目的:探究高龄患者泌尿系统感染大肠埃希菌对常用药物的耐药性,指导临床医生合理用药。方法:按照标准操作规程采集我院泌尿系统感染高龄患者的尿液,作常规尿标本培养和分离,应用微生物分析仪进行细菌鉴定,采用纸片扩散法进行药物敏感试验,采用纸片扩散法和双纸片确证法完成产超广谱β-内酰胺酶菌株的检测。结果:129株大肠埃希菌中,检测出产超广谱β-内酰胺酶菌株有62株,占48.1%;在17种常用抗生素敏感试验中,亚胺培南敏感性最高(100%),无耐药株出现,对第一、二、三和四代头孢类抗生素均出现了不同程度的耐药性,对氨苄西林、四环素和哌拉西林高度耐药(均超过了95%)。结论:泌尿系统感染高龄患者大肠埃希菌对临床常用抗生素耐药性逐年升高,且产超广谱β-内酰胺酶菌株也逐渐增多,这就要求临床医生严格合理应用抗生素,尽量避免耐药菌株的出现。  相似文献

分析2010年1月至2011年12月牡丹江医学院附属红旗医院临床分离的大肠埃希菌的耐药性变化,为指导临床合理用药提供依据。2010至2011年临床各科室送检的全部标本(包括血、尿、便、痰、分泌物等),采用微生物分析仪进行分离鉴定及药物敏感性分析。2010和2011年产ESBLs大肠埃希菌检出率分别为23.8%5、7.4%,呈上升趋势。产ESBLs大肠埃希菌对大多数抗菌药物的耐药率明显高于非产ESBLs大肠埃希菌(P<0.05)。产ESBLs大肠埃希菌检出率越来越高,耐药率日益严重,给临床抗菌治疗带来很大困难,开展产ESBLs大肠埃希菌耐药性分析,指导临床合理应用抗生素,对控制大肠埃希菌对抗菌药物的耐药性增长有重大意义。  相似文献

目的分析女性患者泌尿系统感染大肠埃希菌的耐药性,为临床合理使用抗生素提供依据。方法采用全自动微生物鉴定系统对女性泌尿系统感染患者分离出的病原菌进行鉴定和药敏分析。结果大肠埃希菌非产ESBLs株对氨苄西林、第一、二代头孢菌素类药品高度耐药。大肠埃希菌非产ESBLs株对碳青霉烯类药物美罗培南无耐药,对第三代头孢菌素敏感率高,耐药率低。产ESBLs大肠埃希菌,仅碳青霉烯类抗生素美罗培南敏感。结论女性泌尿系统感染大肠埃希菌的多重耐药性日趋严重,需加强对抗生素使用的规范化管理,合理使用抗菌药物,控制耐药菌的传播和流行。  相似文献

目的 分析2010—2015年重庆地区儿童感染的11039株大肠埃希菌的临床分布特征及耐药性,为合理应用抗菌药物和预防控制医院感染提供依据。方法 分析大肠埃希菌对19种抗菌药物的耐药性,采用BD Phoenix 100 MIC法结合K-B纸片扩散法进行药敏试验,按美国临床实验室标准化委员会(CLSI)标准判断结果。结果 大肠埃希菌总检出率为13.53%;大肠埃希菌在呼吸病房和新生儿病房的检出率高于其他病房;以痰标本多见,占62.22%(6869/11039);大肠埃希菌对氨苄青霉素耐药率最高(90.33%),对美罗培南、亚胺培南的耐药率低,分别为0.81%和0.78%。ESBLs平均检出率为50.98%(5628/11039)。结论 大肠埃希菌的耐药性呈上升趋势,呼吸病房和新生儿病房是预防控制的重点科室。  相似文献

目的对开封市鼓楼区从业人员开展大肠埃希菌耐药性调查。方法对2300名健康从业人员分离到2010株大肠埃希菌用14种抗生素进行药敏试验。结果75.0%以上菌株对青霉素、氨苄西林、氯林克霉素和红霉素耐药,80%以上对先锋V、头孢三嗪、头孢哌酮敏感。71.0%菌株呈多重耐药性,耐药谱以青霉素类、大环内酯类、氨基糖苷类为主。结论导致大肠埃希菌高耐药性的原因是:(1)本地区抗生素使用造成的选择性压力;(2)抗生素的滥用;(3)不良卫生习惯。  相似文献

目的了解深圳市人民医院高产AmpC酶大肠埃希菌的存在现状及其耐药性。方法采用Tris-ED-TA纸片裂菌法检测148株大肠埃希菌的AmpC酶。用琼脂稀释法测定产酶株对11种抗生素的最低抑菌浓度(MIC)。结果 148株大肠埃希菌中6株检出AmpC酶,检出率为4.1%。所有产AmpC酶大肠埃希菌对亚胺培南敏感,MIC为0.12~0.25μg/ml;对头孢吡肟的MIC值也较低,为0.5~32.0μg/ml,仅1株耐头孢吡肟。所有产AmpC酶大肠埃希菌对头孢西丁耐药,MIC为32~128μg/ml;对氨苄西林/舒巴坦、阿莫西林/克拉维酸和哌拉西林/他唑巴坦等加酶抑制剂复合物耐药性较强。结论深圳市人民医院大肠埃希菌高产AmpC酶检出率为4.1%。产酶株对多种抗生素耐药性较高。  相似文献

目的 研究临床分离的大肠埃希菌对常用抗生索的耐药性及其基因分型,了解其耐药性趋势与传播流行情况,为临床合理治疗大肠埃希菌引起的感染提供参考依据。方法 采用常规鉴定技术鉴定细菌;采用K—B纸片扩散法测定77株大肠埃希菌对19种药物的耐药性;K—B法鉴定产超广谱β-内酰胺酶(ESBLs);通过脉冲场凝胶电泳(PFGE)法对其进行基因分型以确定菌株之间的亲缘关系;FINGERPRINT Ⅱ软件进行细菌基因指纹图谱分析。结果 大肠埃希菌对青霉素类、喹诺酮类药物和氨曲南的耐药性明显增高,亚胺培南和美罗培南是大肠埃希菌感染患者的首选药物;经ESBLs确证试验,ESBLs阳性率为28.60%(22/77);产ESBLs大肠埃希菌经PFGE指纹图谱分析,除第62株和第70株相似性系数为78.27%外,其余相似度均低于70.0%;ESBLs大肠埃希菌阴性株中除少数几对菌株相似性系数较高外,其余呈散在分布,且电泳带存有6条以上的不同条带,为流行病学无关的不同克隆。结论 大肠埃希菌对常用抗生索耐药性明显增高,且呈多重耐药趋势;该研究尚不能证明存在大肠埃希菌爆发性流行感染,提示可能存在院内感染大肠埃希菌的优势克隆;PFGE基因分型方法是耐药性与流行状况分析的有效手段。  相似文献

目的 :研究大肠埃希菌 (EC)对多种抗生素的耐药性。方法 :用 AMS法检测 173株 EC临床菌株的耐药性。结果 :EC对亚胺培南、头孢他啶、头胞噻肟、丁胺卡那霉素的耐药率为 1.2 %~ 3.5 % ,对氨曲南、头孢西丁、呋喃妥因的耐药率为 8.1%~ 10 .4% ,对头孢呋辛钠、头孢呋辛脂、妥布霉素、头孢噻吩的耐药率为 11.6 %~ 19.6 % ,对头孢唑啉、替卡西林 /克拉维酸、环丙沙星、头孢噻吩、庆大霉素、替卡西林和美洛西林的耐药率为 2 7.7%~ 5 9.0 % ,对哌拉西林、四环素、氨苄西林、羧苄西林的耐药率为 6 1.3%~ 76 .9%。在五类标本中 ,不同标本分离的 EC对 1~ 13种抗生素的耐药性差异有显著性 ,P<(0 .0 5 0~ 0 .0 0 5 )。在 2 2种抗生素中 ,15种抗生素对 2~ 6组配对标本分离 EC的敏感性差异有显著性 ,(P<(0 .0 5 0~ 0 .0 0 5 )。结论 :EC对亚胺培南、头孢他啶、头孢噻肟、丁氨卡那霉素的耐药率低 ,对羧苄西林、氨苄西林、四环素、哌拉西林的耐药率高 ,不同标本分离 EC对多种抗生素的耐药性差异有显著性。  相似文献

目的研究重症监护病房(ICU)患者标本中分离的大肠埃希菌的耐药情况以及耐药性水平传播的实验研究。方法采取双纸片法(K-B)检测细菌的耐药性;产超广谱β-内酰胺酶(ESBLs)大肠埃希菌为供体菌,耐利福平大肠埃希菌(对其他抗生素敏感)作为受体菌进行接合实验;采用聚合酶链反应(PCR)技术扩增整合子和耐药基因。结果30株大肠埃希菌中产ESBLs菌株检出率为46.7%;接合培养后,接合菌携带23kb和25kb大质粒,而无供体菌中一系列小质粒;供体菌和接合菌均携带I型整合子。结论大肠埃希菌耐药性严重,且呈多重耐药性;产ESBLs菌株可通过质粒和整合子将耐药基因转移给敏感菌,导致耐药性传播。  相似文献

目的研究风湿性疾病中尿路感染大肠埃希菌对左氧氟沙星耐药性及相关危险因素。方法中国医科大学附属第一医院风湿免疫科2009年1月至2013年12月125株住院患者清洁中段尿培养所分离出大肠埃希菌,依据是否耐药分为两组。分析耐药组相关危险因素。结果 5年来大肠埃希菌对左氧氟沙星耐药率呈逐渐上升趋势。耐药组对氨苄西林、环丙沙星、头孢曲松、哌拉西林、复方新诺明的耐药率高于非耐药组,两组间差异有统计学意义(P〈0.05)。慢性病程、既往喹诺酮类药物应用史、既往尿路感染、菌株产ESBLs为耐药危险因素。各风湿性疾病间耐药率比较差异未见统计学意义。结论风湿性疾病尿路感染危险因素与其他疾病尿路感染危险因素类似。临床工作中应加强对危险因素监视和控制。  相似文献

A total of 78 E. coli strains isolated from adults with different types of urinary tract infections were screened by polymerase chain reaction for prevalence of genetic regions coding for virulence factors. The targeted genetic determinants were those coding for type 1 fimbriae ( fimH ), pili associated with pyelonephritis ( pap ), S and F1C fimbriae ( sfa and foc ), afimbrial adhesins ( afa ), hemolysin ( hly ), cytotoxic necrotizing factor ( cnf ), aerobactin ( aer ). Among the studied strains, the prevalence of genes coding for fimbrial adhesive systems was 86 %, 36%, and 23% for fimH, pap , and sfa/foc , respectively. The operons coding for Afa afimbrial adhesins were identified in 14% of strains. The hly and cnf genes coding for toxins were amplified in 23% and 13% of strains, respectively. A prevalence of 54% was found for the aer gene. The various combinations of detected genes were designated as virulence patterns. The strains isolated from the hospitalized patients displayed a greater number of virulence genes and a diversity of gene associations compared to the strains isolated from the ambulatory subjects. A rapid assessment of the bacterial pathogenicity characteristics may contribute to a better medical approach of the patients with urinary tract infections.  相似文献

In 1978, 1,021 Escherichia coli strains were isolated from 105 field broilers (F) and 1,058 strains from 106 broilers in a zootechnical experiment station (Z), and their drug-resistance patterns and the presence of conjugative R plasmids were compared. The resistance markers examined were tetracycline (TC), chloramphenicol (CM), streptomycin (SM), sulfonamides (SA), kanamycin (KM), and ampicillin (APC). The populations of individuals that excreted resistant strains were 100% in F and 58% in Z. Frequencies of isolation of drug-resistant strains among the total isolates were 93% in F and 36% in Z, indicating that the resistant strains are a rather high proportion of the intestinal flora in F but are slightly less prevalent in Z. The resistance pattern to (TC.SM.SA.KM) was seen at the highest frequency in both groups. Conjugative R plasmids were demonstrated more frequently in field broilers (F). The results reflect the wide use of antibiotics in the livestock industry, resulting in the appearance of drug-resistant strains mostly due to the presence of R plasmids.  相似文献

目的探讨大肠埃希菌尿路感染的临床发病特点及对抗生素的耐药情况。方法对2001年1月至2005年12月尿路感染患者尿培养分离出的156株大肠埃希菌进行耐药性分析,用纸片扩散法表型确证试验检测ESBLs。结果大肠埃希菌耐药率最低的抗菌药物是亚胺培南、美罗培南、头孢哌酮/舒巴坦、氧哌嗪青霉素/他唑巴坦,分别为1.28%、1.92%、3.21%、5.13%;对氨苄西林、氟喹诺酮类、庆大霉素、复方新诺明耐药率均>70%。产ESBLs的大肠埃希菌对亚胺培南、美罗培南、头孢哌酮/舒巴坦、氧哌嗪青霉素/他唑巴坦的耐药性均<10%,对氨苄西林、头孢菌素类、氟喹诺酮类均表现出很强的耐药性。结论治疗大肠埃希菌感染时,需根据药敏结果选用碳青霉烯类、β-内酰胺类/β-内酰胺酶抑制剂等。  相似文献

Abstract We have investigated the production of heat-labile enterotoxin (LT), Verotoxin (VT), cytotoxic necrotizing factor (CNF), haemolysis (Hly) and lethal activity for mice in 48 Escherichia coli strains isolated from patients with urinary tract infections. Mannose-resistant and mannose-sensitive haemagglutination in these strains were also studied. Among the total strains investigated, 50% were haemolytic, 50% synthesized CNF and 58% were lethal for mice. A total of 33 (69%) strains were toxigenic, showing positive results at least in one of the tests employed for toxin detection. No strain was positive for LT and VT production. We conclude that, in addition to haemolytic and haemagglutinating activities, the production of CNF was closely associated to virulence in E. coli strains isolated from urinary tract infections.  相似文献

The urinary tract is among the most common sites of bacterial infection and E. coli is by far the most common infecting agent in children and adults of both sexes. In an attempt to evaluate the intrinsic virulence of E. coli uroisolates from children, 54 strains were assessed by using PCR for the presence of five representative genetic determinants coding for adherence systems (pap, sfa/foc, afa), and toxins (hly and cnf). The prevalence of pap, sfa/foc and afa genes was 55%, 54%, and 44%, respectively. Hemolysin-encoding gene hly was detected in 55% strains, while cnf was exhibited by 35% of the screened E. coli isolates. Among the 39 PCR positive strains isolated from children's urine cultures the co-occurrence of the various targeted virulence genes was detected in 30 strains, the virulence profiles identified suggesting the presence of their localization on chromosomal regions known as pathogencity-associated islands. The rapid and reliable detection of the intrinsic virulence potential by this molecular approach could be very useful when evaluating the importance of microorganism pathogenicity versus host's susceptibility for developing an overt symptomatology of infection.  相似文献
