Epidemic control strategies alter the spread of the disease in the host population. In this paper, we describe and discuss mathematical models that can be used to explore the potential of pre-exposure and post-exposure vaccines currently under development in the control of tuberculosis. A model with bacille Calmette-Guerin (BCG) vaccination for the susceptibles and treatment for the infectives is first presented. The epidemic thresholds known as the basic reproduction numbers and equilibria for the models are determined and stabilities are investigated. The reproduction numbers for the models are compared to assess the impact of the vaccines currently under development. The centre manifold theory is used to show the existence of backward bifurcation when the associated reproduction number is less than unity and that the unique endemic equilibrium is locally asymptotically stable when the associated reproduction number is greater than unity. From the study we conclude that the pre-exposure vaccine currently under development coupled with chemoprophylaxis for the latently infected and treatment of infectives is more effective when compared to the post-exposure vaccine currently under development for the latently infected coupled with treatment of the infectives.  相似文献

An susceptible-infective-removed epidemic model incorporating media coverage with time delay is proposed. The stability of the disease-free equilibrium and endemic equilibrium is studied. And then, the conditions which guarantee the existence of local Hopf bifurcation are given. Furthermore, we show that the local Hopf bifurcation implies the global Hopf bifurcation after the second critical value of delay. The obtained results show that the time delay in media coverage can not affect the stability of the disease-free equilibrium when the basic reproduction number is less than unity. However, the time delay affects the stability of the endemic equilibrium and produces limit cycle oscillations while the basic reproduction number is greater than unity. Finally, some examples for numerical simulations are included to support the theoretical prediction.  相似文献

In this paper, we present a deterministic non-linear mathematical model for the transmission dynamics of HIV and TB co-infection and analyze it in the presence of screening and treatment. The equilibria of the model are computed and stability of these equilibria is discussed. The basic reproduction numbers corresponding to both HIV and TB are found and we show that the disease-free equilibrium is stable only when the basic reproduction numbers for both the diseases are less than one. When both the reproduction numbers are greater than one, the co-infection equilibrium point may exist. The co-infection equilibrium is found to be locally stable whenever it exists. The TB-only and HIV-only equilibria are locally asymptotically stable under some restriction on parameters. We present numerical simulation results to support the analytical findings. We observe that screening with proper counseling of HIV infectives results in a significant reduction of the number of individuals progressing to HIV. Additionally, the screening of TB reduces the infection prevalence of TB disease. The results reported in this paper clearly indicate that proper screening and counseling can check the spread of HIV and TB diseases and effective control strategies can be formulated around ‘screening with proper counseling’.  相似文献

A two strain HIV/AIDS model with treatment which allows AIDS patients with sensitive HIV-strain to undergo amelioration is presented as a system of non-linear ordinary differential equations. The disease-free equilibrium is shown to be globally asymptotically stable when the associated epidemic threshold known as the basic reproduction number for the model is less than unity. The centre manifold theory is used to show that the sensitive HIV-strain only and resistant HIV-strain only endemic equilibria are locally asymptotically stable when the associated reproduction numbers are greater than unity. Qualitative analysis of the model including positivity, boundedness and persistence of solutions are presented. The model is numerically analysed to assess the effects of treatment with amelioration on the dynamics of a two strain HIV/AIDS model. Numerical simulations of the model show that the two strains co-exist whenever the reproduction numbers exceed unity. Further, treatment with amelioration may result in an increase in the total number of infective individuals (asymptomatic) but results in a decrease in the number of AIDS patients. Further, analysis of the reproduction numbers show that antiretroviral resistance increases with increase in antiretroviral use.  相似文献

This paper presents a number of deterministic models for theoretically assessing the potential impact of an imperfect prophylactic HIV-1 vaccine that has five biological modes of action, namely “take,” “degree,” “duration,” “infectiousness,” and “progression,” and can lead to increased risky behavior. The models, which are of the form of systems of nonlinear differential equations, are constructed via a progressive refinement of a basic model to incorporate more realistic features of HIV pathogenesis and epidemiology such as staged progression, differential infectivity, and HIV transmission by AIDS patients. The models are analyzed to gain insights into the qualitative features of the associated equilibria. This allows the determination of important epidemiological thresholds such as the basic reproduction numbers and a measure for vaccine impact or efficacy. The key findings of the study include the following (i) if the vaccinated reproduction number is greater than unity, each of the models considered has a locally unstable disease-free equilibrium and a unique endemic equilibrium; (ii) owing to the vaccine-induced backward bifurcation in these models, the classical epidemiological requirement of vaccinated reproduction number being less than unity does not guarantee disease elimination in these models; (iii) an imperfect vaccine will reduce HIV prevalence and mortality if the reproduction number for a wholly vaccinated population is less than the corresponding reproduction number in the absence of vaccination; (iv) the expressions for the vaccine characteristics of the refined models take the same general structure as those of the basic model.  相似文献

In this paper, an epidemiological model with age of infection and disease relapse is investigated. The basic reproduction number for the model is identified, and it is shown to be a sharp threshold to completely determine the global dynamics of the model. By analysing the corresponding characteristic equations, the local stability of a disease-free steady state and an endemic steady state of the model is established. By means of suitable Lyapunov functionals and LaSalle's invariance principle, it is verified that if the basic reproduction number is less than unity, the disease-free steady state is globally asymptotically stable, and hence the disease dies out; if the basic reproduction number is greater than unity, the endemic steady state is globally asymptotically stable and the disease becomes endemic.  相似文献

We derived and analyzed rigorously a mathematical model that describes the dynamics of malaria infection with the recruitment of infected immigrants, treatment of infectives and spray of insecticides against mosquitoes in the population. Both qualitative and quantitative analysis of the deterministic model are performed with respect to stability of the disease free and endemic equilibria. It is found that in the absence of infected immigrants disease-free equilibrium is achievable and is locally asymptotically stable. Using Pontryagin's Maximum Principle, the optimal strategies for disease control are established. Finally, numerical simulations are performed to illustrate the analytical results.  相似文献

In this research article, an epidemiological model is formulated for mosaic disease considering plant and vector populations. Plant host population has been divided into three compartments namely healthy, latently infected and infected ones, and vector population is divided into two compartments: non-infective and infective vectors. The system possesses three equilibria: plant-only, disease-free and endemic equilibrium. Plant-only equilibrium is always unstable; disease-free equilibrium is stable when the basic reproduction number, R0, is less than unity and unstable for when it crosses unity, and ensure existence of an endemic equilibrium which may be stable or can undergo a Hopf bifurcation. Finally, impulse periodic roguing with varied rate and time interval is adopted for cost effective and eco-friendly disease control and future direction of agriculture management. The dynamics of the impulsive system has also been analysed. Detailed numerical simulations are employed to support the analytical results. We found that roguing is most cost effective and useful management for mosaic disease eradication of plants if applied at proper rate and interval.  相似文献

In this paper, a deterministic malaria transmission model in the presence of a drug-resistant strain is investigated. The model is studied using stability theory of differential equations, optimal control, and computer simulation. The threshold condition for disease-free equilibrium is found to be locally asymptotically stable and can only be achieved in the absence of a drug-resistant strain in the population. The existence of multiple endemic equilibria is also established. Both the Sensitivity Index (SI) of the model parameters and the Incremental Cost-Effectiveness Ratio (ICER) for all possible combinations of the disease-control measures are determined. Our results revealed among others that the most cost-effective strategy for drug-resistant malaria control is the combination of the provision of basic amenities (such as access to clean water, electricity, good roads, health care, and education) and treatment of infective individuals. Therefore, more efforts from policy-makers on the provisions of basic amenities and treatment of infectives would go a long way to combat the malaria epidemic.  相似文献

An epidemic model with a limited resource for treatment is proposed to understand the effect of the capacity for treatment. It is assumed that treatment rate is proportional to the number of infectives below the capacity and is a constant when the number of infectives is greater than the capacity. It is found that a backward bifurcation occurs if the capacity is small. It is also found that there exist bistable endemic equilibria if the capacity is low.  相似文献

A new two-strain model, for assessing the impact of basic control measures, treatment and dose-structured mass vaccination on cholera transmission dynamics in a population, is designed. The model has a globally-asymptotically stable disease-free equilibrium whenever its associated reproduction number is less than unity. The model has a unique, and locally-asymptotically stable, endemic equilibrium when the threshold quantity exceeds unity and another condition holds. Numerical simulations of the model show that, with the expected 50 % minimum efficacy of the first vaccine dose, vaccinating 55 % of the susceptible population with the first vaccine dose will be sufficient to effectively control the spread of cholera in the community. Such effective control can also be achieved if 50 % of the first vaccine dose recipients take the second dose. It is shown that a control strategy that emphasizes the use of antibiotic treatment is more effective than one that emphasizes the use of basic (non-pharmaceutical) anti-cholera control measures only. Numerical simulations show that, while the universal strategy (involving all three control measures) gives the best outcome in minimizing cholera burden in the community, the combined basic anti-cholera control measures and treatment strategy also has very effective community-wide impact.  相似文献

Global analysis of an epidemic model with nonmonotone incidence rate   总被引:2,自引:0,他引:2
In this paper we study an epidemic model with nonmonotonic incidence rate, which describes the psychological effect of certain serious diseases on the community when the number of infectives is getting larger. By carrying out a global analysis of the model and studying the stability of the disease-free equilibrium and the endemic equilibrium, we show that either the number of infective individuals tends to zero as time evolves or the disease persists.  相似文献

The susceptible-infected (SI) model is extended by allowing for individual optimal choices of self-protective actions against infection, where agents differ with respect to preferences and costs of self-protection. It is shown that a unique endemic equilibrium prevalence exists when the basic reproductive number of a STD is strictly greater than unity, and that the disease-free equilibrium is the unique steady state equilibrium when the basic reproductive number is less than or equal to one. Unlike in models that take individual behavior as given and fixed, the endemic equilibrium prevalence need not vary monotonically with respect to the basic reproductive number. Specifically, with endogenously determined self-protective behavior, a reduction in the basic reproductive number may in fact increase the endemic equilibrium prevalence. The global stability of the endemic steady state is established for the case of a homogeneous population by showing that, for any non-zero initial disease prevalence, there exists an equilibrium path which converges to the endemic steady state.

The phenomenon of backward bifurcation in disease models, where a stable endemic equilibrium co-exists with a stable disease-free equilibrium when the associated reproduction number is less than unity, has important implications for disease control. In such a scenario, the classical requirement of the reproduction number being less than unity becomes only a necessary, but not sufficient, condition for disease elimination. This paper addresses the role of the choice of incidence function in a vaccine-induced backward bifurcation in HIV models. Several examples are given where backward bifurcations occur using standard incidence, but not with their equivalents that employ mass action incidence. Furthermore, this result is independent of the type of vaccination program adopted. These results emphasize the need for further work on the incidence functions used in HIV models.  相似文献

One major drawback associated with the use of anti-retroviral drugs in curtailing HIV spread in a population is the emergence and transmission of HIV strains that are resistant to these drugs. This paper presents a deterministic HIV treatment model, which incorporates a wild (drug sensitive) and a drug-resistant strain, for gaining insights into the dynamical features of the two strains, and determining effective ways to control HIV spread under this situation. Rigorous qualitative analysis of the model reveals that it has a globally asymptotically stable disease-free equilibrium whenever a certain epidemiological threshold (R t 0) is less than unity and that the disease will persist in the population when this threshold exceeds unity. Further, for the case where R t 0 > 1, it is shown that the model can have two co-existing endemic equilibria, and competitive exclusion phenomenon occurs whenever the associated reproduction number of the resistant strain (R t r) is greater than that of the wild strain (R t w). Unlike in the treatment model, it is shown that the model without treatment can have a family of infinitely many endemic equilibria when its associated epidemiological threshold (R(0)) exceeds unity. For the case when [Formula in text], it is shown that the widespread use of treatment against the wild strain can lead to its elimination from the community if the associated reduction in infectiousness of infected individuals (treated for the wild strain) does not exceed a certain threshold value (in this case, the use of treatment is expected to make R t w < R t r.  相似文献

The demography and infection age play an important role in the spread of slowly progressive diseases. To investigate their effects on the disease spreading, we propose a pairwise epidemic model with infection age and demography on dynamic networks. The basic reproduction number of this model is derived. It is proved that there is a disease-free equilibrium which is globally asymptotically stable if the basic reproduction number is less that unity. Besides, sensitivity analysis is performed and shows that increasing the variance in recovery time and decreasing the variance in infection time can effectively control the diseases. The complex interaction between the death rate and equilibrium prevalence suggests that it is imperative to correctly estimate the parameters of demography in order to assess the disease transmission dynamics accurately. Moreover, numerical simulations show that the endemic equilibrium is globally asymptotically stable.  相似文献

SVIR epidemic models with vaccination strategies   总被引:1,自引:0,他引:1
Vaccination is important for the elimination of infectious diseases. To finish a vaccination process, doses usually should be taken several times and there must be some fixed time intervals between two doses. The vaccinees (susceptible individuals who have started the vaccination process) are different from both susceptible and recovered individuals. Considering the time for them to obtain immunity and the possibility for them to be infected before this, two SVIR models are established to describe continuous vaccination strategy and pulse vaccination strategy (PVS), respectively. It is shown that both systems exhibit strict threshold dynamics which depend on the basic reproduction number. If this number is below unity, the disease can be eradicated. And if it is above unity, the disease is endemic in the sense of global asymptotical stability of a positive equilibrium for continuous vaccination strategy and disease permanence for PVS. Mathematical results suggest that vaccination is helpful for disease control by decreasing the basic reproduction number. However, there is a necessary condition for successful elimination of disease. If the time for the vaccinees to obtain immunity or the possibility for them to be infected before this is neglected, this condition disappears and the disease can always be eradicated by some suitable vaccination strategies. This may lead to over-evaluating the effect of vaccination.  相似文献
