In theory, gene therapy is a simple concept which holds great promise as a cure for disease. In practice, however, considerable obstacles have to be overcome, including problems with safe and efficient gene delivery and stable gene expression. This review gives an overview on the history of gene therapy and analyses some of the problems that have so far prevented the establishment of a successful clinical protocol. Since the revolutionary discovery of RNA interference(RNAi), a remarkable progress has been achieved in understanding and harnessing gene silencing mechanism; especially in small interfering RNA(siRNA) therapeutics. Despite its tremendous potential benefits, major challenges in most siRNA therapeutics remains unchanged—safe, efficient and target oriented delivery of siRNA. Twenty years after the discovery of RNAi, siRNA therapeutics finally charts its way into clinics. As we journey through the decades, we reminisce the history of siRNA discovery and its application in a myriad of disease treatments. Herein,we highlight the breakthroughs in si RNA therapeutics, with special feature on the first FDA approved RNAi therapeutics Onpattro(Patisiran) and the consideration of effective siRNA delivery system focusing on current siRNA nanocarrier in clinical trials. Lastly, we present some challenges and multiple barriers that are yet to be fully overcome in siRNA therapeutics.  相似文献

Thirty-three isolates of Aeromonas from environmental sources and clinical samples were tested and the results, obtained using the pulsed field gel electrophoresis (PFGE) technique, were compared with those obtained by biochemical typing. On the basis of their biochemical characteristics 31 strains was assigned to one of the recognised groups or species within the Aeromonas genus and 2 strains to the species Vibrio fluvialis. These latter were nevertheless found to belong to the Aeromonas genus on the basis of the chromosomal DNA analysis. Among the clinical isolates the biochemical analysis showed greater uniformity. A low correlation between molecular and traditional typing methods was observed with a wider heterogeneity at the genomic level. The results showed the difficulty of discriminating Aeromonas isolates by conventional biochemical methods. The genomic analysis performed by PFGE can be a more effectual technique, which can be used for epidemiological and ecological studies of the microorganisms belonging to the Aeromonas group.  相似文献

Decoding of genomic determinants of nitric oxide biosynthesis in mammals and microorganisms has created a base for the systemic analysis of genetic conditionality of this process in lactobacilli. In the present work computer screening of synthetic ways leading to NO formation has been held in order to prove nitric oxide production through NO-synthase system in Lactobacillus plantarum. By the use of EPR spectroscopy experimental evidence of nitric oxide enzymatic generation from L-arginine, not nitrite, has been received. With a concrete example of NO-synthase from L. plantarum it is shown, that registered functional activity of proteins is a consequence of complex transformation of their genetic program into a real catalytic function under certain conditions.  相似文献

An important by-product of sequencing the human genome has been the development of a novel 'toolbox' for biomarker discovery and development. Genomic medicine is an emerging discipline in the genome sciences that integrates these tools to interrogate genomic variation in well-defined populations in order to develop predictors of disease susceptibility, progression and drug response. Several important classes of biomarkers result from these analyses which, when translated to clinical medicine and drug development, will have an important impact on human health and disease. This review highlights both the opportunities and challenges in bringing biomarkers into clinical medicine.  相似文献

Melanoma in young children is rare; however, its incidence in adolescents and young adults is rising. We describe the clinical course of a 15‐year‐old female diagnosed with AJCC stage IB non‐ulcerated primary melanoma, who died from metastatic disease 4 years after diagnosis despite three lines of modern systemic therapy. We also present the complete genomic profile of her tumour and compare this to a further series of 13 adolescent melanomas and 275 adult cutaneous melanomas. A somatic BRAFV600E mutation and a high mutational load equivalent to that found in adult melanoma and composed primarily of C>T mutations were observed. A germline genomic analysis alongside a series of 23 children and adolescents with melanoma revealed no mutations in known germline melanoma‐predisposing genes. Adolescent melanomas appear to have genomes that are as complex as those arising in adulthood and their clinical course can, as with adults, be unpredictable.  相似文献

An expression signaling victory in agonistic encounters may help to establish status and dominance in a hierarchy. We investigated the possibility that an expression of triumph is such a signal. In Study 1, spontaneous expressions of athletes who had just won a medal at the 2004 Olympic Games were shown to three groups of observers who judged the emotion portrayed. All were given a set of common response alternatives that included Anger, Contempt, Disgust, Fear, Joy, Sadness, Surprise, Neutral, and Other. Group 1 was also given Pride, Group 2 Triumph, and Group 3 both Pride and Triumph. Group 1 labeled some expressions as pride, but Group 2 labeled all the expressions as triumph. For Group 3, some expressions were consistently labeled as triumph, and others as pride. Behavioral coding indicated that the expressions labeled triumph were distinct from other expressions. In Study 2, we replicated this finding with a judgment task involving more positive emotion choices to eliminate the possibility that the previous findings occurred because of a limited number of positive emotion labels. In Study 3, we replicated the findings again in two groups of observers from South Korea using two different judgment tasks. These findings suggest that triumph may have a unique nonverbal expression.  相似文献

Although the population of Hispaniola, Cuba's most similar neighbour in the Caribbean, continues to be threatened by parasitic diseases (including malaria), many tropical parasitic infections in Cuba have been eliminated or controlled. However, some parasitic infections remain important in the Cuban population, and the occurrence of vectors and the high possibility of introduction of parasites mean that Cuban diagnosticians must remain alert. Some key aspects of human parasitology in Cuba are reviewed here, including historical information, comparative data from Hispaniola and Jamaica, and how Cuba strives to maintain and improve its control against parasitic infections. Data from recent key novel parasitology research conducted in Cuba are also described.  相似文献
