Measurements of respiratory exchanges in genuine exercise conditions are undoubtedly of interest to further define the energy needs of endurance horses. However, the equine K4b2, the gas exchanges portable device validated for equines, has not been used in Arabian endurance horses yet. Therefore, the objective of this study was to implement field cardio-respiratory measurements in such horses using the equine K4b2 in order to assess energy expenditure (EE). Measurements of heart rate (HR), oxygen uptake (VO2), carbon dioxide production (VCO2), respiratory frequency (RF), tidal volume (VT) and minute expired volume (VE) were carried out at rest and during a 20-min submaximal incremental field exercise in five trained Arabian endurance horses equipped with the K4b2 system. The relationship between HR and VO2 was determined for each horse. EE of the exercise session was calculated from direct VO2 measurements and individual HR-VO2 regression. Out of the five horses, four tolerated the equipment. Respiratory and metabolic variables at rest and during exercise, as well as EE measured at the different gaits, were consistent with reported values in exercising horses: VO2 ranged from 4.8 to 54.1 ml/min per kg from rest to canter, respectively, and EE from 82 to 1095 J/min per kg BW. The 20-min exercise session EE accounted for 6258 and 6332 J from direct VO2 measurements and individual HR-VO2 regression, respectively, which did not differ significantly. Providing an adaptation period and several technical adjustments, the present equine K4b2 could be used to assess EE in Arabian endurance horses in a controlled environment. The prediction of EE from the individual VO2–HR relationship might be an alternative method to evaluate EE when VO2 measurements are not possible.  相似文献

Robotics-assisted tilt table (RATT) technology provides body support, cyclical stepping movement and physiological loading. This technology can potentially be used to facilitate the estimation of peak cardiopulmonary performance parameters in patients who have neurological or other problems that may preclude testing on a treadmill or cycle ergometer. The aim of the study was to compare the magnitude of peak cardiopulmonary performance parameters including peak oxygen uptake (VO2peak) and peak heart rate (HRpeak) obtained from a robotics-assisted tilt table (RATT), a cycle ergometer and a treadmill. The strength of correlations between the three devices, test-retest reliability and repeatability were also assessed. Eighteen healthy subjects performed six maximal exercise tests, with two tests on each of the three exercise modalities. Data from the second tests were used for the comparative and correlation analyses. For nine subjects, test-retest reliability and repeatability of VO2peak and HRpeak were assessed. Absolute VO2peak from the RATT, the cycle ergometer and the treadmill was (mean (SD)) 2.2 (0.56), 2.8 (0.80) and 3.2 (0.87) L/min, respectively (p < 0.001). HRpeak from the RATT, the cycle ergometer and the treadmill was 168 (9.5), 179 (7.9) and 184 (6.9) beats/min, respectively (p < 0.001). VO2peak and HRpeak from the RATT vs the cycle ergometer and the RATT vs the treadmill showed strong correlations. Test-retest reliability and repeatability were high for VO2peak and HRpeak for all devices. The results demonstrate that the RATT is a valid and reliable device for exercise testing. There is potential for the RATT to be used in severely impaired subjects who cannot use the standard modalities.  相似文献

Abstract

Late gestation energy expenditure (EEgest) originates from energy expenditure (EE) of development of conceptus (EEconceptus) and EE of homeorhetic adaptation of metabolism (EEhomeorhetic). Even though EEgest is relatively easy to quantify, its partitioning is problematic. In the present study metabolizable energy (ME) intake ranges for twin-bearing ewes were 220 – 440, 350 – 700, 350 – 900 kJ per metabolic body weight (W0.75) at week seven, five, two pre-partum respectively. Indirect calorimetry and a linear regression approach were used to quantify EEgest and then partition to EEconceptus and EEhomeorhetic. Energy expenditure of basal metabolism of the non-gravid tissues (EEbmng), derived from the intercept of the linear regression equation of retained energy [kJ/W0.75] and ME intake [kJ/W0.75], was 298 [kJ/W0.75]. Values of the intercepts of the regression equations at week seven, five, and two pre-partum were 311, 398, and 451 [kJ/W0.75], respectively. The difference between the intercepts for different weeks was used to calculate EEhomeorhetic. The remaining part of EEgest was considered to be EEconceptus. In conclusion, the good agreement between our values of EEconceptus and those in the literature indicates the method's validity.  相似文献

Reduced activation of exercise responsive signalling pathways have been reported in response to acute exercise after training; however little is known about the adaptive responses of the mitochondria. Accordingly, we investigated changes in mitochondrial gene expression and protein abundance in response to the same acute exercise before and after 10-d of intensive cycle training. Nine untrained, healthy participants (mean±SD; VO2peak 44.1±17.6 ml/kg/min) performed a 60 min bout of cycling exercise at 164±18 W (72% of pre-training VO2peak). Muscle biopsies were obtained from the vastus lateralis muscle at rest, immediately and 3 h after exercise. The participants then underwent 10-d of cycle training which included four high-intensity interval training sessions (6×5 min; 90–100% VO2peak) and six prolonged moderate-intensity sessions (45–90 min; 75% VO2peak). Participants repeated the pre-training exercise trial at the same absolute work load (64% of pre-training VO2peak). Muscle PGC1-α mRNA expression was attenuated as it increased by 11- and 4- fold (P<0.001) after exercise pre- and post-training, respectively. PGC1-α protein expression increased 1.5 fold (P<0.05) in response to exercise pre-training with no further increases after the post-training exercise bout. RIP140 protein abundance was responsive to acute exercise only (P<0.01). COXIV mRNA (1.6 fold; P<0.01) and COXIV protein expression (1.5 fold; P<0.05) were increased by training but COXIV protein expression was decreased (20%; P<0.01) by acute exercise pre- and post-training. These findings demonstrate that short-term intensified training promotes increased mitochondrial gene expression and protein abundance. Furthermore, acute indicators of exercise-induced mitochondrial adaptation appear to be blunted in response to exercise at the same absolute intensity following short-term training.  相似文献

The purpose of the present study was to examine the effects of active warm-up duration on the diurnal fluctuations in anaerobic performances. Twelve physical education students performed a medical stress test (progressive test up to exhaustion) and four Wingate tests (measurement of peak power [Ppeak], mean power [Pmean], and fatigue index during an all-out 30 s cycling exercise). The tests were performed in separate sessions (minimum interval?=?36?h) in a balanced and randomized design at 08:00 and 18:00?h, either after a 5?min (5-AWU) or a 15?min active warm-up (15-AWU). AWU consisted of pedaling at 50% of the power output at the last stage of the stress exhausting test. Rectal temperature was collected throughout the sessions. A two-way ANOVA (warm-up?×?time of day) revealed a significant interaction for Ppeak (F(1.11)?=?6.48, p?<?0.05) and Pmean (F(1.11)?=?5.84, p?<?0.05): the time-of-day effect was significant (p?<?0.001) in contrast with the effect of warm-up duration (p?>?0.05). Ppeak and Pmean improved significantly from morning to afternoon after both 5-AWU and 15-AWU, but the effect of warm-up duration was significant in the morning only. Indeed, the values of Ppeak or Pmean were the same after both warm-up protocols in the afternoon. For rectal temperature, there was no interaction between time-of-day and warm-up duration. Rectal temperature before and after both the warm-up protocols was higher in the afternoon, and the effect of warm-up duration on temperature was similar at 08:00 and 18:00?h. In conclusion, the interpretation of the results of the anaerobic performance tests should take into account time-of-day and warm-up procedures. Aims

To assess the cross-sectional association between exercise capacity, gas exchange efficiency and endothelial function, as measured by flow-mediated dilation (FMD) and nitroglycerin-mediated dilation (NMD) of the brachial artery, in a large-scale population-based survey.

Methods

The study population was comprised of 1416 volunteers 25 to 85 years old. Oxygen uptake at anaerobic threshold (VO2@AT), peak exercise (peakVO2) and ventilatory efficiency (VE vs. VCO2 slope and VE/VCO2@AT) were assessed on a breath-by-breath basis during incremental symptom-limited cardiopulmonary exercise. FMD and NMD measurements at rest were performed using standardised ultrasound techniques.

Results

Multivariable logistic regression analyses revealed a significant association between FMD and ventilatory efficiency in current smokers but not in ex-smokers or non-smokers. There was no association between FMD and VO2@AT or peak VO2. In current smokers, for each one millimetre decrement in FMD, VE/VCO2@AT improved by -3.6 (95% CI -6.8, -0.4) in the overall population [VE vs. VCO2 slope -3.9 (-7.1, -0.6)]. These results remained robust after adjusting for all major influencing factors. Neither exercise capacity nor ventilatory efficiency was significantly associated with NMD.

Conclusion

In current smokers, FMD is significantly associated with ventilatory efficiency. This result may be interpreted as a potential clinical link between smoking and early pulmonary vasculopathy due to smoking.  相似文献

Objectives

We assessed the prospective association of resting heart rate (RHR) at baseline with peak oxygen uptake (VO2peak) 23 years later, and evaluated whether physical activity (PA) could modify this association.

Background

Both RHR and VO2peak are strong and independent predictors of cardiovascular morbidity and mortality. However, the association of RHR with VO2peak and modifying effect of PA have not been prospectively assessed in population studies.

Methods

In 807 men and 810 women free from cardiovascular disease both at baseline (1984–86) and follow-up 23 years later, RHR was recorded at both occasions, and VO2peak was measured by ergospirometry at follow-up. We used Generalized Linear Models to assess the association of baseline RHR with VO2peak, and to study combined effects of RHR and self-reported PA on later VO2peak.

Results

There was an inverse association of RHR at baseline with VO2peak (p<0.01). Men and women with baseline RHR greater than 80 bpm had 4.6 mL·kg−1·min−1 (95% confidence interval [CI], 2.8 to 6.3) and 1.4 mL·kg−1·min−1 (95% CI, −0.4 to 3.1) lower VO2peak at follow-up compared with men and women with RHR below 60 bpm at baseline. We found a linear association of change in RHR with VO2peak (p = 0.03), suggesting that a decrease in RHR over time is likely to be beneficial for cardiovascular fitness. Participants with low RHR and high PA at baseline had higher VO2peak than inactive people with relatively high RHR. However, among participants with relatively high RHR and high PA at baseline, VO2peak was similar to inactive people with relatively low RHR.

Conclusion

RHR is an important predictor of VO2peak, and serial assessments of RHR may provide useful and inexpensive information on cardiovascular fitness. The results suggest that high levels of PA may compensate for the lower VO2peak associated with a high RHR.  相似文献

Background

Patients with systemic sclerosis (SSc) may develop exercise intolerance due to musculoskeletal involvement, restrictive lung disease, left ventricular dysfunction, or pulmonary vasculopathy (PV). The latter is particularly important since it may lead to lethal pulmonary arterial hypertension (PAH). We hypothesized that abnormalities during cardiopulmonary exercise testing (CPET) in patients with SSc can identify PV leading to overt PAH.

Methods

Thirty SSc patients from the Harbor-UCLA Rheumatology clinic, not clinically suspected of having significant pulmonary vascular disease, were referred for this prospective study. Resting pulmonary function and exercise gas exchange were assessed, including peakVO2, anaerobic threshold (AT), heart rate- VO2 relationship (O2-pulse), exercise breathing reserve and parameters of ventilation-perfusion mismatching, as evidenced by elevated ventilatory equivalent for CO2 (VE/VCO2) and reduced end-tidal pCO2 (PETCO2) at the AT.

Results

Gas exchange patterns were abnormal in 16 pts with specific cardiopulmonary disease physiology: Eleven patients had findings consistent with PV, while five had findings consistent with left-ventricular dysfunction (LVD). Although both groups had low peak VO2 and AT, a higher VE/VCO2 at AT and decreasing PETCO2 during early exercise distinguished PV from LVD.

Conclusions

Previously undiagnosed exercise impairments due to LVD or PV were common in our SSc patients. Cardiopulmonary exercise testing may help to differentiate and detect these disorders early in patients with SSc.  相似文献

The aim of this study was to compare: i) the physiological and perceptual responses of low-load exercise [(moderate intensity exercise (MI)] with different levels of blood flow restriction (BFR), and ii) MI with BFR on the bike with high intensity (HI) exercise without BFR. The protocol involved large muscle mass exercise at different levels of BFR, and this differentiates our study from others. Twenty-one moderately trained males (age: 24.6 ± 2.4 years; VO2peak: 47.2 ± 7.0 ml.kg-1.min-1, mean ± sd) performed one maximal graded exercise test and seven 5-min constant-load cycling bouts. Six bouts were at MI [40% peak power (Ppeak), 60%VO2peak], one without BFR and five with different levels of BFR (40%, 50%, 60%, 70%, 80% of estimated arterial occlusion pressure). The HI bout (70%Ppeak, 90%VO2peak) was without BFR. Oxygen uptake (VO2), heart rate (HR), blood lactate (BLa), rate of perceived exertion (RPE), and tissue oxygen saturation (TSI) were recorded. Regardless of pressure, HR, BLa and RPE during MI-BFR were higher compared to MI (p < 0.05, ES: moderate to very large), and TSI reduction was greater in MI-BFR than MI (p < 0.05, ES: moderate to large). The responses of VO2, HR, BLa, RPE and TSI induced by the different levels of BFR in MI-BFR were similar. Regardless of pressure, the responses of VO2, HR, BLa and RPE induced by MI-BFR were lower than HI (p < 0.05), except for TSI. TSI change was similar between MI-BFR and HI. It appears that BFR equal to 40% of arterial occlusion pressure is sufficient to reduce TSI when exercising with a large muscle mass.  相似文献

Background and Aims

Recent introduction of the Cosmed K4b2 portable metabolic analyzer allows measurement of oxygen consumption outside of a laboratory setting in more typical clinical or household environments and thus may be used to obtain information on the metabolic costs of specific daily life activities. The purpose of this study was to assess the accuracy of the Cosmed K4b2 portable metabolic analyzer against a traditional, stationary gas exchange system (the Medgraphics D-Series) during steady-state, submaximal walking exercise.

Methods

Nineteen men and women (9 women, 10 men) with an average age of 39.8 years (±13.8) completed two 400 meter walk tests using the two systems at a constant, self-selected pace on a treadmill. Average oxygen consumption (VO2) and carbon dioxide production (VCO2) from each walk were compared.

Results

Intraclass Correlation Coefficient (ICC) and Pearson correlation coefficients between the two systems for weight indexed VO2 (ml/kg/min), total VO2 (ml/min), and VCO2 (ml/min) ranged from 0.93 to 0.97. Comparison of the average values obtained using the Cosmed K4b2 and Medgraphics systems using paired t-tests indicate no significant difference for VO2 (ml/kg/min) overall (p = 0.25), or when stratified by sex (p = 0.21 women, p = 0.69 men). The mean difference between analyzers was – 0.296 ml/kg/min (±0.26). Results were not significantly different for VO2 (ml/min) or VCO2 (ml/min) within the study population (p = 0.16 and p = 0.08, respectively), or when stratified by sex (VO2: p = 0.51 women, p = 0.16 men; VCO2: p = .11 women, p = 0.53 men).

Conclusion

The Cosmed K4b2 portable metabolic analyzer provides measures of VO2 and VCO2 during steady-state, submaximal exercise similar to a traditional, stationary gas exchange system.  相似文献

Res. 1999;7:23–33. Objective : Activity self-reports are a commonly used tool in assessing daily physical activity (PA) and associated energy expenditure (EE). This study examined the effect of relative body fatness (%BF) on differences between self-reported and measured duration and associated EE in healthy adults. Research Methods and Procedures: Men and women (n= 115, age 38±9 years), ranging in %BF from 7.9% to 58.9%, spent two separate days (normal and exercise) in a whole-room indirect calorimeter where EE was measured. While in the room calorimeter, subjects reported the type, intensity, and duration of each performed PA. The Compendium of Physical Activity was used to calculate the energy cost of each reported activity. The EE of all self-reported activities (EEr) was categorized into four intensity levels, synchronized, and compared with EE from the room calorimeter (EEm). Results : With increasing %BF, subjects significantly overestimated duration of more strenuous activities (≥4.5), while underestimating moderate activities (2.5 to 4.4 metabolic equivalents (METs)). Misreporting of duration and/or intensity caused an overestimation or underestimation of PA-associated EE at these levels. Reported EE sleep was lower than measured EE sleep, although both had similar durations. As a result, total EEr was similar to EEm. Discussion : Individual variability of daily total PA and associated EE generated from self-reports in adults is high. Persons with a higher %BF report duration and/or intensity of moderate to high levels of PA with lower accuracy than leaner individuals. We conclude using the Compendium of Physical Activity is not suitable for the accurate estimation of self-reported EE of AA in adults with a higher %BF.  相似文献

Impaired cardiorespiratory fitness (CRF) is a hallmark characteristic in obese and lean sedentary young women. Peak oxygen consumption (VO2peak) prediction from the six-minute step test (6MST) has not been established for sedentary females. It is recognized that lower-limb muscle strength and power play a key role during functional activities. The aim of this study was to investigate cardiorespiratory responses during the 6MST and CPX and to develop a predictive equation to estimate VO2peak in both lean and obese subjects. Additionally we aim to investigate how muscle function impacts functional performance. Lean (LN = 13) and obese (OB = 18) women, aged 20–45, underwent a CPX, two 6MSTs, and isokinetic and isometric knee extensor strength and power evaluations. Regression analysis assessed the ability to predict VO2peak from the 6MST, age and body mass index (BMI). CPX and 6MST main outcomes were compared between LN and OB and correlated with strength and power variables. CRF, functional capacity, and muscle strength and power were lower in the OB compared to LN (<0.05). During the 6MST, LN and OB reached ~90% of predicted maximal heart rate and ~80% of the VO2peak obtained during CPX. BMI, age and number of step cycles (NSC) explained 83% of the total variance in VO2peak. Moderate to strong correlations between VO2peak at CPX and VO2peak at 6MST (r = 0.86), VO2peak at CPX and NSC (r = 0.80), as well as between VO2peak, NSC and muscle strength and power variables were found (p<0.05). These findings indicate the 6MST, BMI and age accurately predict VO2peak in both lean and obese young sedentary women. Muscle strength and power were related to measures of aerobic and functional performance.  相似文献

The purpose of this study was to assess the effect of relative exercise intensity on various plasma trace elements in euhydrated endurance athletes.Twenty-seven well-trained endurance athletes performed a cycloergometer test: after a warm-up of 10 min at 2.0 W kg−1, workload increased by 0.5 W kg−1 every 10 min until exhaustion. Oxygen uptake, blood lactate concentration ([La]b), and plasma ions (Zn, Se, Mn and Co) were measured at rest, at the end of each stage, and 3, 5 and 7 min post-exercise. Urine specific gravity (USG) was measured before and after the test, and subjects drank water ad libitum. Fat oxidation (FATOXR), carbohydrate oxidation (CHOOXR), energy expenditure from fat (EEFAT), from carbohydrates (EECHO) and total EE (EET) were estimated using stoichiometric equations. A repeated measure (ANOVA) was used to compare plasma ion levels at each exercise intensity level. The significance level was set at P < 0.05.No significant differences were found in USG between, before, and after the test (1.014 ± 0.004 vs. 1.014 ± 0.004 g cm−3) or in any plasma ion level as a function of intensity. There were weak significant correlations of Zn (r = 0.332, P < 0.001) and Se (r = 0.242, P < 0.01) with [La]b, but no relationships were established between [La]b, VO2, FATOXR, CHOOXR, EEFAT, EECHO, or EET and plasma ion levels.Acute exercise at different submaximal intensities in euhydrated well-trained endurance athletes does not provoke a change in plasma trace element levels, suggesting that plasma volume plays an important role in the homeostasis of these elements during exercise.  相似文献

High altitude natives are reported to have outstanding work capacity in spite of the challenge of oxygen transport and delivery in hypoxia. To evaluate the developmental effect of lifelong exposure to hypoxia on aerobic capacity, VO2peak was measured on two groups of Peruvian Quechua subjects (18–35 years), who differed in their developmental exposure to altitude. Male and female volunteers were recruited in Lima, Peru (150 m), and were divided in two groups, based on their developmental exposure to hypoxia, those: a) Born at sea‐level individuals (BSL), with no developmental exposure to hypoxia (n = 34) and b) Born at high‐altitude individuals (BHA) with full developmental exposure to hypoxia (n = 32), but who migrated to sea‐level as adults (>16‐years‐old). Tests were conducted both in normoxia (BP = 750 mm Hg) and normobaric hypoxia at sea‐level (BP = 750 mm Hg, FiO2 = 0.12, equivalent to 4,449 m), after a 2‐month training period (in order to control for initial differences in physical fitness) at sea‐level. BHA had a significantly higher VO2peak at hypoxia (40.31 ± 1.0 ml/min/kg) as compared to BSL (35.78 ± 0.96 ml/min/kg, P = 0.001), adjusting for sex. The decrease of VO2peak at HA relative to SL (ΔVO2peak) was not different between groups, controlling for baseline levels (VO2peak at sea‐level) and sex (BHA = 0.35 ± 0.04 l/min, BSL = 0.44 ± 0.04 l/min; P = 0.12). Forced vital capacity (controlling for height) and the residuals of VO2peak (controlling for weight) had a significant association in the BHA group only (r = 0.155; P = 0.031). In sum, results indicate that developmental exposure to altitude constitutes an important factor to determine superior exercise performance. Summary. The effect of beta-alanine (β-Ala) alone or in combination with creatine monohydrate (Cr) on aerobic exercise performance is unknown. The purpose of this study was to examine the effects of 4 weeks of β-Ala and Cr supplementation on indices of endurance performance. Fifty-five men (24.5 ± 5.3 yrs) participated in a double-blind, placebo-controlled study and randomly assigned to one of 4 groups; placebo (PL, n = 13), creatine (Cr, n = 12), beta-alanine (β-Ala, n = 14), or beta-alanine plus creatine (CrBA, n = 16). Prior to and following supplementation, participants performed a graded exercise test on a cycle ergometer to determine VO2peak, time to exhaustion (TTE), and power output, VO2, and percent VO2peak associated with VT and LT. No significant group effects were found. However, within groups, a significant time effect was observed for CrBa on 5 of the 8 parameters measured. These data suggest that CrBA may potentially enhance endurance performance.  相似文献

Associations between body mass index (BMI), peak oxygen consumption (VO2peak), and metabolic syndrome (MetS) risk factors have not been adequately studied in Japanese children. Here the relationships between these parameters and the threshold aerobic fitness level necessary for low MetS risk were determined. The participants (299 children; 140 boys and 159 girls, aged 9.1 ± 0.3 years) were divided into four groups using the medians of predicted VO2peak (pVO2peak) and BMI. MetS risk scores were calculated using z-scores. Receiver Operating Characteristic analysis was used to determine the threshold aerobic fitness level necessary for low MetS risk. The MetS risk score of the High BMI group was significantly higher than that of the Low BMI group for both sexes (p < 0.0001). However, the High BMI/High Fitness group had a significantly lower MetS risk score than the High BMI/Low Fitness group for both sexes. The pVO2peak cut-off values for low MetS risk were 47.9 and 44.9 ml/kg/min for boys and girls, respectively. Our results suggest that improvements in both fatness and aerobic fitness are important for decreasing MetS risk. We also confirmed the pVO2peak of cut-off values necessary for low MetS risk in Japanese children.  相似文献

Aging-induced deterioration of arterial stiffness is decreased by regular exercise, and increased nitric oxide (NO) production participates in this effect. Apelin regulates endothelial NO synthase in endothelial cells, promoting NO production. However, the effect of aerobic exercise training on circulating apelin levels in healthy middle-aged and older adults remains unknown. Accordingly, this study aimed to clarify the effects of regular aerobic exercise on apelin concentrations in middle-aged and older adults. Thirty-four healthy middle-aged and older subjects (67.0 ± 1.3 years) were randomly divided into two groups: exercise intervention and sedentary controls. Subjects in the training group completed 8-week of aerobic exercise training (60–70% peak oxygen uptake [VO2peak] for 45 min, 3 days/week). Before and after the intervention, we evaluated plasma apelin and nitrite/nitrate (NOx) concentrations, VO2peak, and arterial stiffness index. In the training group, VO2peak was significantly increased, and carotid β-stiffness was significantly decreased, after the intervention (P<0.05). Moreover, plasma apelin and NOx levels were significantly increased in the training group after the intervention (P<0.05). Additionally, there was a correlation between the training effects of plasma apelin levels and carotidβ-stiffness (r = −0.508, P = 0.032) and plasma NOx levels (r = 0.494, P = 0.037). By contrast, none of these parameters changed significantly in the control group. These results suggest that the increased in plasma apelin levels may be associated with exercise training-induced alternation of arterial stiffness in middle-aged and older adults.  相似文献

Background

Although aerobic fitness is regarded as an overall prognostic measure of morbidity and mortality, its evaluation in the chronic progressive sinopulmonary disease primary ciliary dyskinesia (PCD) has been infrequently and inconsistently reported. Here we assessed peak oxygen uptake (VO2peak) in a large well-characterized cohort of PCD patients, and explored whether VO2peak was associated with parameters of pulmonary function, self-reported physical limitations, and physical activity level.

Methods

VO2peak, spirometry, diffusing capacity, whole-body plethysmography, and nitrogen multiple breath inert gas washout (N2 MBW) were assessed in a cross-sectional, single-occasion study of clinically stable children and young adults with PCD. We used a questionnaire including self-reported physical limitations in everyday life or in vigorous activities, and estimation of weekly hours of strenuous physical activity. VO2peak in PCD patients was compared with that in matched, healthy control subjects and a national reference.

Results

Forty-four PCD patients aged 6–29 years exhibited reduced VO2peak compared to healthy controls (P<0.001) and the national reference. VO2peak was abnormal (z-score <–1.96) in 34% of PCD patients. Spirometric values, RV/TLC, and indices of N2 MBW were significantly abnormal, but VO2peak only correlated with FEV1 and DLCO/VA. VO2peak correlated with complaints of moderate or significant limitations in vigorous activities (P = 0.0001), exhibited by 39% of PCD patients.

Conclusion

One-third of PCD patients exhibited substantially lower aerobic fitness than healthy subjects. Aerobic fitness correlated with FEV1, DLCO/VA and self-reported complaints of limitations in vigorous physical activity. These findings are most likely explained by PCD pulmonary disease and its impact on pulmonary function and physical ability. Considering fitness as an important outcome and including regular strenuous physical activity in PCD treatment would probably altogether increase pulmonary clearance, lung function, aerobic fitness, and quality of life, and prevent lifestyle-related diseases.  相似文献

目的:探讨阻塞性睡眠呼吸暂停低通气综合征(OSAHS)血压变异性及运动心肺功能与病情的相关性分析。方法:选取2017年2月~2018年11月期间我院收治的OSAHS患者119例为研究对象,根据每小时睡眠呼吸暂停次数(AHI)将患者分为轻度组(AHI:5~20次/h,n=45)、中度组(AHI:21~40次/h,n=39)、重度组(AHI:40次/h,n=35),检测所有研究对象的血压变异性及运动心肺功能,并作组间比较。采用Pearson积矩相关分析OSAHS患者血压变异性及运动心肺功能与病情的相关性。结果:OSAHS患者收缩压(SBP)昼、舒张压(DBP)夜以及中度组、重度组SBP夜均高于对照组(P0.05);重度组DBP夜以及中度组、重度组SBP昼、SBP夜高于轻度组(P0.05);重度组SBP夜、SBP昼、DBP夜高于中度组(P0.05);而各组DBP昼整体比较差异无统计学意义(P0.05)。OSAHS患者最大摄氧量占预计值百分比(VO2max%pred)、最大摄氧量峰值占预计值百分比(peak VO2%pred)、无氧阈值(AT)、摄氧量功率比值(VO2/WR)、呼吸储备(VEmax/MVV)均低于对照组,且随病情严重程度的增加而降低(P0.05),OSAHS患者二氧化碳通气当量(VE/VCO2)高于对照组,且随病情严重程度的增加而升高(P0.05)。Pearson积矩相关分析结果显示,OSAHS患者病情严重程度与SBP昼、SBP夜、DBP夜、VE/VCO2呈正相关(P0.05),与VO2max%pred、peakVO2%pred、AT、VEmax/MVV、VO2/WR呈负相关(P0.05),与DBP昼无相关性(P0.05)。结论:血压变异性及运动心肺功能指标可反映OSAHS患者病情严重程度,可考虑作为早期监测指标并参考指导治疗。  相似文献

Objective: To determine the influence of dietary glycemic index on exercise training‐induced adaptations in substrate oxidation in obesity. Design and Methods: Twenty older, obese individuals undertook 3 months of fully supervised aerobic exercise and were randomized to low‐ (LoGIX) or high‐glycemic (HiGIX) diets. Changes in indirect calorimetry (VO2; VCO2) were assessed at rest, during a hyperinsulinemic‐euglycemic clamp, and during submaximal exercise (walking: 65% VO2max, 200 kcal energy expenditure). Intramyocellular lipid (IMCL) was measured by 1H‐magnetic resonance spectroscopy. Results: Weight loss (?8.6 ± 1.1%) and improvements (P < 0.05) in VO2max, glycemic control, fasting lipemia, and metabolic flexibility were similar for both LoGIX and HiGIX groups. During submaximal exercise, energy expenditure was higher following the intervention (P < 0.01) in both groups. Respiratory exchange ratio during exercise was unchanged in the LoGIX group but increased in the HiGIX group (P < 0.05). However, fat oxidation during exercise expressed in relation to changes in body weight was increased in the LoGIX group (+10.6 ± 3.6%; P < 0.05). Fasting IMCL was unchanged, however, extramyocellular lipid was reduced (P < 0.05) after LoGIX. Conclusions: A LoGIX/exercise weight‐loss intervention increased fat utilization during exercise independent of changes in energy expenditure. This highlights the potential therapeutic value of low‐glycemic foods for reversing metabolic defects in obesity.  相似文献
