目的:探讨剖宫产术后产褥感染的相关危险因素,为临床制定感控措施提供参考。方法:回顾性分析我院接受剖宫产术的1760例孕妇的临床资料,采用单因素卡方检验及多因素Logistic回归方法对剖宫产术后发生产褥感染的相关危险因素进行统计学分析。结果:1760例接受剖宫产手术的孕妇,术后发生产褥感染的有68例,发生率为3.86%;发生产褥感染最常见的部位是急性子宫内膜炎、子宫肌炎,其构成比占57.4%;经过单因素卡方检验及多因素Logistic回归分析,最终筛选出手术时间(OR=1.351)、血红蛋白(OR=1.759)、胎膜早破(OR=2.247)、孕期生殖道感染反复发作(OR=2.047)、妊娠糖尿病(OR=1.473)、前置胎盘反复阴道出血(OR=1.584)与宫产术后发生产褥感染呈正相关(P0.05)。结论:手术时间、血红蛋白、胎膜早破、孕期生殖道感染反复发作、妊娠糖尿病、前置胎盘反复阴道出血是剖宫产后发生产褥感染的高危因素,临床应针对这些高危因素制定干预措施以降低感染的发生率。  相似文献

目的了解我市某三级甲等医院急危重症患者压力性损伤发生情况,分析其影响因素,并提出干预措施。方法选取我市某三级甲等医院急危重症患者450例,记录患者压力性损伤发生情况,并采用自行设计的急危重症患者压力性损伤危险因素调查表进行危险因素调查分析。结果急危重症患者压力性损伤发生率为7.78%。采用Logistic回归分析显示,患者年龄、体重、住院前感知能力、住院前活动能力、住院时长、使用医疗器具、使用预防性敷料情况和皮肤情况是压力性损伤发生的危险因素。结论急危重症患者压力性损伤受自身和院内多种危险因素影响,医护人员应对住院患者进行全面评估,给予针对性的防护措施,以降低压力性损伤的发生,为患者后期的康复奠定基础。  相似文献

目的:探讨北京市2型糖尿病(T2DM)患者抑郁症发病情况及其危险因素,为T2DM合并抑郁症的防治提供依据。方法:采取横断面调查的方法对2016年1月-2016年12月中国人民解放军总医院健康管理研究院体检中患T2DM的3000例患者随机抽取1200例进行调查,所有患者进行Zung抑郁自评量表(SDS)测评,根据SDS结果分为T2DM合并抑郁症组和T2DM非抑郁症组。应用单因素和多因素Logistic回归法分析T2DM合并抑郁症的危险因素。结果:北京市T2DM患者抑郁症患病率为21.93%,不同性别T2DM患者抑郁症患病率比较无统计学差异(P0.05)。单因素分析显示,北京市T2DM患者抑郁症发病与糖尿病病程、运动情况、胰岛素治疗、并发症情况、糖化血红蛋白有关(P0.05),与饮食控制、血糖监测情况、收缩压、舒张压、胆固醇、甘油三酯无关(P0.05)。多因素Logistic回归分析显示糖尿病病程≥10年、胰岛素治疗、并发症≥3个、糖化血红蛋白≥8.0%是北京市T2DM患者抑郁症发病的危险因素(P0.05),经常运动是北京市T2DM患者抑郁症发病的保护性因素(P0.05)。结论:北京市T2DM患者抑郁症患病率为21.93%,糖尿病病程≥10年、胰岛素治疗、并发症≥3个、糖化血红蛋白≥8.0%是北京市T2DM患者抑郁症发病的危险因素,经常运动是北京市T2DM患者抑郁症发病的保护性因素。  相似文献

目的:探讨2型糖尿病患者发生下肢血管病变的危险因素.方法:回顾性分析我院2009年12月~2011年12月收治的98例2型糖尿病患者的临床资料,通过彩色多普勒超声检查其下肢血管,按下肢血管有无病变将其分为单纯2型糖尿病租和2型糖尿病合并下肢血管病变组,通过非条件Logistic回归分析导致2型糖尿病患者发生下肢血管病变的危险因素.结果:彩色多普勒超声检查结果显示,98例2型糖尿病患者中,61例发生不同程度下肢血管病变,占62.24%.与单纯2型糖尿病组比较,下肢血管病变组患者年龄、病程、收缩压及低密度脂蛋白胆固醇的差异均有统计学意义(P<0.05),经非条件Logistic回归分析,患者的年龄、病程、SBP及LDL-L是下肢血管病变的独立危险因素(P<0.05).结论:2型糖尿病患者下肢血管病变的发生与患者的年龄、病程、血压及血脂情况密切相关.  相似文献

目的:探讨颅内动脉瘤开颅术后发生肺部感染的危险因素。方法:回顾性分析在我院接受开颅手术治疗的211例颅内动脉瘤患者的性别、年龄、吸烟史、糖尿病史、高血压病史、Hunt-Hess分级、动脉瘤部位、动脉瘤直径、手术时机及术后肺部感染的情况,对可能导致肺感染的因素行X2检验及Logistic回归分析。结果:单因素分析显示影响颅内动脉瘤患者术后肺感染的因素主要包括年龄、吸烟、糖尿病、Hunt-Hess分级(P0.05)。多因素Logistic回归分析结果显示影响颅内动脉瘤患者开颅术后发生肺部感染的因素为吸烟和Hunt-Hess分级。结论:吸烟、高Hunt-Hess分级是影响颅内动脉瘤开颅术后发生肺部感染的独立危险因素。  相似文献

目的:了解新生儿发生毛细血管渗漏综合征(CLS)的相关危险因素。方法:对620例新生儿进行回顾性分析,采用Logistic回归模型分析毛细血管渗漏综合症发生的危险因素。结果:620例新生儿中有168例诊断为CLS,单因素分析显示严重感染、呼吸窘迫综合征、早产、重度窒息、发病前低体温对新生儿CLS的发生有影响(P<0.05),经多元Logistic回归分析显示严重感染、呼吸窘迫综合征、早产是发生CLS的独立危险因素(P<0.05)。结论:严重感染、呼吸窘迫综合征、早产是发生CLS的独立危险因素。  相似文献

目的:探讨甲状腺乳头状癌发生颈部淋巴结转移的危险因素。方法:回顾性分析我院自2017年1月至2017年5月手术治疗甲状腺乳头状癌病人177例的临床资料,采用卡方检验、t检验及logistic回归分析甲状腺乳头状癌发生颈部淋巴结的危险因素。结果:甲状腺乳头状癌淋巴结转移率为45.8%,术后并发症发生率为6.2%,淋巴结转移组较未转移组年龄更小、癌结节更大(P0.05),两组比较性别无统计学差异(P0.05),0.5 cm癌结节≤1 cm组较癌结节≤0.5 cm组更易出现淋巴结转移(P0.05),年龄大于45岁、癌结节≤0.5 cm的患者,9.6%发生淋巴结转移。Logistic回归分析提示年龄、癌结节大小是淋巴结转移的独立危险因素(P0.05)。结论:年龄、癌结节大小是淋巴结转移的独立危险因素,年龄越小、癌结节越大的甲状腺乳头状癌患者,越容易出现颈部淋巴结转移。  相似文献

目的:探讨脑梗死患者颈动脉粥样硬化的危险因素。方法:选取148例脑梗死患者作为实验组和同期住院的80例非脑梗死疾病患者为对照组,检测并比较各组年龄、吸烟、血压、饮酒、糖尿病、血脂、同型半胱氨酸和磷脂酶A2等指标,然后进行卡方检验、相关性和Logistic回归分析。结果:实验组颈动脉粥样硬化检出率为82.43%,对照组为12.5%,其差异具有统计学意义(P0.05)。高龄、吸烟、高血压、高血脂、高同型半胱氨酸血症、高磷脂酶A2(LP-PLA2)血症是经动脉粥样硬化的危险因素,且血浆同型半胱氨酸水平、LP-PLA2水平、血压与颈动脉斑块稳定性有关(P0.05)。结论:颈动脉粥样硬化是脑梗死的病理基础,与高龄、吸烟、高血压、高血脂、高同型半胱氨酸血症、高磷脂酶A2(LP-PLA2)血症有关,对高同型半胱氨酸血症、LP-PLA2和高血压患者要关注其斑块的稳定性。  相似文献

目的:调查北京市通州区永顺社区老年人的慢性肾脏病(CKD)患病率,并分析其危险因素。方法:采用横断面调查方法,对该社区长期居住的65岁及以上的通州区户籍老年人进行问卷调查、肾脏损伤指标及其他实验室指标的检测,收集资料并分析CKD患病的危险因素。结果:本次调查共获得948例该社区常住老年人的完整资料,参与研究的老年人平均年龄为70.86±4.89岁;镜下血尿患病率为9.39%,白蛋白尿患病率为22.15%,5.70%的老年人出现肾功能下降,该人群中CKD患病率为36.81%(349/948)。单因素分析结果显示,两组吸烟史、饮酒史、血肌酐、体质量指数(BMI)、尿微量白蛋白肌酐比(ACR)、糖尿病、甘油三酯、总胆固醇及血尿酸水平对比无显著性差异(P0.05),而CKD组较非CKD组年龄更大、合并高血压的比例更高、女性占比更高(P0.05),多因素logistic回归分析显示年龄、女性与高血压是通州区永顺社区老年人CKD患病的危险因素(OR=1.432、1.163、1.335,P0.05)。结论:北京市通州区永顺社区老年人CKD患病率较高,其危险因素为年龄、女性与高血压。  相似文献

目的:了解油田工作人员高尿酸血症(HUA)患病率并分析其影响因素。方法:于2018年1月至2018年12月通过随机抽样法选取300例华北油田的工作人员。统计所有油田工作人员的HUA患病率,并根据HUA发生与否分成HUA组41例与非HUA组259例。采用单因素分析及多因素Logistic回归分析HUA的影响因素。结果:300例油田工作人员HUA患病率为13.67%(41/300),其中男性油田工作人员血清尿酸水平及HUA患病率均高于女性(均P0.05)。HUA组肥胖、饮酒、高血压、高血糖、高血脂症人数占比高于非HUA组(均P0.05)。HUA组尿素氮(BUN)、血肌酐(Scr)水平均高于非HUA组,而肾小球滤过率(eGFR)水平低于非HUA组(均P0.05)。多因素Logistic回归分析显示,男性、肥胖、饮酒、高血压、高血糖、高血脂症及BUN5.0 mmol/L、Scr130.0μmol/L、eGFR130.0 m L/(min·1.73m2)均是油田工作人员HUA的独立危险因素(均P0.05)。结论:油田工作人员HUA患病率较高,应予以重视。男性、肥胖、饮酒、高血压、高血糖、高血脂症、BUN、Scr水平较高以及eGFR较低的人群HUA发生的风险较高,可通过针对上述因素予以干预,继而降低HUA的发生风险。  相似文献

Peridomestic exposure to Borrelia burgdorferi-infected Ixodes scapularis nymphs is considered the dominant means of infection with black-legged tick-borne pathogens in the eastern United States. Population level studies have detected a positive association between the density of infected nymphs and Lyme disease incidence. At a finer spatial scale within endemic communities, studies have focused on individual level risk behaviors, without accounting for differences in peridomestic nymphal density. This study simultaneously assessed the influence of peridomestic tick exposure risk and human behavior risk factors for Lyme disease infection on Block Island, Rhode Island. Tick exposure risk on Block Island properties was estimated using remotely sensed landscape metrics that strongly correlated with tick density at the individual property level. Behavioral risk factors and Lyme disease serology were assessed using a longitudinal serosurvey study. Significant factors associated with Lyme disease positive serology included one or more self-reported previous Lyme disease episodes, wearing protective clothing during outdoor activities, the average number of hours spent daily in tick habitat, the subject’s age and the density of shrub edges on the subject’s property. The best fit multivariate model included previous Lyme diagnoses and age. The strength of this association with previous Lyme disease suggests that the same sector of the population tends to be repeatedly infected. The second best multivariate model included a combination of environmental and behavioral factors, namely hours spent in vegetation, subject’s age, shrub edge density (increase risk) and wearing protective clothing (decrease risk). Our findings highlight the importance of concurrent evaluation of both environmental and behavioral factors to design interventions to reduce the risk of tick-borne infections.  相似文献

It is the aim of the present study to assess factors associated with time spent in class among working college students. Eighty-two working students from 21 to 26 years old participated in this study. They were enrolled in an evening course of the University of São Paulo, Brazil. Participants answered a questionnaire on living and working conditions. During seven consecutive days, they wore an actigraph, filled out daily activity diaries (including time spent in classes) and the Karolinska Sleepiness Scale every three hours from waking until bedtime. Linear regression analyses were performed in order to assess the variables associated with time spent in classes. The results showed that gender, sleep length, excessive sleepiness, alcoholic beverage consumption (during workdays) and working hours were associated factors with time spent in class. Thus, those who spent less time in class were males, slept longer hours, reported excessive sleepiness on Saturdays, worked longer hours, and reported alcohol consumption. The combined effects of long work hours (>40 h/week) and reduced sleep length may affect lifestyles and academic performance. Future studies should aim to look at adverse health effects induced by reduced sleep duration, even among working students who spent more time attending evening classes.

Several attributes of the work schedule can increase the risk of occupational injuries and accidents, health impairments, and reduced social participation. Although previous studies mainly focused on the effects of shiftwork and long working hours on employee health and safety, there is little evidence of a potential negative impact of working Sundays on the incidence of occupational accidents, health impairments, and work-life balance. A representative sample of employed workers in 31 member and associated states of the European Union (n?=?23,934) served as the database for a cross-sectional analysis. The sample was collected via face-to-face interviews in the year 2005. The association of the risks of occupational accidents, health impairments, and decreases in work-life balance with working Sundays was calculated using logistic regression models, controlling for potential confounders, such as shiftwork, workload, and demographic characteristics. The results indicated that working one or more Sundays/month was associated with increase both in the risk of reporting one or more health impairments (odds ratio [OR]: 1.17, 95% confidence interval [CI]: 1.06-1.29) and poorer work-life balance (OR: 1.15, 95% CI: 1.02-1.28). These effects remained after controlling for potentially confounding factors, such as other work schedule attributes, intensity of physical and mental workload, and individual characteristics. Furthermore, working Sundays was also related to increased risk of occupational accidents within the last year (OR: 1.34, 95% CI: 1.03-1.73). Controlling again for individual, workload, and working-time characteristics, a significant association with accident risk, however, remained only in work sectors with low a priori risk of occupational accidents (OR: 1.40, 95% CI: 1.02-1.91), although the increased risk could be observed for both medium and high a priori risk sectors working Sundays (without controlling for additional confounders). The results thus indicate that the detrimental effects of working Sundays on safety, health, and social well-being should be taken into account when designing work schedules. The potential hazards to employees' safety, health, and work-life balance, in particular, should be considered in discussions concerning extending work on Sundays in certain sectors, e.g., retail.  相似文献

OBJECTIVE--To assess whether the threat of unemployment affects risk factors for cardiovascular disease. DESIGN--Longitudinal study of a cohort of middle aged shipyard workers followed up for a mean of 6.2 (SD 1.9) years and a group of controls observed for the same period. The first investigation took place during a period of relative economic stability for the shipyard and the second during the phase of its closure. SETTING--An age cohort health screening programme in Malmö, Sweden. PARTICIPANTS--715 Male shipyard workers and 261 age matched male controls. MAIN OUTCOME MEASURES--Changes in 19 variables related to the risk of cardiovascular disease, and psychological variables, alcohol consumption, smoking, and dietary habits as assessed by questionnaire. RESULTS--Serum cholesterol concentrations increased more (mean 0.25 (SD 0.68) mmol/l v 0.08 (0.66) mmol/l) and serum calcium concentrations decreased less (-0.06 (0.10) mmol/l v -0.08 (0.09) mmol/l) in the shipyard workers than in the controls. A correlation was found between scores for sleep disturbance and changes in serum cholesterol concentration. In the whole series there was a greater increase in serum cholesterol concentrations among men threatened with unemployment (437/976; 44.8%) than among those who were not. In stepwise regression analysis the change in serum cholesterol concentration was correlated with changes in haemoglobin concentration, body weight, and serum triglyceride and calcium concentrations. A positive correlation was found between change in cholesterol concentration and change in blood pressure, indicating that the overall risk profile had worsened among men with increased serum cholesterol concentrations. CONCLUSIONS--Risk of unemployment increases the serum cholesterol concentration in middle aged men, the increase being more pronounced in those with sleep disturbance. The increase in serum cholesterol is related to changes in other established risk factors for cardiovascular disease. These findings might partly explain the excessive mortality due to cardiovascular disease recorded among the unemployed and people with sleep disturbance.  相似文献

There is considerable evidence showing that night work is associated with increased morbidity, but only a few studies have focused on its relation to mortality. This study investigates the relationship between the type of working-time arrangement (weekly night work/daytime work) and total and cause-specific mortality among men and women. The data consist of a representative working conditions survey of Finnish employees conducted in 1984 (2286 men/2216 women), which has been combined with register-based follow-up data from Statistics Finland covering the years 1985-2008. In the 1984 survey, the employees were asked if they worked during the night (23:00-06:00 h) and if so, how often. In this study, the authors compare employees who worked at night (121 men/89 women) to daytime employees who did not do night work (1325 men/1560 women). The relative risk of death was examined by Cox proportional hazards analyses adjusted for background (age, level of education, family situation, and county), health (longstanding illness, pain symptoms, smoking status, and psychological symptoms), and work-related factors (weekly working hours, physical and psychological demands, demands of learning at work, and perceived job insecurity). Female employees working at night had a 2.25-fold higher risk of mortality than female dayworkers (95% confidence interval [CI] 1.20-4.20) after adjustment for background and health- and work-related factors. In addition to total mortality, night work was also associated with tumor mortality. Female night workers had a 2.82-fold higher risk of tumor mortality than female dayworkers (95% CI 1.20-6.65) in the adjusted model. Among men, no such significant association was observed. The present study indicated that female night workers had a higher risk of both total and tumor mortality compared to female daytime employees. Additional research on the potential factors and mechanisms behind the association between night work and mortality is required.  相似文献

Cardiovascular disease is the leading cause of morbidity and mortality in the industrialized world. Familial aggregation of cardiovascular risk factors is a frequent finding, but genetic factors affecting its presentation are still poorly understood. The calpain 10 gene (CAPN10) has been associated with type 2 diabetes (T2DM), a complex metabolic disorder with increased risk of cardiovascular disease. Moreover, the CAPN10 gene has been associated with the presence of metabolic syndrome (MS) in T2DM and in polycystic ovary syndrome (PCOS). In this work, we have analysed whether the polymorphisms UCSNP44, -43, -19 and -63 are related to several cardiovascular risk factors in the context of MS. Molecular analysis of CAPN10 gene was performed in 899 individuals randomly chosen from a cross-sectional population-based epidemiological survey. We have found that CAPN10 gene in our population is mainly associated with two indicators of the presence of insulin resistance: glucose levels two hours after a 75-g oral glucose tolerance test (OGTT) and HOMA values, although cholesterol levels and blood pressure values are also influenced by CAPN10 variants. In addition, the 1221/1121 haplogenotype is under-represented in individuals that fulfil the International Diabetes Federation (IDF) diagnostic criteria for MS. Our results suggest that CAPN10 gene is associated with insulin resistance phenotypes in the Spanish population.  相似文献

OBJECTIVE: Our purpose was to assess the extent to which early weaning and other weaning-management factors affect development of postweaning chronic diarrhea in captive rhesus monkeys at the California Regional Primate Research Center between 1992 and 1995. METHODS: Data for weaning, management, and onset of diarrhea were obtained from daily records. The Cox proportional hazard model was used to assess whether the risk of chronic diarrhea was related to early weaning. RESULTS: Monkeys that were lighter at weaning had a threefold increase in risk of postweaning chronic diarrhea (P = 0.07), compared with that in heavier monkeys. An episode of preweaning diarrhea increased the risk of postweaning chronic diarrhea twofold (P = 0.08). Relocation of monkeys to outdoor facilities in the fall was associated with a fivefold decrease in risk (P < 0.001), compared with that of other seasons, and weaning in 1993 was associated with a twofold decrease in risk, compared with that of other years (P = 0.04). CONCLUSIONS: Multiple factors need to be considered for prevention of postweaning chronic diarrhea, including weaning weight, preweaning diarrhea, season weaned, and weaning conditions that change from year to year.  相似文献

Obesity is associated with lower levels of serum 25-hydroxyvitamin D (25(OH)D). Obese individuals might need higher doses of vitamin D supplementation than the general population. In this cross-sectional study, associations between 25(OH)D serum levels, body mass index (BMI), and outdoor exercise were assessed in a population of 291 ambulatory patients attending the Osteoporosis Center at the University of Miami, mean age 62 ± 13.48 years. Obesity was defined as BMI ≥ 30 kg/m2 and hypovitaminosis D as 25(OH)D ≤ 30 ng/ml. Overall, prevalence of obesity was 14.1% and of hypovitaminosis D was 42.4%. Among Hispanics, there was a significantly higher prevalence of hypovitaminosis D in obese (63.2%) compared to non-obese individuals (35.8%). Outdoor exercise had a significant effect on the prevalence of hypovitaminosis D in Hispanics, with a lower prevalence in those performing outdoor exercise (24.1%) compared to those who did not (47.9%). After adjusting for age, gender, and ethnicity, those reporting outdoor exercise were 47% less likely to have hypovitaminosis D, while those with obesity had more than twice the risk. Since outdoor exercise may protect overweight individuals from hypovitaminosis D, prevention programs involving higher doses of vitamin D and/or outdoor exercise may result in additional metabolic and functional benefits in this population.  相似文献

ABSTRACT

There is no study on the relationship between working hours and nonalcoholic fatty liver disease (NAFLD). The objective of the present study was to determine the relationship between working hours and NAFLD by sleep duration using a large set of abdominal ultrasonography examination data. Data from 194,625 patients who underwent health examinations from 2015 to 2017 were analyzed. Chi-square tests, linear-by-linear association and ANOVA were performed to compare general characteristics according to working hours. Multivariate logistic regression analysis was performed to determine the relationship between working hours and NAFLD by sleep duration. There was no significant relationship between working hours and NAFLD prevalence in the group of short sleep duration of ≤5 hours or the group of long sleep duration of ≥7 hours. The risk of NAFLD in the >52 working hour group was significantly higher (aOR, 1.09; 95% CI, 1.04–1.14) than that in the 40- to 52-hour working hour group after adjusting for confounding factors in the 5- to 6-hour sleep duration group. There was no significant difference between ≤40 working hours and 40 ~ 52 working hours in the 5 ~ 6 hours sleep duration group (aOR, 1.02; 95% CI, 0.97–1.06). In general, working hours were significantly related to NAFLD. There was a difference in the relationship between working hours and NAFLD according to sleep duration.  相似文献

AIMS: The FINDRISC questionnaire is a screening tool to estimate the risks for type 2 diabetes as well as asymptomatic type 2 diabetes. We aimed to evaluate its performance to predict diabetes in a German population and to compare its predictive and detective ability in the same population. METHODS: A total of 552 subjects with increased risk of type 2 diabetes were investigated. All individuals completed the FINDRISC questionnaires and underwent an oral glucose tolerance test (OGTT). All individuals were followed for 3 years and underwent an OGTT again. The performance of the opportunistic screening was assessed with the area under the receiver operating characteristics curve (AUC). An intervention program was carried out for all diabetic and IFG/IGT patients at baseline. RESULTS: For identification, the asymptomatic type 2 DM was named Condition 1; prediction of type 2 DM risk in the follow-up survey as Condition 2; and diabetes risk predicting in a hypothetical case of survey without intervention program as Condition 3. The ROC-AUC in the three condition were AUC (FINDRISC1)=0.745, AUC (FINDRISC2)=0.789, and AUC (FINDRISC3)=0.775, respectively. A significant association between FINDRISC and evolution of disease was found, but the variation of plasma glucose during the three years follow-up was not associated with FINDRISC. People in the intervention group with an improvement of glucose tolerance had a smaller FINDRISC score than persons with an unchanged or progressive condition of disease. CONCLUSION: FINDRISC was validated in our study as a simple tool with high performance to predict diabetes risk and less efficient to identify asymptomatic type 2 diabetes. People with lower FINDRISC score will benefit easier from preventive intervention.  相似文献
