Femoroacetabular impingement syndrome (FAIS) consists of abnormal hip joint morphology and pain during activities of daily living. Abnormal gait mechanics and potentially abnormal muscle forces within FAI patients leads to articular cartilage damage. Therefore, there is a necessity to understand the effects of FAI on hip joint muscle forces during gait and the link between muscle forces, patient reported outcomes (PRO) and articular cartilage health. The purposes of this study were to assess: (1) hip muscle forces between FAI patients and healthy controls and (2) the associations between hip muscle forces with PRO and cartilage composition (T/T2 mapping) within FAI patients. Musculoskeletal simulations were used to estimate peak muscle forces during the stance phase of gait in 24 FAI patients and 24 healthy controls. Compared to controls, FAI patients ambulated with lower vasti (30% body-weight, p = 0.01) and higher sartorius (4.0% body-weight, p < 0.01) forces. Within FAI patients, lower peak gluteus medius, gluteus minimus, sartorius and iliopsoas forces were associated with worse hip joint pain and function (R = 0.43–0.70, p = 0–0.04), while lower muscle forces were associated with increased T and T2 values (i.e. altered cartilage composition) within the hip joint cartilage (R = −0.44 to −0.58, p = 0.006–0.05). Although FAI patients demonstrate abnormal muscle forces, it is unknown whether or not these altered muscle force patterns are associated with pain avoidance or weak musculature. Further investigation is required in order to better understand the effects of FAI on hip joint muscle forces and the associations with hip joint cartilage degeneration.  相似文献

Patellofemoral pain syndrome (PFPS) is a disorder of the patellofemoral (PF) joint in which abnormal tracking is often cited as a factor in pain development. PF tracking is partially dependent on passive stabilizers (ex: PF geometry). Relations amongst PFPS, PF tracking, and contact mechanics are poorly understood. In-vivo investigation of passive PF joint stabilizers including PF tracking, contact mechanics, cartilage thickness, and patellar shape will allow structural characterization of the PF joint and may highlight differences associated with PFPS. This study examined the role that passive stabilizers play in PFPS (n=10) versus healthy subjects (n=10). PF tracking (contact area centroid migration), cartilage thickness, shape, congruence, and contact patterns were quantified using magnetic resonance imaging during isometric loading at 15°, 30°, and 45° of knee flexion. Distinct relationships were identified between patellar shape and tracking and contact, particularly at low flexion (15–30°). Healthy subjects exhibited distinct PF tracking and contact patterns related to Type I patella shape (80%) with increasing total contact area (p<0.001) and proximal centroid migration (15–30° p=0.012; 30–45° p<0.001) for increasing knee angles. PFPS subjects deviated from these patterns at low flexion, demonstrating higher total contact area than healthy subjects (p=0.046 at 15°), lack of proximal centroid migration (15–30°), and more Type II (30%) and III (20%) patella shapes. This study highlights a new finding that patellar shape combined with low degrees of flexion (15–30°) may be important to consider, as this is where PFPS tracking and contact patterns deviate from healthy.  相似文献

Computational models may have the ability to quantify the relationship between hip morphology, cartilage mechanics and osteoarthritis. Most models have assumed the hip joint to be a perfect ball and socket joint and have neglected deformation at the bone-cartilage interface. The objective of this study was to analyze finite element (FE) models of hip cartilage mechanics with varying degrees of simplified geometry and a model with a rigid bone material assumption to elucidate the effects on predictions of cartilage stress. A previously validated subject-specific FE model of a cadaveric hip joint was used as the basis for the models. Geometry for the bone-cartilage interface was either: (1) subject-specific (i.e. irregular), (2) spherical, or (3) a rotational conchoid. Cartilage was assigned either a varying (irregular) or constant thickness (smoothed). Loading conditions simulated walking, stair-climbing and descending stairs. FE predictions of contact stress for the simplified models were compared with predictions from the subject-specific model. Both spheres and conchoids provided a good approximation of native hip joint geometry (average fitting error ~0.5 mm). However, models with spherical/conchoid bone geometry and smoothed articulating cartilage surfaces grossly underestimated peak and average contact pressures (50% and 25% lower, respectively) and overestimated contact area when compared to the subject-specific FE model. Models incorporating subject-specific bone geometry with smoothed articulating cartilage also underestimated pressures and predicted evenly distributed patterns of contact. The model with rigid bones predicted much higher pressures than the subject-specific model with deformable bones. The results demonstrate that simplifications to the geometry of the bone-cartilage interface, cartilage surface and bone material properties can have a dramatic effect on the predicted magnitude and distribution of cartilage contact pressures in the hip joint.  相似文献

Measurements of hip kinematics inherently depend on the coordinate system in which they are derived, yet the effect of the coordinate system definition on calculations of hip angles is not well-understood. Herein, hip angles calculated during dynamic activities were compared using coordinate systems described in the literature. In-vivo kinematic data of 24 participants (13 males) were analyzed during gait and the anterior impingement test using dual fluoroscopy and model-based tracking. Two coordinate systems for the pelvis (anterior pelvic plane, International Society of Biomechanics [ISB]) and three coordinate systems for the femur (table top plane with two definitions of the superior-inferior axis, ISB) were evaluated. Bony landmarks visible on computed tomography (CT) images were identified to establish each coordinate system and used as the basis to calculate differences in hip angles between coordinate system pairs. In the analysis during gait, the maximum differences derived from various coordinate system definitions were 6.7° ± 5.5° for flexion, 7.7° ± 2.1° for rotation, and 5.5° ± 0.7° for adduction. For the anterior impingement test, the differences were 8.1° ± 5.9°, 7.1° ± 1.2°, and 5.3° ± 0.7°, respectively. Landmark-based analysis using CT images could estimate these dynamic differences with errors less than 1.0°. Our results indicate that hip angles can be accurately transformed to angles calculated in different coordinate systems by accounting for the inherent bony anatomy. This information may aid in the interpretation of results across biomechanical studies of the hip.  相似文献

Abstract

Finite element modeling (FEM) can predict hip cartilage contact mechanics. This study investigated how subject-specific boundary conditions and joint geometry affect acetabular cartilage contact mechanics using a multi-scale workflow. For two healthy subjects, musculoskeletal models calculated subject-specific hip kinematics and loading, which were used as boundary conditions for FEM. Cartilage contact mechanics were predicted using either generic or subject-specific FEM and boundary conditions. A subject-specific mesh resulted in a more lateral contact. Effects of subject-specific boundary conditions varied between both subjects. Results highlight the complex interplay between loading and kinematics and their effect on cartilage contact mechanics.  相似文献

The biomechanics of the patellofemoral (PF) joint is complex in nature, and the aetiology of such manifestations of PF instability as patellofemoral pain syndrome (PFPS) is still unclear. At this point, the particular factors affecting PFPS have not yet been determined. This study has two objectives: (1) The first is to develop an alternative geometric method using a three-dimensional (3D) registration technique and linear mapping to investigate the PF joint contact stress using an indirect measure: the depth of virtual penetration (PD) of the patellar cartilage surface into the femoral cartilage surface. (2) The second is to develop 3D PF joint models using the finite element analysis (FEA) to quantify in vivo cartilage contact stress and to compare the peak contact stress location obtained from the FE models with the location of the maximum PD. Magnetic resonance images of healthy and PFPS subjects at knee flexion angles of 15°, 30° and 45° during isometric loading have been used to develop the geometric models. The results obtained from both approaches demonstrated that the subjects with PFPS show higher PD and contact stresses than the normal subjects. Maximum stress and PD increase with flexion angle, and occur on the lateral side in healthy and on the medial side in PFPS subjects. It has been concluded that the alternative geometric method is reliable in addition to being computationally efficient compared with FEA, and has the potential to assess the mechanics of PFPS with an accuracy similar to the FEA.  相似文献

The basic stress pathway above the acetabular dome is important for the maintenance of implant stability in press-fit acetabular reconstruction of total hip arthroplasty. However, information on the basic stress pathway and its impact factors remains unclear. The objective of this study was to investigate the effects of the orientations and positions of the acetabular component on the basic stress pathway. The basic stress pathway above the acetabular dome was defined as two parts: 3D basic trabecular bone stress distribution and quantified basic cortical bone stress level, using two subject-specific finite element normal hip models. The effects were then analysed by generating 32 reconstructed acetabular cases with different cup abduction and anteversion angles within a range of 35–50° and 10–25°, respectively, and 12 cases with different hip centre heights within a range of 0–15 mm above the acetabular dome. The 3D trabecular stress distribution decreased remarkably in all cases, while the 80% of the basic cortical bone stress level was maintained in cases when the acetabular component was positioned at 10° or 15° anteversion and 40° or 45° abduction angles. The basic stress pathway above the acetabular dome was disturbed when the superior displacement of the hip centre exceeded 5 mm above the anatomical hip centre. Positioning the acetabular component correctly contributes to maintain the stress balance between the acetabular cup and the bone during acetabular reconstruction, thus helping restore the normal hip biomechanics and preserve the stability of the implants.  相似文献

Hip osteoarthritis may be initiated and advanced by abnormal cartilage contact mechanics, and finite element (FE) modeling provides an approach with the potential to allow the study of this process. Previous FE models of the human hip have been limited by single specimen validation and the use of quasi-linear or linear elastic constitutive models of articular cartilage. The effects of the latter assumptions on model predictions are unknown, partially because data for the instantaneous behavior of healthy human hip cartilage are unavailable. The aims of this study were to develop and validate a series of specimen-specific FE models, to characterize the regional instantaneous response of healthy human hip cartilage in compression, and to assess the effects of material nonlinearity, inhomogeneity and specimen-specific material coefficients on FE predictions of cartilage contact stress and contact area. Five cadaveric specimens underwent experimental loading, cartilage material characterization and specimen-specific FE modeling. Cartilage in the FE models was represented by average neo-Hookean, average Veronda Westmann and specimen- and region-specific Veronda Westmann hyperelastic constitutive models. Experimental measurements and FE predictions compared well for all three cartilage representations, which was reflected in average RMS errors in contact stress of less than 25 %. The instantaneous material behavior of healthy human hip cartilage varied spatially, with stiffer acetabular cartilage than femoral cartilage and stiffer cartilage in lateral regions than in medial regions. The Veronda Westmann constitutive model with average material coefficients accurately predicted peak contact stress, average contact stress, contact area and contact patterns. The use of subject- and region-specific material coefficients did not increase the accuracy of FE model predictions. The neo-Hookean constitutive model underpredicted peak contact stress in areas of high stress. The results of this study support the use of average cartilage material coefficients in predictions of cartilage contact stress and contact area in the normal hip. The regional characterization of cartilage material behavior provides the necessary inputs for future computational studies, to investigate other mechanical parameters that may be correlated with OA and cartilage damage in the human hip. In the future, the results of this study can be applied to subject-specific models to better understand how abnormal hip contact stress and contact area contribute to OA.  相似文献

实验在8只大棕蝠(Eptesicusfuscus)上进行.选取了5个声源方位记录部位对侧80°(C-80°)、40°(C-40°)、0°和同侧40°(I-40°)、80°(I-80°).下丘听神经元的频率调谐曲线的形状有三种V型、封闭型和U型.以C-40°的频率调谐曲线为对照,声源方位改变时,多数神经元频率调谐曲线的形状和最佳频率不变.多数神经元在声源方位由记录部位对侧转向同侧时,频率调谐曲线的锐度(以QndB值为指标)由小变大;神经元在I-80°、I-40°的QndB值显著高于C-80°、C-40°、0°的QndB值.在所观察的47个神经元中,5个声源方位的最低阈值的平均值有显著差异,由高到低的分布为MTI-80°>MTI-40°>MT0°>MTc-80°>MTc-40°.我们推测,声源方位对神经元频率调谐特性影响的机制,可能是抑制性和兴奋性输入的比例不同而相互整合的结果.  相似文献

Combining musculoskeletal simulations with anatomical joint models capable of predicting cartilage contact mechanics would provide a valuable tool for studying the relationships between muscle force and cartilage loading. As a step towards producing multibody musculoskeletal models that include representation of cartilage tissue mechanics, this research developed a subject-specific multibody knee model that represented the tibia plateau cartilage as discrete rigid bodies that interacted with the femur through deformable contacts. Parameters for the compliant contact law were derived using three methods: (1) simplified Hertzian contact theory, (2) simplified elastic foundation contact theory and (3) parameter optimisation from a finite element (FE) solution. The contact parameters and contact friction were evaluated during a simulated walk in a virtual dynamic knee simulator, and the resulting kinematics were compared with measured in vitro kinematics. The effects on predicted contact pressures and cartilage–bone interface shear forces during the simulated walk were also evaluated. The compliant contact stiffness parameters had a statistically significant effect on predicted contact pressures as well as all tibio-femoral motions except flexion–extension. The contact friction was not statistically significant to contact pressures, but was statistically significant to medial–lateral translation and all rotations except flexion–extension. The magnitude of kinematic differences between model formulations was relatively small, but contact pressure predictions were sensitive to model formulation. The developed multibody knee model was computationally efficient and had a computation time 283 times faster than a FE simulation using the same geometries and boundary conditions.  相似文献

Fuji film has been widely used in studies aimed at obtaining the contact mechanics of articular joints. Once sealed for practical use in biological joints, Fuji Pressensor film has a total effective thickness of 0.30 mm, which is comparable to the cartilage thickness in the joints of many small animals. The average effective elastic modulus of Fuji film is approximately 100 MPa in compression, which is larger by a factor of 100-300 compared to that of normal articular cartilage. Therefore, inserting a Pressensor film into an articular joint will change the contact mechanics of the joint. The measurement precision of the Pressensor film has been determined systematically; however, the changes in contact mechanics associated with inserting the film into joints have not been investigated. This study was aimed at quantifying the changes in the contact mechanics associated with inserting sealed Fuji Pressensor film into joints. Spherical and cylindrical articular joint contact mechanics with and without Pressensor film and for varying degrees of surface congruency were analyzed and compared by using finite element models. The Pressensor film was taken as linearly elastic and the cartilage was assumed to be biphasic, composed of a linear elastic solid phase and an inviscid fluid phase. The present analyses showed that measurements of the joint contact pressures with Fuji Pressensor film will change the maximum true contact pressures by 10-26 percent depending on the loading, geometry of the joints, and the mechanical properties of cartilage. Considering this effect plus the measurement precision of the film (approximately 10 percent), the measured joint contact pressures in a joint may contain errors as large as 14-28 percent.  相似文献

Knee joints are subject to large compression forces in daily activities. Due to artefact moments and instability under large compression loads, biomechanical studies impose additional constraints to circumvent the compression position–dependency in response. To quantify the effect of compression on passive knee moment resistance and stiffness, two validated finite element models of the tibiofemoral (TF) joint, one refined with depth-dependent fibril-reinforced cartilage and the other less refined with homogeneous isotropic cartilage, are used. The unconstrained TF joint response in sagittal and frontal planes is investigated at different flexion angles (0°, 15°, 30° and 45°) up to 1800 N compression preloads. The compression is applied at a novel joint mechanical balance point (MBP) identified as a point at which the compression does not cause any coupled rotations in sagittal and frontal planes. The MBP of the unconstrained joint is located at the lateral plateau in small compressions and shifts medially towards the inter-compartmental area at larger compression forces. The compression force substantially increases the joint moment-bearing capacities and instantaneous angular rigidities in both frontal and sagittal planes. The varus–valgus laxities diminish with compression preloads despite concomitant substantial reductions in collateral ligament forces. While the angular rigidity would enhance the joint stability, the augmented passive moment resistance under compression preloads plays a role in supporting external moments and should as such be considered in the knee joint musculoskeletal models.  相似文献

Modern computerized planning tools for periacetabular osteotomy (PAO) use either morphology-based or biomechanics-based methods. The latter relies on estimation of peak contact pressures and contact areas using either patient specific or constant thickness cartilage models. We performed a finite element analysis investigating the optimal reorientation of the acetabulum in PAO surgery based on simulated joint contact pressures and contact areas using patient specific cartilage model. Furthermore we investigated the influences of using patient specific cartilage model or constant thickness cartilage model on the biomechanical simulation results. Ten specimens with hip dysplasia were used in this study. Image data were available from CT arthrography studies. Bone models were reconstructed. Mesh models for the patient specific cartilage were defined and subsequently loaded under previously reported boundary and loading conditions. Peak contact pressures and contact areas were estimated in the original position. Afterwards we used a validated preoperative planning software to change the acetabular inclination by an increment of 5° and measured the lateral center edge angle (LCE) at each reorientation position. The position with the largest contact area and the lowest peak contact pressure was defined as the optimal position. In order to investigate the influence of using patient specific cartilage model or constant thickness cartilage model on the biomechanical simulation results, the same procedure was repeated with the same bone models but with a cartilage mesh of constant thickness. Comparison of the peak contact pressures and the contact areas between these two different cartilage models showed that good correlation between these two cartilage models for peak contact pressures (r = 0.634 ∈ [0.6, 0.8], p < 0.001) and contact areas (r = 0.872 > 0.8, p < 0.001). For both cartilage models, the largest contact areas and the lowest peak pressures were found at the same position. Our study is the first study comparing peak contact pressures and contact areas between patient specific and constant thickness cartilage models during PAO planning. Good correlation for these two models was detected. Computer assisted planning with FE modeling using constant thickness cartilage models might be a promising PAO planning tool when a conventional CT is available.  相似文献

Vocal fold geometry plays an important role in human phonation. The intraglottal quasi-steady pressure and velocity distributions depend upon the shape, size, and diameter of the glottis. This study reports the effects of the variation of glottal shapes on intraglottal pressures and velocities using a Plexiglas model with a glottis having nine symmetric glottal angles (uniform, as well as convergent and divergent 5°, 10°, 20° and 40°), while the minimal glottal diameter was held constant at 0.06 cm. The empirical data were supported by penalty finite element computational results. The results suggest that larger convergent glottal angles correspond to increased pressures and decreased velocities in the glottis upstream of the minimum glottal location, with a reversal of this pattern at the minimal glottal diameter location. The pressure dip near the glottal entrance for divergent glottal angles was greatest for the 10° divergence angle condition, and was sequentially less for 5°, 20°, and 40°. Flow resistance was greater for a convergent angle than a divergent angle of the same value, and least for the 10° divergent condition. Pressure recovery in the glottis suggested that the optimal glottal diffuser angle was near 10°. Results suggest that the glottal geometry has a critical relationship with phonation (especially for vocal efficiency), and therefore important significance to understanding artistic voice and clinical voice management.  相似文献

Multi-body musculoskeletal models that can be used concurrently to predict joint contact pressures and muscle forces would be extremely valuable in studying the mechanics of joint injury. The purpose of this study was to develop an anatomically correct canine stifle joint model and validate it against experimental data. A cadaver pelvic limb from one adult dog was used in this study. The femoral head was subjected to axial motion in a mechanical tester. Kinematic and force data were used to validate the computational model. The maximum RMS error between the predicted and measured kinematics during the complete testing cycle was 11.9 mm translational motion between the tibia and the femur and 4.3° rotation between patella and femur. This model is the first step in the development of a musculoskeletal model of the hind limb with anatomically correct joints to study cartilage loading under dynamic conditions.  相似文献

A finite element analysis (FEA) modeling technique has been developed to characterize how varying the orientation of the patellar tendon influences the patellofemoral pressure distribution. To evaluate the accuracy of the technique, models were created from MRI images to represent five knees that were previously tested in vitro to determine the influence of hamstrings loading on patellofemoral contact pressures. Hamstrings loading increased the lateral and posterior orientation of the patellar tendon. Each model was loaded at 40°, 60°, and 80° of flexion with quadriceps force vectors representing the experimental loading conditions. The orientation of the patellar tendon was represented for the loaded and unloaded hamstrings conditions based on experimental measures of tibiofemoral alignment. Similar to the experimental data, simulated loading of the hamstrings within the FEA models shifted the center of pressure laterally and increased the maximum lateral pressure. Significant (p < 0.05) differences were identified for the center of pressure and maximum lateral pressure from paired t-tests carried out at the individual flexion angles. The ability to replicate experimental trends indicates that the FEA models can be used for future studies focused on determining how variations in the orientation of the patellar tendon related to anatomical or loading variations or surgical procedures influence the patellofemoral pressure distribution.  相似文献

This study examined the effect of hip flexion angle on the stiffness of the adductor longus (AL) muscle during isometric hip flexion. Seventeen men were recruited. Ten participants performed submaximal voluntary contraction at 0%, 25%, 50%, and 75% of maximal voluntary contraction (MVC) during isometric hip flexion after performing MVC at 0°, 40°, and 80° of hip flexion. Seven participants performed submaximal voluntary tasks during isometric hip extension in addition to hip flexion task. The shear modulus of the AL muscle was used as the index of muscle stiffness, and was measured using ultrasound shear-wave elastography during the tasks at each contraction intensity for each hip flexion angle. During hip flexion, the shear modulus of the AL muscle was higher at 0° than at 40° and 80° of hip flexion at each contraction intensity (p < 0.016). Conversely, a significant effect was not found among hip flexion angle during hip extension at 75% of MVC (p = 0.867). These results suggest that mechanical stress of the AL muscle may be higher at 0° of hip flexion during isometric hip flexion.  相似文献

A novel technique of “anterior offsetting” of the humeral head component to address posterior instability in total shoulder arthroplasty has been proposed, and its biomechanical benefits have been previously demonstrated experimentally. The present study sought to characterize the changes in joint mechanics associated with anterior offsetting with various amounts of glenoid retroversion using cadaver specimen-specific 3-dimensional finite element models. Specimen-specific computational finite element models were developed through importing digitized locations of six musculotendinous units of the rotator cuff and deltoid muscles based off three cadaveric shoulder specimens implanted with total shoulder arthroplasty in either anatomic or anterior humeral head offset. Additional glenoid retroversion angles (0°, 10°, 20°, and 30°) other than each specimen׳s actual retroversion were modeled. Contact area, contact force, peak pressure, center of pressure, and humeral head displacement were calculated at each offset and retroversion for statistical analysis. Anterior offsetting was associated with significant anterior shift of center of pressure and humeral head displacement upon muscle loading (p<0.05). Although statistically insignificant, anterior offsetting was associated with increased contact area and decreased peak pressure (p > 0.05). All study variables showed significant differences when compared between the 4 different glenoid retroversion angles (p < 0.05) except for total force (p < 0.05). The study finding suggests that the anterior offsetting technique may contribute to joint stability in posteriorly unstable shoulder arthroplasty and may reduce eccentric loading on glenoid components although the long term clinical results are yet to be investigated in future.  相似文献

This study tested the hypothesis that the effect of hip joint angle on concentric knee extension torque depends on knee joint angle during a single knee extension task. Twelve men performed concentric knee extensions in fully extended and 80° flexed hip positions with maximal effort. The angular velocities were set at 30° s−1 and 180° s−1. The peak torque and torques attained at 30°, 50°, 70° and 90° (anatomical position = 0°) of the knee joint were compared between the two hip positions. Muscle activations of the vastus lateralis, medialis, rectus femoris and biceps femoris were determined using surface electromyography. The peak torque was significantly greater in the flexed than in the extended hip position irrespective of angular velocity. The torques at 70° and 90° of the knee joint at both angular velocities and at 50° at 180° s−1 were significantly greater in the flexed than in the extended hip position, whereas corresponding differences were not found at 30° (at either angular velocity) and 50° (at 30° s−1) of the knee joint. No effect of hip position on muscle activation was observed in any muscle. These results supported our hypothesis and may be related to the force–length and force–velocity characteristics of the rectus femoris.  相似文献
