目的:探索肺超声(LUS)在获得性肺炎(CAP)患儿诊断中的临床应用价值。方法:选取自2013年4月至2014年11月我院确诊为CAP的患儿95例,以腋前线、腋后线为界,将患儿肺脏分为前、后、侧三个区域,对所有患儿进行LUS检查,观察胸膜线、B线、肺实变程度及胸腔积液,并对支气管充气征进行检查。结果:95例CAP患儿经LUS检查后,诊断为阳性的有93例,阴性有2例,准确率为97.89%;CAP患儿的LUS的主要表现为肺泡-间质综合征、支气管充气症、胸膜线异常、胸腔积液;80例为综合表现(84.21%),15例为单一表现(15.79%)。结论:LUS诊断CAP患儿具有较高的准确率,结果安全可靠,可以应用于为临床诊断CAP。  相似文献

Virus detection methods are important to cope with the SARS-CoV-2 pandemics. Apart from the lung, SARS-CoV-2 was detected in multiple organs in severe cases. Less is known on organ tropism in patients developing mild or no symptoms, and some of such patients might be missed in symptom-indicated swab testing. Here, we tested and validated several approaches and selected the most reliable RT-PCR protocol for the detection of SARS-CoV-2 RNA in patients’ routine diagnostic formalin-fixed and paraffin-embedded (FFPE) specimens available in pathology, to assess (i) organ tropism in samples from COVID-19-positive patients, (ii) unrecognized cases in selected tissues from negative or not-tested patients during a pandemic peak, and (iii) retrospectively, pre-pandemic lung samples. We identified SARS-CoV-2 RNA in seven samples from confirmed COVID-19 patients, in two gastric biopsies, one small bowel and one colon resection, one lung biopsy, one pleural resection and one pleural effusion specimen, while all other specimens were negative. In the pandemic peak cohort, we identified one previously unrecognized COVID-19 case in tonsillectomy samples. All pre-pandemic lung samples were negative. In conclusion, SARS-CoV-2 RNA detection in FFPE pathology specimens can potentially improve surveillance of COVID-19, allow retrospective studies, and advance our understanding of SARS-CoV-2 organ tropism and effects.  相似文献

目的:生存素基因(survivin)是一种新近发现的抗凋亡基因,在肿瘤组织中呈现表达。本文旨在探讨和比较肺癌性胸腔积液和结核性胸腔积液中生存素基因的表达情况,以及其联合细胞学检查对判断肺癌性胸腔积液的敏感度。方法:应用逆转录酶-聚合酶链反应法(RT-PCR)检测2007年06月~2008年03月42例肺癌患者癌性胸腔积液标本,及同时期28例结核性胸腔积液标本的生存素mRNA表达情况,并联合细胞学检查结果进行对比分析。结果:42例肺癌患者胸腔积液标本中生存素mRNA的阳性率为52138%(22/42);癌细胞的检出率为30.95%(13/42);生存素mRNA检测联合细胞学检查诊断肺癌的敏感性为61.90%(26/42),显著高于单独胸腔积液细胞学检测的敏感性(P〈0.001)。28例结核性胸腔积液标本的生存素mRNA阳性率为7.14%(2/28),显著低于肺癌患者胸腔积液标本生存素mRNA的阳性率(P〈0.001)。结论:运用RT—PCR方法检测胸腔积液中生存素mRNA的表达在判断肺癌性胸腔积液中具有一定的敏感性和特异性,可能作为肺癌辅助诊断的一个新检测指标。  相似文献

目的研究探讨基质金属蛋白酶2(MMP-2)及血管内皮生长因子(VEGF)在胸腔积液、痰液中肺腺癌细胞的不同表达及二者在肺癌细胞侵袭转移过程中的相互关系。方法选择胸腔积液、痰液共计264例癌性及异型增生细胞标本经免疫细胞化学方法分别检测MMP-2 VEGF的表达情况。结果免疫细胞化学结果显示:MMP-2在胸腔积液中腺癌细胞、异型增生上皮细胞的表达率分别为71.7%(99/138)、16.7%(6/36),在胸膜炎和结核病变典型良性胸腔积液增生上皮细胞中不表达;在痰腺癌细胞中的表达率为39.1%(27/69),统计结果显示MMP-2在恶性胸腔积液腺癌细胞中的表达率明显高于在异型增生的上皮、增生的上皮及痰腺癌细胞的表达率(P均0.05)。VEGF在胸腔积液中腺癌细胞、异型增生上皮细胞的表达率分别为89.1%(123/138)、33.3%(12/36),在胸膜炎和结核病变典型良性胸腔积液增生上皮细胞中不表达;在痰腺癌细胞中的表达率为47.8%(33/69),VEGF在恶性胸腔积液腺癌细胞中表达率明显高于在异型增生的上皮细胞、增生的上皮细胞及痰腺癌细胞的表达率(P均0.05),且MMP-2同VEGF总阳性表达率之间成正相关(r=0.867,P=0.049)。结果 MMP-2 VEGF在胸水腺癌细胞中高表达,可能与肺腺癌的转移、侵袭有关;两者联合做免疫细胞化学检查对肺腺癌细胞病理诊断有辅助意义。  相似文献

Recent advancements in Artificial Intelligence (AI) and Machine Learning (ML) technology have brought on substantial strides in predicting and identifying health emergencies, disease populations, and disease state and immune response, amongst a few. Although, skepticism remains regarding the practical application and interpretation of results from ML-based approaches in healthcare settings, the inclusion of these approaches is increasing at a rapid pace. Here we provide a brief overview of machine learning-based approaches and learning algorithms including supervised, unsupervised, and reinforcement learning along with examples. Second, we discuss the application of ML in several healthcare fields, including radiology, genetics, electronic health records, and neuroimaging. We also briefly discuss the risks and challenges of ML application to healthcare such as system privacy and ethical concerns and provide suggestions for future applications.  相似文献

The presence of vascular endothelial growth factor (VEGF) was examined by enzyme immunoassay in 60 cytology-documented malignant pleural effusions associated with primary lung cancer and 51 other benign and malignant pleural effusions. Exudative pleural effusions contained significantly higher amounts of VEGF than transudative pleural effusions. Among exudative pleural effusions, levels of VEGF in malignant pleural effusions associated with lung cancer were significantly higher than those of benign exudative pleural effusions. There was no significant difference in pleural VEGF in patients with different histological types or clinical stages of lung cancer. Serial measurement of pleural VEGF levels was performed in six lung cancer patients treated with intrapleural instillation of recombinant interferon γ, and reduction of pleural effusion was associated with decreasing pleural VEGF levels. These findings suggest that VEGF has a role in the accumulation of exudative pleural effusions, especially that of malignant pleural effusion associated with lung cancer. 目的探讨缺氧诱导因子-1(hypoxia inducible factor-1,HIF-1)在非小细胞肺癌胸水细胞中的表达及其与肿瘤血管生成的关系。方法利用免疫组织化学、免疫细胞化学分别检测1180例非小细胞肺癌标本中HIF-1α和HIF-1β蛋白的表达以及非小细胞肺癌胸水细胞中血管生成标记物CD34和VEGF的表达情况,其中包括1060例非小细胞肺癌胸水标本以及用于对照的120例肺穿刺非小细胞肺癌组织标本。结果 HIF-1α在非小细胞肺癌穿刺组织中的表达率72.50%(87/120)明显高于非小细胞肺癌胸水中表达率17.55%(186/1060),差异有统计学意义(P0.05);HIF-1β在非小细胞肺癌穿刺组织中的表达率为77.50%(93/120),而在非小细胞肺癌胸水中表达率为81.42%(863/1060),差异无统计学意义(P0.05);CD34、VEGF在非小细胞肺癌胸水细胞中阳性表达率分别为77.92%(826/1060)、82.92%(879/1060)。HIF-1α与HIF-1β的表达呈显著正相关(r=0.093,P=0.002),HIF-1α与VEGF的表达呈显著正相关(r=104,P=0.001),HIF-1β与VEGF的表达无相关性(P0.05)。结论 HIF-1α在非小细胞肺癌穿刺组织与非小细胞肺癌胸水中的差异性表达,可能与非小细胞肺癌胸水细胞缺氧适应性调节有关,HIF-1与肿瘤血管生成密切相关。  相似文献

Pleural effusion, an accumulation of pleural fluid, contains proteins originated from plasma filtrate and, especially when tissues are damaged, parenchyma interstitial spaces of lungs and/or other organs. This study details protein profiles in human pleural effusion from 43 lung adenocarcinoma patients by a two-dimensional nano-high performance liquid chromatography electrospray ionization tandem mass spectrometry (2D nano-HPLC-ESI-MS/MS) system. The experimental results revealed the identification of 1415 unique proteins from human pleural effusion. Among these 124 proteins identified with higher confidence levels, some proteins have not been reported in plasma and may represent proteins specifically present in pleural effusion. These proteins are valuable for mass identification of differentially expressed proteins involved in proteomics database and screening biomarker to further study in human lung adenocarcinoma. The significance of the use of proteomics analysis of human pleural fluid for the search of new lung cancer marker proteins, and for their simultaneous display and analysis in patients suffering from lung disorders has been examined.  相似文献

Morphological changes in the pleura and adjacent pulmonary tissue were studied in 50 rabbits exposed to various doses of low-frequency ultrasound. When the exposure did not exceed 20 sec/cm2, no changes in pleural structure were noted. A longer period of exposure was followed by serous effusion. The contact of a wave-guide with the lung tissue caused the necrosis of the pleura and adjacent tissue.  相似文献

目的:探讨自发性气胸患者术后并发胸腔积液与肺部感染、胸水葡糖糖(Glu)水平的相关性,为自发性气胸患者术后并发胸腔积液的预防诊治提供参考。方法:选择我院2016年6月至2018年3月收治的96例行胸腔镜手术的自发性气胸患者作为研究对象,按照术后是否并发胸腔积液将其分为对照组和观察组两组,其中对照组患者56例,术后无并发症,观察组患者40例,术后并发胸腔积液。采用单因素分析法对两组患者的一般资料进行分析,并通过Pearson分析法对上述资料进行相关性分析。对术后肺部感染患者的病原菌分布及构成比进行分析,观察两组患者手术前后外周血炎症因子变化。结果:观察组患者感染率、住院时间、术后恢复时间、胸水Glu水平明显高于对照组,差异显著具有统计学意义(P0.05);对上述具有显著性差异的一般资料进行Pearson分析显示,肺部感染与SP患者术后并发胸腔积液呈现正相关,胸水Glu与SP患者术后并发胸腔积液呈现负相关(P0.05)。17例感染病例中,革兰氏阴性菌为11例,构成比64.71%,革兰氏阳性菌为6例,构成比35.29%,无真菌感染病例发生。两组患者治疗前各外周血炎症因子水平差异不具有统计学意义,具有可比性(P0.05);两组患者治疗后降钙素原(PCT)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)以及白细胞介素-10(IL-10)水平均明显升高,且观察组患者上述指标升高更为显著,差异具有统计学意义(P0.05)。结论:自发性气胸患者术后并发胸腔积液与胸水Glu水平呈现负相关,与肺部感染呈现正相关,且肺部感染患者中革兰氏阴性菌相对较多,对自发性气胸患者术后并发胸腔积液临床诊治具有一定的借鉴意义。  相似文献

Abstract

Objective: To evaluate the diagnostic value of sB7-H4 and CEA in both serum and pleural effusion of lung cancer patients.

Methods: Levels of sB7-H4 and CEA in 90 patients with malignant pleural effusion due to lung cancer and 58 patients with benign pleural effusion were measured by ELISA.

Results: The sB7-H4 and CEA levels in pleural effusion, serum and their ratio (F/S) were higher in lung cancer group than that in benign group (p?<?0.01). The diagnostic efficiency of sB7-H4 combined CEA was superior to either sB7-H4 or CEA.

Conclusions: Measurement of sB7-H4 and CEA might be useful diagnostic value for malignant effusion.  相似文献

目的:探讨肺癌患者血清及胸腔积液中的糖蛋白抗原19-9(CA19-9)、鳞状细胞癌抗原(SCC-Ag)和细胞角蛋白19片段(CYFRA21-1)对肺癌的诊断意义。方法:选取2016年1月到2017年6月在我院接受治疗的肺癌患者67例作为肺癌组,另选取我院同期收治的肺良性病变患者55例纳入良性病变组。采用电化学发光法检测并对比两组患者血清及胸腔积液中的CA19-9、SCC-Ag和CYFRA21-1水平,比较所有研究对象血清及胸腔积液中CA19-9、SCC-Ag和CYFRA21-1的阳性率并分析其诊断价值。结果:肺癌组患者血清及胸腔积液中的CA19-9、SCC-Ag和CYFRA21-1水平显著高于良性病变组,有统计学差异(P0.05)。CA19-9、SCC-Ag和CYFRA21-1在胸腔积液中的阳性率高于在血清中的阳性率,有统计学差异(P0.05)。胸腔积液中CA19-9、SCC-Ag和CYFRA21-1单项检测对肺癌的灵敏度显著高于血清检测,血清及胸腔积液中CA19-9、SCC-Ag和CYFRA21-1三项联合检测的灵敏度、特异性、阳性预测值均高于单项检测,差异均有统计学意义(P0.05)。结论:肺癌患者血清及胸腔积液中CA19-9、SCC-Ag和CYFRA21-1呈现高表达,三项指标联合检测可提高诊断肺癌的灵敏度、特异性和阳性预测值。  相似文献

目的探讨胸腹水细胞学检验在恶性肿瘤细胞诊断中的应用价值。方法回顾我院2014年1月~2016年6月376例胸腹水脱落细胞学的检验结果,分析胸腹水细胞学检验在恶性肿瘤诊断中的应用价值。结果 376例胸腹水脱落细胞学检验患者中,发现疑似肿瘤细胞3例,恶性肿瘤细胞74例,病理组织学证实恶性肿瘤患者79例,胸腹水细胞学检验的正确率高达97.47%,假阴性率为2.53%,假阳性率为0%。结论胸腹水脱落细胞学检验在恶性肿瘤诊断中具有重要的临床价值,值得临床推广。  相似文献

 The effect of intrapleural instillation of recombinant human interferon γ (IFNγ) at increasing doses of (1–12) × 106 U was examined in six patients with cytologically positive pleural effusion due to lung cancer. Intrapleural instillation was repeated up to three times. Clinically, no reaccumulation of pleural effusion was observed in one patient and disappearance of lung cancer cells from the pleural effusion was seen in two other patients. No severe side-effects were observed. Considerable levels of IFNγ remained in the pleural effusion as well as in patients’ serum up to 7 days after instillation of 2 × 106 U and higher doses. The total cell number showed a transient decrease on day 1 of therapy. Levels of pro-inflammatory cytokines, such as tumor necrosis factor α, interleukin(IL)-1β and IL-6, in the pleural effusion remained almost stable after IFNγ instillation. On the other hand, intrapleural IL-1 receptor antagonist levels were remarkably elevated by the instillation of IFNγ. IL-2- and IL-12-inducible killer activity of pleural mononuclear cells tended to increase slightly. Despite the inability of IFNγ to control pleural effusion in this treatment schedule, IFNγ instilled by an intrapleural route had a potential local antitumor activity. Moreover, since IFNγ persists in pleural effusions for a long time after a single instillation, such a therapy in combination with other fibrogenic biological response modifiers can be promising. 目的应用免疫细胞化学对胸腔积液中的肺非小细胞癌分类与恶性间皮瘤的鉴别诊断。方法利用液基薄层细胞学自动涂片技术方法对筛查出的胸腔积液可疑瘤细胞及瘤细胞标本1158例进行细胞包埋连续切片,分别作肺非小细胞癌(NSCLC)肿瘤细胞标记物CK7、CK5&6、TTF-1、E—Ca及恶性间皮瘤标记物MC(MesothelialCell,MC)、CR(Calfetinin,CR)、P53、Vimentin免疫细胞化学染色。结果1158例胸腔积液患者确诊为肺腺癌581例,鳞癌509例,腺鳞癌48例,恶性间皮瘤20例。TTF-1在腺癌中有明显高表达,阳性表达率为92.43%;CK58L6在鳞癌中有明显高表达,阳性表达率为97.45%;MC、CR在恶性间皮瘤中有明显高表达,阳性表达率为100.00%和95.00%。结论液基细胞学与免疫细胞化学技术相结合在胸腔积液鉴别诊断中有很重要的临床意义,CK7、CK58L6、TTF-1、E—ca联合应用可用于胸腔积液中NSCLC之间的分类与诊断,CK58L6、MC、CR、P53、Vimentin联合应用可用于胸腔积液中间皮瘤的定性诊断,值得在临床细胞病理学诊断中推广应用。  相似文献

In this study we applied narrow‐range peptide IEF to plasma or pleural effusion prior to LC/MS/MS. Two methods for narrow‐range IEF were run; IPG strips and free‐flow electrophoresis. Data from this study was compared with cell line data to evaluate the method performance in body fluids. To test the methods potential in quantitative biomarker discovery studies, plasma and pleural effusion from patients with lung adenocarcinoma (n=3) were compared with inflammatory pleuritis (n=3) using iTRAQ quantification. Using narrow‐range IEF on the peptide level we were able to identify and quantify 282 proteins in plasma and 300 proteins in pleural effusion. These body fluid proteomes demonstrated high degree of overlap; however, more proteins significantly differently altered levels related to adenenocarcinoma were found in pleural effusion compared with plasma, suggesting enrichment of lung tissue‐related proteins in pleural effusion. Nine proteins were chosen for initial validation with Western blot, and one protein (NPC2) was chosen for further validation using imunohistochemistry. Overall, the quantitative results from IEF/LC/MS/MS showed good correlation with the results from Western blot and imunohistochemistry, showing the potential of this methodology in quantitative biomarker discovery studies.  相似文献

The Ca1 antibody was used in an immunohistochemical procedure on smears of cells from 40 patients with malignant pleural effusion. The control group consisted of 25 benign pleural effusions with a high percentage of reactive mesothelial cells. The Ca1 Mc Ab was positive in 19 (79%) of the 24 pleural effusions with positive malignant cytology. In all the benign cases the Ca1 Mc Ab was negative (100% specificity). The Ca1 Mc Ab detected malignant mesothelial cells in two cases and was negative with reactive mesothelial cells and other nucleated cells present in the pleural effusion. We conclude that the Ca1 antibody offers a useful diagnostic method for malignant pleural effusions, when the morphological interpretation is doubtful.  相似文献

Pleural epithelial adaptations to mechanical stress are relevant to both normal lung function and parenchymal lung diseases. Assessing regional differences in mechanical stress, however, has been complicated by the nonlinear stress–strain properties of the lung and the large displacements with ventilation. Moreover, there is no reliable method of isolating pleural epithelium for structural studies. To define the topographic variation in pleural structure, we developed a method of en face harvest of murine pleural epithelium. Silver-stain was used to highlight cell borders and facilitate imaging with light microscopy. Machine learning and watershed segmentation were used to define the cell area and cell perimeter of the isolated pleural epithelial cells. In the deflated lung at residual volume, the pleural epithelial cells were significantly larger in the apex (624 ± 247 μm2) than in basilar regions of the lung (471 ± 119 μm2) (p < 0.001). The distortion of apical epithelial cells was consistent with a vertical gradient of pleural pressures. To assess epithelial changes with inflation, the pleura was studied at total lung capacity. The average epithelial cell area increased 57% and the average perimeter increased 27% between residual volume and total lung capacity. The increase in lung volume was less than half the percent change predicted by uniform or isotropic expansion of the lung. We conclude that the structured analysis of pleural epithelial cells complements studies of pulmonary microstructure and provides useful insights into the regional distribution of mechanical stresses in the lung.  相似文献
