Over the past decade and more, Western practices bearing on social life have been marked by a turn against Cartesian dualism. In particular, sociological analysts, political activists, and many ordinary Westerners have come increasingly to advocate an appreciation of the person in terms of the mind-body construed as inextricably one. To account for this, comparative material from Inuit hunter-gatherers and the Western New Age movement is brought to bear. Using this material, the holistic person is interpreted as a discourse about individual moral frailty, ideologically pertinent in societies where egalitarianism of outcome is the dominant cultural ethos. This analysis is related to Giddens's account of 'late modernity', specifically as this is informed by 'life politics'.  相似文献

Dental enamel hypoplasias are increasingly being used in epidemiological studies as indicators of health within both modern and prehistoric populations. This symptom of growth disruption is used here to examine possible changes in health occurring at the transition between the Bronze Age and Iron Age in Jordan, through examination of enamel hypoplasias in skeletal remains from two tombs at the archaeological site of Pella. A small but not statistically significant difference in the prevalence and frequency of hypoplastic defects was found between the two time periods. These results suggest that the political and economic changes occurring at this time were not sufficiently stressful to cause a dramatic deterioration in health at the onset of the Early Iron Age.  相似文献

This article considers the role of telemedicine in the production of biomedical health care using three specific theoretical constructs as lenses through which to examine this phenomenon: (1) Foucault's medical "gaze"; (2) the political economy of health; and (3) deterritorialization and multisite ethnography. This examination focuses first on the changing corporate structure of health care and changing political attitudes toward telemedicine. Second, it documents the current use of telemedicine in prisons, the military, and in cross-cultural settings. Third, it discusses responses to telemedicine of individual physicians, health care staff, and patients, finding that these responses are broadly conditioned by an individual's mode of articulation with biomedical institutions and that they are mediated by personal experience.  相似文献

In this article, I examine the process by which some biomedical physicians and nurses in Australia have come to adopt various alternative therapies in their regimens of practice, largely in response to (1) the growing interest on the part of many Australians in what is generally called "complementary medicine", and (2) a recognition that biomedicine is not particularly effective in treating an array of chronic ailments. Some Australian biomedical physicians and nurses have come to embrace "integrative medicine," which purports to blend the best of biomedicine and complementary medicine, and have even created an Australasian Integrative Medical Association and established integrative medical training programs and centers. I argue that the adoption of alternative therapies and the development of integrative medicine on the part of Australian biomedical physicians and nurses constitute another national manifestation of the co-option of complementary and alternative medicine.  相似文献

This article examines how a group of public health physicians in the urban Amazon values medicinal plant knowledge. As biomedical health care providers, physicians routinely draw on scientific plant knowledge. At the same time, as residents of the Amazon and health care providers to the poor, they are aware of and sometimes participate in local systems of plant knowledge. When discussing medicinal plant use, physicians repeatedly mention three themes: science, superstition, and biopiracy. The way in which physicians construct and negotiate these themes is part of the process of maintaining and legitimating their expertise and authority. This analysis finds that context is key to understanding whether, when, and why physicians value certain bodies of knowledge. Locally, in clinics, scientific plant knowledge is constructed as superior. In a global context, however, local plant knowledge is explicitly valued. This situational valuation/devaluation of plant knowledge relates to the positions of power physicians occupy in each context.  相似文献

Naturopathic medicine in the United States had its inception around the turn of the 20th century. Subsequently, it underwent a process of relatively rapid growth until around the 1930s, followed by a period of gradual decline almost to the point of extinction due to biomedical opposition and the advent of "miracle drugs." Because its therapeutic eclecticism had preadapted it to fit into the holistic health movement that emerged in the 1970s, it was able to undergo a process of organizational rejuvenation during the last two decades of the century. Nevertheless, U.S. naturopathy as a professionalized heterodox medical system faces several dilemmas as it enters the new millennium. These include (1) the fact that it has succeeded in obtaining licensure in only two sections of the country, namely, the Far West and New England; (2) increasing competition from partially professionalized and lay naturopaths, many of whom are graduates of correspondence schools; and (3) the danger of cooptation as many biomedical practitioners adopt natural therapies.  相似文献

This article discusses the perspectives of Chinatown’s traditional Chinese medical practitioners on tuberculosis among New York City’s Chinese laborers. The practice of traditional Chinese medicine (TCM) in the United States is neither regulated nor well understood. Some public health providers have expressed their concern that the use of TCM could prevent Chinese tuberculosis patients from receiving proper, biomedical treatment. Contrary to the suspicion of public health providers, the traditional Chinese medical practitioners in the context of New York City’s Chinatown provide diverse methods of health care, many being familiar with the biomedical explanation for tuberculosis. All TCM informants in this study stated that biomedicine is more effective than Chinese medicine in treating tuberculosis. TCM in tuberculosis therapy is said to complement biomedicine and to restore bodily balance and the general health of patients. This study discusses the political–economic context shaping the explanation and treatment of tuberculosis among traditional Chinese medical practitioners and broadens our understanding of the various contexts in which TCM and biomedicine can be integrated. Furthermore, it is suggested that an opportunity exists for tuberculosis control programs to incorporate TCM practitioners in the effort to control the disease within New York City.  相似文献

Croatia never had a separate vocation (occupation) of a health educator. Health education is one of the main tasks in the long tradition of preventive work of doctors-school medicine specialists. Additionally, in the school curriculum in the Republic of Croatia the health-educational contents are integrated into various subjects, and are conducted by teachers. However, there are requests to introduce a new subject into schools called Health Education. We asked physicians of school medicine their opinion on the introduction of this new subject. 30% of the physicians were dissatisfied with the current condition, 10% were satisfied, while there were no very satisfied school medicine doctors. They believe that health education goals are oriented solely to passing on knowledge (facts), while efforts are not done to change habits and attitudes of young people. They recognize themselves as persons mostly involved in health education in schools. Half of the school medicine doctors believe that the school curriculum should contain both a separate subject as well as integration of health education into other subjects. Before introducing any changes into healthcare or education system, it is necessary to examine the attitudes of students and parents, to direct the changes towards the promotion of the cooperation between the healthcare system, education, civil society, school and community where investing into the health of young people is done through comprehensive and holistic programmes.  相似文献

Historically, the United States and Aotearoa/New Zealand symbolize opposite poles of an individualist-collectivist welfare state continuum. Until recently, Aotearoa/New Zealand was known as a "cradle-to-grave" welfare state, with "universal" employment and coverage in health and education. U.S. history, in contrast, is marked by an unabashed individualism and a residualist approach to welfare. Recent neoliberal reforms, however, have engendered a convergence between the two countries in the conceptualization and organization of assistance for poor single mothers. Most notable are the "workfare" provisions of legislative changes made in 1996 in the two countries, which work to reconstitute poor mothers as potential able-bodied workers. In this article we analyze welfare reform in the United States and Aotearoa/New Zealand, with particular reference to how poor single mothers respond to, comply and cope with, or resist neoliberal strategies. Analysis is based on participant-observation, interviews, and focus groups conducted between 1989 and 1999. Drawing on ethnographic material from a government hospital in Papua New Guinea (PNG), this paper examines the relationship between power and visibility in two kinds of bureaucratic practice: hospital managers’ performance of institutional ‘transparency’ and patients’ careful management of their government health documents. In both examples, people engage with bureaucratic and biomedical technologies of visibility to entice governing actors to see and enter into a relationship with them. Against dominant Foucauldian theories of power‐vision, the article shows that, in the institutional crevices of a ‘weak state’, bureaucratic technologies operate as relational technologies that elicit affective motivations rather than as disciplinary technologies that transform the self.  相似文献

Concerns about conflicts of interest have driven a wedge between academia and the pharmaceutical and devices industries. Although elevated concern for bias is justified, particularly when academics may affect drug sales, partnerships between industry and academia are essential to achieve the full promise of health improvement from the public investment in biomedical research. The study of charcoal produced by five burning episodes that occurred in a rapid succession within a ritual pit dating to the late Iron Age at Raffin Fort, Co. Meath, Ireland, reveals considerable variation in the charcoal assemblages resulting from each burning episode. Wood selection processes are considered against the background of information on woodland composition and land-use history provided by a detailed pollen diagram from nearby Emlagh Bog, the chronology of which is based on both AMS 14C dates and tephra analysis. A human skull fragment lay on top of the charcoal layers but the radiocarbon evidence indicates that the skull predated the burnings by at least a century. This and other evidence indicate a ritual pit with the skull as a human relic. It is suggested that, in this instance, wood selection was neither random nor determined solely by availability or combustibility, but instead may have been informed by socio-religious belief systems pertaining to trees and wood. Early Irish documentary sources, which reveal a complex ethnography of wood and trees in later prehistoric and early historic Ireland, are reviewed. The results shed fresh light on aspects of late Iron Age archaeology in a part of Europe that was outside the direct influence of the Roman world. New information is provided on a distinctive feature in late Holocene Irish pollen records namely the Late Iron Age Lull (ca. a.d. 1–500). During this time, widespread regeneration of woody vegetation took place. In the subsequent early Medieval period renewed farming activity resulted in substantial decline in woodland, a pattern also seen at many other locations in Ireland.  相似文献

A need for more in‐depth understanding of the oral health of that age group has pointed to a need for more qualitative investigations. Objective: To explore experiences and perceptions of oral health and oral health care among an ethnically‐mixed sample of older New Zealanders. Methods: In‐depth interviews were conducted with 24 older people in two communities in New Zealand’s South Island. Thematic analysis of transcribed data was undertaken. Results: Three main themes that emerged were: (1) the processes of negotiating a tension between cost and convenience of access; (2) the experiential constraining of oral health maintenance; and (3) trusting in dental professionals. These serve to organise processes such as normalising, justifying and social comparisons that create an equilibrium or tolerance and acceptance of what might otherwise be considered to be relatively poor oral health. Conclusions: We identified a number of shared experiences which affect older people’s ability to maintain their oral health in the face of material and social barriers to oral health care. Because expectations were generally lower, there was greater concordance between experience and expectation, and people tended to be fairly satisfied with their oral health and the care they had received.  相似文献

Background

Although its incidence has been decreasing during the last decade, malaria is still a major public health issue in Madagascar. The use of Long Lasting Insecticidal Nets (LLIN) remains a key malaria control intervention strategy in Madagascar, however, it encounters some obstacles. The present study aimed to explore the local terminology related to malaria, information channels about malaria, attitude towards bed nets, and health care seeking practices in case of fever. This article presents novel qualitative findings about malaria. Until now, no such data has been published for Madagascar.

Methods

A comparative qualitative study was carried out at four sites in Madagascar, each differing by malaria epidemiology and socio-cultural background of the populations. Seventy-one semi-structured interviews were conducted with biomedical and traditional caregivers, and members of the local population. In addition, observations of the living conditions and the uses of bed net were conducted.

Results

Due to the differences between local and biomedical perspectives on malaria, official messages did not have the expected impact on population in terms of prevention and care seeking behaviors. Rather, most information retained about malaria was spread through informal information circulation channels. Most interviewees perceived malaria as a disease that is simple to treat. Tazomoka (“mosquito fever”), the Malagasy biomedical word for malaria, was not used by populations. Tazo (“fever”) and tazomahery (“strong fever”) were the terms more commonly used by members of the local population to refer to malaria related symptoms. According to local perceptions in all areas, tazo and tazomahery were not caused by mosquitos. Each of these symptoms required specific health recourse. The usual fever management strategies consisted of self-medication or recourse to traditional and biomedical caregivers. Usage of bed nets was intermittent and was not directly linked to protection against malaria in the eyes of most Malagasy people.

Conclusions

This article highlights the conflicting understanding of malaria between local perceptions and the biomedical establishment in Madagascar. Local perceptions of malaria present a holistic vision of the disease that includes various social and cultural dimensions, rather than reflecting one universal understanding, as in the biomedical image. The consideration of this “holistic vision” and other socio-cultural aspects surrounding the understanding of malaria is essential in implementing successful control intervention strategies.  相似文献

The age distribution of 865 lactating New Zealand sea lions (NZSLs; Phocarctos hookeri ) was investigated over 3 yr (1999–2001) at two breeding colonies, Sandy Bay and Dundas Island, New Zealand. Lactating females were aged between 3 and 26 yr with a maximum observed age of 28 yr. The mean age of lactating females was 11.1 (SE = 0.16) yr. Age distributions peaked at ages 8 and 9 with a strong skew toward younger females, likely indicative of maximum recruitment into the breeding population by this age. There were significant intersite differences in age structure and also significant interannual differences in age distributions at Sandy Bay, but not at Dundas Island. Given that the two colonies are less than 10 km apart, have some interchange, and share foraging areas, these differences are surprising. However, the colony at Dundas Island is almost four times larger than Sandy Bay and may therefore be less sensitive to demographic or environmental stochasticity. That age distributions of NZSLs vary significantly over small temporal and spatial scales has important implications for the extrapolation of data from one site or year to the population level, and hence for their management and conservation.  相似文献

This article considers contributions that the medical humanities have made to biomedical ethics. Philosophy has contributed methods of ethical justification to case analysis and has given birth to the New Professionalism movement. Taking biography as its paradigmatic resource, this movement has refocused medical education on the formation of physicians who not only have certain responsibilities to their patients, but also a regard for the role of the medical profession in working toward social justice. However, reliance on biography is now giving way to a renewed emphasis on autobiography, as educators seek to support medical students through the personal and spiritual journey that confrontation with death, disease, and injustice naturally entails. Reflective methods and techniques characterize this emerging era as educators and students try to preserve and reinvigorate the soul of medicine.  相似文献

João de Deus (John of God) is a Brazilian faith healer who has become increasingly well‐known outside Brazil. In 2006 alone he was invited to conduct healing events in Germany, the United States of America and New Zealand. He returned to the United States of America and New Zealand in 2007, and in 2008 was once again in the United States of America. There are plans for him to come to Australia in the near future. This article is based on ethnographic research conducted in Brazil, New Zealand and Australia and on interviews with Australians in Sydney. Here, I explore the stories of Australian followers of John of God and their reasons for seeking his healing system. I argue that Western New Age and alternative medicine concepts of healing produce a disposition ( Bourdieu 1972 ) towards ‘traditional’ healing from exotic lands. I show that the reinstatement of a connection between healing and religion, the construction of context that gives meaning to illness, and empowerment through surrendering to a higher power are three important factors that help to explain why people shun Western medicine in favour of alternative practices.  相似文献
