Phenomena related to chronic kidney disease, such as atherosclerosis, aggravate with the introduction of dialysis. Matrix
metalloproteinases (MMP) and factors modifying their activity, such as their tissue inhibitors (TIMP) or neutrophil gelatinase-associated
lipocalin (NGAL), take part in the matrix turnover and the endothelial damage characteristic for atherogenesis. However, there
are no data on the associations between these parameters and other known pro-atherogenic factors, or on the impact of various
dialysis modalities on them. The aim of our study was to assess the serum concentrations of NGAL, MMP-7, MMP-9, and TIMP-1,
as well as their correlations with human heat shock proteins (Hsp90α, anti-Hsp60), endothelial dysfunction (sE-selectin),
and inflammation (hsCRP) in pediatric patients chronically dialyzed. Twenty-two children on automated peritoneal dialysis
(APD), 17 patients on hemodialysis (HD) and 24 controls were examined. The serum concentrations of NGAL, MMP-7, MMP-9, TIMP-1,
Hsp90α, anti-Hsp60, and sE-selectin were assessed by enzyme-linked immunosorbent assay (ELISA). The median values of NGAL,
MMP-7, MMP-9, TIMP-1, and MMP-9/NGAL ratio were significantly elevated in all dialyzed children vs. controls and were higher
in HD than in APD. The values of MMP-9/TIMP-1 and MMP-7/TIMP-1 ratios in the HD subjects were lower than those in the APD
children. Hsp90α and anti-Hsp60 predicted the values of NGAL, MMPs, and TIMP-1. Additionally, sE-selectin was a predictor
of NGAL levels, whereas NGAL predicted the MMP and TIMP-1 concentrations. The increased concentrations of examined parameters
indicate the dysfunction of MMP/TIMP/NGAL system in the dialyzed children, more pronounced on hemodialysis. The discrepancies
between dialysis modalities and correlations with heat shock proteins (HSPs) suggest that NGAL may be considered a novel stress
protein, whereas MMP-7, MMP-9, and TIMP-1 may be regarded as indicators of stress response in the pediatric population on

目的探讨MMP-9、TIMP-1及细菌L型在卵巢上皮性肿瘤中的表达及临床意义。方法采用原位杂交和免疫组化及革兰染色方法检测97例卵巢乳头状癌及23例卵巢乳头状瘤组织中MMP-9、TIMP-1的表达及细菌L型检出率,并用2χ检验进行统计学处理。结果卵巢乳头状癌中MMP-9及TIMP-1的表达率均明显高于良性肿瘤(P<0.005)。MMP-9在卵巢乳头状癌中临床分期Ⅲ、Ⅳ期中的表达率明显高于Ⅰ、Ⅱ期(P<0.005~P<0.01),随着病理分级增高而显著增加(P<0.005~P<0.05),腹腔淋巴结有转移和有腹水者均高于无腹腔淋巴结转移和无腹水者(P<0.005~P<0.05)。而TIMP-1阳性表达与MMP-9阳性表达相反,呈负相关。细菌L型检出阳性率与病理分级及临床分期差异有显著性,腹腔淋巴结有转移比无转移者、有腹水比无腹水者差异有显著性(P<0.005)。结论MMP-9、TIMP-1基因及蛋白在卵巢肿瘤中有不同程度的异常表达,两者均可作为判断卵巢肿瘤生物学行为及患者预后参考指标。L型感染极有可能成为诱发肿瘤因素之一,它与MMP-9、TIMP-1可能有协同致瘤及恶性肿瘤侵袭和转移作用。研究细菌L型感染与肿瘤的关系,具有重要的临床应用价值。 相似文献

目的:对比分析血液透析(HD)、血液透析联合血液透析滤过(HD+HDF)、腹膜透析(PD)三种方式对老年尿毒症患者皮肤瘙痒症的疗效,为老年尿毒症患者皮肤瘙痒症的治疗提供参考。方法:选取2010年3月至2014年3月来我院就诊的老年尿毒症皮肤瘙痒症患者共100例作为研究对象,采用随机数表法随机分为HD组(33例)、联合组(33例)、PD组(34例),在初次透析前后、透析后1个月、透析后2个月记录患者皮肤瘙痒评分,并检测患者全段甲状旁腺激素(i PTH)、β2-微球蛋白(β2-MG)的水平。透析2个月后比较三组对皮肤瘙痒症的疗效。结果:联合组和PD组透析2月后β2-MG水平、i PTH水平、瘙痒症评分明显低于HD组,差异具有统计学意义(P<0.05)。PD组透析2月后β2-MG水平低于联合组,但差异不具有统计学意义(P>0.05)。PD组透析2月后i PTH水平明显低于联合组,差异具有统计学意义(P<0.05)。PD组透析2月后评分明显低于联合组,差异具有统计学意义(P<0.05)。PD组、联合组有效率明显高于HD组,差异具有统计学意义(P<0.05);PD组有效率高于联合组,但差异怒具有统计学意义(P>0.05)。结论 :血液透析联合血液透析滤过与腹膜透析均可以明显降低患者全段甲状旁腺激素、β2-微球蛋白的水平,缓解患者瘙痒症状,提升患者生活质量,值得临床推广。 相似文献

真菌性腹膜炎是影响腹膜透析的主要因素之一,与细菌性腹膜炎相比,其病亡率更高。尽管随着技术的改进和管理的提升,真菌性腹膜炎的发病率明显降低,全球发病率为2.0%~23.8%,但对于患者个人来说腹膜炎仍是影响其生存及预后的主要因素。国际腹膜透析协会建议一旦确诊为真菌性腹膜炎,需要尽早拔管并继续进行抗感染治疗至少2周,最终才能继续腹膜透析。早期预防和及时规范诊治是维持长期腹膜透析成功的关键,本研究结合近年来国内外的真菌性腹膜炎诊治的研究进展作一综述。 相似文献

目的 观察肝纤维化形成过程中基质金属蛋白酶MMP-1及其抑制剂TIMP-1的表达变化,从细胞外基质降解代谢的角度研究四氯化碳(CCl4)中毒性肝纤维化发生的机制.方法 雄性Wistar大鼠20只,分为正常组和肝纤维化模型组.肝纤维化组采用CCl4、饮酒、高脂低蛋白饮食等复合病因刺激制备肝纤维化动物模型,造模时间为8周.实验结束后测定肝脏指数、血清透明质酸(HA)、谷丙转氨酶(ALT)及尿羟脯氨酸(HYP)排出量,光镜下观察肝组织纤维化程度,并用免疫组化SABC法检测肝组织中Ⅰ、Ⅲ型胶原蛋白及MMP-1、TIMP-1的表达,同时用荧光实时定量PCR(RT-PCR)的方法检测肝组织中MMP-1、TIMP-1 mRNA的表达.结果 与正常对照组比较,肝纤维化模型组大鼠肝脏指数、血清HA及ALT显著增高,尿羟脯氨酸的排出量明显增加,病理组织学检查发现肝组织内纤维结缔组织增生明显,有假小叶形成;免疫组化的结果显示肝组织内Ⅰ、Ⅲ型胶原蛋白、MMP-1及TIMP-1的表达较正常组显著增加.结论 肝组织中MMP-1及TIMP-1的表达变化可能是导致肝纤维化的重要机制之一. 相似文献

Despite the strong rationale for combining cytoreductive surgery (CRS) with hyperthermic intraperitoneal chemotherapy (HIPEC) in patients with peritoneal carcinomatosis, thermotolerance and chemoresistance might result from heat shock protein overexpression. The aim of the present study was thus to determine whether the heat shock protein 27 (Hsp27), a potential factor in resistance to treatment, could have a higher level in serum from patients under this combined therapy. Patients receiving CRS plus HIPEC for peritoneal carcinomatosis (group 1), patients with cancer or a history of cancer undergoing abdominal surgery (group 2), and patients without malignancies undergoing abdominal surgery (group 3) were included. Hsp27 serum levels were determined before and at different times following CRS and HIPEC using enzyme-linked immunosorbent assay. In group 1 ( n = 25), the high Hsp27 levels, observed at the end of surgery compared with before ( p < 0.0001), decreased during HIPEC, but remained significantly higher than before surgery ( p < 0.0005). In groups 2 ( n = 11) and 3 ( n = 15), surgery did not significantly increase Hsp27 levels. A targeted molecular strategy, inhibiting Hsp27 expression in tumor tissue, could significantly reduce resistance to the combined CRS plus HIPEC treatment. This approach should be further assessed in a clinical phase I trial. 相似文献

目的:探究尿毒症皮肤瘙瘁患者采取腹膜透析与血液透析治疗的临床效果,并为该病的临床治疗提供经验积累。方法:选取我院肾内科于2005年1月-2012年12月收治的52例尿毒症皮肤瘙痒患者,利用随机数字表法进行分组,分别设为研究组和对照组。其中对照组实施血液透析治疗,而研究组开展腹膜透析治疗。记录两组在治疗前和治疗后第8周末血生化客观指标及皮肤瘙痒程度,并做好对比。结果:两组在治疗前的皮肤瘙痒评分、β2-MG、PTH、p3,BUN、SCr值差异无统计学意义(P〉0.05);治疗后,研究组皮肤瘙瘁评分为(3.4±0.8)分,对照组为(5.8±0.9)分,差异有统计学意义(P〈0.05)。治疗后,研究组β2-MG、PTH及Ps.均低于对照组,差异有统计学意义(P〈0.05)。结论:尿毒症皮肤瘙痒患者的主要致敏因子为大中分子,采取腹膜透析能够有效清除大分子物质,进而达到瘙痒程度的有效改善。 相似文献

目的:探究百令胶囊联合厄贝沙坦片对膜性肾病患者基质金属蛋白酶-9、3和金属蛋白酶组织抑制物-1影响。方法:收集我院肾内科收治的膜性肾病患者98例,根据随机对照表分为对照组和试验组,每组49例。对照组给予厄贝沙坦片治疗,试验组联合百令胶囊治疗。对比分析两组患者的临床疗效、血清Scr、BUN、UA、Ccr、尿蛋白、MMP-9、MMP-3及TIMP-l水平以及不良反应的发生情况。结果:治疗后,对照组临床总有效率为81.63%,显著低于试验组的95.92%(P0.05)。两组治疗后血清Scr、BUN、UA、MMP-9、MMP-3、TIMP-l水平均显著降低,且试验组显著低于对照组(P0.05),Ccr水平升高,且试验组显著高于对照组(P0.05)。对照组不良反应发生率为10.20%,试验组为6.25%,差异无统计学意义(P0.05)。结论:百令胶囊联合厄贝沙坦片对膜性肾病患者的临床疗效显著,安全性较高,可能与其显著降低MMP-9、MMP-3和TIMP-1水平有关。 相似文献

Background/aim: Elevated relative expression of insulin-like growth factor-II (IGF-II) was observed in hepatocellular carcinoma (HCC) liver tissues with a role in neovascularization and associated with poor prognosis. IGF-II is influenced by the proteolytic cleavage of IGF-binding protein 3 and by matrix metalloproteinases (MMP), which are further regulated by their tissue inhibitors tissue inhibitor of metalloprotienase-1 (TIMP-1). Our aim is to study new molecular markers for HCC. Patients/methods: RNA was extracted from the peripheral blood for evaluating the relative expression of IGF-II, MMP-9, and TIMP-1 in correlation with clinical staging of 39 HCC patients and 15 healthy controls using TaqMan real-time PCR. Results: The relative expression of IGF-II and MMP-9 mRNA were significantly elevated in HCC patients compared with healthy controls; P-value <0.0001 for both. There was a significant correlation between MMP-9 and different HCC stages. On the other hand, TIMP-1 was significantly down-regulated in HCC patients; P?=?0.0003 with the elevation of the IGF-II/TIMP-1 ratio. Significant correlation between TIMP-1 and HCC Stage III and Stage IV was found; P-value?=?0.0138. Conclusion: These results highlight the importance of profiling the expression of IGF-II, MMP-9, and TIMP-1 in the peripheral blood as prognostic molecular biomarkers in HCC. 相似文献

Matrix metalloproteinases (MMPs) are enzymes that are responsible for degradation of extracellular matrix (ECM); they are involved in the pathogenesis of ischemia-re-perfusion (I-R) injury. We investigated the possible preventive effect of alpha-lipoic acid (LA) in a renal I-R injury model in rats by assessing its reducing effect on the expression and activation of MMP-2 and MMP-9 induced by I-R. Rats were assigned to four groups: control, sham-operated, I-R (saline, i.p.) and I-R+ LA (100 mg/kg, i.p.). After a right nephrectomy, I-R was induced by clamping the left renal pedicle for 1 h, followed by 6 h re-perfusion. In the sham group, a right nephrectomy was performed and left renal pedicles were dissected without clamping and the entire left kidney was excised after 6 h. LA pretreatment was started 30 min prior to induction of ischemia. Injury to tubules was evaluated using light and electron microscopy. The expressions of MMP-2 and MMP-9 were determined by immunohistochemistry and their activities were analyzed by gelatin zymography. Serum creatinine was measured using a quantitative kit based on the Jaffe colorimetric technique. Malondialdehyde (MDA) and glutathione (GSH) were analyzed using high performance liquid chromatography. Tissue inhibitor of metalloproteinase (TIMP)-2 and TIMP-1 were assessed using enzyme-linked immunosorbent assay (ELISA). I-R caused tubular dilatation and brush border loss. LA decreased both renal dysfunction and abnormal levels of MDA and GSH during I-R. Moreover, LA decreased significantly both MMP-2 and MMP-9 expressions and activations during I-R. TIMP-1 and TIMP-2 levels were increased significantly by LA administration. LA modulated increased MMP-2 and MMP-9 activities and decreased TIMP-1 and TIMP-2 levels during renal I-R. 相似文献

Context: Chronic kidney disease (CKD) is characterized by immunocompetent cell migration and inflammation. Monocyte chemoattractant protein (MCP )-1 and macrophage colony-stimulating factor (MCSF) stimulate monocyte migration and transition into macrophages with subsequent release of neopterin. Objective: The aim of the study was to analyze these parameters in children with various stages of CKD. Material and methods: The study group consisted of 41 CKD children, 19 patients on haemodialysis (HD), 22 children on automated peritoneal dialysis (APD) and 23 controls. Serum concentrations of MCP-1, MCSF and neopterin were assessed by ELISA. Correlations to matrix metalloproteinases (MMPs) and their tissue inhibitors (TIMPs) were analyzed. Results: MCP-1, MCSF and neopterin were significantly elevated in all patients versus controls and the highest values concerned HD children. A single HD session lessened the concentrations of all parameters, yet they rose back before the next HD session. All markers correlated with MMPs and TIMPs in different combinations. Conclusions: Systemic inflammation and cell migration are triggered by CKD and additionally aggravated by chronic dialysis, with the more evident negative impact of HD than APD. Discrepancies in MCP1, MCSF and neopterin serum concentrations suggest they may serve as new markers of cellular and inflammatory responses in children with CKD. 相似文献

目的:比较腹膜透析和血液透析对慢性肾衰竭患者微炎症状态和生活质量的影响。方法:选取我院2014年11月~2016年9月因慢性肾衰竭需住院治疗患者96例,按入院先后顺序分为观察组和对照组,每组48例。观察组予以腹膜透析,对照组予以血液透析,比较两组治疗前后血清超敏C反应蛋白(hc-C-reactive protein,hs-CRP)、白细胞介素-6(interleukin-6,IL-6)、肿瘤坏死因子-α(Tumor necrosis factor-α,TNF-α)、血尿素氮(blood urea nitrogen,BUN)、血肌酐(Serum creatinine,Scr)和白蛋白(albumin,ALB)水平、血红蛋白(hemoglobin,HB)和红细胞(red blood cell,RBC)值、SF-36量表生活质量评分(the MOS item short from health survey)的变化及并发症的发生情况。结果:治疗后,观察组血清hs-CRP、IL-6、TNF-α、BUN、Scr和ALB水平均显著低于对照组,HB和RBC值明显高于对照组,差异均具有统计学意义(P0.05)。观察组治疗后并发心脑血管疾病的发生率显著低于对照组,并发低蛋白血症率和感染得发生率显著高于对照组,差异具有统计学意义(P0.05),但两组总并发症发生率比较差异无统计学意义(P0.05)。观察组治疗6个月后体力功能、总体健康、心理健康评分与对照组比较差异无统计学意义(P0.05),躯体疼痛、生命活力、社会职能、生理职能和情感职能评分均显著高于对照组,差异具有统计学意义(P0.05)。结论:腹膜透析对慢性肾衰竭患者的临床效果较好,较血液透析可更有效抑制患者的微炎症状态,提高小分子物质清除率,改善生活质量,降低心脑血管疾病发生率,但治疗中易发生低蛋白血症和感染等并发症,需及时补充白蛋白和加强预防感染措施等对症处理。 相似文献

目的:探讨兔后交叉韧带(Posterior cruciate ligament,PCL)断裂对外侧胫骨平台组织学的影响。方法:48只家兔膝关节随机配对为实验侧和对照侧造模,造模后第4、8、16、24周各随机处死12只,行外侧胫骨平台大体观察、HE染色、免疫组化检测基质金属蛋白酶13(matrix metalloproteinase-13,MMP-13)、基质金属蛋白酶抑制剂1(Tisse inhibitor-1 of matrix metalloproteinasel,TIMP-1)表达。结果:①大体观察,随时间延长,实验组外侧胫骨平台软骨出现磨损,呈灰黄色,弹性差,骨赘形成。②组织学观察,随时间延长,胫骨平台软骨纤维化,细胞排列紊乱,簇聚细胞出现频率增加。⑧实验组MMP-13、TIMP—1表达均高于对照组,有显著性差异。P〈0.05。④实验组MMP-13、TIMP-1表达阳性率第4、8、16周逐渐升高,24周下降,各组比较有显著性差异,P〈0.05。结论:①兔膝关节PCL断裂会引起外侧胫骨平台软骨退行性变,且该退变随着时间的推移逐渐加重。②MMP-13与TIMP-1在PCL断裂膝关节外侧胫骨平台中的表达呈现先高后低的变化规律,造成MMP-13与T1MP-1的失衡,加速软骨退变。⑨MMP-13与T/MP-1表达增高提示MMP-13与TIMP—1可能参与了PCL断裂后外侧胫骨平台软骨的退变过程。 相似文献

目的:探讨血必净注射液对脓毒症早期肺组织损伤的影响及其可能作用机制,为临床脓毒症时急性肺损伤的治疗提供新思路。方法:选用健康雄性S-D大鼠30只,随机分3组,即假手术组(Sham组)、盐水组(NS组)和血必净组(XBJ组)。应用盲肠结扎穿孔法(CLP)建立脓毒症大鼠模型,术后6 h处死动物,通过HE染色光镜观察组织形态学改变及透射电镜观察肺组织超微结构,测定各组肺组织湿干重比,采用RT-PCR检测肺组织内皮素-1(ET-1)、诱导型一氧化氮合酶(iNOS)、基质金属蛋白酶-9(MMP-9)、组织金属蛋白酶抑制物-1(TIMP-1)mRNA的表达。结果:XBJ组肺泡及肺间质水肿和内皮细胞超微结构改变均较NS组明显减轻。术后6 h,NS组大鼠肺组织湿干重比(W/D)显著高于Sham组(5.37±0.12 vs 4.33±0.06,P0.01),XBJ组肺组织W/D较NS组显著降低(4.67±0.09 vs 5.37±0.12,P0.05)。此外,XBJ组大鼠肺组织ET-1(0.511±0.111 vs 0.705±0.122,P0.01)、iNOS(0.456±0.075 vs 0.548±0.098,P0.05),MMP-9(0.617±0.079 vs 0.732±0.131,P0.05)、TIMP-1(0.438±0.043vs 0.515±0.049,P0.01)mRNA的表达均较NS组明显降低,而NS组大鼠肺组织ET-1(0.705±0.122 vs 0.400±0.033,P0.01)、iNOS(0.548±0.098 vs 0.334±0.027,P0.01)、MMP-9(0.732±0.131 vs 0.352±0.061,P0.01)、TIMP-1mRNA(0.515±0.049 vs 0.365±0.068,P0.01)水平均较Sham组明显升高。结论:血必净治疗对脓毒症大鼠的肺组织损伤有一定的保护作用,可能与其降低肺组织ET-1、iNOS、MMP-9、TIMP-1的表达有关。 相似文献

目的探讨维生素A缺乏(vitamin A deficiency,VAD)对大鼠肺脏发育的影响及其可能的机制。方法建立VAD大鼠模型,取出生后2周正常及VAD幼鼠肺脏,HE、Weigert染色分别观察其肺脏组织形态变化和弹力纤维变化;Image Pro Plus5.0软件做肺脏形态学定量分析,蛋白质免疫印记检测大鼠肺组织MMP-9及TIMP-1蛋白含量变化。结果①VAD组大鼠肺上皮细胞堆积,出现明显的肺气肿,并发弥散的间质性肺炎,肺组织弹力纤维显著减少;②VAD组大鼠肺MMP-9的蛋白质表达水平明显高于对照组,TIMP-1水平变化不大,MMP-9与TIMP-1的比值升高。结论VAD导致大鼠肺脏正常发育受损,肺脏组织形态异常,可能通过上调MMP-9的表达诱发大鼠肺气肿。 相似文献

The effects of strenuous physical exercise on the serial changes in the haematological, biochemical and hormonal markers were investigated. A group of 14 soldiers, aged 24–36 years, took part in a military training course for about 13 weeks. After severe exercise stress, an increase (90%) in the number of peripheral blood leucocytes was observed. The degree of leucocytosis showed a close correlation with the values of some serum parameters, such as concentrations of aspartate aminotransferase (AST; r = 0.747), lactate dehydrogenase (LD; r = 0.748), blood urea nitrogen ( r = 0.756), creatine kinase (CK; r = 0.637), manganese-superoxide dismutase (Mn-SOD; r = 0.508), alanine aminotransferase (ALT; r = 0.542) and uric acid ( r = 0.538), and concentrations of urinary parameters, such as vanilmandelic acid ( r = 0.429) and free cortisol ( r = 0.437). The subjects showing prominent leucocytosis over 9500 cells · l –1 exhibited a lower concentration of serum cholinesterase than those who showed milder leucocytosis. The serum Mn-SOD concentration was closely correlated with the serial changes in serum concentrations of AST, ALT, LD and CK, indicating exercise-induced muscle and liver damage. The change in peripheral leucocyte number was assumed to be diagnostically informative and may be a prognostic marker, reflecting organ damage and restoration after strenuous physical exercise. 相似文献
