Performances de la TEP au 18F-FDG dans la caractérisation initiale de la lésion primitive du cancer du sein

Objective¹⁸F-FDG PET/CT is for the moment not recommended for stage T of the TNM classification of breast cancer. The aim of our study was to evaluate the performance of ¹⁸F-FDG PET/CT in the initial staging of breast tumors. Tumor size, skin involvement and inflammation as well as the relationship between primary tumor maximum standardized uptake value (SUVmax) and histopathological grade (SBR), molecular tumor subtypes (luminal A and B, Her2 enriched, triple negative), estrogen receptors (ER), progesterone receptors (PR) and focality were evaluated.MethodsHistological reports of patients operated for breast cancer, without neoadjuvant chemotherapy, were compared to preoperative ¹⁸F-FDG PET/CT.ResultsSeventy-four patients who underwent surgery in 2016 were included. ¹⁸F-FDG PET/CT was able to visualize primary tumors in 91% and to correctly classify the T stage of the TNM classification in 81% of the cases, to detect multifocality in 73% and cutaneous and inflammatory breast cancers in 100%. The uptake intensity of ¹⁸F-FDG (SUVmax) was significantly correlated with histo-prognostic factors such as SBR grade (P = 0.02), lack of expression of estrogen receptors (ER) (P = 0.01) and progesterone (PR) (P = 0.02), positive HER2 status (P = 0.01) or triple negative subtype tumors (P = 0.02).Conclusion¹⁸F-FDG PET/CT provides relevant elements for local assessment, in particular, tumor focality and inflammatory character in addition to ensuring the regional and extension assessment.     

Recherches sur la conservation du virus de la densonucléose

Résumé Le virus de la densonucléose (VDN) conserve une partie de sa virulence en solution aqueuse entre 0°C et 30°C pendant deux mois. Son pouvoir infectieux se maintient à—10°C pendant deux mois mais est supprimé après dix jours de dessication. On peut done considérer ceParvovirus d’Insectes comme un virus relativement résistant aux températures que l’on rencontre ordinairement dans la nature et au laboratoire.

Summary The persistence of DNV is described. This virus retains a part of its infectivity in aqueous solution between 0°C and 30°C. The log LD₅₀ initially 7,15 drops to 2 after two months. The virus keeps its infectivity at 10°C but it loses all its pathogenicity after 10 days in dry conditions.

Fonctions histoenzymatiques lors de l'évolution et de la lutéolyse du corps jaune de la lapine

Résumé Trente cinq enzymes sont recherchées lors de l'étude comparative de la lutéinisation et la lutéolyse. Vingt six sont histochimiquement décelables dans les corps jaunes gestatifs leur permettant de stocker et métaboliser glucides, corps gras, nucléotides, amines, phosphates, sulfates et d'autres métabolites. Les activités importantes des enzymes de la voie des pentoses, des diaphorases et de la stéroïde 3 ol DH témoigneraient d'une stéroïdogenèse active. Les cellules lutéiniques thécales semblent histoenzymatiquement mieux pourvues au début de la luteinisation; les cellules lutéiniques granuleuses le sont pendant les stades ultérieurs.La lutéolyse est marquée par une diminution de la plupart des activités enzymatiques. Les activités tant qu'elles existent, sont topographiquement limitées (ilôts actifs extra lutéaux) et surtout métaboliquement restreintes. L'équipement enzymatique des structures lutéales très involuées suggère plutôt qu'une stéroïdogenèse même résiduelle des processus typiques et une activité des éléments conjonctifs (fibrillogenèse).

Histoenzymatic functions of the evolution and the luteolysis of the corpus luteum of the rabbit

Summary 35 enzymes were investigated in the course of a comparative study on the luteinisation and luteolysis. 26 of them were found in the corpora lutea during gestation. They were able as a consequence to stock and the metabolize glucides, lipides, nucleotides, amines, phosphates, sulphates as well as other metabolites. The strong activities of the pentose shunt's enzymes, of the tetrazolium reductases and ⁵ 3 OH-steroide DH indicated most probably an active steroidogenesis. The theca-luteinic cells appeared having a stronger histoenzymatic activity in the beginning of the luteinisation; the granuloso-luteinic cells showed a stronger activity in the later stades.The luteolysis is characterized by a decrease of the activity of the preponderant number of the enzymes mainly in the degenerated luteal cells. The enzymes activities as far as such existed were topographically limited (active extraluteal islands) and restricted mainly in their metabolizing possibilities. The author's results suggested as more probable the existence of a process of lysis and an activity of the connective tissue elements (fibrillogenesis) than a steroidogenesis, even though in a residual or terminal form.

     

Génétique moléculaire de la transposition du bras long du chromosome x sur l'y lors de la spéciation humaine

Five cosmidic probes (7 b; 8 j; 22 b; CB 16 and 115 i 1) recognizes site of simple-copy DNA sequence homology between the human X and Y chromosomes. Genomic DNAS restricted with Eco RI from the great apes (Gorilla and Pan) were examined for sequences homologous to these probes, by molecular hybridization studies. Experiments reported here revealed, by genetic dosage with sex, that the great apes have homologous sequences of these five probes on their X chromosomes only. The simplest explanation of these results is that the corresponding sequences were transposed from the X chromosome to the Y chromosome after the human line diverged, like probe pDP 34 for locus DXY S1 (Page et al., 1984).

Summary The densonucleosis virus is thermoresistant. It is not inactivated after treatment with lipid solvents. Attempts to demonstrate hemagglutination of DNV, using different erythrocytes, were unsuccessful. These properties are compared to those of Parvoviruses.

     

Nouveautés de la flore halophile du Littoral croate

Summary The archipelago opposite Senj (Croatian Coast) is very exposed to the Bora hurricanes accompanied by air dryness, salt spray, and periodic coast icing. Therefore the xerohalophytic flora of this area is very interesting. Four new taxa from there are described:Astragalus glacialis n. spec. subsp.glacialis s. s. and subsp.uraganicus n. subsp.,Allium horvatii n. spec., andPinus nigra Arn. subsp.croatica n. subsp., a stenoendemic relict near extinction on the isle of Prvi.     

L'effet de β-mercaptoéthanol et de l'urée sur la structure du phragmoplaste

Résumé Les fibrilles constitutives du phragmoplaste sont de nature protéique. L'action du-mercaptoéthanol permet de conclure que ces protéines sont liées par des ponts S-S. En présence d'urée, la structure du phragmoplaste est détruite, ce qui indique l'existence de liaisons hydrogènes.

Der Effekt des-Merkaptoäthanols gestattet die Annahme, daß die Proteine mit S-S-Brücken verbunden sind. 3. Bei Anwesenheit von Harnstoff wird die Phragmoplaststruktur zerstört, was für das Vorhandensein von H-Brücken spricht.

Anomalies postnatales du développement de la spermatogenèse associées aux troubles de la migration testiculaire

Cryptorchidism, a non-descended testis in its physiological intrascrotal location, is one of the most frequent congenital anomalies of the male genital system. The mechanisms of the normal descent of the testis are still unclear. Several etiological hypotheses have been proposed for cryptorchidism. Cryptorchidism is associated with a greater risk of testis cancer, and is also a cause of impairment in sperm parameters and fertility in the adult age. In this article, we review the cellular and hormonal events occurring from birth to puberty in isolated cases of congenital cryptorchidism that will later, in adulthood, alter both spermatogenesis and fertility.     

Épidémiologie du cancer de la prostate: Article de revue

Prostate cancer is the first cancer in incidence and the second in mortality for men in France. In 2000, the estimated number of newly diagnosed cases was 40,309. Crude and standardised on world population yearly incidence rates were 141.4/100,000 and 75.3/100,000, respectively. The French Society of Urology recommends individual screening for prostate cancer. Information and consent is required before screening. The Haute Autorité de Santé has published an information for men seeking for screening. Mass screening is not recommended. Screening tests consist of prostate specific antigen (PSA) dosage, rectal examination, yearly for men aged 50 to 75 or starting from 45 in case of risk factors. Currently, large studies are ongoing to estimate the benefit of mass screening on morbidity and mortality. France participates to a European study (ERSPC). Results will be available in years 2008–2010.     

L’analyse automatisée de la morphologie du spermatozoïde

Examination of sperm morphology is one factor of evaluation of sperm function, but it can also be considered as a biomarker of testicular function. All publications showed a high variability in observed results, in relation with different methods of staining slides and classifying sperm morphology, and a large subjectivity in the visual assessment. Automated sperm morphology analysis (ASMA) have the potential to provide more objective, accurate, and precise morphometric measurements of spermatozoa. Standardisation of the methods of slides preparation is first essential. Analysis of the sperm head morphometry appears the more accessible for the ASMA and could give selective parameters in the evaluation of fertility, in complement with motion sperm analysis. In the other hand automated analysis of all sperm abnormalities appears illusory with actual instruments, because the midpiece or the flagellum is a little structure weakly stained, and thus difficult to be identified by the computer. Until more rigorous and consistent definitions of sperm features can be developped, in relation with testicular function, the pronostic value of existing sperm abnormalities classifications is limited.     

La forme scléronodulaire de la maladie de Hodgkin: expérience du CHU de Sherbrooke

Scleronodular type of Hodgkin''s disease: experience at the Centre hospitalier universitaire de SherbrookeThe nodular sclerosis type of Hodgkin''s disease appears to be a distinct clinical entity. However, the incidence, the initial localization of the tumour and the survival of the patients are variable. The present study was carried out on a group of 17 patients, all French Canadians living in the province of Quebec, from a total of 31 with Hodgkin''s disease, an incidence of 55%. There were more males (10) than females (7). The mean age of the group was 37 years, but that of the females was lower than that of the males. The mediastinum was involved at the onset in 47% of the patients. The initial staging (according to the classification of Rye) in 76% of the patients was I or II.Four patients showed disease below the diaphragm. The lungs were infiltrated three times, the spleen six times, and the liver five times. The duration of survival of the 17 patients was twice that of the patients with the three other types of the disease.

Le role des autorités publiques dans la réalisation de projets de biomethanisation: l'exemple du Ministere de l'Energie du Burundi et sa coopération avec la Belgique,la Chine et la RFA

Résumé Le programme biogaz au Burundi a démarré par la création d'un Département chargé de la mise au point de stratégies de vulgarisation de nouvelles sources d'énergie. Au sein de ce Département on a établi les constats suivants: (1) la nécessité de former des cadres et d'établir un inventaire de la biomasse biodégradable au Burundi; (2) un problème de collecte des déchets et de distribution du gaz et des effluents dû à l'habitat dispersé; (3) l'existence de collectivités (écoles, hôpitaux, casernes); et (4) l'évacuation dans le lac Tanganyka des déchets des usines agro-alimentaires. En conséquence le Burundi a fait appel à trois formes de coopération. Un financement de l'Administration Belge de la Coopération au Développement a permis l'éstablissement d'un centre de démonstration et d'un laboratoire responsables de recherches, cours de formation, démonstration et suivis scientifiques et techniques. Un financement du Gouvernement Burundais permet à trois experts chinois la réalisation de digesteurs pilotes à raison de cinq à six par province. Un financement de la GTZ permet la diffusion de la technologie par le biais de la mise en place d'une structure (bureaux régionaux, formation de techniciens maçons et de multiplicateurs potentiels) et la construction d'installations.

Summary The Burundi biogas programme started with the setting up of a new Department with the overall responsibility for the popularization of new sources of energy. The biogas programme started because : (1) it was felt necessary to train staff and to establish an inventory of the biodegradable biomass in Burundi; (2) a largely scattered habitat complicated both the collection of domestic waste and the distribution of gas; (3) there existed large communities like schools, hospitals, etc.; and (4) the fact that Lake Tanganyka was more and more used as a dumping area for the wastes of the agro-industrial industry. Thus three cooperation programmes were started. From Belgium a funding by BADC allowed setting up of a demonstration centre and laboratory to promote research on biomethanation, to train personnel and permit scientific and technical follow-up of biomethanation projects. Three Chinese experts were recruited to help realize pilot digesters in all the provinces. And finally a programme funded by West Germany realized the dissemination of the technology through the country with the training of peasants via local training centres.

Resumen El programa de biogas de Burundi empezó con la creación de un nuevo departamento con la misión de popularizar nuevas fuentes de energía. El programa de biogas empezó porque (1) se creyó necesaria la formación de personal y el establecimiento de un inventario de la biomasa biodegradable en Burundi, (2) el hábitat muy desperdigado complica tanto la recolección de basuras domésticas como la distribución de gas, (3) existen comunidades numerosas como escuelas, hospitales etc. y (4) el lago Tanganika se estaba utilizando cada vez más como un basurero para los desechos de la industria agro-industrial. Así pues se iniciaron tres programas de cooperación: Bélgica mediante la financiación del BADC permitió la creación de un centro de demostración y de un laboratorio para promocionar la investigación en biometanación, formar al personal y permitir un seguimiento científico y técnico de los proyectos de biometanación. Se ha obtenido la colaboración de tres expertos chinos para ayundar a la realización de digestores piloto en todas las provincias. Y finalmente un programa financiado por Alemania Occidental se encarga de la distridución de la tecnología por todo el país formando campesinos a través de centros de formación locales.

     

Performances diagnostiques actuelles du coroscanner dans la détection de la maladie coronaire

Recent advances in multislice computed tomography (MSCT), which might represent a minimally invasive alternative to coronary angiography (CA), have allowed the ability to analyze coronary arteries. In this review, we present the current diagnostic performance of MSCT, as compared with the reference standard (CA), for the assessment of native coronary arteries, coronary in-stent restenosis and coronary artery bypass graft.     

Étude de la cytodifférenciation du rein de l'Epinoche femelle après traitement par la méthyltestostérone

Résumé Au microscope électronique, l'action de la méthyltestostérone sur les cellules rénales de l'Epinoche femelle se traduit par une cytodifférenciation conduisant à la formation de cellules glandulaires muqueuses. Elle a lieu simultanément à deux niveaux distincts:Au niveau du tubule proximal, le premier signe visible de cytodifférenciation est une activation du nucléole, accompagnée par une augmentation de taille des cellules. Puis on assiste à un développement de l'ergastoplasme et de l'appareil de Golgi et à l'élaboration de deux types de sécrétions: d'abord des granules de 2000–2500 Å, ensuite des grains de mucigène typiques, qui subissent rapidement une transformation muqueuse.Une cytodifférenciation régressive intervient en même temps. Elle concerne la pinocytose apicale qui disparaît.Au niveau des tubules collecteurs, la cytodifférenciation se traduit par la formation d'un mucus hyalin d'origine golgienne. Elle s'accompagne également d'une disparition de la pinocytose.La méthyltestostérone est capable de provoquer, chez la femelle, une cytodifférenciation rénale identique à celle que l'on observe chez le mâle pendant la période de reproduction. La transformation muqueuse des cellules rénales est donc sous le contrôle de la seule testostérone, qui déclenche au niveau cellulaire un ensemble de processus conduisant à la formation de mucus.Au microscope électronique, on constate que l'élaboration de mucus prêt à l'excrétion est achevée au bout de trois jours dans les tubules proximaux alors que dans les tubules collecteurs elle ne demande que 24 heures.

Action of methyl testosterone on the cytodifferentiation of the kidney of the female three-spined Stickleback

Summary At the microscopic level, the action of methyl testosterone on the cells of the kidney of the female three-spined Stickleback gives raise to a cytodifferentiation which leads to the formation of mucous glandular cells. This action is evident at two different levels:At the level of the proximal tubule, the first visible sign of cytodifferentiation is an activation of the nucleolus, accompanied by a growth of the cell size. Then a rapid development of the ergastoplasm and the Golgi apparatus takes place, which leads to the elaboration of two types of secretory particles: granules of 2000–2500 Å in diameter appear first, then typical mucigen granules become visible. These latter undergo a rapid mucous transformation.A regressive cytodifferentiation occurs at the same time. It concerns the apical pinocytosis which disappears in the cells undergoing the glandular differentiation.At the level of the collecting tubules, the cytodifferentiation is characterized by the elaboration of a clear mucus which originates in the Golgi apparatus and migrates to the apex of the cells. A disappearance of the pinocytosis is also noticeable.In the kidney of the female, methyl testosterone induces a cytodifferentiation which is identical to that occuring in the male during the breeding period. So the mucous transformation of renal cells is under the control of a single hormone: the testosterone, which is able to give raise to a succession of phenomena leading to the formation of a mucous secretion.With the electron microscope, it is possible to demonstrate that three days are necessary for the elaboration of the mucus ready to be discharged in the lumen of the renal proximal tubules. In the collecting tubules, the reaction occurs more quickly, after only two days of treatment.
