Detailed knowledge about loading of the knee joint is essential for preclinical testing of implants, validation of musculoskeletal models and biomechanical understanding of the knee joint. The contact forces and moments acting on the tibial component were therefore measured in 5 subjects in vivo by an instrumented knee implant during various activities of daily living.Average peak resultant forces, in percent of body weight, were highest during stair descending (346% BW), followed by stair ascending (316% BW), level walking (261% BW), one legged stance (259% BW), knee bending (253% BW), standing up (246% BW), sitting down (225% BW) and two legged stance (107% BW). Peak shear forces were about 10–20 times smaller than the axial force. Resultant forces acted almost vertically on the tibial plateau even during high flexion. Highest moments acted in the frontal plane with a typical peak to peak range ?2.91% BWm (adduction moment) to 1.61% BWm (abduction moment) throughout all activities. Peak flexion/extension moments ranged between ?0.44% BWm (extension moment) and 3.16% BWm (flexion moment). Peak external/internal torques lay between ?1.1% BWm (internal torque) and 0.53% BWm (external torque).The knee joint is highly loaded during daily life. In general, resultant contact forces during dynamic activities were lower than the ones predicted by many mathematical models, but lay in a similar range as measured in vivo by others. Some of the observed load components were much higher than those currently applied when testing knee implants.  相似文献

Orthodontic miniscrews are exposed to three mechanical loading phases during clinical use: torsional loading upon insertion, flexural loading during anchorage function, and torsional loading upon removal. The aim of this study was to simulate clinical loading conditions for different types of orthodontic miniscrews in vitro to quantify the effects of combined torsional and bending stress. Various orthodontic miniscrew systems (Lomas, Dual-top, Aarhus anchorage, Tomas-pin and T.I.T.A.N.-pin) comprising 10 samples each were subjected to the following loading sequences in vitro: a torsional load corresponding to manual insertion with limited torque; and flexural loading at two different insertion depths. For all screw systems with torsional pre-loading (simulating insertion), subsequent flexural loading (simulating anchorage) yielded permanent deformations of approximately 0.15-0.25 mm, depending on the insertion depth. Since EDX analysis revealed comparable elemental compositions for the different screw systems, the differences in mechanical properties are attributed to screw design. Torsional loading during screw insertion may cause premature mechanical weakening and needs to be minimized. Unless fully inserted, screws show pronounced plastic deformation and hence fracture risk under subsequent flexural loading.  相似文献

Three-dimensional load measurements in an external fixator   总被引:1,自引:0,他引:1  
On the basis of a six-degree-of-freedom adjustable fracture reduction hexapod external fixator, a system which can be used for measuring axial and shear forces as well as torsion and bending moments in the fixator in vivo was developed. In a pilot study on 9 patients (7 fresh fractures and 2 osteotomies of the tibia), the load in the fixator during the healing process was measured after 2, 4, 8 and 12 weeks and at fixator removal. The measured values enabled both the type of fracture to be determined as well as the monitoring of the healing process. In well-reduced type A3 fractures small axial (direction of the bone axis) forces were found in the fixator. A2, B2 and C3 fractures showed distinct axial forces, which decreased during the healing process, according to an increasing load transfer over the bone. Bending moments in the fixator showed good correspondence with the clinical healing process, except in the case of a C3 fracture. A combination of bending moment and axial force proved to be particularly suitable to assess fracture healing. In transverse fractures, the well-known resorption phenomenon of bone in the fracture gap at approximately 4 weeks was detected by the system. Compared with other external fixator load measurements in vivo, the hexapod offers the advantage of being able to measure all forces and moments in the fixator separately and with a relatively simple mechanical arrangement. In our opinion, it will be possible to control fracture healing using this system, thereby minimizing radiation exposure from radiographs. Furthermore, the measurement system is a step towards the development of external fixator systems that enable automatic adjustments of the callus mechanical situation ("automatic dynamization") and inform the patients about the optimal weight bearing of their extremity ("intelligent fixator").  相似文献

In vitro multi-axial bending testing using pure moment loading conditions has become the standard in evaluating the effects of different types of surgical intervention on spinal kinematics. Simple, cable-driven experimental set-ups have been widely adopted because they require little infrastructure. Traditionally, “fixed ring” cable-driven experimental designs have been used; however, there have been concerns with the validity of this set-up in applying pure moment loading. This study involved directly comparing the loading state induced by a traditional “fixed ring” apparatus versus a novel “sliding ring” approach. Flexion-extension bending was performed on an artificial spine model and a single cadaveric test specimen, and the applied loading conditions to the specimen were measured with an in-line multiaxial load cell. The results showed that the fixed ring system applies flexion-extension moments that are 50–60% less than the intended values. This design also imposes non-trivial anterior–posterior shear forces, and non-uniform loading conditions were induced along the length of the specimen. The results of this study indicate that fixed ring systems have the potential to deviate from a pure moment loading state and that our novel sliding ring modification corrects this error in the original test design. This suggests that the proposed sliding ring design should be used for future in vitro spine biomechanics studies involving a cable-driven pure moment apparatus.  相似文献

The development of a novel instrumented implant for ulnar head replacement is presented in this study. This implant was instrumented with strain gauges to quantify bending moments about the anatomic axes of the distal ulna, and subsequently the distal radioulnar joint (DRUJ) reaction force magnitude. The implant was surgically inserted in seven cadaveric upper extremities, which were subsequently mounted in a custom joint simulator. Simulated active unresisted pronation and supination motion trials were conducted using computer-controlled pneumatic actuators to simulate forearm musculature. Passive (unloaded) trials were also conducted. The reaction force across the DRUJ ranged from 2 to 10 N in magnitude during this unresisted motion. Increased bending moment magnitudes were measured when the forearm was positioned in supination compared to pronation. The magnitude of joint bending moments showed a consistent pattern with forearm position, regardless of simulated active or passive rotation, or supination and pronation motion trials. This result illustrates that the primary influence on joint load is likely the position and contact with the radial articulation. This study of DRUJ loading should be useful for biomechanical modeling, implant design considerations and improved knowledge of articular mechanics.  相似文献

Ultimate strength of the lumbar spine in flexion--an in vitro study   总被引:2,自引:0,他引:2  
The ultimate strength in flexion of 16 lumbar functional spinal units (FSU) was determined. The specimens were exposed to a combined static load of bending and shearing in the sagittal plane until overt rupture occurred (simulated flexion-distraction injuries). The biomechanical response of the FSU was measured with a force and moment platform. Mechanical displacement gauges were used to measure vertical displacements (flexion angulation) of the specimens. Photographs were taken after each loading step for determination of horizontal displacements and the centre of rotation. The lumbar FSU could resist a combination of bending moment and shear force of 156 Nm and 620 N respectively, before complete disruption occurred. The tension force acting on the posterior structures was 2.8 kN. The flexion angulation just before failure was 20 degrees and the anterior horizontal displacement between the upper and lower vertebrae was 9 mm. The centre of rotation was located in the posterior part of the lower vertebral body. The bone mineral content in the vertebrae appeared to be a good predictor of ultimate strength of the lumbar FSU. Knowledge of the biomechanical response of the lumbar spine under different static traumatic loads is a first step to better understand the injury mechanisms of the spine in traffic accidents.  相似文献

The effects of the rib cage on thoracic spine loading are not well studied, but the rib cage may provide stability or share loads with the spine. Intervertebral disc pressure provides insight into spinal loading, but such measurements are lacking in the thoracic spine. Thus, our objective was to examine thoracic intradiscal pressures under applied pure moments, and to determine the effect of the rib cage on these pressures. Human cadaveric thoracic spine specimens were positioned upright in a testing machine, and Dynamic pure moments (0 to ±5 N·m) with a compressive follower load of 400 N were applied in axial rotation, flexion - extension, and lateral bending. Disc pressures were measured at T4-T5 and T8-T9 using needle-mounted pressure transducers, first with the rib cage intact, and again after the rib cage was removed. Changes in pressure vs. moment slopes with rib cage removal were examined. Pressure generally increased with applied moments, and pressure-moment slope increased with rib cage removal at T4-T5 for axial rotation, extension, and lateral bending, and at T8-T9 for axial rotation. The results suggest the intact rib cage carried about 62% and 56% of axial rotation moments about T4-T5 and T8-T9, respectively, as well as 42% of extension moment and 36–43% of lateral bending moment about T4-T5 only. The rib cage likely plays a larger role in supporting moments than compressive loads, and may also play a larger role in the upper thorax than the lower thorax.  相似文献

After spinal stabilization with an implant, patients want to know whether carrying loads will endanger their spine and their implant. Therefore, the effect of carrying a weight on the loading of internal spinal fixation devices was determined in 10 patients using instrumented, telemeterized internal fixators. Patients carried a 5kg dumbbell in one hand as well as up to 20kg in both hands. Compared to normal standing carrying a weight caused only a slight increase of the fixator loads in all patients. The maximum flexion bending moment in the fixators was then lower than during walking. Carrying a weight even decreased the axial compression force in the fixators of the patients with the T11 or T12 vertebra bridged. Patients also raised their arms in anteversion while holding weights of up to 5kg with both hands. This increased the flexion bending moment in the fixators. Only a small part of a carried weight is taken over by internal fixators. Carrying a shopping bag only slightly increases the risk of pedicle screw breakage, however, since the spine has to support most of the additional weight, it represents a risk with regard to correction loss and bone sintering.  相似文献

A three-dimensional nonlinear finite element model of the lumbar spine was developed. Paired threaded cages and a monosegmental internal spinal fixation device were integrated into the computer model. The model was loaded with such forces as apply during standing, as well as with pure moments in the three main anatomical planes, plus an additional preload. The latter was generated by shortening the distance between the pedicle screws on the longitudinal rod of the fixator. With the exception of torsional loading, an implant appreciably reduces the mobility in the segment concerned. At the loads studied, cages had only a minor impact on the movements and stresses in the adjacent regions, but a strong influence on the stresses in the endplates in contact with them. A preload increases these stresses dramatically. Contact conditions between vertebral body and cages also have a marked effect on the stress distribution in the corresponding vertebral endplate, especially in the case of extension loading. Owing to the preload, maximum stresses were higher for the rigid bond than when contact elements were used.  相似文献

Pure moment testing has become a standard protocol for in vitro assessment of the effect of surgical techniques or devices on the bending rigidity of the spine. Of the methods used for pure moment testing, cable-driven set-ups are popular due to their low requirements and simple design. Fixed loading rings are traditionally used in conjunction with these cable-driven systems. However, the accuracy and validity of the loading conditions applied with fixed ring designs have raised some concern, and discrepancies have been found between intended and prescribed loading conditions for flexion-extension. This study extends this prior work to include lateral bending and axial torsion, and compares this fixed ring design with a novel "3D floating ring" design. A complete battery of multi-axial bending tests was conducted with both rings in multiple different configurations using an artificial lumbar spine. Applied moments were monitored and recorded by a multi-axial load cell at the base of the specimen. Results indicate that the fixed ring design deviates as much as 77% from intended moments and induces non-trivial shear forces (up to 18 N) when loaded to a non-destructive maximum of 4.5 Nm. The novel 3D floating ring design largely corrects the inherent errors in the fixed ring design by allowing additional directions of unconstrained motion and producing uniform loading conditions along the length of the specimen. In light of the results, it is suggested that the 3D floating ring set-up be used for future pure moment spine biomechanics applications using a cable-driven apparatus.  相似文献

The mechanical coupling behaviour of the thoracic spine is still not fully understood. For the validation of numerical models of the thoracic spine, however, the coupled motions within the single spinal segments are of importance to achieve high model accuracy. In the present study, eight fresh frozen human thoracic spinal specimens (C7-L1, mean age 54 ± 6 years) including the intact rib cage were loaded with pure bending moments of 5 Nm in flexion/extension (FE), lateral bending (LB), and axial rotation (AR) with and without a follower load of 400 N. During loading, the relative motions of each vertebra were monitored. Follower load decreased the overall ROM (T1-T12) significantly (p < 0.01) in all primary motion directions (extension: −46%, left LB: −72%, right LB: −72%, left AR: −26%, right AR: −26%) except flexion (−36%). Substantial coupled motion was found in lateral bending with ipsilateral axial rotation, which increased after a follower load was applied, leading to a dominant axial rotation during primary lateral bending, while all other coupled motions in the different motion directions were reduced under follower load. On the monosegmental level, the follower load especially reduced the ROM of the upper thoracic spine from T1-T2 to T4-T5 in all motion directions and the ROM of the lower thoracic spine from T9-T10 to T11-T12 in primary lateral bending. The facet joints, intervertebral disc morphologies, and the sagittal curvature presumably affect the thoracic spinal coupled motions depending on axial compressive preloading. Using these results, the validation of numerical models can be performed more accurately.  相似文献

Activities with high spinal loads should be avoided by patients with back problems. Awareness about these activities and knowledge of the associated loads are important for the proper design and pre-clinical testing of spinal implants. The loads on an instrumented vertebral body replacement have been telemetrically measured for approximately 1000 combinations of activities and parameters in 5 patients over a period up to 65 months postoperatively. A database containing, among others, extreme values for load components in more than 13,500 datasets was searched for 10 activities that cause the highest resultant force, bending moment, torsional moment, or shear force in an anatomical direction. The following activities caused high resultant forces: lifting a weight from the ground, forward elevation of straight arms with a weight in hands, moving a weight laterally in front of the body with hanging arms, changing the body position, staircase walking, tying shoes, and upper body flexion. All activities have in common that the center of mass of the upper body was moved anteriorly. Forces up to 1650 N were measured for these activities of daily life. However, there was a large intra- and inter-individual variation in the implant loads for the various activities depending on how exercises were performed. Measured shear forces were usually higher in the posterior direction than in the anterior direction. Activities with high resultant forces usually caused high values of other load components.  相似文献

Simplified loading modes (pure moment, compressive force) are usually applied in the in vitro studies to simulate flexion-extension, lateral bending and axial rotation of the spine. The load magnitudes for axial rotation vary strongly in the literature. Therefore, the results of current investigations, e.g. intervertebral rotations, are hardly comparable and may involve unrealistic values. Thus, the question 'which in vitro applicable loading mode is the most realistic' remains open. A validated finite element model of the lumbar spine was employed in two sensitivity studies to estimate the ranges of results due to published load assumptions and to determine the input parameters (e.g. torsional moment), which mostly affect the spinal load and kinematics during axial rotation. In a subsequent optimisation study, the in vitro applicable loading mode was determined, which delivers results that fit best with available in vivo measurements. The calculated results varied widely for loads used in the literature with potential high deviations from in vivo measured values. The intradiscal pressure is mainly affected by the magnitude of the compressive force, while the torsional moment influences mainly the intervertebral rotations and facet joint forces. The best agreement with results measured in vivo were found for a compressive follower force of 720N and a pure moment of 5.5Nm applied to the unconstrained vertebra L1. The results reveal that in many studies the assumed loads do not realistically simulate axial rotation. The in vitro applicable simplified loads cannot perfectly mimic the in vivo situation. However, the optimised values lead to the best agreement with in vivo measured values. Their consequent application would lead to a better comparability of different investigations.  相似文献

The Achilles tendon and epimuscular connective tissues mechanically link the triceps surae muscles. These pathways may cause joint moments exerted by each muscle individually not to sum linearly, both in magnitude and direction. The aims were (i) to assess effects of sagittal plane ankle angle (varied between 150° and 70°) on isometric ankle moments, in both magnitude and direction, exerted by active rat triceps surae muscles, (ii) to assess ankle moment summation between those muscles for a range of ankle angles and (iii) to assess effects of sagittal plane ankle angle and muscle activation on Achilles tendon length. At each ankle angle, soleus (SO) and gastrocnemius (GA) muscles were first excited separately to assess ankle-angle moment characteristics and subsequently both muscles were excited simultaneously to investigate moment summation. The magnitude of ankle moment exerted by SO and GA, the SO direction in the transverse and sagittal planes, and the GA direction in the transverse plane were significantly affected by ankle angle. SO moment direction in the frontal and sagittal planes were significantly different from that of GA. Nonlinear magnitude summation varied between 0.6±2.9% and −3.6±2.9%, while the nonlinear direction summation varied between 0.3±0.4° and −0.4±0.7° in the transverse plane, between 0.5±0.4° and 0.1±0.4° in the frontal plane, and between 3.0±7.9° and 0.3±2.3° in the sagittal plane. Changes in tendon length caused by SO contraction were significantly lower than those during contraction of GA and GA+SO simultaneously. Thus, moments exerted by GA and SO sum nonlinearly both in the magnitude and direction. The limited degree of nonlinear summation may be explained by different mechanisms acting in opposite directions.  相似文献

Contact forces and moments act on orthopaedic implants such as joint replacements. The three forces and three moment components can be measured by six internal strain gauges and wireless telemetric data transmission. The accuracy of instrumented implants is restricted by their small size, varying modes of load transfer, and the accuracy of calibration. Aims of this study were to test with finite element studies design features to improve the accuracy, to develop simple but accurate calibration arrangements, and to select the best mathematical method for calculating the calibration constants. Several instrumented implants, and commercial and test transducers were calibrated using different loading setups and mathematical methods. It was found that the arrangement of flexible elements such as bellows or notches between the areas of load transfer and the central sensor locations is most effective to improve the accuracy. Increasing the rigidity of the implant areas, which are fixed in bones or articulate against joint surfaces, is less effective. Simple but accurate calibration of the six force and moment components can be achieved by applying eccentric forces instead of central forces and pure moments. Three different methods for calculating the measuring constants proved to be equally well suited. Employing these improvements makes it possible to keep the average measuring errors of many instrumented implants below 1-2% of the calibration ranges, including cross talk. Additional errors caused by noise of the transmitted signals can be reduced by filtering if this is permitted by the sampling rate and the required frequency content of the loads.  相似文献

Asymmetrical lifting and lowering are predominant activities in the workplace. Mechanical causes are suggested for many back injuries and the dynamic conditions within which spine loading occurs are related to spine loading increase. More data on tridimensional biomechanical lumbar spine loading during asymmetrical lifting and lowering are needed. A tridimensional dynamic multisegment model was developed to compute spinal loading for asymmetrical box-handling situations. The tridimensional positions of the anatomical markers were generated by a direct linear transformation algorithm adapted for the processing of data from two real and two virtual views (mirrors). Two force platforms measured the external forces. Five male subjects performed three variations (slow, fast and accelerated) of asymmetric lifting and two variations (slow and fast) of asymmetric lowering. The torsional, extension/flexion and lateral bending net muscular moments at the L5/S1 joint were computed and peak values selected for statistical analysis. For the lifting task, the fast and accelerated conditions showed significant increases over the slow condition for torsion, extension/flexion and lateral-bending moments. The accelerated condition also showed significant increases over the fast condition for extension. A comparison between lifting and lowering tasks showed equivalent loadings for torsion and extension. The moments were compared to average maximal values measured on equivalent male subject populations by isokinetic dynamometry. This showed torsional and extension values of 30 and 83% of the maximal possible subject capacity, respectively. These results demonstrated that dynamic factors do influence the load on the spine and highlighted the influence of both lifting and lowering on the loading of the spine. This suggested that for a more complete analysis of asymmetrical handling, the maximal velocity and acceleration produced during lifting should be included.  相似文献

Little work has been done to examine the deep squat position (>130° sagittal knee flexion). In baseball and softball, catchers perform this squat an average of 146 times per nine-inning game. To alleviate some of the stress on their knees caused by this repetitive loading, some catchers wear foam knee supports.ObjectivesThis work quantifies the effects of knee support on lower-body joint kinematics and kinetics in the deep squat position.MethodsSubjects in this study performed the deep squat with no support, foam support, and instrumented support. In order to measure the force through the knee support, instrumented knee supports were designed and fabricated. We then developed an inverse dynamic model to incorporate the support loads. From the model, joint angles and moments were calculated for the three conditions.ResultsWith support there is a significant reduction in the sagittal moment at the knee of 43% on the dominant side and 63% on the non-dominant side compared to without support. These reductions are a result of the foam supports carrying approximately 20% of body weight on each side.ConclusionKnee support reduces the moment necessary to generate the deep squat position common to baseball catchers. Given the short moment arm of the patella femoral tendon, even small changes in moment can have a large effect in the tibial-femoral contact forces, particularly at deep squat angles. Reducing knee forces may be effective in decreasing incidence of osteochondritis dissecans.  相似文献

A wide range of loading conditions involving external forces with varying magnitudes, orientations and locations are encountered in daily activities. Here we computed the effect on trunk biomechanics of changes in force location (two levels) and orientation (5 values) in 4 subjects in upright standing while maintaining identical external moment of 15 Nm, 30 N m or 45 Nm at the L5–S1. Driven by measured kinematics and gravity/external loads, the finite element models yielded substantially different trunk neuromuscular response with moderate alterations (up to 24% under 45 Nm moment) in spinal loads as the load orientation varied. Under identical moments, compression and shear forces at the L5–S1 as well as forces in extensor thoracic muscles progressively decreased as orientation of external forces varied from downward gravity (90°) all the way to upward (−25°) orientation. In contrast, forces in local lumbar muscles followed reverse trends. Under larger horizontal forces at a lower elevation, lumbar muscles were much more active whereas extensor thoracic muscle forces were greater under smaller forces at a higher elevation. Despite such differences in activity pattern, the spinal forces remained nearly identical (<6% under 45 Nm moment). The published recorded surface EMG data of extensor muscles trend-wise agreed with computed local muscle forces as horizontal load elevation varied but were overall different from results in both local and global muscles when load orientation altered. Predictions demonstrate the marked effect of external force orientation and elevation on the trunk neuromuscular response and spinal forces and questions attempts to estimate spinal loads based only on consideration of moments at a spinal level.  相似文献

Two distal femoral replacements were instrumented to measure axial force, torque and bending moments in the prosthesis shaft. Data are reported up to 2.5 years for the following activities: uni- and bi-lateral standing, walking, stair climbing and descending, treadmill walking, jogging and jumping. In the first subject the greatest averaged peak shaft forces found were: jogging 3.6Bodyweight (BW), stair descending 3.1BW, walking 2.8BW, treadmill walking 2.75BW, and stair ascending 2.8BW. Bending moments about the antero-posterior axis (varus-valgus) and medio-lateral axis (flexion-extension) peaked in the range 8.5-9.8 and 4.7-7.6BWcm respectively, over the follow-up period. Axial torques peaked in the range 0.2-1.3BWcm, outwardly directed. At most follow-up sessions, forces and moments during jogging were generally greater than those for other gait activities. In the second subject forces and moments were generally only 45-70% of those in the first subject, due to inadequate musculature around the knee. The data can be applied to the design and testing of distal femoral replacements and even to total knee replacements, and contributes to the knowledge of forces acting in the distal femur during activity.  相似文献

The Bacterial flagellar filament can undergo a polymorphic phase transition in response to both mechanical and chemical variations in vitro and in vivo environments. Under mechanical stimuli, such as viscous flow or forces induced by motor rotation, the filament changes its phase from left-handed normal (N) to right-handed semi-coiled (SC) via phase nucleation and growth. Our detailed mechanical analysis of existing experiments shows that both torque and bending moment contribute to the filament phase transition. In this paper, we establish a non-convex and non-local continuum model based on the Ginzburg-Landau theory to describe main characteristics of the filament phase transition such as new-phase nucleation, growth, propagation and the merging of neighboring interfaces. The finite element method (FEM) is adopted to simulate the phase transition under a displacement-controlled loading condition (rotation angle and bending deflection). We show that new-phase nucleation corresponds to the maximum torque and bending moment at the stuck end of the filament. The hysteresis loop in the loading and unloading curves indicates energy dissipation. When the new phase grows and propagates, torque and bending moment remain static. We also find that there is a drop in load when the two interfaces merge, indicating a concomitant reduction in the interfacial energy. Finally, the interface thickness is governed by the coefficients of the gradient of order parameters in the non-local interface energy. Our continuum theory and the finite element method provide a method to study the mechanical behavior of such biomaterials.  相似文献
