目的 应用统计过程控制理论分析指导医院服务质量持续改进。方法 联合使用多个控制图方法对医院服务质量评价指标的变化趋势进行监控,及时发现过程中的异常波动,制定针对性措施实施过程改进。结果 找出了影响医院服务质量数据波动的主要原因,制定改进措施,取得了良好的管理效果。结论 控制图可以准确全面反映医院服务质量的总体情况和过程控制情况,根据数据异常分布状态找到潜在的影响服务质量的原因并进行持续改进,是有效的过程管理工具。  相似文献

ObjectivesTo assess the growth of quality of life measures and to examine the availability of measures across specialties.DesignSystematic searches of electronic databases to identify developmental and evaluative work relating to health outcome measures assessed by patients.Results3921 reports that described the development and evaluation of patient assessed measures met the inclusion criteria. Of those that were classifiable, 1819 (46%) were disease or population specific, 865 (22%) were generic, 690 (18%) were dimension specific, 409 (10%) were utility, and 62 (1%) were individualised measures. During 1990-9 the number of new reports of development and evaluation rose from 144 to 650 per year. Reports of disease specific measures rose exponentially. Over 30% of evaluations were in cancer, rheumatology and musculoskeletal disorders, and older people''s health. The generic measures—SF-36, sickness impact profile, and Nottingham health profile—accounted for 612 (16%) reports.ConclusionsIn some specialties there are numerous measures of quality of life and little standardisation. Primary research through the concurrent evaluation of measures and secondary research through structured reviews of measures are prerequisites for standardisation. Recommendations for the selection of patient assessed measures of health outcome are needed.

ObjectivesTo calculate socioeconomic and health status measures for the primary care groups in London and to examine the association between these measures and hospital admission rates.DesignCross sectional study.Setting66 primary care groups in London, total list size 8.0 million people.ResultsStandardised hospital admission ratios varied from 74 to 116 for total admissions and from 50 to 124 for emergency admissions. Directly standardised admission rates for asthma varied from 152 to 801 per 100 000 (mean 364) and for diabetes from 235 to 1034 per 100 000 (mean 538). There were large differences in the mortality, socioeconomic, and general practice characteristics of the primary care groups. Hospital admission rates were significantly correlated with many of the measures of chronic illness and deprivation. The strongest correlations were with disability living allowance (R=0.64 for total admissions and R=0.62 for emergency admissions, P<0.0001). Practice characteristics were less strongly associated with hospital admission rates.ConclusionsIt is feasible to produce a range of socioeconomic, health status, and practice measures for primary care groups for use in needs assessment and in planning and monitoring health services. These measures show that primary care groups have highly variable patient and practice characteristics and that hospital admission rates are associated with chronic illness and deprivation. These variations will need to be taken into account when assessing performance.  相似文献

Abstract

Background: The purpose of the review is to summarize the literature surrounding the use of muscle vibration as it relates to modifying human gait.

Methods: After a brief introduction concerning historical uses and early research identifying the effect of vibration on muscle activation, we reviewed 32 articles that used muscle vibration during walking. The review is structured to address the literature within four broad categories: the effect of vibration to ‘trigger’ gait-like lower limb motions, the effect of vibration on gait control of healthy individuals and individuals with clinical conditions in which gait disorders are a prominent feature, and the effect of vibration training protocols on gait.

Results: The acute effects of vibration during gait involving healthy participants is varied. Some authors reported differences in segmental kinematic and spatiotemporal measures while other authors reported no differences in these outcome measures. The literature involving participants with clinical conditions revealed that vibration consistently had a significant impact on gait, suggesting vibration may be an effective rehabilitation tool. All of the studies that used vibration therapy over time reported significant improvement in gait performance.

Conclusions: This review highlights the difficulties in drawing definitive conclusions as to the impact of vibration on gait control, partly because of differences in walking protocols, site of vibration application, and outcome measures used across different investigative teams. It is suggested that the development of common investigative methodologies and outcome measures would accelerate the identification of techniques that may provide optimal rehabilitation protocols for individuals experiencing disordered gait control.  相似文献

IntroductionThe purpose of this study was to examine the relative and absolute between-day reliability of the motor unit number index (MUNIX).MethodsYoung, healthy adults (n = 19) attended two testing sessions separated by 4-weeks where their maximal pinch-grip strength, MUNIX, and motor unit size index (MUSIX) were assessed in the abductor pollicis brevis muscle. Reliability was assessed by intraclass correlation coefficients (ICC), coefficient of variation (CV) and limits of agreement (LOA).ResultsNo mean differences were observed for MUNIX or MUSIX. The CV for the MUNIX and MUSIX measures were between 13.5% and 17.5%. The ICC for both measures were moderate to moderately-high (0.73–0.76), The LOA for both indicated a homoscedastic relationship.DiscussionOur findings indicate moderate to moderately-high reliability for both MUNIX and MUSIX. Future work is needed to ensure both measures are reliable in other muscles and cohorts, and further investigations are required to examine the validity of MUNIX.  相似文献

ObjectiveTo assess the use of Global Positioning System receiver (GPS) derived performance measures for differentiating between: 1) different outdoor activities in healthy dogs; 2) healthy dogs and those with osteoarthritis; 3) osteoarthritic dogs before and after treatment with non-steroidal anti-inflammatory analgesia.DesignProspective study.AnimalsTen healthy dogs and seven dogs with osteoarthritis of the elbow joint (OA dogs).ProcedureHealthy dogs were walked on a standard route on-lead, off-lead and subjected to playing activity (chasing a ball) whilst wearing a GPS collar. Each dog was walked for five consecutive days. Dogs with OA were subjected to a single off-lead walk whilst wearing a GPS collar, and then administered oral Carprofen analgesia daily for two weeks. OA dogs were then subjected to the same walk, again wearing a GPS collar.ResultsGPS derived measures of physical performance could differentiate between on-lead activity, off-lead activity and playing activity in healthy dogs, and between healthy dogs and OA dogs. Variation in the performance measures analysed was greater between individual dogs than for individual dogs on different days. Performance measures could differentiate healthy dogs from OA dogs. OA Dogs treated with Carprofen analgesia showed improvements in their physical performance, which returned to values indistinguishable from those of healthy dogs on nearly all the measures assessed.

Conclusions and Clinical Relevance

GPS derived measures of physical performance in dogs are objective, easy to quantify, and can be used to gauge the effects of disease and success of clinical treatments. Specific stimuli can be used to modulate physical performance beyond the self-governed boundaries that dogs will naturally express when allowed to exercise freely without stimulation.  相似文献

Capsule Expert‐based classification of bird species as habitat specialists and as generalists agrees with objective measures of species’ habitat requirements based on large‐scale monitoring data.

Aims To compare habitat specialization of 137 common bird species breeding in the Czech Republic using three different measures and to test their relationships to species’ abundance and habitat associations.

Methods Data on bird abundance and surveyed habitats were collected through a standardized monitoring scheme of common breeding species in the Czech Republic. From these data we calculated a quantitative species specialization index (SSI). Canonical correspondence analysis (CCA) was applied to calculate species’ habitat niche breadth and the level of association of each species to the main habitats. A panel of 11 local bird experts classified each species as habitat generalist or habitat specialist.

Results Species classified as habitat specialists by expert opinion showed higher habitat specialization according to the SSI, as well as according to CCA‐based habitat niche breadth. These species were also more closely associated with one of the main habitat types. These relationships were significant even after controlling for abundance.

Conclusions As expert opinion accords with the level of species’ habitat specialization expressed using two quantitative objective measures, we suggest that these characteristics reflect real interspecific variation in the breadth of habitat requirements in birds. Interspecific differences in habitat specialization are not caused solely by the variability in abundance among species.  相似文献

Background: Therapeutic benefits of Kinect-based virtual reality (VR) game training in rehabilitation encourage its use to improve motor function.

Objective: To assess the effects of Kinect-based VR training on motor recovery of the upper extremity and functional outcomes in patients with chronic stroke.

Methods: In this randomized controlled trial, group A received 20 sessions of physical therapy (PT)?+?20 sessions of Kinect-based VR training and group B received only 20 sessions of PT. Clinical outcome measures were assessed at baseline and at the end of the treatments. Primary outcome measures that assess stroke patients’ motor function included upper extremity (UE) Fugl-Meyer Assessment (FMA). Secondary outcome measures were Brunnstrom Recovery Stages (BRS), Modified Ashworth Scale (MAS), Box and Block test (BBT), Motricity index (MI), and active range of motion (AROM) measurement.

Results: Statistically significant improvements in game scores (p?p?Conclusions: To conclude, our results suggest that the adjunct use of Kinect-based VR training may contribute to the improvement of UE motor function and AROM in chronic stroke patients. Further studies with a larger number of subjects with longer follow-up periods are needed to establish its effectiveness in neurorehabilitation.  相似文献

IntroductionAdequate neuromuscular control of the lumbar spine is required to prevent lumbar injuries. A trunk postural stability test has been proposed earlier, using a chair wobbling on a central pivot and four springs with adjustable positions to modulate task difficulty. An inertial sensor is fixed on the chair to measure postural sway. The aim of this study is to assess the criterion validity and between-day reliability of the calibration and testing components.MethodsThirty six subjects (with and without low back pain) followed a calibration procedure, four practice trials and three 60-s trials on 2 days. The criterion validity of the inertial sensor was tested against an optoelectronic system and a force platform. The reliability of 38 body sway measures obtained from the inertial sensor angular measures was estimated.ResultsThe inertial sensor led to valid estimates of postural sway. The reliability of the calibration procedure was moderate. Practically no learning effect was detected except for a few body sway measures in patients with CLBP. Three 60-s trials provided acceptable reliability for approximately half of the body sway measures, although this is more difficult to achieve in patients with CLBP.DiscussionThe use of an easy to use inertial sensor led to valid measures of postural sway. A number of body sway measures were identified as reliable tools for individual follow-ups but inter-subject comparisons were anticipated as more difficult when patients with CLBP are involved.  相似文献

Purpose

This work provides an unambiguous conceptual framework for inclusion of nutrition in Life Cycle Assessments (LCAs) of food that enables the distinction between two different roles of nutrition, namely serving as the basis of food comparisons via the functional unit and as an impact pathway that links food ingestion with human health effects.

Methods

We compare how nutritional aspects have been considered in the functional unit of published LCAs of food with the procedural requirements for ensuring comparability of the functional units. We consider the relevance of nutrient profiling models for assessing food- and diet-related health damages and benefits in the context of LCAs of food. We finally recommend a method that will enable a systematic, comparative, and holistic assessment of the marginal health effect of food products on human health.

Results and discussion

Satiety is proposed as a central attribute for comparisons of food products, while weighted measures of nutrient content are suggested to be largely misplaced as part of the functional unit. In contrast, nutritional measures have a large role to play in assessing the human health impacts of the marginal ingestion of specific food products. Such measures should enable a direct quantification of human health effect and benefits and should take advantage of robust epidemiological evidence.

Conclusions

Nutritional measures enter into both the functional unit in the form of satiety measures and into the calculation of impacts in the form of the marginal influence of the specific food item on the human health impact of the overall diet. To enhance the differentiation of health impacts at the level of individual food items, it is recommended to combine the nutrient balance indicator with the DALY Nutritional Index (DANI) in each specific dietary context.

目的 探讨影响临床路径完成率的因素并采取相应的改进措施,提高临床路径完成率。方法 采用鱼骨图,分析影响临床路径完成率的根本原因,针对原因依据PDCA循环持续改进,不断提高临床路径完成率。结果 临床路径完成率与医务人员对临床路径的认识、临床路径系统的设置、临床路径质控方法等因素有关,采取相应改进措施后,临床路径完成率由29.4%增加到80.0%。结论 正确认识临床路径并采取有效的管理措施能够提高临床路径完成率,从而保证临床路径实施质量,为提高医疗质量和保障患者安全打基础。  相似文献

BackgroundSnakebite is a priority neglected tropical disease and causes a range of complications that vary depending on the snake species. Randomised clinical trials have used varied outcome measures that do not allow results to be compared or combined. In accordance with the Core Outcomes Measurements in Effectiveness Trials (COMET) initiative, this systematic review aims to support the development of a globally relevant core outcome set for snakebite.MethodsAll randomised controlled trials, secondary analyses of randomised controlled trials and study protocols investigating the efficacy of therapeutics for human snakebite envenoming were eligible for inclusion. Study screening and data extraction were conducted in duplicate by two independent reviewers. All primary and secondary outcome measures were extracted and compiled, as were adverse event outcome measures. Similar outcome measures were grouped into domains. The study was prospectively registered with PROSPERO: CRD42020196160.ResultsThis systematic review included 43 randomised controlled trials, two secondary analyses and 13 study protocols. A total of 382 outcome measures were extracted and, after duplicates were merged, there were 153 unique outcomes. The most frequently used outcome domain (‘venom antigenaemia’) was included in less than one third of the studies. The unique outcomes were classified into 60 outcome domains. Patient-centred outcomes were used in only three of the studies.DiscussionSignificant heterogeneity in outcome measures exists in snakebite clinical trials. Consensus is needed to select outcome measures that are valid, reliable, patient-centred and feasible. The results of this systematic review strongly support the development of a core outcome set for use in snakebite clinical trials.  相似文献

BackgroundThe predictive coding model is rapidly gaining attention in schizophrenia research. It posits the neuronal computation of residual variance (‘prediction error’) between sensory information and top-down expectation through multiple hierarchical levels. Event-related potentials (ERP) reflect cortical processing stages that are increasingly interpreted in the light of the predictive coding hypothesis. Both mismatch negativity (MMN) and repetition suppression (RS) measures are considered a prediction error correlates based on error detection and error minimization, respectively.MethodsTwenty-five schizophrenia patients and 25 healthy controls completed auditory tasks designed to elicit MMN and RS responses that were investigated using repeated measures models and strong spatio-temporal a priori hypothesis based on previous research. Separate correlations were performed for controls and schizophrenia patients, using age and clinical variables as covariates.ResultsMMN and RS deficits were largely replicated in our sample of schizophrenia patients. Moreover, MMN and RS measures were strongly correlated in healthy controls, while no correlation was found in schizophrenia patients. Single-trial analyses indicated significantly lower signal-to-noise ratio during prediction error computation in schizophrenia.ConclusionsThis study provides evidence that auditory ERP components relevant for schizophrenia research can be reconciled in the light of the predictive coding framework. The lack of any correlation between the investigated measures in schizophrenia patients suggests a disruption of predictive coding mechanisms in general. More specifically, these results suggest that schizophrenia is associated with an irregular computation of residual variance between sensory input and top-down models, i.e. prediction error.  相似文献

Capsule Skylarks breeding in Ireland prefer extensive grassland habitats and almost completely avoid tillage habitats.

Aims To describe the distribution and habitat use of breeding Skylarks in Ireland, particularly in lowland agricultural habitats, and to use this information to inform conservation measures for this species.

Methods Countryside Bird Survey (CBS) and Farmland Bird Project (FBP) data were examined to determine large-scale (national) distribution and habitat selection, in addition to smaller-scale (farm- and field-level) habitat use. The CBS is a national breeding bird monitoring scheme involving 397 1-km squares. The FBP collected detailed bird and habitat data from 122 farms.

Results CBS and FBP data both showed significant regional differences in breeding Skylark densities, with the highest relative abundances in the northwest and west. Dry grassland/grass moor habitats supported the highest densities of breeding Skylarks in the CBS, which were significantly higher than in improved grassland or tillage. At the farm-level, Skylark numbers were positively related to wetland habitats but negatively associated with trees in field boundaries, dense ground vegetation and overall density of farm boundaries. At the field-scale, larger fields and unimproved grasslands were preferred.

Conclusion Agri-environment measures tailored to region-specific requirements and to the relatively local habitat preferences of target species are required if population declines of species of conservation concern, including Skylarks, are to be reversed.  相似文献
