Since the initial demonstration in 1982 of profound phenotypic effects stemming from the expression of a single transgene, genetic engineering has revolutionized fundamental biological and biomedical research. The application of transgenic technology to farm animals has held the promise of being able to improve animal agriculture significantly and has resulted in a new industry, i.e., the successful expression of foreign proteins in the mammary gland for the pharmaceutical industry. Work over the last few years in model species (e.g., the mouse) and new technical developments such as cloning have now set the stage for the initial application of transgenic technology for the improvement of farm animals. Major limitations that remain are the lack understanding of which genes we should transfer in order to alter quantitative production traits usefully and the low efficiency of producting transgenic founders. Furthermore, more research is needed concerning the consequences and potential problems arising from the integration of transgenes into populations with varying genetic backgrounds. Recent advances suggest that within the first decade of the 21 st century the first transgenic animals will become available to the livestock industry, with acceptance depending upon their cost versus their potential economic benefit to the producers.  相似文献

AIMS: To assess the evolution and patterns of obesity in countries of the WHO European Region with a particular focus on the Eastern European countries, and to discuss the health and economic implications of obesity for those countries. METHODS: The available data on overweight and obesity in children and adults for the countries of the WHO European countries were collated from the International Obesity TaskForce database and considered in the light of estimates for the costs of obesity-related ill health. RESULTS: Overweight and obesity in most countries of Europe show rising secular trends, and are predicted to continue rising if not addressed. Estimates of the costs to the health services and to economic productivity indicate that some countries may find it hard to cope with the burden of obesity: up to 6% of total health care costs and as much in indirect costs of lost productivity could be attributed to obesity and its associated illnesses. CONCLUSIONS: Transition, despite the many benefits it has undoubtedly conferred to the population living in the Region, has also entailed the collateral damage of a fast growing obesity challenge. Policy-makers in the new and candidate EU countries as well as other countries of the European Region can learn from the negative Western European and global experience, act now to stem the obesity epidemic from further developing and in so doing, reduce the substantial economic losses associated with obesity. Local, national and international strategies will be needed to combat the problem.  相似文献

Background

There is considerable international interest in exploiting the potential of digital solutions to enhance the quality and safety of health care. Implementations of transformative eHealth technologies are underway globally, often at very considerable cost. In order to assess the impact of eHealth solutions on the quality and safety of health care, and to inform policy decisions on eHealth deployments, we undertook a systematic review of systematic reviews assessing the effectiveness and consequences of various eHealth technologies on the quality and safety of care.

Methods and Findings

We developed novel search strategies, conceptual maps of health care quality, safety, and eHealth interventions, and then systematically identified, scrutinised, and synthesised the systematic review literature. Major biomedical databases were searched to identify systematic reviews published between 1997 and 2010. Related theoretical, methodological, and technical material was also reviewed. We identified 53 systematic reviews that focused on assessing the impact of eHealth interventions on the quality and/or safety of health care and 55 supplementary systematic reviews providing relevant supportive information. This systematic review literature was found to be generally of substandard quality with regards to methodology, reporting, and utility. We thematically categorised eHealth technologies into three main areas: (1) storing, managing, and transmission of data; (2) clinical decision support; and (3) facilitating care from a distance. We found that despite support from policymakers, there was relatively little empirical evidence to substantiate many of the claims made in relation to these technologies. Whether the success of those relatively few solutions identified to improve quality and safety would continue if these were deployed beyond the contexts in which they were originally developed, has yet to be established. Importantly, best practice guidelines in effective development and deployment strategies are lacking.

Conclusions

There is a large gap between the postulated and empirically demonstrated benefits of eHealth technologies. In addition, there is a lack of robust research on the risks of implementing these technologies and their cost-effectiveness has yet to be demonstrated, despite being frequently promoted by policymakers and “techno-enthusiasts” as if this was a given. In the light of the paucity of evidence in relation to improvements in patient outcomes, as well as the lack of evidence on their cost-effectiveness, it is vital that future eHealth technologies are evaluated against a comprehensive set of measures, ideally throughout all stages of the technology''s life cycle. Such evaluation should be characterised by careful attention to socio-technical factors to maximise the likelihood of successful implementation and adoption. Community structure or species composition of benthic invertebrates has frequently been used in environmental monitoring and assessment of aquatic systems. Three general approaches have been taken: the saprobic approach, which requires detailed knowledge of taxonomy and is most effective in measuring impacts from sewage effluents; diversity indices, which do not require detailed knowledge of species requirements but ignore information provided by important species and tend to lose information; and biotic indices, which combine both approaches. In the past few years considerable advances have been made by applying multivariate statistical techniques to large data matrices and relating benthic community structure to key environmental variables. Using these techniques it is possible to establish reference communities for a set of environmental conditions, to predict the benthic community that should occur at new sites and thus measure deviation from an expected community type. This suggests that environmental criteria and objectives can be established based on biological variables as opposed to the more traditional chemical approach.Measurement of ecosystem health using functional attributes of benthic invertebrates is generally in the development stage. In the future, functional measures of ecosystem health, such as chronic measures of toxicity or stress, should be incorporated into any assessment process.  相似文献

A significant quantum of crude oil is trapped in reservoirs and often unrecoverable by conventional oil recovery methods. Further downstream, the petroleum industry is facing challenges to remove sulfur, metal, nitrogen as well as undesirable organic compounds from the crude.Conventional secondary recovery methods such as water and gas injections helped to increase the productivity of the well, while chemical and physical refinery processes such as hydrodesulfurization, desalting, and high-pressure high-temperature hydrotreating remove most inorganic impurities. The increasing demand for oil in the world coupled with very stringent environmental laws piled economical and technical pressure upon the refinery industry to further improve crude oil recovery as well as reduce sulfur, metal and nitrogen concentration to the low ppm levels.In the search for economical and environmentally friendly solutions, growing attention has been given to biotechnology such as the use of microbial enhanced oil recovery (MEOR). MEOR is an alternate recovery method that uses microorganisms and their metabolic products. In addition, the emerging field of crude oil refining and associated industrial processes such as biodesulfurization, biodemetallation, biodenitrogenation and biotransformation are also covered.This review aims to provide an overview on MEOR and biorefining relevant to the petroleum industry and highlights challenges that need to be overcome to become commercially successful. Literature pertaining to laboratory experiments, field trials and patents are included in view of industrial applications and further developments.  相似文献
