Although the biomechanical behavior of the acetabular cup (AC) implant is determinant for the surgical success, it remains difficult to be assessed due to the multiscale and anisotropic nature of bone tissue. The aim of the present study was to investigate the influence of the anisotropic properties of peri-implant trabecular bone tissue on the biomechanical behavior of the AC implant at the macroscopic scale. Thirteen bovine trabecular bone samples were imaged using micro-computed tomography (μCT) with a resolution of 18 μm. The anisotropic biomechanical properties of each sample were determined at the scale of the centimeter based on a dedicated method using asymptotic homogenization. The material properties obtained with this multiscale approach were used as input data in a 3D finite element model to simulate the macroscopic mechanical behavior of the AC implant under different loading conditions. The largest stress and strain magnitudes were found around the equatorial rim and in the polar area of the AC implant. All macroscopic stiffness quantities were significantly correlated (R2 > 0.85, p < 6.5 e-6) with BV/TV (bone volume/total volume). Moreover, the maximum value of the von Mises stress field was significantly correlated with BV/TV (R2 > 0.61, p < 1.6 e-3) and was always found at the bone-implant interface. However, the mean value of the microscopic stress (at the scale of the trabeculae) decrease as a function of BV/TV for vertical and torsional loading and do not depend on BV/TV for horizontal loading. These results highlight the importance of the anisotropic properties of bone tissue.  相似文献

Factors related to micromovements at bone-implant interface have been studied because they are considered adverse to osseointegration. Simplifications are commonly observed in these FEA evaluations. The aim of this study was to clarify the influence of FEA parameters (boundary conditions and bone properties) on the stress distribution in peri-implant bone tissue when micromovements are simulated in implants with different geometries. Three-dimensional models of an anterior section of the jaw with cylindrical or conical titanium implants (4.1 mm in width and 11 mm in length) were created. Micromovement (50, 150, or 250 μm) was applied to the implant. The FEA parameters studied were linear vs. non-linear analyses, isotropic vs. orthogonal anisotropic bone, friction coefficient (0.3) vs. frictionless bone-implant contact. Data from von Mises, shear, maximum, and minimum principal stresses in the peri-implant bone tissue were compared. Linear analyses presented a relevant increase of the stress values, regardless of the bone properties. Frictionless contact reduced the stress values in non-linear analysis. Isotropic bone presented lower stress than orthogonal anisotropic. Conical implants behave better, in regard to compressive stresses (minimum principal), than cylindrical ones, except for nonlinear analyses when micromovement of 150 and 250 μm were simulated. The stress values raised as the micromovement amplitude increased. Non-linear analysis, presence of frictional contact and orthogonal anisotropic bone, evaluated through maximum and minimum principal stress should be used as FEA parameters for implant-micromovement studies.  相似文献

Micromotion-induced interstitial fluid flow at the bone-implant interface has been proposed to play an important role in aseptic loosening of cementless implants. High fluid velocities are thought to promote aseptic loosening through activation of osteoclasts, shear stress induced control of mesenchymal stem cells differentiation, or transport of molecules. In this study, our objectives were to characterize and quantify micromotion-induced fluid flow around a cementless femoral stem using finite element modeling. With a 2D model of the bone-implant interface and full-factorial design, we first evaluated the relative influence of material properties, and bone-implant micromotion and gap on fluid velocity. Transverse sections around a femoral stem were built from computed tomography images, while boundary conditions were obtained from experimental measurements on the same femur. In a second step, a 3D model was built from the same data-set to estimate the shear stress experienced by cells hosted in the peri-implant tissues. The full-factorial design analysis showed that local micromotion had the most influence on peak fluid velocity at the interface. Remarkable variations in fluid velocity were observed in the macrostructures at the surface of the implant in the 2D transverse sections of the stem. The 3D model predicted peak fluid velocities extending up to 2.2 mm/s in the granulation tissue and to 3.9 mm/s in the trabecular bone. Peak shear stresses on the cells hosted in these tissues ranged from 0.1 to 12.5 Pa. These results offer insight into mechanical stimuli encountered at the bone-implant interface.  相似文献

Due to the increasing adoption of immediate implantation strategies and the rapid development of the computer aided design/computer aided manufacturing technology, a therapeutic concept based on patient-specific implant dentistry has recently been reintroduced by many researchers. However, little information is available on the designs of custom-made dental implant systems, especially their biomechanical behavior. The influence of the custom-made implant designs on the biomechanical performance for both an immediate and a delayed loading protocol in the maxillary esthetic zone was evaluated by means of the finite element (FE) method. FE models of three dental implants were considered: a state of the art cylindrical implant and two custom-made implants designed by reverse engineering technology, namely a root-analogue implant and a root-analogue threaded implant. The von Mises stress distributions and micro-motions around the bone-implant interfaces were calculated using ANSYS software. In a comparison of the three implant designs for both loading protocols, a favorable biomechanical performance was observed for the use of root-analogue threaded implant which approximated the geometry of natural anterior tooth and maintained the original long-axis. The results indicated that bone-implant interfacial micro-motion was reduced and a favorable stress distribution after osseointegration was achieved.  相似文献

Dental implantology has high success rates, and a suitable estimation of how stresses are transferred to the surrounding bone sheds insight into the correct design of implant features. In this study, we estimate stress transfer properties of four commercial implants (GMI, Lifecore, Intri and Avinent) that differ significantly in macroscopic geometry. Detailed three-dimensional finite element models were adopted to analyse the behaviour of the bone-implant system depending on the geometry of the implant (two different diameters) and the bone-implant interface condition. Occlusal static forces were applied and their effects on the bone, implant and bone-implant interface were evaluated. Large diameters avoided overload-induced bone resorption. Higher stresses were obtained with a debonded bone-implant interface. Relative micromotions at the bone-implant interface were within the limits required to achieve a good osseointegration. We anticipate that the methodology proposed may be a useful tool for a quantitative and qualitative comparison between different commercial dental implants.  相似文献

Lower survival rates were observed for the implant placed in the anterior maxilla. The purpose of this study was to investigate the influence of different implant lengths on the stress distribution around osseointegrated implants under a static loading condition in the anterior maxilla using a three-dimensional finite element analysis. The diameter of 4.0 mm external type implants of different lengths (8.5 mm, 10.0 mm, 11.5 mm, 13.0 mm, 15.0 mm) was used in this study. The anterior maxilla was assumed to be D3 bone quality. All the material was assumed to be homogenous, isotropic and linearly elastic. The implant–bone interface was constructed using a rigid element for simulating the osseointegrated condition. Then, 176 N of static force was applied on the middle of the palatoincisal line angle of the abutment at a 120°angle to the long axis of abutment. The von Mises stress value was measured with an interval of 0.25 mm along the bone–implant interface. Incremental increase in implant length causes a gradual reduction of maximum and average von Mises stress at the labial portion within the implant. In the bone, higher stress was concentrated within cortical bone area and more distributed at the labial cortex, while cancellous bone showed relatively low stress concentration and even distribution. An increase in implant length reduced stress gradients at the cortical peri-implant region. Implant length affects the mechanisms of load transmission to the osseointegrated implant. On the basis of this study the biomechanical stress-based performance of implants placed in the anterior maxilla improves when using longer implants.  相似文献

The aim of study was to evaluate the stress distribution in implant-supported prostheses and peri-implant bone using internal hexagon (IH) implants in the premaxillary area, varying surgical techniques (conventional, bicortical and bicortical in association with nasal floor elevation), and loading directions (0°, 30° and 60°) by three-dimensional (3D) finite element analysis. Three models were designed with Invesalius, Rhinoceros 3D and Solidworks software. Each model contained a bone block of the premaxillary area including an implant (IH, Ø4 × 10 mm) supporting a metal-ceramic crown. 178 N was applied in different inclinations (0°, 30°, 60°). The results were analyzed by von Mises, maximum principal stress, microstrain and displacement maps including ANOVA statistical test for some situations. Von Mises maps of implant, screws and abutment showed increase of stress concentration as increased loading inclination. Bicortical techniques showed reduction in implant apical area and in the head of fixation screws. Bicortical techniques showed slight increase stress in cortical bone in the maximum principal stress and microstrain maps under 60° loading. No differences in bone tissue regarding surgical techniques were observed. As conclusion, non-axial loads increased stress concentration in all maps. Bicortical techniques showed lower stress for implant and screw; however, there was slightly higher stress on cortical bone only under loads of higher inclinations (60°).  相似文献

The aim of this study was to assess stress/strain of different implant modeling simplifications by 3D-FEA. Three variation of external hexagon implant (Ø3.75?×?10 mm) supporting one molar crown were simulated: A (no threads); B (slightly threads simplification); C (original design). 200 N (axial) and 100 N (oblique) were applied. Cortical bone was evaluated by maximum principal stress and microstrain qualitatively and quantitatively (ANOVA and Tukey post hoc (p < 0.05)). Higher stress levels (p < 0.05) were observed in model A. Models B and C presented similar stress transmission. It was possible to conclude that slightly simplification should be used for studies evaluating stress transferring for bone tissue.  相似文献

Silver-based devices activated by electric current are of interest in biomedicine because of their broad-spectrum antimicrobial activity. This study investigates the in vitro antibacterial efficacy and cytotoxicity of a low intensity direct current (LIDC)-activated silver–titanium implant system prototype designed for localized generation and delivery of silver ions at the implantation site. First, the antibacterial efficacy of the system was assessed against methicillin-resistant Staphylococcus aureus (MRSA) over 48 h at current levels of 3 and 6 µA in Mueller–Hinton broth. The cytotoxicity of the system was then evaluated over 48 h in two phases using an in vitro model with in which the activated electrodes were suspended in growth medium in a cell-seeded tissue culture plate. In phase-1, the system was tested on human osteosarcoma (MG-63) cell line and compared to titanium controls. In phase-2, the cytotoxicity characteristics were validated with normal human diploid osteoblast cells. The LIDC-activated system demonstrated high antimicrobial efficacy against MRSA, but was also toxic to human cells immediately surrounding the electrodes. The statistical analysis showed that the cytotoxicity was a result of the presence of silver, and the electric activation did not make it worse.  相似文献

Development of surface treatments has enabled secure attachment of dental implants in less than 1 month. Consequently, it is necessary to characterize accurately the osseointegration of the implant surface in the region of the bone-implant contact (BIC). We developed a method for sample preparation that preserves both bone and BIC to permit analysis of the contact interface. We prepared eight nanotextured implants and implanted them in rabbit tibias. After healing for 30 days, outcomes were analyzed using both our bone preservation protocol and routine decalcification followed by preparation of histological sections stained by hematoxylin and eosin (H & E). Pull-out tests for implant osseointegration were performed after healing. Non-implanted samples of rabbit mandible were used as a control for assessing organic and mineralized bone characteristics and bone structure. Our bone preservation protocol enabled evaluation of many of the same bone characteristics as histological sections stained with H & E. Our protocol enables analysis of implant samples, implant surfaces and osseointegration without risk of BIC damage.  相似文献

Dental implant failure is mainly the consequence of bone loss at peri-implant area. It usually begins in crestal bone. Due to this gradual loss, implants cannot withstand functional force without bone overload, which promotes complementary loss. As a result, implant lifetime is significantly decreased. To estimate implant success prognosis, taking into account 0.2 mm annual bone loss for successful implantation, ultimate occlusal forces for the range of commercial cylindrical implants were determined and changes of the force value for each implant due to gradual bone loss were studied. For this purpose, finite element method was applied and von Mises stresses in implant–bone interface under 118.2 N functional occlusal load were calculated. Geometrical models of mandible segment, which corresponded to Type II bone (Lekholm & Zarb classification), were generated from computed tomography images. The models were analyzed both for completely and partially osseointegrated implants (bone loss simulation). The ultimate value of occlusal load, which generated 100 MPa von Mises stresses in the critical point of adjacent bone, was calculated for each implant. To estimate longevity of implants, ultimate occlusal loads were correlated with an experimentally measured 275 N occlusal load (Mericske-Stern & Zarb). These findings generally provide prediction of dental implants success.  相似文献

The placenta plays a major role in embryo-fetal defects and intrauterine growth retardation after maternal alcohol consumption. Our aims were to determine the oxidative status and cellular and molecular oxidative stress effects on uterine myometrium and trophoblast-decidual tissue following perigestational alcohol intake at early organogenesis. CF-1 female mice were administered with 10% alcohol in drinking water for 17 days prior to and up to day 10 of gestation. Control females received ethanol-free water. Treated mice had smaller implantation sites compared to controls (p < 0.05), diminished maternal vascular lumen, and irregular/discontinuous endothelium of decidual vessels. The trophoblast giant cell layer was disorganized and presented increased abnormal nuclear frequency. The myometrium of treated females had reduced nitrite content, increased superoxide dismutase activity, and reduced glutathione (GSH) content (p < 0.05). However, the trophoblast-decidual tissue of treated females had increased nitrite content (p < 0.05), increased GSH level (p < 0.001), increased thiobarbituric acid-reactive substance concentration (p < 0.001), higher 3-nitrotyrosine immunoreaction, and increased apoptotic index (p < 0.05) compared to controls. In summary, perigestational alcohol ingestion at organogenesis induced oxidative stress in the myometrium and trophoblast-decidual tissue, mainly affecting cells and macromolecules of trophoblast and decidual tissues around early organogenesis, in CF-1 mouse, and suggests that oxidative-induced abnormal early placental formation probably leads to risk of prematurity and fetal growth impairment at term.  相似文献

There is accumulating evidence that leptin may be directly involved in mammalian reproduction, however, the potential role of obesity gene/obesity gene long form receptor (ob/ob-Rb) system in porcine implantation is poorly understood. To further confirm this role, mRNA and protein expression of ob/ob-Rb in implantation site and inter-implantation sites of porcine uterus on pregnancy day 13, 18 and 24 were compared in this study. Ob mRNA level went up with the advance of pregnancy and was higher in implantation site than inter-implantation site (P < 0.05). But ob-Rb mRNA, which was negative-regulated by leptin, went down with the advance of pregnancy and lessened in implantation site compared with inter-implantation site (P < 0.05). During the three implantation phase, leptin protein peaked at day 18 pregnancy (P < 0.05) and leptin protein at implantation site were always higher than inter-implantation site (P < 0.05). The higher ob-Rb protein in implantation site compared with inter-implantation site (P < 0.05) only appeared at day 18 pregnancy. Localization of ob/ob-Rb protein in porcine uterus was assayed using immunohistochemistry and found that ob/ob-Rb protein mainly located in luminal epithelium and glandular epithelium in pregnant pigs, but distinct immune-staining of leptin also detected in stroma in non-pregnancy porcine uterus except for luminal epithelium and glandular epithelium. In conclusion, the peak of leptin and the peak of ob-Rb protein in implantation site specifically appeared on day 18 pregnancy of pig. Another funning discovery is ob-Rb mRNA in porcine endometrium was mainly negative-regulated by leptin. The space–time difference of gene and protein expression for ob/ob-Rb confirmed ob/ob-Rb system role as delicate regulator of porcine implantation process.  相似文献

The retaining screw of the implant-supported dental prosthesis is the weakest point of the crown/implant system. Furthermore, crown height is another important factor that may increase the lever arm. Therefore, the aim of this study was to assess the stress distribution in implant prosthetic screws with different heights of the clinical crown of the prosthesis using the method of three-dimensional finite element analysis. Three models were created with implants (3.75 mm × 10 mm) and crowns (heights of 10, 12.5 and 15 mm). The results were visualised by means of von Mises stress maps that increased the crown heights. The screw structure exhibited higher levels of stresses in the oblique load. The oblique loading resulted in higher stress concentration when compared with the axial loading. It is concluded that the increase of the crown was damaging to the stress distribution on the screw, mainly in oblique loading.  相似文献

The aim of this study was to analyze the influence of three different transmucosal heights of the abutments in single and multiple implant-supported prostheses through the finite element method. External hexagon implants, MicroUnit, and EsthetiCone abutments were scanned and placed in an edentulous maxillary model obtained from a tomography database. The simulations were divided into two groups: (1) one implant with 3.75 × 10 mm placed in the upper central incisor, simulating a single implant-supported fixed prosthesis with an EsthetiCone abutment; and (2) two implants with 3.75 × 10 mm placed in the upper lateral incisors with MicroUnit abutments, simulating a multiple implant-supported prosthesis. Subsequently, each group was subdivided into three models according to the transmucosal height (1, 2, and 3 mm). A static oblique load at an angle of 45 degrees to the long axis of the implant in palatal-buccal direction of 150 and 75 N was applied for multiple and single implant-supported prosthesis, respectively. The implants and abutments were assessed according to the equivalent Von Mises stress analyses while the bone and ceramics were analyzed through maximum and minimum principal stresses. The total deformation values increased in all models, while the transmucosal height was augmented. The transmucosal height of the abutments influences the stress values at the bone, ceramics, implants, and abutments of both the single and multiple implant-supported prostheses, with the transmucosal height of 1 mm showing the lowest stress values.  相似文献

The determining factors for the fixation of uncemented screws in bone are the bone-implant interface and the peri-implant bone. The goal of this work was to explore the role of the peri-implant bone architecture on the mechanics of the bone-implant system. In particular, the specific aims of the study were to investigate: (i) the impact of the different architectural parameters, (ii) the effects of disorder, and (iii) the deformations in the peri-implant region. A three-dimensional beam lattice model to describe trabecular bone was developed. Various microstructural features of the lattice were varied in a systematic way. Implant pull-out tests were simulated, and the stiffness and strength of the bone-implant system were computed. The results indicated that the strongest decrease in pull-out strength was obtained by trabecular thinning, whereas pull-out stiffness was mostly affected by trabecular removal. These findings could be explained by investigating the peri-implant deformation field. For small implant displacements, a large amount of trabeculae in the peri-implant region were involved in the load transfer from implant to bone. Therefore, trabecular removal in this region had a strong negative effect on pull-out stiffness. Conversely, at higher displacements, deformations mainly localized in the trabeculae in contact with the implant; hence, thinning those trabeculae produced the strongest decrease in the strength of the system. Although idealized, the current approach is helpful for a mechanical understanding of the role played by peri-implant bone.  相似文献

A numerical analysis is developed to study the interaction phenomena between endousseus titanium dental implants and surrounding jawbone tissue. The interest is focused on the most appropriate evaluation of the stress state arising in the tissue because of the implant under physiological loading. The problem is considered with regard to linear elastic response of the one and to short time effect. Different configurations of bone-implant system are described, using axial-symmetrical and three-dimensional models, by means of finite and geometric element method. The investigation attains to the stress states induced in bone that lead to a limit condition near the effective failure surface. The parameter commonly adopted in literature, such as the Von Mises stress, represents an excessive simplification of problem formulation, leading to an incorrect evaluation of the real failure risk for the implant, due to the assumption of the isotropic and deviatoric nature of the adopted stress measure. More suitable criterion can be assumed, such as the Tsai-Wu criterion, to take into account the anisotropy that characterises the response of bone, as well as the influence of a hydrostatic stress state. The analysis developed offers a comparison of results by using different criteria, leading to an evaluation of reliability of the procedure to be followed and addressing also to an evaluation of a risk factor for the implant investigated.  相似文献

This study combines finite element method and animal studies, aiming to investigate tissue remodelling processes around dental implants inserted into sika deer antler and to develop an alternative animal consuming model for studying bone remodelling around implants. Implants were inserted in the antlers and loaded immediately via a self-developed loading device. After 3, 4, 5 and 6 weeks, implants and surrounding tissue were taken out. Specimens were scanned by μCT scanner and finite element models were generated. Immediate loading and osseointegration conditions were simulated at the implant-tissue interface. A vertical force of 10 N was applied on the implant. During the healing time, density and Young’s modulus of antler tissue around the implant increased significantly. For each time point, the values of displacement, stresses and strains in the osseointegration model were lower than those of the immediate loading model. As the healing time increased, the displacement of implants was reduced. The 3-week immediate loading model (9878 ± 1965 μstrain) illustrated the highest strains in the antler tissue. Antler tissue showed similar biomechanical properties as human bone in investigating the bone remodelling around implants, therefore the use of sika deer antler model is a promising alternative in implant biomechanical studies.  相似文献

With the resurgence of composite materials in orthopaedic applications, a rigorous assessment of stress is needed to predict any failure of bone-implant systems. For current biomechanics research, strain gage measurements are employed to experimentally validate finite element models, which then characterize stress in the bone and implant. Our preliminary study experimentally validates a relatively new nondestructive testing technique for orthopaedic implants. Lock-in infrared (IR) thermography validated with strain gage measurements was used to investigate the stress and strain patterns in a novel composite hip implant made of carbon fiber reinforced polyamide 12 (CF/PA12). The hip implant was instrumented with strain gages and mechanically tested using average axial cyclic forces of 840 N, 1500 N, and 2100 N with the implant at an adduction angle of 15 deg to simulate the single-legged stance phase of walking gait. Three-dimensional surface stress maps were also obtained using an IR thermography camera. Results showed almost perfect agreement of IR thermography versus strain gage data with a Pearson correlation of R(2) = 0.96 and a slope = 1.01 for the line of best fit. IR thermography detected hip implant peak stresses on the inferior-medial side just distal to the neck region of 31.14 MPa (at 840 N), 72.16 MPa (at 1500 N), and 119.86 MPa (at 2100 N). There was strong correlation between IR thermography-measured stresses and force application level at key locations on the implant along the medial (R(2) = 0.99) and lateral (R(2) = 0.83 to 0.99) surface, as well as at the peak stress point (R(2) = 0.81 to 0.97). This is the first study to experimentally validate and demonstrate the use of lock-in IR thermography to obtain three-dimensional stress fields of an orthopaedic device manufactured from a composite material.  相似文献

There are numerous types of bone replacement materials used to regenerate atrophic alveolar processes before the elective intraosseous implantation. Properties of these materials differ one from another, therefore the choice of material should be thoroughly analysed as well as its type and texture in regard of intraoral conditions and the objective to be achieved. The study involved reconstruction of atrophic alveolar processes with allogeneic bone following unsuccessful use of synthetic and animal materials. The procedure of bone regeneration was performed with frozen bone block (case 1) and allogeneic bone granulate (cases 2, 3, 4) radiation-sterilised with 35 kGy prepared by the Tissue Bank. In all of the presented cases after 3-month implant reorganisation optimal width of the process was obtained, which allowed implant embedment (case 1) or correct implant submergence in the osseous tissue, when implantation took place at the same time (case 2, 3, 4). Allogeneic bone material both, in the form of a block as well as granulate, seems to be an adequate alternative for other materials used in order to widen the bone of the alveolar process, particularly in difficult, complicated cases, where the first regeneration procedure was not successful.  相似文献
