Objective: To examine the relationship between BMI and patient satisfaction with health care providers using a nationally representative survey. Research Methods and Procedures: This analysis examined the 9914 adult patients who completed the 2000 Medical Expenditure Panel Survey and had visited a health care provider within 12 months of the survey. Linear regression models were employed with patient satisfaction as the dependent variable. The patient satisfaction scale was based on ratings from five questions assessing the quality of provider interactions. The independent variable was BMI, with adjustments for the domains of demographics, social‐economic status, health attitudes and behavior, health status, and health care use. BMI (weight in kilograms/square of height in meters) was classified as normal weight (18.5 to 24.9), overweight (25.0 to 29.9), or obese (≥30.0). Hierarchical models were used to evaluate how each domain modified the BMI‐satisfaction association. Results: Obese patients reported significantly greater satisfaction with their health care providers than their normal‐weight counterparts did (p < 0.05). There were no significant differences in satisfaction between normal‐weight and overweight patients or between overweight and obese patients. The health status domain produced the largest modification in the BMI‐satisfaction relationship. Examination of interaction effects revealed that the association between BMI and satisfaction was confined to older persons. Discussion: In this nationally representative sample of individuals, obese persons were more satisfied than their normal‐weight counterparts. This finding counters those of previous studies. Incomplete adjustments for health care use and insurance status may have led to those conclusions.  相似文献

Hypertension is a multi-factorial process, prevalent in developed as well as in developing countries. Different antioxidants and free radicals play an important role in cardiovascular system. In present study, total antioxidant power in terms of FRAP (ferric reducing activity of plasma), free radicals and different antioxidants have been studied in essential hypertensives (n = 50) and normal subjects (n = 50). Levels of total cholesterol, low-density lipids-cholesterol, malonialdehyde, very low-density lipids (VLDL), uric acid, plasma homocysteine and low-density lipids (LDL), were significantly higher in hypertensives as compared to normotensive. HDL-cholesterol, SOD, GPx, reduced glutahione, total glutathione, oxidized glutathione, total thiols, protein thiols, non protein thiols, RNI, total antioxidant power, vitamin A, ascorbic acid and glutahione-S-transferase (GST) were decreased significantly in normotensive. We observed significantly low nitric oxide levels in hypertensive patients. No correlation was observed between severity of disease and plasma nitric oxide levels. There was a significant decrease in plasma FRAP value in essential hypertensives as compared to normotensive controls, which showed a negative correlation with diastolic blood pressure. In conclusion, our study revealed that there was a consistent significant difference between essential hypertensives versus controls with respect to most of the parameters. These complex changes are consistent in the view that essential hypertension is associated with an abnormal level of antioxidant status compared to normal response to oxidative stress or both.  相似文献

Objective: To provide evidence‐based guidelines on the psychological and behavioral screening of weight loss surgery (WLS) candidates and the impact of psychosocial factors on behavior change after gastric bypass surgery. Research Methods and Procedures: The members of the Behavioral and Psychological subgroup of the Multidisciplinary Care Task Group conducted searches of MEDLINE and PubMed for articles related to WLS, behavior changes, and mental health, including quality of life (QOL) and behavior modification. Pertinent abstracts and literature were reviewed for references. A total of 198 abstracts were identified; 17 papers were reviewed in detail. Search periods were from 1980 to 2004. Results: We found a high incidence of depression, negative body image, eating disorders, and low QOL in severely obese patients. Our task subgroup recommended that all WLS candidates be evaluated by a licensed mental health care provider (i.e., psychiatrist, psychologist, or social worker), experienced in the treatment of severely obese patients and working within the context of a multidisciplinary care team. We also recommended development of pre‐ and postsurgical treatment plans that address psychosocial contraindications for WLS and potential barriers to postoperative success. Discussion: The psychological consequences of obesity can range from lowered self‐esteem to clinical depression. Rates of anxiety and depression are three to four times higher among obese individuals than among their leaner peers. A comprehensive multidisciplinary program that incorporates psychological and behavior change services can be of critical benefit in enhancing compliance, outcome, and QOL in WLS patients.  相似文献

Species distribution models (SDMs) in river ecosystems can incorporate climate information by using air temperature and precipitation as surrogate measures of instream conditions or by using independent models of water temperature and hydrology to link climate to instream habitat. The latter approach is preferable but constrained by the logistical burden of developing water temperature and hydrology models. We therefore assessed whether regional scale, freshwater SDM predictions are fundamentally different when climate data versus instream temperature and hydrology are used as covariates. Maximum entropy (MaxEnt) SDMs were built for 15 freshwater fishes using one of two covariate sets: 1) air temperature and precipitation (climate variables) in combination with physical habitat variables; or 2) water temperature, hydrology (instream variables) and physical habitat. Three procedures were then used to compare results from climate vs instream models. First, equivalence tests assessed average pairwise differences (site‐specific comparisons throughout each species’ range) among climate and instream models. Second, ‘congruence’ tests determined how often the same stream segments were assigned high habitat suitability by climate and instream models. Third, Schoener's D and Warren's I niche overlap statistics quantified range‐wide similarity in predicted habitat suitability from climate vs instream models. Equivalence tests revealed small, pairwise differences in habitat suitability between climate and instream models (mean pairwise differences in MaxEnt raw scores for all species < 3 × 10–4). Congruence tests showed a strong tendency for climate and instream models to predict high habitat suitability at the same stream segments (median congruence = 68%). D and I statistics reflected a high margin of overlap among climate and instream models (median D = 0.78, median I = 0.96). Overall, we found little support for the hypothesis that SDM predictions are fundamentally different when climate versus instream covariates are used to model fish species’ distributions at the scale of the Columbia Basin.  相似文献

Comparative phylogeographic investigations have identified congruent phylogeographic breaks in co‐distributed species in nearly every region of the world. The qualitative assessments of phylogeographic patterns traditionally used to identify such breaks, however, are limited because they rely on identifying monophyletic groups across species and do not account for coalescent stochasticity. Only long‐standing phylogeographic breaks are likely to be obvious; many species could have had a concerted response to more recent landscape events, yet possess subtle signs of phylogeographic congruence because ancestral polymorphism has not completely sorted. Here, we introduce Phylogeographic Concordance Factors (PCFs), a novel method for quantifying phylogeographic congruence across species. We apply this method to the Sarracenia alata pitcher plant system, a carnivorous plant with a diverse array of commensal organisms. We explore whether a group of ecologically associated arthropods have co‐diversified with the host pitcher plant, and identify if there is a positive correlation between ecological interaction and PCFs. Results demonstrate that multiple arthropods share congruent phylogeographic breaks with S. alata, and provide evidence that the level of ecological association can be used to predict the degree of similarity in the phylogeographic pattern. This study outlines an approach for quantifying phylogeographic congruence, a central concept in biogeographic research.  相似文献

[Purpose] This study aimed to investigate the effect of education based on the health belief model on the physical activity of the staff of the University of Medical Sciences.[Methods] This semi-experimental study was conducted on 130 university staff aged 25-50 years from the Hamadan University of Medical Sciences. Inclusion criteria were having at least 1 year of work experience, lack of acute and chronic physical and mental illnesses, and not using drugs that affect physical activity. The samples were randomly divided into two groups. The experimental group received three training sessions based on the health belief model. Before and 2 months after training, the control and experimental groups were evaluated via the following questionnaires: (1) demographic information questionnaire, (2) Health Belief Model Questionnaire, and (3) International Physical Activity Questionnaire. Finally, data were analyzed statistically.[Results] The training process resulted in a significant increase in the mean scores of the health belief model constructs in the experimental group, but changes in the control group were not significant. Self-efficacy was the strongest predictor of physical activity.[Conclusion] The health belief model is a useful model for improving individuals'' understanding of the benefits of physical activity.  相似文献

Summary. Earth mounds called ‘murundus’ are conspicuous features in the semi-arid region of Brazil, where their distribution and origin remain largely unknown. We adopted an approach based on spatially explicit mapping and modeling to determine the current and potential geographical distribution of murundus. We set up a systematic grid survey to map their current distribution using high-resolution satellite imagery available via free open source software. To obtain insights into the origin of these earth mounds, predictive models were used to test their geographical congruence with six mound-building termites (genera Cornitermes Wasmann, 1897, and Syntermes Holmgren, 1909). The Maxent algorithm was used to generate the potential distribution models based on a set of five variables related to primary environmental regimes that determine the fundamental niche of termite species. To analyze if there was some congruence between associated environmental variables, we cross-checked the results obtained from the statistical tests using MAXENT software. Murundus mostly occur in the south-central region of the Caatinga biome (i.e. seasonally dry tropical forest) in areas with moderate annual mean temperature (20–24°C) and precipitation (600–910 mm), and soils with moderate to severe constraints concerning nutrient availability. We observed a strong geographical congruence between the potential distributions of densely packed murundus and four mound-building termites [i.e. Cornitermes bequaerti Emerson, 1952, C. silvestrii Emerson, 1949, Syntermes dirus (Burmeister, 1839) and S. wheeleri (Emerson, 1925)]. Based on the significant association of those distributions with the climate and soil nutrient availability, we concluded that the formation of murundus in the semi-arid domain might be an adaptive response of mound-building termites to particular regional environmental conditions.  相似文献

Biodiversity reduction and loss continues to progress at an alarming rate, and thus, there is widespread interest in utilizing rapid and efficient methods for quantifying and delimiting taxonomic diversity. Single‐locus species delimitation methods have become popular, in part due to the adoption of the DNA barcoding paradigm. These techniques can be broadly classified into tree‐based and distance‐based methods depending on whether species are delimited based on a constructed genealogy. Although the relative performance of these methods has been tested repeatedly with simulations, additional studies are needed to assess congruence with empirical data. We compiled a large data set of mitochondrial ND4 sequences from horned lizards (Phrynosoma) to elucidate congruence using four tree‐based (single‐threshold GMYC, multiple‐threshold GMYC, bPTP, mPTP) and one distance‐based (ABGD) species delimitation models. We were particularly interested in cases with highly uneven sampling and/or large differences in intraspecific diversity. Results showed a high degree of discordance among methods, with multiple‐threshold GMYC and bPTP suggesting an unrealistically high number of species (29 and 26 species within the P. douglasii complex alone). The single‐threshold GMYC model was the most conservative, likely a result of difficulty in locating the inflection point in the genealogies. mPTP and ABGD appeared to be the most stable across sampling regimes and suggested the presence of additional cryptic species that warrant further investigation. These results suggest that the mPTP model may be preferable in empirical data sets with highly uneven sampling or large differences in effective population sizes of species.  相似文献

Seven tetraploid species ofElymus, viz.E. sibiricus, E. caninus, E. gmelinii, E. semicostatus, E. caucasicus, E. parviglume, andE. longearistatus subsp.canaliculatus, representing five sections were studied morphologically and used in interspecific hybridizations. The aim was to investigate whether the present sectional delimitation of the genus was in agreement with genomic data and if there was a correlation between genome constitution and morphology. The study revealed: (i) further information on the genomic affinities between the different species, (ii) that there is no congruence between genome constitution of the species and current sectional delimitation, and (iii) that there is a correlation between genome constitution and morphology in the palea apex shape and in the size of cilia of the palea.  相似文献

Coevolutionary processes that drive the patterns of host–parasite associations can be deduced through congruence analysis of their phylogenies. Feather lice and their avian hosts have previously been used as typical model systems for congruence analysis; however, such analyses are strongly biased toward nonpasserine hosts in the temperate zone. Further, in the Afrotropical region especially, cospeciation studies of lice and birds are entirely missing. This work supplements knowledge of host–parasite associations in lice using cospeciation analysis of feather lice (genus Myrsidea and the Brueelia complex) and their avian hosts in the tropical rainforests of Cameroon. Our analysis revealed a limited number of cospeciation events in both parasite groups. The parasite–host associations in both louse groups were predominantly shaped by host switching. Despite a general dissimilarity in phylogeny for the parasites and hosts, we found significant congruence in host–parasite distance matrices, mainly driven by associations between Brueelia lice and passerine species of the Waxbill (Estrildidae) family, and Myrsidea lice and their Bulbul (Pycnonotidae) host species. As such, our study supports the importance of complex biotic interactions in tropical environments.  相似文献

For a neuron, firing activity can be in synchrony with that of others, which results in spatial correlation; on the other hand, spike events within each individual spike train may also correlate with each other, which results in temporal correlation. In order to investigate the relationship between these two phenomena, population neurons’ activities of frog retinal ganglion cells in response to binary pseudo-random checker-board flickering were recorded via a multi-electrode recording system. The spatial correlation index (SCI) and temporal correlation index (TCI) were calculated for the investigated neurons. Statistical results showed that, for a single neuron, the SCI and TCI values were highly related—a neuron with a high SCI value generally had a high TCI value, and these two indices were both associated with burst activities in spike train of the investigated neuron. These results may suggest that spatial and temporal correlations of single neuron’s spiking activities could be mutually modulated; and that burst activities could play a role in the modulation. We also applied models to test the contribution of spatial and temporal correlations for visual information processing. We show that a model considering spatial and temporal correlations could predict spikes more accurately than a model does not include any correlation.  相似文献

The aim of this investigation is to compare different mathematical models of the liver in the context of in vitro-in vivo correlation. We reanalyze drugs from the Houston reviews [1, 2], and compare the mathematical models. For the well-stirred model, a particular form of the distributed tubes model, and the dispersion model, fits are done to in vitro and in vivo intrinsic clearance data from microsomal and hepatocyte experiments. The distributed and dispersion models have decreased residuals as compared to the well-stirred model, but neither is to be clearly preferred over theother. It seems likely that drug-specific factors have a major impact on the quality of IVIVC correlations. While new experiments are needed to validate IVIVC models, our results indicate that improved correlation of in vitroand in vivo data is possible for high clearance drugs by using either a dispersion or distributed tube model rather than a well-stirred model.  相似文献

Three EST-derived microsatellite loci from Vitis vinifera were amplified and sequenced across eight species of Vitaceae from four different genera. Phylogenetic analysis of the microsatellite’s flanking regions produced informative results in congruence with previous studies. Generic relationships were respected and the data produced sufficient inter-specific variation to distinguish between Cayratia acris and Cayratia saponaria, two very closely related species. Overall, the sequence alignments showed that priming sites were conserved, whereas microsatellite repeats were present in most cases but structurally variable. The sequence data provided information on the evolutionary patterns of various microsatellite repeats and their correlation to evolutionary relationships among taxa. The stability of complex patterns of geographic variation was investigated by assessing the congruence between multivariate ordinations derived from randomly chosen real characters. Two series of populations were analysed representing two situations with complex patterns of geographic variation. The first, a ‘Eurasian’ series of populations, showed a strongly structured hierarchical pattern, the second, an ‘eastern’ series of populations, showed a more subtle complex pattern of smooth clines and steps. The characters were selected from a total of 81 (Eurasian) or 61 (eastern) within-population independent characters from six different systems. The congruence between ordinations of the geographical populations was measured by the rotational fit statistic, R2. Three procedures were used to compare ordinations based on from two to up to 80 characters randomly chosen to give: A, completely independent character sets; B, subsets compared to the total set; and C, potentially overlapping sets. All three procedures showed that congruence between the ordinations was asymptotic in relation to character number. This relationship was described by one of two mathematical models (procedure B did not result in a hyperbolic model as found with simple patterns of geographic variation). Generally speaking, once a sufficient number of characters are used, the complex patterns of geographic variation are stable, reliable and predictive and not substantially influenced by character choice. The strongly structured hierarchical pattern required 15 or so characters to achieve reliability whilst the more subtle patterns required 20 or so characters. However, the addition of further characters does improve reliability in both cases. The greater percentage of variance portrayed by three-dimensional ordinations compared to two-dimensional ordinations is achieved at the cost of lower congruence when a sufficient number of characters are used. If case studies of geographic variation were to adopt these procedures (preferably using completely independent character sets; procedure A) the reliability of their results would be indicated).  相似文献

Summary A surrogate marker (S) is a variable that can be measured earlier and often more easily than the true endpoint (T) in a clinical trial. Most previous research has been devoted to developing surrogacy measures to quantify how well S can replace T or examining the use of S in predicting the effect of a treatment (Z). However, the research often requires one to fit models for the distribution of T given S and Z. It is well known that such models do not have causal interpretations because the models condition on a postrandomization variable S. In this article, we directly model the relationship among T, S, and Z using a potential outcomes framework introduced by Frangakis and Rubin (2002, Biometrics 58 , 21–29). We propose a Bayesian estimation method to evaluate the causal probabilities associated with the cross‐classification of the potential outcomes of S and T when S and T are both binary. We use a log‐linear model to directly model the association between the potential outcomes of S and T through the odds ratios. The quantities derived from this approach always have causal interpretations. However, this causal model is not identifiable from the data without additional assumptions. To reduce the nonidentifiability problem and increase the precision of statistical inferences, we assume monotonicity and incorporate prior belief that is plausible in the surrogate context by using prior distributions. We also explore the relationship among the surrogacy measures based on traditional models and this counterfactual model. The method is applied to the data from a glaucoma treatment study.  相似文献

Much of the variation in individual growth rates can be attributed to differences in individual feeding rates. Therefore, in order to build predictive models of individual, or population growth, the factors influencing an individual's feeding rate must be described. An important determinant of the feeding rate is the relationship between the local abundance of food and the individual's ingestion rate – otherwise known as the functional response. We determined functional responses for two species of invertebrate grazers: the snail Potamopyrgus antipodarum and the mayfly Deleatidium sp., by measuring their assimilation rate with increasing densities of radiolabelled periphyton. The assimilation rates were consistent with the Holling Type II or Michelis Menten functional response curve. The parameters of the functional response yielded estimates of the search area and handling time for the stream invertebrates. Our functional response data indicate that the half-saturation food density for P. antipodarum and Deleatidium sp. were 980 mg and 3200 mg AFDM m–2, respectively, suggesting that Deleatidium growth may be subject to food limitation more often than is P. antipodarum – despite the lower assimilation efficiency of the latter species.  相似文献

为了寻求有效的草地地上生物量估测方法和精确估测黄土高原典型草原草原地上生物量。于2014年8月中旬,在黄土高原典型草原草原地上生物量达到最大值,分别从单株水平和种群水平进行野外调查。以株高(H)和盖度(C)的复合因子(C×H)为自变量,通过回归分析,建立地上生物量估测模型,采用留一法对其精确性进行评估;并通过校正系数以及群落总生物量估测值和实测值比较单株水平和种群水平所建模型的精确性。结果表明:黄土高原典型草原草地,无论在单株水平还是种群水平,线性和幂函数对该区域生物量的拟合效果更好。估测模型检验结果表明,在单株水平各个物种的生物量估测值与实测值相关性较好,均达到了显著水平(P0.05),其r值均大于0.6,总相对误差RS均小于10%,平均相对误差绝对值RMA(average absolute value of relative error)均小于30%,总生物量的实测值与估测值比较接近,校正系数均接近1;而在种群水平上,虽然各物种的生物量估测值与实测值相关性均达到了显著水平(P0.05),但多数物种平均相对误差绝对值RMA大于30%,总相对误差RS(total relative error)均大于10%,总生物量的估测值均大于实测值,校正系数均偏离了1,说明在黄土高原典型草原通过单株水平建立的物种生物量估测模型的精度优于种群水平建立的物种生物量估测模型的精度。  相似文献

Individual illness belief systems form a cognitive structure which lies beneath the cultural and social aspect of health care in a community. Popular belief systems are different from, yet linked to, expert belief systems. Popular illness terms often help support a stable cultural milieu by linking concepts of causes and significance of types of illness problems with a set of health care seeking choices; as well as linking typical physical and psychological symptoms with associated social problems. This study presents an example of how illness beliefs perform these functions in urban, mainstream America. One hundred and seventeen people with biomedically defined hypertension were interviewed following the Explanatory Models format. The belief held by 72% of this sample was that they had Hyper-Tension, a physical illness characterized by excessive nervousness caused by untoward social stress. The data are used to derive a composite diagram of the cognitive domain of Hyper-Tension in America which demonstrates the various options people have for interpreting their experiences and choosing appropriate therapeutic actions. They use this illness belief system to justify otherwise unwarranted social behavior and to assume various aspects of the sick role. This popular model is compared and contrasted with the professional model of the disease hypertension and with other less frequent models which were observed in this sample.University of Washington  相似文献
