目的:通过对冠心病患者经皮冠状动脉介入治疗(PCI)前后血清Caspase-3,9水平变化的观察,探讨冠状动脉介入治疗对冠心病患者血清Caspase-3,9水平的影响。方法:冠状动脉介入组(PCI组)30例和单纯造影组(CAG组)29例,在术前0.5h,术后3h,术后6h,术后12h,术后24h,术后48h分别测定血清Caspase-3,9水平。结果:CAG组术前与术后的血清Caspase-3,9水平差异均无统计学意义,PCI组术后3h、6h的Caspase-3水平较术前0.5h均有降低趋势,差异均有统计学意义(P<0.05),PCI组术后48h的Caspase-9水平与术后3h、6h相比均有升高趋势,差异均有统计学意义(P<0.05)。结论:PCI术后狭窄或闭塞的冠状动脉血管重新恢复血供,血清中Caspase-3,9水平在PCI术后先降低后又逐渐升高,提示再灌注后可出现细胞的凋亡程度加重,血清中Caspase-3,9的水平可作为心肌再灌注损伤后细胞凋亡程度的评估指标并可作为诊断临床症状不典型的再灌注损伤的一个指标。PCI术可有效改善心肌血供,减少因术前缺血缺氧造成的细胞凋亡,但PCI术后再灌注损伤可进一步加重细胞凋亡。  相似文献

目的:探讨高负荷剂量氯吡格雷联合替罗非班在高危非ST段抬高型急性冠状动脉综合征(acute coronarysyndrome,ACS)患者经皮冠状动脉介入治疗(percutaneous coronary intervention,PCI)中的作用及安全性。方法:入选208例高危非ST段抬高型ACS患者,根据PCI术前氯吡格雷负荷量不同,随机分为观察组和对照组。对照组:112例,以氯吡格雷300 mg+替罗非班(用法同观察组)治疗,观察组:96例,以氯吡格雷600 mg+替罗非班0.4μg/(kg·min)在30 min内静脉注射。继以0.1 g/(kg·min)由微量泵静脉持续泵入至手术后36 h,分别测定两组最大血小板聚集率(MPAR)变化、PCI术后TIMI心肌灌注分级(TMPG);随访住院期间和30 d内不良心血管事件和出血事件。结果:两组临床情况、PCI前负荷剂量氯吡格雷给药时间、支架置入数目比较差异无统计学意义(P〉0.05);用药后2 h、6 h、术后24 h两组MPAR均较用药前明显降低(P〈0.05);用药后6 h两组MPAR均低于用药后2 h(P〈0.05);用药后2 h时观察组MPAR低于对照组(P〈0.05)。PCI术后TMPG 3级获得率观察组高于对照组(P〈0.05);30 d内复合心血管事件发生率、出血并发症两组比较差异均无统计学意义(P〉0.05)。结论:PCI术前负荷氯吡格雷600 mg可以更有效、更迅速地抑制患者的MPAR,联合应用替罗非班时仍可改善心肌灌注,且安全可行。  相似文献

摘要 目的:探讨血清CX3C趋化因子配体1 (CX3CL1)、脂联素(APN)、同型半胱氨酸(Hcy)及超敏C反应蛋白(hs-CRP)水平对急性心肌梗死(AMI)患者经皮冠状动脉介入治疗(PCI)术后心肌低灌注的预测价值,并构建其预测模型。方法:选择2020年10月至2022年4月我院行PCI术治疗的AMI患者106例,根据患者术后冠状动脉血流分级结果将其分为正常灌注组(n=82)和低灌注组(n=24),比较两组临床资料、PCI治疗情况及血清CX3CL1、APN、Hcy及hs-CRP水平。应用单因素和多因素Logistic回归分析PCI术后心肌低灌注的相关因素,并构建AMI患者PCI术后心肌低灌注预测模型,应用受试者工作特征(ROC)曲线分析新模型及各指标对AMI患者PCI术后心肌低灌注的预测价值。结果:低灌注组年龄显著高于正常灌注组,左心室射血分数(LVEF)显著低于正常灌注组(P<0.05)。低灌注组血清CX3CL1、Hcy及hs-CRP水平显著高于正常灌注组,APN水平显著低于正常灌注组(P<0.05)。多因素Logistic回归分析显示,年龄≥62.62岁、LVEF≤59.05%、血清CX3CL1≥1954.37 ng/mL、血清APN≤6.69 μg/L、血清Hcy≥11.86 μg/mL、血清hs-CRP≥5.18 mg/L是心肌梗死患者PCI术后心肌低灌注的危险因素(P<0.05)。ROC曲线分析显示基于血清CX3CL1、APN、Hcy及hs-CRP建立的预测模型对PCI术后心肌低灌注有较高的敏感度、特异度。结论:血清CX3CL1、Hcy、hs-CRP水平增高及APN水平降低是心肌梗死PCI术后心肌低灌注的危险因素,基于以上指标构建的预测模型对心肌梗死PCI术后心肌低灌注的预测具有一定价值。  相似文献

目的:探讨血清白细胞介素-18(IL-18)、氧化低密度脂蛋白(ox-LDL)与急诊经皮冠状动脉介入治疗(PCI)术后支架内再狭窄的关系。方法:75例急性心梗急诊介入术后8~12个月内接受冠状动脉造影复查,其中9例有再狭窄作为再狭窄组,66例无再狭窄作为对照组。2组术后均接受阿司匹林、氯吡格雷、他汀类等药物治疗。取2组患者PCI术前、术后冠状动脉造影复查时血清标本,采用酶联免疫吸附法(EL ISA)检测血清IL-18、ox-LDL水平。结果:①再狭窄组PCI术后IL-18、ox-LDL水平较术前均明显升高[(2.37±0.22):(0.85±0.19)mg/L、(6.99±0.98):(2.38±1.06)mg/L],均P<0.01;对照组PCI后IL-18、ox-LDL水平较术前明显下降[(0.48±0.11):(1.23±0.09)mg/L、(1.39±0.54):(4.45±0.87)mg/L],P<0.05。②再狭窄组和对照组PCI术前IL-18、ox-LDL水平差异无统计学意义,再狭窄组PCI术后IL-18、ox-LDL水平显著高于对照组(均P<0.01)。④再狭窄组和对照组术前、术后IL-18和o...  相似文献

目的:探讨冠心病患者血清白介素18(Interleukin-18,IL-18)、白细胞分化抗原40配体(CD40L)、及高敏C反应蛋白(hs-CRP)水平在经皮冠状动脉介入术治疗前后的变化和意义。方法:选择经冠状动脉造影确诊的冠心病患者85例,根据病变程度分为单支病变组(n=32)、双支病变组(n=28)和多支病变组(n=25),采用双抗体夹心ELISA法测定PCI术前术后血清IL-18、CD40L和hs-CRP水平。结果:血清IL-18水平测定结果:多支病变组高于双支病变组,双支病变组高于单支病变组;支架置入术后显著高于术前,差异均有统计学意义(P〈0.01)。血清CD40L水平测定结果:多支病变组高于双支病变组和单支病变组,差异均有统计学意义(P〈0.01),双支病变组与单支病变组间差异无统计学意义(P〉0.05);支架置入术后较术前显著升高,差异有统计学意义(P〈0.01)。血清hs-CRP水平测定结果:多支病变组高于双支病变组,双支病变组高于单支病变组;支架置入术后显著高于术前,差异均有统计学意义(P〈0.01)。结论:冠心病患者血清IL-18、CD40L和hs-CRP与冠脉病变程度密切相关,介入治疗可使冠心病患者血清IL-18、CD40L和hs-CRP水平升高,监测血清中IL-18、CD40L和hs-CRP水平变化可了解治疗效果和炎症程度。  相似文献

目的:探讨血清胱抑素C(Cys C)、心肌营养素-1(CT-1)联合冠状动脉血流储备分数(FFR)值预测冠状动脉中度狭窄冠心病患者经皮冠状动脉介入治疗(PCI)术后主要心脏不良事件(MACE)发生风险的预测价值。方法:选取2020年1月~2022年3月南京鼓楼医院收治的冠状动脉中度狭窄冠心病患者260例,根据PCI术后1年是否发生MACE分为MACE组36例和非MACE组224例。检测血清Cys C、CT-1水平和计算FFR值。构建多因素Logistic回归模型分析影响冠状动脉中度狭窄冠心病患者PCI术后MACE发生的风险因素,绘制受试者工作特征(ROC)曲线分析血清Cys C、CT-1联合FFR值对冠状动脉中度狭窄冠心病患者PCI术后MACE发生风险的预测价值。结果:随访1年,无失访病例,260例冠状动脉中度狭窄冠心病患者PCI术后MACE发生率为13.85%(36/260)。与非MACE组比较,MACE组血清Cys C、CT-1水平升高,FFR值降低(P<0.05)。多因素Logistic回归分析显示,KILLIP分级≥Ⅲ级、Cys C升高、CT-1升高为影响冠状动脉中度狭窄...  相似文献

目的:探讨缺血后适应对大鼠局灶性脑缺血/再灌注损伤后caspase-3表达的影响。方法:大脑中动脉线拴法复制大鼠局灶性脑缺血/再灌注损伤动物模型。将30只雄性SD大鼠随机分为3组(n=10):假手术组(sham组)、缺血/再灌注(I/R)组和缺血后适应(IP)组。利用原位缺口末端标记法观察神经细胞凋亡的变化。应用Western blot检测大鼠局灶性脑缺血/再灌注损伤后caspase-3蛋白表达水平的变化。结果:大鼠脑缺血/再灌注后凋亡细胞数量和caspase-3蛋白表达水平均显著升高,而缺血后适应组凋亡细胞数量和caspase-3蛋白表达水平均显著低于缺血/再灌注组(P〈0.01)。结论:缺血后适应可抑制大鼠脑缺血/再灌注后细胞凋亡的发生,此作用可能与下调caspase-3蛋白表达有关。  相似文献

目的:基于冠状动脉斑块定量分析探讨经皮冠状动脉介入(PCI)术后心肌损伤(PMI)的影响因素及预测价值。方法:回顾性分析2020年8月-2022年12月在我院住院并经择期PCI手术治疗的患者251例作为研究对象,根据PCI术后有无血清生物标志物的升高分为PMI阳性组(n=106)和PMI阴性组(n=145)。所有患者术前1周均行冠状动脉CT血管成像(CCTA)检查,且在PCI术前和术后24 h内均测量患者高敏心肌肌钙蛋白T(hs-cTnT)水平。采用半自动斑块分析软件标注并分析斑块特征,主要测量冠脉解剖学狭窄程度并对斑块成分和形态特征进行定量分析。采用单因素方差分析比较两组患者的临床资料和斑块定量指标。通过多因素Logistic回归分析确定PCI术后PMI发生的危险因素。采用受试者工作特征曲线(ROC)分析冠脉斑块定量指标对PCI术后PMI发生的诊断价值。结果:单因素方差分析显示,最大面积狭窄率、斑块总体积、斑块负荷、脂质斑块体积、脂质斑块占比和纤维斑块体积与PCI术后PMI的发生有关(P<0.05);而斑块长度、管腔最窄处面积、纤维斑块占比、钙化斑块体积、钙化斑块占比、斑点状钙...  相似文献

摘要 目的:探讨急性冠状动脉综合征(ACS)患者术前血清脂蛋白相关磷脂酶A2(Lp-PLA2)、可溶性生长刺激表达基因2蛋白(sST2)表达情况,分析术前血清Lp-PLA2、sST2与经皮冠脉介入术(PCI)术后冠状动脉慢血流/无复流(CSF/CNF)的关系。方法:选择2019年9月至2022年9月在徐州医科大学附属连云港医院心内科进行PCI治疗的ACS患者386例为ACS组,根据术后心肌梗死溶栓实验(TIMI)血流分级分为CSF/CNF组(138例)和正常血流组(248例);另选择同期在本院体检的健康对象174例为健康对照组。采用酶联免疫吸附法(ELISA)检测ACS患者术前及健康志愿者体检时血清Lp-PLA2、sST2水平,比较ACS组与健康对照组血清Lp-PLA2、sST2水平。采用单因素及多因素logistic回归模型分析CSF/CNF的影响因素,采用受试者工作特征(ROC)曲线分析术前血清Lp-PLA2、sST2对ACS患者PCI术后CSF/CNF的预测价值。结果:术前血清Lp-PLA2、sST2水平ACS组高于健康对照组(P<0.05);单因素分析结果显示,CSF/CNF组术前血肌酐(Scr)、超敏C反应蛋白(hs-CRP)、D-二聚体(D-D)、Lp-PLA2、sST2水平均高于正常血流组(P<0.05);多因素logistic回归模型分析结果显示,术前血清Lp-PLA2、sST2水平升高是ACS患者PCI术后发生CSF/CNF的独立危险因素(P<0.05)。ROC曲线分析结果显示,两指标联合预测ACS患者PCI术后CSF/CNF的曲线下面积(AUC)显著高于术前血清Lp-PLA2、sST2单独检测。结论:PCI术后发生CSF/CNF的ACS患者术前血清Lp-PLA2、sST2水平异常升高,术前检测血清Lp-PLA2、sST2能较准确预测CSF/CNF,两者联合检测的预测效能更高。  相似文献

目的:观察大鼠心肌缺血/再灌注损伤对血清和心肌组织瘦素(Leptin)表达的影响,探讨Leptin在心肌缺血/再灌注损伤中的作用。方法:建立大鼠心肌缺血/再灌注模型,检测血清乳酸脱氢酶(LDH)和Leptin浓度,并用HE染色和免疫组织化学观察心肌组织病理学及Lepfin表达水平。结果:缺血组、再灌注组血清LDH水平显著升高(P〈0.05),表明该模型制作成功,造成心肌局部一定程度的损伤。缺血组血清Leptin含量(6.34±2.49)ng/ml显著低于对照组(7.50±2.93ng/ml,P〈0.05);再灌注后Leptin水平缓慢恢复,于再灌注2h时Leptin达到(8.32±1.74)ng/ml,恢复到损伤前水平(8.38±2.56)ng/ml,且随再灌注时间延长有升高趋势。免疫纽化显示与假手术纽心肌Leptin蛋白表达水平相比,其他四组均有显著降低(P〈0.01),按缺血45min后再灌注1h组、缺血45min后再灌注3h组、单纯缺血45min组、缺血45min后再灌注2h组依次递减。结论:Leptin在心肌缺血/再灌注损伤后早期45min血中有明显减少,心肌组织中也明显表达下降。心肌组织病理损伤与Leptin的改变可能有一定的关系。  相似文献

Abstract

Objective: To investigate the dynamic changes in serum tryptase levels and their association with clinical data in patients with acute ST-elevation myocardial infarction (STEMI) treated with primary percutaneous coronary intervention (PCI).

Methods: Serum tryptase levels were measured in 99 STEMI patients and 25 control subjects.

Results: Tryptase levels were significantly increased at admission, and descended after primary PCI. Tryptase levels at 0.5, 2 and 6?h after PCI were negatively correlated with the percentage of ST-segment resolution (STR) as well as left ventricular ejection fraction (LVEF).

Conclusions: High tryptase levels after PCI were associated with poor myocardial reperfusion and poor cardiac function.  相似文献

目的:探讨黄芪皂苷Ⅳ对大鼠心肌缺血/再灌注损伤的保护作用及抗凋亡作用。方法:研究黄芪皂苷Ⅳ对大鼠收缩压和舒张压的作用;建立大鼠心肌缺血/再灌注模型,在缺血前给予黄芪皂苷Ⅳ处理,观察心律失常的改变,测定血液中乳酸脱氢酶(LDH)、超氧化物歧化酶(SOD)和丙二醛(MDA)的变化,检测计算凋亡心肌细胞百分比及对P-STAT1、P-STAT3蛋白表达的调控作用。结果:黄芪皂苷Ⅳ可降低大鼠收缩压和舒张压,心肌缺血/再灌注前,预先给予黄芪皂苷Ⅳ有抗心律失常作用,降低血液中LDH和MDA含量,提高SOD活性,降低凋亡心肌细胞百分比,显著增加P-STAT1蛋白表达而同时降低P-STAT3蛋白表达。结论:黄芪皂苷Ⅳ对心肌缺血/再灌注损伤具有一定的保护作用,减少心肌细胞凋亡,其机制可能与抑制P—STAT1,诱导P—STAT3表达有关。  相似文献

Aim

In patients with ST-segment elevation myocardial infarction (STEMI), it is uncertain whether atrial fibrillation has prognostic implications. There may be a difference between atrial fibrillation before and after reperfusion therapy.

Methods and results

In patients with STEMI treated with primary percutaneous coronary intervention (PCI), ECGs were analysed before and after primary PCI. Of the 1623 patients with electrocardiographic data before primary PCI, 53 patients (3.3%) had atrial fibrillation. Patients with atrial fibrillation were older, were more often female, and less often had anterior MI location. Of the 1728 patients with electrocardiographic data after primary PCI, 52 patients (3.0%) had atrial fibrillation. Atrial fibrillation was more common in older patients and in those with Killip class >1. Also patients with occlusion of the right coronary artery or TIMI flow 0 before primary PCI more commonly had AF after the procedure. Not successful reperfusion was also associated with a higher incidence of AF after primary PCI. Although both atrial fibrillation before and after primary PCI were associated with increased mortality, multivariable analyses, adjusting for differences in age, gender and Killip class on admission, revealed that atrial fibrillation after PCI (OR 3.69, 95% CI 1.87–7.29) but not before PCI (OR 1.86, 95% CI 0.89–3.90) was independent and statistically significantly associated with long-term mortality.

Conclusion

In patients with STEMI, atrial fibrillation after but not before primary PCI has independent prognostic implications. Possibly, atrial fibrillation after the PCI is a symptom of failed reperfusion and a sign of heart failure.  相似文献

Context: Angiopoietin-1 (Ang-1) and angiopoietin-2 (Ang-2) play divergent roles in myocardial ischemia and reperfusion injury.

Objective: To investigate serum Ang-1 and Ang-2 levels in ST-segment elevation myocardial infarction (STEMI) patients treated with primary percutaneous coronary intervention (PCI).

Methods: Serum Ang-1 and Ang-2 were measured in 85 STEMI patients in the first week after PCI.

Results: Ang-1, Ang-2 and Ang-2/Ang-1 ratio (Ang-2/1) were all increased at admission, and had dynamic changes after PCI. Ang-2 and Ang-2/1 at admission and 2 h after PCI were positively correlated with peak cardiac troponin T levels.

Conclusion: The extent of myocardial damage may be linked to circulating Ang-2 and Ang-2/1.  相似文献

目的:观察急性心肌梗死(AMI)患者围手术期血浆apelin的表达变化,分析AMI合并2型糖尿病(T2DM)患者血浆apelin的表达与预后的相关性,探讨apelin在冠脉介入治疗(PCI)中的心脏保护作用。方法:72例于2012年2月~8月在我院心内科接受冠状动脉造影确诊为AMI并成功完成PCI的冠心病患者,分别在术前、术后0小时、术后4小时、术后24小时收集血清,酶联免疫吸附法测定血浆apelin-13水平;进一步对糖尿病及非糖尿病AMI患者(每组各20例)进行亚组分析,随访两组患者在术后6个月时主要不良心脑血管事件(MACCE)。结果:AMI患者术后0 h组apelin水平与术前基线水平明显降低(31.54±5.48 vs35.15±6.48 ng/L,P0.05);术后4小时及24小时组apelin水平较术前明显升高(39.65±5.48 vs 35.15±6.48 ng/L,43.93±5.37 vs35.15±6.48 ng/L,P0.05)。糖尿病与非糖尿病组apelin水平术前无明显差异;糖尿病组在术后各时间点的apelin水平均明显高于非糖尿病组(31.12±5.50 vs 29.21±6.53 ng/L,40.57±5.37 vs 33.49±3.89 ng/L,43.50±7.41 vs 34.54±3.52 ng/L,P0.05)。两组术后6个月随访T2DM组LVEF值改善明显高于NT2DM组,但MACCE事件无明显差异。结论:AMI患者PCI术后存在血浆apelin表达的升高,其中糖尿病患者在术后血浆apelin表达较非糖尿病患者明显增高,提示PCI冠脉血运重建可促进糖尿病患者apelin分泌,调节胰岛素抵抗改善预后。  相似文献

BACKGROUND: In acute myocardial infarction (AMI) treated with percutaneous coronary intervention (PCI), myocardial injury results from complex processes during both ischemia and reperfusion. Release of reactive oxygen species (ROS) may contribute to the accumulated myocardial damage. AIMS: To examine by frequent sampling of peripheral blood oxidative stress and early inflammation in patients undergoing primary PCI for AMI. Secondly, to assess whether a correlation exists between these parameters and the extent of myocardial damage. METHODS: Sixteen patients undergoing primary PCI within 6 h of AMI onset were included. Peripheral blood was sampled at start of procedure (t0) and repeatedly over 24 h following reperfusion. Main plasma analyses were: 8-iso-PGF2alpha (oxidative stress), 15-keto-dihydro-PGF2alpha (cyclooxygenase-mediated inflammation); and troponin-T (myocardial injury). Additional analyses included: total antioxidant status (TAS); vitamins; hsCRP and lipids. RESULTS: 8-Iso-PGF2alpha increased following restoration of blood flow, returned to t0 values after 3 h and was reduced below t0 the following day. TAS decreased significantly from t0 to the next day. There was no significant correlation between 8-iso-PGF2alpha and troponin T values. 15-Keto-dihydro-PGF2alpha was elevated during the first hour. There was a major rise in hsCRP after 24 h. CONCLUSION: Following reperfusion by primary PCI in AMI, oxidative stress and an inflammatory response are induced immediately. A rise in 8-iso-PGF2a during ischemia indicate that ROS generation may also take place during severely reduced coronary blood flow and hypoxia. No direct relationship between 8-iso-PGF2alpha or 15-keto-dihydro-PGF2alpha and troponin T was evident. The present study adds to the increasingly complex pathophysiological roles of ROS acting both as signal molecules and as mediators of tissue injury.  相似文献

目的:观察急性心肌梗死(AMI)患者围手术期血浆apelin的表达变化,分析AMI合并2型糖尿病(T2DM)患者血浆apelin的表达与预后的相关性,探讨apelin在冠脉介入治疗(PCI)中的心脏保护作用。方法:72例于2012年2月-8月在我院心内科接受冠状动脉造影确诊为AMI并成功完成PCI的冠心病患者,分别在术前、术后0小时、术后4小时、术后24小时收集血清,酶联免疫吸附法测定血浆apelin-13水平;进一步对糖尿病及非糖尿病AMI患者(每组各20例)进行亚组分析,随访两组患者在术后6个月时主要不良心脑血管事件(MACCE)。结果:AMI患者术后0h组apelin水平与术前基线水平明显降低(31.54±5.48VS35.15±6.48ng/L,P〈0.05);术后4小时及24小时组apelin水平较术前明显升高(39.65±5.48VS35.15±6.48ng/L,43.93±5.37VS35.15±6.48ng/L,P〈0.05)。糖尿病与非糖尿病组apelin水平术前无明显差异;糖尿病组在术后各时间点的apelin水平均明显高于非糖尿病组(31.12±5.50VS29.21±6.53ng/L,40.57+5.37VS33.49±3.89ng/L,43.50±7.41VS34.54±3.52ng/L,P〈0.05)。两组术后6个月随访T2DM组LVEF值改善明显高于NT2DM组,但MACCE事件无明显差异。结论:AMI患者PCI术后存在血浆apelin表达的升高,其中糖尿病患者在术后血浆apelin表达较非糖尿病患者明显增高,提示PCI冠脉血运重建可促进糖尿病患者apelin分泌,调节胰岛素抵抗改善预后。  相似文献
