目的 评价口服双歧杆菌活菌散剂治疗肠道菌群失调所致的急慢性腹泻的疗效和安全性.方法 采用阳性药物平行对照、分层区组随机、双盲双模拟、多中心临床试验设计方法.其中急性腹泻136例,慢性腹泻125例;每次口服双歧杆菌活菌散剂1包(1.0 g,含双歧杆菌活菌1.0亿个)或丽珠肠乐胶囊2粒,每日2次;急性腹泻疗程3 d,慢性腹泻疗程14 d,观察治疗后临床症状和大便常规的恢复情况,慢性腹泻中39例进行肠道菌群分析.结果 疗程结束时急性腹泻总有效率:口服双歧杆菌活菌散剂组为89.6%,丽珠肠乐胶囊组为91.2%,2组疗效相似,差异无显著性(P>0.05);慢性腹泻总有效率:口服双歧杆菌活菌散剂组为88.1%,丽珠肠乐胶囊组为79.0%,2组疗效相似,差异无显著性(P>0.05);慢性腹泻组中39例治疗前后进行的肠道菌群分析表明,2组双歧杆菌、拟杆菌和真杆菌治疗后比治疗前均有显著增加(P=0.00~0.02),但治疗前后增加值2组间比较差异无显著性(P>0.05).安全性研究未发现不良反应.结论 口服双歧杆菌活菌散剂能有效治疗急慢性腹泻,疗效和安全性与丽珠肠乐胶囊相当.  相似文献

Background  

The macrolid antibiotic tylosin has been widely used to treat canine chronic diarrhea, although its efficacy is based on anecdotal reports and experimental studies in dogs and not on strong scientific evidence. The term tylosin-responsive diarrhea (TRD) refers to diarrheal disorders responding to tylosin therapy within a few days. In TRD, the stool remains normal as long as tylosin treatment continues, but diarrhea reappears in many dogs within weeks after discontinuation. The aim of our trial was to assess the effect of tylosin on fecal consistency compared with a placebo treatment in dogs with suspected TRD and additionally to establish whether tylosin in dogs with recurrent diarrhea is as effective as empirical studies and anecdotal reports suggest.  相似文献

Introduction

To design effective national diarrhea control programs, including oral rehydration solution (ORS) and therapeutic zinc supplementation, information is needed on local perceptions of illness, external care seeking behaviors, and home treatment practices.

Methods

A cross-sectional, community-based household survey was conducted in the Orodara Health District, Burkina Faso. Caregivers of 10,490 children <27 months were interviewed to assess child diarrhea prevalence and related care practices. Characteristics of households, caregivers, children, and reported illnesses were compared for those caregivers who did or did not recognize the presence of diarrhea, as defined according to clinical criteria (≥3 liquid or semi-liquid stools/day). Multiple logistic regression models were used to examine factors associated with illness recognition and treatment.

Results

Clinically defined diarrhea was present in 7.6% (95% CI: 7.1–8.1%) of children during the 24 hours preceding the survey but recognized by only 55% of caregivers. Over half (55%) of the caregivers of 1,067 children with a clinically defined diarrhea episode in the past 14 days sought care outside the home; 78% of those seeking care attended a public sector clinic. Care was sought and treatment provided more frequently for children with fever, vomiting, anorexia, longer illness duration, and those living closer to the health center; and care was sought more frequently for male children. 80% of children with recent diarrhea received some form of treatment; only 24% received ORS, whereas 14% received antibiotics. Zinc was not yet available in the study area.

Conclusions

Caregivers frequently fail to recognize children''s diarrhea, especially among younger infants and when illness signs are less severe. Treatment practices do not correspond with international recommendations in most cases, even when caregivers consult with formal health services. Child caregivers need additional assistance to recognize diarrhea correctly, and both caregivers and health care providers need updated training on current diarrhea treatment recommendations.  相似文献

Introduction

Programs aimed at reducing the burden of diarrhea among children under-five in low-resource settings typically allocate resources to training community-level health workers, but studies have suggested that provider knowledge does not necessarily translate into adequate practice. A diarrhea management program implemented in Bihar, Gujarat and Uttar Pradesh, India trained private sector rural medical practitioners (RMPs) and public sector Accredited Social Health Activists (ASHAs) and Anganwadi workers (AWWs) in adequate treatment of childhood diarrhea with oral rehydration salts (ORS) and zinc. We used cross-sectional program evaluation data to determine the association between observed diarrhea treatment practices and reported knowledge of ORS and zinc among each provider cadre.

Methods

We conducted principal components analysis on providers’ responses to diarrhea treatment questions in order to generate a novel scale assessing ORS/zinc knowledge. We subsequently regressed a binary indicator of whether ORS/zinc was prescribed during direct observation onto the resulting knowledge scores, controlling for other relevant knowledge predictors.

Results

There was a positive association between ORS/zinc knowledge score and prescribing ORS and zinc to young children with diarrhea among private sector RMPs (aOR: 2.32; 95% CI: 1.29-4.17) and public sector ASHAs and AWWs (aOR 2.48; 95% CI: 1.90-3.24). Controlling for knowledge score, receipt of training in the preceding 6 months was a good predictor of adequate prescribing in the public but not the private sector. In the public sector, direct access to ORS and zinc supplies was also highly associated with prescribing.

Conclusions

To enhance the management of childhood diarrhea in India, programmatic activities should center on increasing knowledge of ORS and zinc among public and private sector providers through biannual trainings but should also focus on ensuring sustained access to an adequate supply chain.  相似文献

The persistence and transmission of Aeromonas in a duckweed aquaculture-based hospital sewage water treatment plant in Bangladesh was studied. A total of 670 samples from different sites of the hospital sewage water treatment plant, from feces of hospitalized children suffering from diarrhea, from environmental control ponds, and from feces of healthy humans were collected over a period of three years. In total, 1,315 presumptive Aeromonas isolates were biochemically typed by the PhenePlate rapid screening system (PhP-AE). A selection of 90 representative isolates was further analyzed with PhenePlate (PhP) extended typing (PhP-48), fatty acid methyl ester analysis, and amplified fragment length polymorphism (AFLP) fingerprinting. In addition, the prevalence of the putative virulence factors hemolysin and cytotoxin and the presence of the cytolytic enterotoxin gene (AHCYTOEN) were analyzed. Aeromonas was found at all sites of the treatment plant, in 40% of the samples from environmental control ponds, in 8.5% of the samples from hospitalized children suffering from diarrhea, and in 3.5% of samples from healthy humans. A significantly high number of Aeromonas bacteria was found in duckweed, which indicates that duckweed may serve as a reservoir for these bacteria. PhP-AE typing allowed identification of more than 192 distinct PhP types, of which 18 major PhP types (MTs) were found in multiple sites and during several occasions. AFLP fingerprinting revealed the prevalence of genotypically indistinguishable Aeromonas isolates among certain PhP MTs recovered from different sampling occasions and/or at multiple sites. Hemolytic and cytotoxic activities were observed in 43% of the tested strains, whereas 29% possessed the cytolytic enterotoxin gene AHCYTOEN. Collectively, two specific MTs associated with diarrhea were shown to exhibit high cytotoxicity. Furthermore, all tested isolates of these major types were positive for the cytolytic enterotoxin gene. In conclusion, our data indicate that certain phenotypically and genotypically stable clonal lineages of Aeromonas have persisted in the treatment system for a prolonged period and might spread from the hospitalized children suffering from diarrhea to fish produced for human consumption through the sewage water treatment system.  相似文献

Background  

Rotavirus is a worldwide cause of infectious infantile diarrhea that claims over 600,000 lives annually. Recently, two new vaccine candidates have been developed but their efficacy in developing countries, still remains to be proven. Oral delivery of specific immunoglobulins provides passive immunity and is a fast acting treatment for rotavirus diarrhea. Probiotic bacteria have also gained considerable attention lately as treatment for rotavirus diarrhea. 目的观察和评价参苓白术散加减联合酪酸梭菌活菌散治疗小儿腹泻的临床疗效。方法选取玉田县医院60例患有腹泻的儿童,随机分成观察组和对照组。对照组给予补液纠正水、电解质紊乱等常规治疗,观察组在常规治疗基础上服用参苓白术散加减联合酪酸梭菌活菌散,其中酪酸梭菌活菌散剂量为0.5g/次,3次/d,急性腹泻疗程为3~7d,慢性腹泻疗程为14~21d,观察疗效。结果观察组显效率为63.3%,有效率为33.3%,总有效率为96.6%;对照组显效率为46.7%,有效率为30.O%,总有效率为76.7%,两组比较差异有统计学意义(P〈0.05),观察组疗效明显优于对照组。结论参苓白术散加减联合酪酸梭菌活菌散治疗小儿腹泻疗效显著,能迅速恢复肠道菌群平衡和肠道生理功能,值得临床推广。  相似文献

Campylobacter jejuni isolated from patas monkeys with diarrhea   总被引:3,自引:0,他引:3  
Campylobacter jejuni was isolated from 11 (46%) of 24 patas monkeys with chronic diarrhea. Eight of these 11 (73%) monkey were characterized clinically by mucohemorrhagic diarrhea for periods up to a month followed by loose, semi-formed feces for a 12-month period. Half of the monkeys were treated with erythromycin for 10 days and the other half with tetracycline for 10 days, with all responding to treatment. Despite treatment, all monkeys again had an outbreak of mucohemorrhagic diarrhea. Biopsy specimens were taken from all eight monkeys over a period of 3 months. The clinical signs, treatment, and the gross and microscopic lesions seen in these monkeys were similar to those reported in humans and animals infected with Campylobacter jejuni.  相似文献

The microflora of the gastrointestinal tract (GIT) are a complex ecosystem, performing a number of beneficial functions. Irinotecan causes both early and late diarrhea, the latter possibly caused, in part, by changes in the microflora of the GIT. Female DA rats were given atropine subcutaneously, prior to a single 200 mg/kg intraperitoneal dose of irinotecan. Animals were monitored for diarrhea and killed at 30 and 60 mins, 2, 6, 12, 24, 48, and 72 hrs after chemotherapy administration. Control rats received no treatment. Fecal samples and stomach, jejunum, and colon samples were collected and stored at -70 degrees C until required. Standard microbiological culture techniques were used to grow and isolate the flora. Biochemical tests were used to identify the bacteria. The level of growth was noted for relative comparison between time points and graded accordingly. Early diarrhea was observed in the rats from 2-6 hrs after treatment, after which time the diarrhea resolved. Late onset diarrhea was apparent 72 hrs after treatment. Changes were seen in the flora of the stomach, jejunum, colon and feces. The majority of microflora changes were seen 6, 12, and 24 hrs after treatment, with a relative increase or decrease in the presence of bacteria in comparison with control rats. In some rats bacteria were not observed at all time points, and different bacteria not seen in control animals were identified in rats treated with irinotecan. These changes were observed up to 72 hrs after treatment. In conclusion, irinotecan treatment causes changes in the flora of the stomach, jejunum, colon, and feces of rats and is associated with the development of diarrhea. These changes in flora may have systemic effects and in particular may contribute to the development of chemotherapy-induced mucositis.  相似文献

目的 探讨益生菌对婴幼儿肺炎并发腹泻的治疗作用。方法 回顾分析6 5例婴幼儿肺炎并发腹泻患儿,使用益生菌+黏膜保护剂(治疗组4 9例)及单用黏膜保护剂(对照组16例)对腹泻的治疗效果。所有患儿均行大便常规及轮状病毒检测,大便常规正常,轮状病毒阴性。结果 治疗组及对照组在肺炎出现腹泻时间、腹泻后开始治疗时间及重型腹泻所占比例上差异无显著性( P>0 .0 5 )。治疗组有效率为98% ,明显高于对照组的75 %。结论 益生菌联合黏膜保护剂治疗婴幼儿肺炎并发腹泻,疗效高,值得推广。  相似文献

目的研究双歧杆菌四联活菌片中的蜡样芽胞杆菌对小鼠腹泻治疗作用的影响。方法将SPF小鼠采用抗生素联合伤寒沙门菌和志贺菌诱导建立腹泻模型,取112只造模成功后的小鼠随机分为7组,其中三组分别喂养3种不同剂量的四联活菌片,三组分别喂养3种不同剂量的不含蜡样芽胞杆菌的三联活菌片,余下一组为对照组灌胃生理盐水。比较各组小鼠腹泻的治愈时间,测定粪便菌群、结肠组织耗氧能力及氧化还原电势、肠内sIgA含量及脾脏指数变化。结果相同剂量条件下,含蜡样芽胞杆菌的四联活菌片治愈腹泻时间比三联活菌片缩短48~72h。治疗48h后,四联活菌片组肠内sIgA水平明显增加,与三联活菌片组相比差异有统计学意义(t=5.5783,P=0.0002);治疗结束后,四联活菌片组的脾脏指数也显著高于三联活菌片组(t=18.8648,P=0.0001)。结论含蜡样芽胞杆菌的四联活菌片对小鼠腹泻的治疗及肠道菌群的恢复作用比三联活菌片效果更佳,能显著促进腹泻小鼠肠黏膜局部免疫力的提高。  相似文献

【目的】本试验研究百里酚和香芹酚对滩羊羔羊腹泻、肠道短链脂肪酸和微生物组成的影响,旨在为利用植物提取物手段治疗羔羊腹泻提供可用的添加剂和理论基础。【方法】本研究选择腹泻发病时间为2-3 d的10只腹泻羔羊,随机分为2组,分为腹泻羔羊组(CON组)和百里酚和香芹酚处理组(SCAT组),试验期间SCAT组羔羊补充200 mg/只/d的百里酚和香芹酚1:1混合品,处理时间为7 d。【结果】百里酚和香芹酚处理能够显著提高腹泻羔羊日增重,降低羔羊腹泻指数和腹泻率(P<0.05),降低血清促炎因子表达水平(P<0.05),提高肠道短链脂肪酸(short-chain fatty acids,SCFA)丁酸浓度(P<0.05);同时百里酚和香芹酚能够改变腹泻羔羊肠道微生物组成,提高肠道中双歧杆菌(Bifidobacteriaceae)和阿克曼氏菌(Akkermansiaceae)的相对丰度,降低肠杆菌科(Enterobacteriaceae)相关菌的相对丰度。【结论】百里酚和香芹酚能够提高肠道有益菌相对丰度和有益代谢物丁酸的浓度,降低致病菌相对丰度,从而有效抑制羔羊腹泻,可作为新型替...  相似文献

An outbreak of diarrhea spanning 3 months occurred in a breeding colony of scid/Trp53 knockout mice. Approximately a third of the 150 mice were clinically affected, with signs ranging from mucoid or watery diarrhea to severe hemorrhagic diarrhea with mortality. Helicobacter bilis and the newly recognized urease-negative organism H. rodentium were isolated from microaerobic culture of feces or cecal specimens from affected mice. Dual infection with H. bilis and H. rodentium were confirmed by culture and polymerase chain reaction (PCR) in several animals. Both Helicobacter species rapidly colonized immunocompetent sentinel mice exposed to bedding from cages containing affected mice, but the sentinel remained asymptomatic. Mice with diarrhea had multifocal to segmental proliferative typhlitis, colitis, and proctitis. Several affected mice had multifocal mucosal necrosis with a few focal ulcers in the cecum, colon, and rectum. Mice with diarrhea were treated with antibiotic food wafers (1.5 mg of amoxicillin, 0.69 mg of metronidazole, and 0.185 mg of bismuth/mouse per day) previously shown to eradicate H. hepaticus in immunocompetent mice. Antibiotic treatment resulted in resolution of diarrhea, but not eradication of H. bilis and H. rodentium; mice continued to have positive PCR results after a 2-week treatment regimen, and clinical signs of diarrhea returned in some mice when treatment was suspended. To the authors' knowledge, this is the first report of natural infection with either H. bilis and/or H. rodentium causing acute diarrheal disease and suggests that H. bilis and/or H. rodentium can be an important pathogen for scid mice.  相似文献

Estimation of serum zinc and copper in children with acute diarrhea   总被引:2,自引:0,他引:2  
Diarrhea is, in reality, as much a nutritional disease as one of fluid and electrolyte loss. Children who die from diarrhea, despite good management of dehydration, are usually malnourished and often severely so. In this study, we determined the serum levels of zinc and copper before and after standard oral rehydration solution (ORS) therapy in children with acute diarrhea and correlated it with diarrheal duration and severity. One hundred ten children suffering from acute diarrhea were included. Serum zinc and copper levels of these children were estimated at the time of enrollment and after treatment with standard ORS therapy. This study shows that children suffering from acute diarrhea show a statistically significant decrease of 13.1% and 12.8% in serum zinc and copper concentrations, respectively, compared to normal. These levels further decrease by 22.6% and 22.4%, respectively, after treatment with standard ORS therapy. Our study shows that children with the lower plasma zinc and copper levels suffered with more severe and longer duration of diarrhea. Zinc and copper supplementation could be added to standard ORS therapy for the reduction in morbidity and mortality associated with acute diarrhea in children.  相似文献

目的进行戊糖乳杆菌活菌制剂防治仔猪腹泻的活体实验。方法以8头健康长白母猪所产的90头仔猪为实验材料,随机分组进行腹泻预防实验,设空白为对照。结果戊糖乳杆菌活菌制剂预防组的腹泻率为8.51%,对照组为78.57%,差异有非常显著性(P〈0.01)。对空白组出现腹泻的仔猪进行治疗比较实验,庆大霉素为对照。结果戊糖乳杆菌制剂治疗组的总有效率为100%,治愈率为76.47%,平均疗程5d;庆大霉素治疗组的有效率为93.75%,治愈率仅为31.25%,平均疗程6d,差异有显著性(P〈0.05)。此外,与对照比较,戊糖乳杆菌制剂预防组仔猪的精神状态好,毛色光亮,粪便成型;即使个别出现腹泻情况,连续灌服戊糖乳杆菌制剂2d(1次/d),即可痊愈。结论戊糖乳杆菌制剂可有效预防和治疗仔猪腹泻。  相似文献

A 20-year-old man from Simanjiro district in northern Tanzania presented with a 3-year history of splenomegaly, fatigue, cachexia, skin maculae, and recent onset of watery diarrhea at Kilimanjaro Christian Medical Centre (KCMC) in Northern Tanzania. Due to laboratory findings of pancytopenia, diagnostic workup included bone marrow aspiration cytology and biopsy. Although the rapid test (IT LEISH, rK39 RDT) was negative, blood smear showed amastigote forms of leishmaniasis in macrophages. Repeat bone marrow aspiration and PCR eventually confirmed visceral leishmaniasis (VL). The patient denied travel to known endemic areas of VL. Treatment was initiated with Amphotericin B, but the patient died on the fourth day of treatment from respiratory insufficiency. An autopsy revealed massive organ manifestations of VL. This is the first reported autochthonous case of VL in Tanzania. Clark and colleagues detected the vector Phlebotomus martini in Northern Tanzania in 2013, in a region bordering the district of our patient. The negative rapid test draws attention to the fact that sensitivity and specificity were found to be low in East African VL patients as displayed earlier by a Kenyan study. Therefore, tissue samples (spleen or bone marrow) remain necessary for diagnosis. The variety of symptoms in this presented case was remarkable, including the occurrence of post-kala-azar dermal leishmaniasis (PKDL) and VL at the same time. This has been described in East African VL cases before as well as the occurrence of chronic diarrhea. An elongated undiagnosed period likely led to a mixed clinical picture that included hepato-splenomegaly, PKDL, cachexia, and diarrhea.  相似文献

目的:探讨小檗碱对甲状腺功能亢进(甲亢)性腹泻大鼠结肠及血浆中神经肽Y的影响,进而研究小檗碱治疗甲亢性腹泻的机制.方法:制备甲亢性腹泻大鼠模型.应用小檗碱对其干预,采用免疫组化方法测定甲亢性腹泻组、小檗碱治疗剂量组、健康组大鼠结肠神经肽Y的定位,采用酶联免疫方法检测各组大鼠血浆中神经肽Y含量的变化.结果:在结肠组织中,甲亢性腹泻组与健康对照组比较NPY表达明显增多,经小檗碱治疗后NPY表达较甲亢性腹泻组表达减少,接近健康对照组的表达.在血浆中,NPY的表达在甲亢性腹泻组与健康对照组中增多,经小檗碱治疗后NPY表达较甲亢性腹泻组表达减少,接近健康对照组的表达.结论:甲亢性腹泻大鼠结肠及血浆中NPY的表达明显增多,小檗碱治疗后NPY的表达又减少,接近健康对照组.NPY可能参与甲亢性腹泻的发生机制.  相似文献
