In the chronic experiments on dogs by means of modified method of multiple drainage of the intestinal fistulas, the effect of the Treitz ligament transection on dynamics of solid carbohydrate food evacuation from the duodenum was studied. It was established that in the norm, evacuation from the duodenum was submitted to exponential law and depended on absolute quantity of food evacuated from the stomach. After the Treitz ligament transection, the evacuation through intestine had a non-exponential character. The operation led to disturbance in co-ordination between evacuatory function of the stomach and that of duodenum, and also had the depressive effect on intestinal emptying. The conclusion is made of the functional role of the Treitz ligament as active regulator of solid food evacuation from the duodenum.  相似文献

Dynamics of the solid carbohydrate food evacuation from the duodenum has been determined in chronic experiments on dogs using modified technique for multiple drainage of intestinal fistulas. It is established that dynamics of the duodenal chyme emptying depends on the absolute quantity of the stomach-evacuated food and includes three phases: increase in the evacuation rate, exponential phase and that of accelerated evacuation. The highest rate of the duodenal contents is observed in the period from the 90th to 210th minute after food intake, the lowest one--at the end of the digestive cycle. A conclusion is made concerning the high degree of coordination of the evacuatory stomach and duodenum function.  相似文献

Electrical stimulation of the vagal trunk with 10 Hz in frequency, 3 ms in duration and 15 volt in intensity for 10 s in cats produced an excitatory response of the stomach and the response was composed of two phases, an initial rapid excitation during stimulation period and the late multi-peak response after stimulation period. The initial response was inhibited by the administrations of hexamethonium (10 mg/kg, i.v.) and atropine (100 micrograms/kg, i.v.). The late response was not inhibited by hexamethonium but was inhibited by atropine (100 micrograms/kg, i.v.). The hexamethonium-sensitive initial excitation was not affected by the administration of morphine and gamma-aminobutyric acid (GABA). On the other hand, the hexamethonium-resistant late response was attenuated by the treatment with morphine (1 to 10 mg/kg, i.v.) and GABA (100 to 500 micrograms/kg, i.v.). Such inhibitory actions of morphine and GABA on the late response were antagonized by picrotoxin. From these results, it was concluded that morphine might inhibit specifically the hexamethonium-resistant late excitatory response of the stomach without affecting the hexamethonium-sensitive initial excitatory response and the inhibitory effect of morphine on the late response of stomach might be due to action of GABA released from the intramural neurons of gastric walls in cats.  相似文献

The authors of the present paper outline the results of studying the use of routine transabdominal sonography and ultrasound irrigoscopy (UI) in the diagnosis of acute colonic obstruction (ACO) in 70 patients. The cause of ACO was tumor-induced colonic luminal obturation in 55 patients and colonic evacuatory dysfunction due to congenital anomalies and acquired constrictions of inflammatory genesis in 15 cases. UI was used to determine the presence, degree, and causes of colonic obstruction. The paper details the UI procedure developed by the authors and the ultrasound semiotics of ACO, shown by routine transabdominal sonography and UI. Analysis of the results of the studies allows the authors to state that UI is as highly informative as X-ray irrigoscopy in the diagnosis of ACO and may be included into a diagnostic algorithm in these patients.  相似文献

The complex examination of 72 patients with acute ileus (AI) of nontumor nature with different severity of endotoxicosis was carried out. The study revealed that AI was accompanied by deep suppression of the immunosecretory and motor evacuatory function of the small intestine, as well as by its pronounced bacterial contamination, mainly due to the significant quantitative prevalence of Gram-negative microflora. The combination of these factors played the key role in the increase of the permeability of the enteric barrier for symbiotic microflora and its massive translocation from the intestinal tract to the internal organs of the body (peritoneal exudate, portal bed), which directly correlated with the severity of endotoxicosis in AI patients. The deficiency of the barrier function of the liver was accompanied by the penetration of infective agents into the general blood stream, thus causing the development of endotoxic shock in AI patients. The analysis of the results thus obtained made it possible to determine the main ways for the elimination of intestinogenic intoxication in AI; they should be aimed at the bacterial decontamination of the small intestine, the restoration of its motor evacuatory and protective barrier functions, the liquidation of portal and systemic bacteremia, the correction of the functional deficiency of the liver.  相似文献

By using 237 verified cases of diffuse carcinoma and benign diseases of the stomach as an example, the authors assess the capacities of routine transabdominal ultrasound study (USS) of the stomach in the diagnosis of tumors and present their modification of the procedure facilitating the imaging of problem areas. In the authors' opinion, the symptoms of thickening of the wall of the stomach and abnormalities in its normal five-layered structure indicate diffuse gastric carcinoma. The authors consider USS to be a supplement to the basic diagnostic techniques--X-ray study and gastroscopy.  相似文献

Canonical Wnt signalling is known to be involved in the regulation of differentiation and proliferation in the context of endodermal organogenesis. Wnt mediated β-catenin activation is understood to be modulated by secreted Frizzled-related proteins, such as XsFRP5, which is dynamically expressed in the prospective liver/ventral pancreatic precursor cells during late neurula stages, becoming liver specific at tailbud stages and shifting to the posterior stomach/anterior duodenum territory during tadpole stages of Xenopus embryogenesis. These expression characteristics prompted us to analyse the function of XsFRP5 in the context of endodermal organogenesis. We demonstrate that XsFRP5 can form a complex with and inhibit a multitude of different Wnt ligands, including both canonical and non-canonical ones. Knockdown of XsFRP5 results in transient pancreatic hypoplasia as well as in an enlargement of the stomach. In VegT-injected animal cap explants, XsFRP5 can induce expression of exocrine but not endocrine pancreatic marker genes. Both, its expression characteristics as well as its interactions with XsFRP5, define Wnt2b as a putative target for XsFRP5 in vivo. Knockdown of Wnt2b results in a hypoplastic stomach as well as in hypoplasia of the pancreas. On the basis of these findings we propose that XsFRP5 exerts an early regulatory function in the specification of the ventral pancreas, as well as a late function in controlling stomach size via inhibition of Wnt signalling.  相似文献

Summary Variations in stomach weights, plasma glucose and insulin levels in the portal vein, and in subcellular structures of rat pancreatic islet B-, A- and D-cells were examined over 24 h. The variation in stomach weights paralleled plasma glucose levels, indicating that the levels may be influenced by intestinal glucose absorption. Plasma insulin levels increased from the late dark to the early light periods, whereas they decreased from the late light to the early dark periods. The variation in plasma insulin levels was in the opposite sense to that in the relative numbers of B-cell granules. The decrease in the relative numbers of A-cell granules occurred between the late dark and early light periods. The relative numbers of D-cell granules decreased before and after the decreases in B- and A-cell granules. The variation in D-cell granules appears to correspond to the inhibitory effect of somatostatin. The relative amounts of rough endoplasmic reticulum (rER) in each cell varied in a reciprocal manner to those of B-cell granules. Moreover, the variation in plasma insulin levels coincided with variations in rER of hepatocytes and pancreatic acinar cells. The changes in rER of each cell may correlate with the trophic effect of insulin.  相似文献

目的探讨不同方式干预孕期运动与妊娠期糖代谢异常的关系及对妊娠结局的影响。方法将300例孕14周单胎孕妇按阶段分为两组各150例,干预组对孕妇进行评估,建立孕妇档案,孕期通过个体化运动干预,给予孕妇个体化、具体的适度运动处方及给予督导规律运动;对照组采用传统的产前检查及常规护理。观察比较两组孕妇血糖变化、体重增长及妊娠结局。结果干预组空腹,餐后2h血糖值,妊娠末期体重,发生糖代谢异常,早产儿、新生儿窒息、巨大儿以及剖宫产率均低于对照组,差异有统计学意义(P0.05)。结论通过个体化护理对孕期运动进行干预,有效控制血糖值,降低母婴并发症的发生。  相似文献

目的 :端粒酶的激活与胃癌发生密切相关 ,通过检测胃癌组织中端粒酶活性表达水平 ,为胃癌及其发生的早期诊断提供一种有效的方法。方法 :应用PCR ELISA技术 ,把端粒酶重复序列扩增技术与微孔液相杂交酶联呈色技术有机结合 ,检测胃肠癌组织和腹水标本中的端粒酶活性表达。结果 :1 7例晚期胃癌组织标本中检出 1 3例端粒酶活性表达 ,阳性率为 76 47% ;2 3例中期胃肠癌组织标本中检出 1 9例端粒酶活性表达 ,阳性率为 78 2 6% ;66例早期胃癌组织标本中检出5 7例端粒酶活性表达 ,阳性率为 86 36% ;36例癌旁近端组织标本中检出 1 1例端粒酶活性表达 ,阳性率为 30 5 6% ;36例癌旁远端组织标本中检出 4例端粒酶活性表达 ,阳性率为 1 1 1 1 % ;2 6例胃肠癌腹水标本检出 1 9例端粒酶活性表达 ,阳性率为 73 0 8% ;5 4例正常人胸腹水标本中没有检出端粒酶活性表达。结论 :胃癌组织标本中有较高的端粒酶活性表达 ,端粒酶活性表达可以作为胃癌发生的有效指标 ,而PCR ELISA技术可以定量评价作为端粒酶活性表达从而作为诊断胃癌发生的一种快速简便有效的方法。  相似文献

IntroductionThe ablation of ventricular tachycardia, including premature ventricular contractions, is an approved, albeit infrequent procedure in pediatric patients. Data are scarce regarding the outcomes of this procedure. The purpose of this study was to share a high-volume center experience and patient outcomes for catheter ablation of ventricular ectopy and ventricular tachycardia in pediatric population.MethodsData were retrieved from the institutional data bank. Outcomes over time were evaluated, and procedural details were compared.ResultsA total of 116 procedures were performed on 102 pediatric patients between July 2009 and May 2021 at the Rajaie Cardiovascular Medical and Research Center in Tehran, Iran. Ablation was not performed in 4 procedures (3.4%) due to high-risk substrates. Of the remaining 112 ablations performed, 99 (88.4%) were successful. However, one patient died due to a coronary complication. There were no significant differences observed in early ablation results based on patients' age, sex, cardiac anatomy, or ablation substrates (P > 0.05). Follow-up records were available for 80 procedures, and 13 (16.3%) of those experienced recurrence. During long-term follow-up, none of the variables mentioned above were statistically different between patients with or without arrhythmia recurrence.ConclusionThe overall success rate of pediatric ventricular arrhythmia ablation is favorable. We found no significant predictor for the procedural success rate concerning acute and late outcomes. Larger multicenter studies are needed to elucidate the predictors and outcomes of the procedure.  相似文献

The purpose of this study was to evaluate results of combined phacoemulsification, intraocular lens implantation and pars plana vitrectomy in patients with advanced diabetic retinopathy. We retrospectively evaluated postoperative outcomes and complications in 102 eyes of 102 patients who underwent a combined procedure. All patients had a visually significant cataract. Forty two patients had vitreous hemorrhage and mild proliferative diabetic retinopathy. Sixty patients had a mild tractional retinal detachment. The median follow up was 14 months (range 6-36 months). 80% of patients had an increase of visual acuity of at least 2 Snellen lines. The most frequent early postoperative complication was elevated intraocular pressure, followed by mild fibrinous reaction. The most frequent late postoperative complication was the presence of posterior synechia, followed by glaucoma. Eleven patients required a repeated pars plana vitrectomy. Combined phacoemulsification, posterior chamber intraocular lens implantation and pars plana vitrectomy can be successfully performed in selected diabetic patients with favorable postoperative outcome.  相似文献

Proceeding from analysis of over 2000 ultrasound investigations (USI) the role of this method in the diagnosis of stomach tumors was defined (employed during routine investigation of the abdominal cavity and as a procedure with liquid filling of the stomach cavity). Ultrasound semiotics of a neoplastic process of the stomach was described. USI can be used for screening and specification. Various methodological approaches were proposed with regard to purposes to be achieved. Routine methods (x-ray and endoscopy) were shown to remain the chief methods. USI can be used in doubtful cases or for obtaining detailed information. The most valuable are its potentialities in showing details of intraparietal changes in endophytic stomach carcinomas which are difficult for diagnosis.  相似文献

Two groups of hatchery-reared juveniles of Japanese flounder Paralichthys olivaceus (Temminck and Shlegel) were experimentally released in Wada Beach, Fukui Prefecture, Japan and their feeding state examined in comparison to wild juveniles to investigate the optimal release season and the relationship between food availability and feeding performance of juveniles. We released two groups of 40,000 juveniles of ca. 50 mm size, one group in the early (29 May) and the other in late (2 July) seasons, periods with predicted high and low densities of prey mysids, respectively, although the abundance of mysids fluctuated markedly during the season. Food availability was higher for the early release group than the late release group. Although the main stomach contents of released and wild juveniles were mainly mysids and fish, the feeding state differed between the early and late release groups. The early release group had a stomach content index (SI) similar to that of the wild juveniles, but the SI of the late group was significantly less than that of the early release group and wild juveniles. The percentage of fish with empty stomachs was much higher in the late release group than in the other two groups. In addition, the density decrease rate was significantly more rapid for the late release group than the early release group. Clear correspondence between the food availability and feeding state of the released flounder juveniles indicates that the release of juveniles in an appropriate season when mysids are abundant is crucial for successful stocking.  相似文献

Diel feeding patterns of herring Clupea harengus and mackerel Scomber scombrus in the southern Gulf of St Lawrence were examined based on samples obtained by midwater trawling between 19 and 26 June 2001. Within 3 h time periods, stomach contents tended to be more similar between fish from the same tow than between fish from different tows. Thus, in contrast to previous diet studies, which have used individual fish stomachs as independent observations, tow was used as the experimental unit in statistical analyses in this study. Diel patterns in stomach fullness were identified using generalized additive models. Two peaks in stomach fullness occurred for herring, one in the morning and the other in the evening. Mackerel showed an increase in feeding intensity throughout the day with a peak in mid‐afternoon. The diel changes in stomach contents suggested rapid gastric evacuation rates for both species, especially for herring. The estimate of the instantaneous evacuation rate for herring was twice that for mackerel. Calanus copepods (mainly C. hyperboreus ), fishes (mainly capelin Mallotus villosus ) and euphausiids were the main prey found in the stomachs of both species. Calanus copepods dominated the diet of herring regardless of time period. They also dominated the diet of mackerel during the late afternoon, evening and night while fishes and euphausiids were dominant during the morning and early afternoon. These diel patterns emphasize the need for sampling throughout the day and night in order to estimate ration and diet composition for bioenergetic and ecosystem models.  相似文献

Bariatric surgery includes a variety of procedures that are performed on obese people and aim at decreasing the intake of food and calories. This goal is usually pursued by reducing stomach capacity and/or absorbing capability. Adjustable gastric banding is the most common and successful operation. In general, bariatric surgical procedures are effective, but are often associated with major complications.Surgical procedure and post-surgical conformation of the stomach are usually defined on clinical and surgical basis only. Instead, the optimal configuration should be identified by analyzing the mechanical functionality of the stomach and the surrounding structures, and the relationship between food intake, nutrient adsorption, mechanical stimulation of stomach wall and feeling of satiety.A novel approach to bariatric surgery is required, integrating competences in the areas of biomechanics, physiology and surgery, based on a strong interaction between engineers and clinicians. Preliminary results from coupled experimental and computational investigations are here reported. The analyses aim to develop computational tools for the investigation of stomach mechanical functionality in pre- and post-surgical conformations.  相似文献

To understand the mechanisms of infection and to attempt to simulate human infection by the Anisakidae family, many in vivo experimental approaches have been developed. The aim was to develop and present a technique for the induction of an oral infection through the use of an intra-gastric gavage of live Anisakis simplex in mice. A commercial pediatric gastric tube (No. 4) was cut longitudinally to produce a 3-cm slit at the distal end where the larva was placed to then be administered to the stomach of the mouse. There were no abnormal clinical complications before, during or after the procedure. In conclusion oral infection through the direct delivery of larvae in the stomach is simple and effective.  相似文献

Substance P, neurokinin A and calcitonin gene-related peptide (CGRP) were determined in the stomach and small intestine of rats during late foetal development and up to 35 days postnatal life. Concentrations of substance P in stomach and intestine increased from 14 gestational days to 3 days postpartum, and declined thereafter. Concentrations of neurokinin A in stomach declined from 14 days gestation over the period 3-35 postnatal days. In the intestine, concentrations of neurokinin A increased steadily from 14 days gestation to 21-35 postnatal days. Concentrations of CGRP in stomach and intestine declined from 14 days gestation to 7 postnatal days. Thereafter, concentrations of CGRP increased in both stomach and intestine. Total contents of each of the three peptides increased progressively with gestational and postnatal age in parallel with increasing stomach and intestinal weights. The results demonstrate different patterns of change in the concentrations of substance P, neurokinin A and CGRP during the dynamic phases of growth and maturation of the gastrointestinal tract in the foetal and postnatal rat.  相似文献

Juvenile and adult anadromous trout Salmo trutta utilize the sea for feeding during the winter in the Skagerrak. This finding conflicts with the traditional view that anadromous trout overwinter in fresh water. Adults, just prior to spawning, were captured at sea in October to December, and spent fish were caught at sea from October to April, showing that the fish may leave the stream and move to sea just after spawning and spend the winter there. During mid‐winter (January to February), the feeding probability (chance of finding a fish with food in its stomach) increased markedly with increasing body length, with no similar effect during early and late winter (October to December and March to April). Among individuals with food in their stomach (72·5%), there was no evidence for variation in feeding intensity [stomach fullness = (mass of stomach content)(fish body mass)−1] among early, mid‐, and late winter.  相似文献
