, geographic distributions, and specimen source.

Results

S. aureus was the most prevalent species isolated from inpatient specimens (18.7% of all bacterial isolates) and the second most prevalent (14.7%) from outpatient specimens. In March 2005 MRSA rates were 59.2%, 55%, and 47.9% for strains from non-ICU inpatients, ICU, and outpatients, respectively. This trend was noted in all nine US Bureau of Census regions and multi-drug resistance phenotypes (resistance to ≥ 3 non-beta-lactams) was common among both inpatient MRSA (59.9%) and outpatient MRSA (40.8%). Greater than 90% of multi-drug resistant MRSA were susceptible to trimethoprim-sulfamethoxazole, linezolid, and vancomycin.

Conclusion

Prevalence of MRSA among both inpatient and outpatient specimens continues to increase with multi-drug resistance as a common phenotype. Continued emergence of outpatient MRSA that exhibit multi-drug resistant phenotypes has important implications for developing and evolving outpatient treatment guidelines.  相似文献
