股骨头坏死是一种常见的疾病,在30岁至60岁年龄段的人群中较为常见,临床的症状包括疼痛以及髋部不适等,股骨头坏死在早期很难发现,由于没有得到准确的诊断,耽误了最佳的治疗时间和有效的治疗,随着病情的发展,最终将会造成股骨头变形以及塌陷,从而引起骨性关节炎,对髋关节功能的影响是很大的,甚至会丧失髋关节的基本功能。股骨头坏死的病状体征和早期症状存在一定的隐蔽性,因此,造成误诊的情况频繁发生。此外,有些疾病的症状表现为髋关节疼痛,最后反而容易被误诊为股骨头坏死。  相似文献

We were interested in whether or not the biomechanical status of the hip influences the course of avascular necrosis of the femoral head. To investigate this, we used a computer aided system based on a three dimensional mathematical model for determining the stress distribution in the hip joint from standard anteroposterior rentgenographs (X-ray images) of both hips and pelvis. Based on the results of our study, we suggest that the biomechanical parameters of the hip play an important role in the outcome of treatment of hips affected by avascular necrosis of the femoral head.  相似文献

目的初步探讨血清病动物激素性骨坏死的发病机制。方法将新西兰大白兔分为三组,A组联合应用马血清和地塞米松磷酸钠诱导骨坏死模型,B组单纯应用地塞米松磷酸钠,C组应用生理盐水作为对照组,对三组实验兔股骨头进行HE、MRI观察,并对各组兔的血脂、AST和ALT进行检测,分析比较。结果A组出现了典型的股骨头缺血坏死改变,血脂、AST和ALT与另外两组相比差异有显著性。单纯应用激素组和对照组未出现股骨头缺血坏死。结论运用马血清和地塞米松磷酸钠可成功诱导出兔骨坏死模型,血管炎、血脂代谢异常和肝功能损害共同导致股骨头缺血坏死发生,短期应用大剂量激素难以诱导出骨坏死模型。  相似文献

Three clinical cases of canine avascular femoral head necrosis and 4 cases of hip dysplasia were examined using intraosseus femoral neck venography. The contrast medium passed initio the diaphyseal bone marrow in all cases. Three growing dogs and 7 growing pigs were examined with the same method, before and after experimental ligation of the femoral veins. Before the venous tamponade, no contrast medium was visible in the femoral neck. The venography performed immediately after ligation showed contrast flow into the femoral neck similar to that seen in the clinical cases of Legg Perthes disease or hip dysplasia. However, a collateral circulation developed within 7 weeks. No more contrast-flow into the diaphysis was observed after that time. Although osteomedullography shows that both in Perthes disease and in hip dysplasia venous drainage has failed, venous tamponade may not induce the onset of the disease.  相似文献

目的:观察髋关节置换术治疗股骨头缺血性坏死临床可行性和疗效。方法:后侧入路小切口行全款关节置换术,选取55例患者作为临床研究对象共62髋,进行12个月以上随访平均随访(15.5±2.5)个月。结果:术后患者功能恢复较好,假体位置及周围组织结合良好,Harris评分比术前提高45分以上,临床效果值得肯定。结论:通过外后侧入路髋关节置换治疗股骨头缺血性坏死可达到预期疗效,术前的充分准备对手术成功具有积极作用,可以作为减轻患者自身痛苦和降低医疗费用临床治疗方式。  相似文献

Avascular necrosis of the femoral head is one of the common problems in orthopedic practice in Taiwan. The subchondral bone loses its blood supply which weakens its biomechanical support. Steroid overuse is one of many possible etiologies in reducing blood flow to the femoral head. Laser Doppler velocimeter is a precise monitor of regional blood flow of bone which is expressed in perfusion units (PU). In the control group the rabbits were injected with normal saline and there were no statistical differences between blood flow to the right hip (39.26±5.64 PU) and left hip (38.58±4.35 PU). In group B a weekly injection of methylprednisolone into rabbits for 6 weeks demonstrated the reduction of blood flow of femoral head (24.74±3.13 PU) by the laser Doppler velocimeter. The flow decreased further (15.93±2.33 PU) by 12 weeks of steroid treatment. In group C after a weekly injection of steroid for 6 weeks the flow became 31.63±4.79 PU. The steroid was then discontinued for 3 weeks and the flow was 34.6±1.34 PU. In group D the blood flow was 25.89±4.01 PU after 6 weeks of steroid treatment and we stopped the steroid for 6 weeks, the blood flow became 29.86±2.59 PU. The merit of our experiment established a model of study in avascular necrosis of the femoral head in rabbits.  相似文献

目的 研究自体骨髓单个核细胞移植对股骨头坏死患者缺血状态的改善程度和治疗效果.方法 选取2004 年7 月至2010 年11 月期间187 例252 髋股骨头坏死患者,应用自体骨髓单个核细胞移植治疗,分别采集 187 例患者骨髓200 -360 ml,采用 Ficoll 密度梯度离心法分离单个核细胞,单个核细胞总数为(2.4 ~ 7.8)× 108 个,流式细胞仪检测 CD34+ 细胞和 CD133+ 细胞在所分离出的干细胞悬液中的含量分别为2.47﹪± 0.58﹪和1.29﹪± 0.35﹪,然后将单个核细胞用生理盐水制备成细胞悬液 20 - 30 ml,使用数字减影血管造影技术(DSA)行超选择性股骨头供血动脉内干细胞移植术.按世界骨循环研究学会(ARCO)对骨坏死分期,设自身前后对照观察疗效.移植术后第 3、6、12、24、36 和48 个月,根据髋关节 Harris 评分评价疗效,移植术后6 个月通过复查患者股骨头供血动脉 DSA,观察其新生血管形成情况,以后每隔 6 个月采用影像学方法观察股骨头形态学变化.结果 (1)临床疗效:对接受自体骨髓单个核细胞移植治疗的187 例患者随访 3 ~ 48(24.2 ± 4.5)个月,其中髋关节疼痛缓解者 158 例(占患者总数的 84.5﹪),髋关节功能改善者 146 例(占患者总数的 78.1﹪),行走间距延长者 149 例(占患者总数的 79.7﹪);(2)影像学检查:干细胞移植术后 6 个月187 例患者中54 例行股骨头供血动脉 DSA 检查,48 例显示供血动脉较移植术前明显增多、增粗,血流速度增快,12 ~ 24个月后 72 例患者股骨头区骨质病变获得改善.结论 超选择性动脉内骨髓单个核细胞移植方法简便、安全有效,对因缺血导致坏死的股骨头无再次损伤,能够有效治疗缺血性股骨头坏死.  相似文献

Four of 10 children with SLE kept under observation over the past nine years have developed avascular necrosis (AN) of the femoral head. The symptoms of AN are insidious and unpredictable and predate the radiologic diagnosis by weeks to months. In a comparison of these children with SLE, with and without AN, with a group of patients with nephrosis treated with corticosteroids and a group with glomerulonephritis treated with azathioprine, AN was related to the duration of daily steroid therapy rather than the total duration of steroid treatment; this was not true for azathioprine. The occurrence of AN in our patients while they were on alternate-day steroid therapy, or coincident with a relapse, suggests that its development is determined by underlying disease.  相似文献

A female patient, 36 years of age, with a metastasised left breast cancer received several courses of chemotherapy for aggressive local tumour growth and multiple metastatic activity. In the current patient, surgical ablation of the left breast was carried out. Also loco-regional radio-therapy was conducted. To facilitate the administration of chemotherapy courses and prevent thrombophlebitis a vascular access port (port-a-cath) was surgically inserted via the right subclavian vein. After a few successful administrations of chemotherapeutic drugs the vascular port stopped functioning. It was demonstrated that a detached catheter fragment had dislodged into the right ventricle. Successful percutaneous, transvenous removal of the entrapped catheter fragment by the Gooseneck retrieval loop snare from the right ventricle was performed via the right femoral vein access. The procedure was uncomplicated and the patient tolerated the procedure well.  相似文献

目的:探讨人工髋关节置换术在治疗股骨头缺血性坏死(ANFH)中的临床疗效。方法:选择2007年2月-2011年2月我院收治的320例(340髋)股骨头缺血性坏死患者,均采用人工髋关节置换术对患者进行治疗,其中160例(172髋)患者应用骨水泥型假体进行治疗,另外160例(168)患者采用非骨水泥型假体进行治疗。采用Harris评分对患者手术前后的髋关节功能情况进行评价,并比较骨水泥治疗组和非骨水泥治疗组的临床疗效。结果:患者均获得随访,随访时间为3~18个月。全部患者手术后的Harris评分明显高于手术前,差异有统计学意义(P〈0.05)。骨水泥治疗组和非骨水泥治疗组在术后出血量、术后Harris评分及住院时间方面的差异无统计学意义(P〉0.05),但非骨水泥治疗组的并发症发生率明显低于骨水泥治疗组(P〈0.05)。结论:采用人工髋关节置换术治疗ANFH疗效显著,能明显改善患者的生活质量,骨水泥型假体与非骨水泥型假体的治疗效果相当,应根据患者的具体情况进行合理的选择。  相似文献

目的:探讨介入治疗股骨头缺血坏死的临床应用价值。方法:采用Seldinger技术对28例患者行超选择股骨头供血动脉插管,动脉造影、溶通治疗。结果:28例患者治疗后髋部疼痛及关节功能障碍均有不同程度的减轻及改善,有效率96.4%;治疗后病变区血管分支增多、增粗;影像随访显示股骨头骨质不同程度修复。结论:介入治疗具有创伤小、并发症低、血管再通率高、临床症状改善明显的优势,能有效的改善股骨头的局部血液循环和髋关节的疼痛、活动功能,应大力推广。  相似文献

Introduction

Treatment of femoral neck fractures in young adults may require total hip arthroplasty or hip hemiarthroplasty using a bipolar cup. The latter can, however, result in migration of the femoral head and poor long-term results.

Case presentation

We report a case of femoral head migration after hemiarthroplasty performed for femoral neck fracture that had occurred 22 years earlier, when the patient (a Japanese man) was 20 years old. He experienced peri-prosthetic fracture of the femur, subsequent migration of the prosthesis, and a massive bone defect of the pelvic side acetabular roof. After bone union of the femoral shaft fracture, the patient was referred to our hospital for reconstruction of the acetabular roof. Intra-operatively, we placed two alloimplants of bone from around the transplanted femoral head into the weight-bearing region of the acetabular roof using an impaction bone graft method. We then implanted an acetabular roof reinforcement plate and a cemented polyethylene cup in the position of the original acetabular cup. Eighteen months post-operatively, X-rays showed union of the transplanted bone.

Conclusions

Treatment of femoral neck fractures in young adults is usually accomplished by osteosynthesis, but it may be complicated by femoral head avascular necrosis or by infection or osteomyelitis. In such cases, once an infection has subsided, either hip hemiarthroplasty using a bipolar cup or total hip arthroplasty may be required. However, if the acetabular side articular cartilage is damaged, a bipolar cup should not be used. Total hip arthroplasty should be performed to prevent migration of the implant.  相似文献

目的:探讨病灶清除植骨联合动力髋螺钉(DHS)内固定术治疗股骨近端囊肿的临床疗效及意义。方法:自2006年3月至2012年6月共有17例股骨近端囊肿患者在我院接受治疗并有完整随访,全部患者均应用病灶清除植骨联合DHS内固定术手术治疗。结果:平均随访时间18个月(14-60个月),病灶植骨处平均愈合时间7个月(6-8个月)。平均手术时间182分钟(90分钟~282分钟),术中平均出血量340 mL(100~700 mL)。切口全部一期愈合,术后病灶处均达到骨性愈合,骨密度均匀,17例均未出现骨囊肿复发及内固定断裂,未发生股骨头坏死及股骨近端骨折,按Harris髋关节功能评分标准,优良率为100%。结论:病灶清除植骨术联合DHS内固定治疗股骨近端囊肿操作简单,术中出血少,并发症少,疗效确切,是治疗股骨近端骨囊肿,降低其复发率和预防病理性骨折的一种有效方法。  相似文献

目的:分析与股骨头缺血坏死相关临床与影像资料,以期提高影像科医生对该病的认识水平,提高股骨头缺血坏死的诊断正确率,防止与其相关的髋关节疾病的误诊。方法:回顾性分析26例误诊为股骨头缺血坏死的髋关节疾患,归纳分类疾病的种类,找出原因。结果:类风湿性髋关节炎、强直性脊柱炎髋关节炎和痛风性髋关节炎、先天性髋臼发育不良、股骨颈干角发育异常、髋关节骨关节病等疾病易与股骨头缺血坏死相混淆,引起误诊。结论:病史采集不详细、思路狭窄,对类风湿、强直性脊柱炎及痛风性关节炎所致髋关节病变认识不足或不了解、股骨头缺血坏死病理过程不清楚、临床误导等因素与股骨头缺血坏死的误诊有关。  相似文献

目的:观察营养动脉结扎和液氮冷冻对股骨头内局部氧分压的影响,为后期制作极度低氧股骨头缺血坏死模型打下基础。方法:9只雄性大耳白兔随机平均分为对照组、液氮冷冻组和动脉结扎+液氮冷冻组,对照组动物不做任何处理,液氮冷冻组动物行股骨头液氮冷冻使其发生缺血坏死,动脉结扎+液氮冷冻组动物先结扎旋股内外侧动脉,再对股骨头进行液氮冷冻。造模后立刻活体观察股骨头内氧分压变化。结果:液氮冷冻组股骨头内氧分压较对照组下降一半左右,2组比较有显著性差异(P〈0.05);动脉结扎+液氮冷冻组动物股骨头内氧分压进一步下降到对照组的1/8左右,与其他2组相比有显著性差异(P〈0.05)。结论:液氮冷冻和旋股内、外侧动脉结扎均可明显降低股骨头内氧分压,两者合用具有重叠效应,可共同促使股骨头内氧分压下降。  相似文献

Cancer cells escape apoptosis by intrinsic or acquired mechanisms of drug resistance. An alternative strategy to circumvent resistance to apoptosis could be through redirection into other death pathways, such as necrosis. However, necrosis is a nonspecific, nontargeted process resulting in cell lysis and inflammation of both cancer and normal cells and is therefore not a viable alternative. Here, we report that a C-terminal peptide of p53, called p53p-Ant, induced targeted necrosis only in multiple mutant p53 human prostate cancer lines and not normal cells, because the mechanism of cytotoxicity by p53p-Ant is dependent on the presence of high levels of mutant p53. Topotecan- and paclitaxel-resistant prostate cancer lines were as sensitive to p53p-Ant-induced targeted necrosis as parental lines. A massive loss of ATP pools and intracellular generation of reactive oxygen species was involved in the mechanism of targeted necrosis, which was inhibited by O(2)(.) scavengers. We hypothesize that targeted necrosis by p53p-Ant is dependent on mutant p53, is mediated by O(2)(.) loss and ATP, and can circumvent chemotherapy resistance to apoptosis. Targeted necrosis, as an alternative pathway for selective killing of cancer cells, may overcome the problems of nonspecificity in utilizing the necrotic pathway.  相似文献

Early stages of avascular necrosis of the femoral head (AVNFH) can be conservatively treated with hyperbaric oxygen therapy (HBOT). This study investigated how HBOT modulates inflammatory markers and reactive oxygen species (ROS) in patients with AVNFH. Twenty-three male patients were treated with two cycles of HBOT, 30 sessions each with a 30 days break between cycles. Each session consisted of 90?minutes of 100% inspired oxygen at 2.5 absolute atmospheres of pressure. Plasma levels of tumor necrosis factor alfa (TNF-α), interleukin 6 (IL-6), interleukin 1 beta (IL-1β) and ROS production were measured before treatment (T0), after 15 and 30 HBOT sessions (T1 and T2), after the 30-day break (T3), and after 60 sessions (T4). Results showed a significant reduction in TNF-α and IL-6 plasma levels over time. This decrease in inflammatory markers mirrored observed reductions in bone marrow edema and reductions in patient self-reported pain.  相似文献

A new experimental model for free-flap transfer has been developed in the rat. This "thigh flap" is an osteomyocutaneous free flap of bone (femur), muscle (thigh), and skin (groin) based on the femoral vessels. The flap is harvested from the left groin and thigh of an inbred female rat and is transferred to a subcutaneous pocket in the left groin of a male rat of the same inbred strain. The femoral vessels supplying the flap are anastomosed end-to-end with the femoral vessels of the recipient. Thirty flaps have been transferred, with 5 technical failures. Three of the remaining 25 flaps developed necrosis within 24 hours. The other 22 flaps remained viable until the rat was sacrificed at 7 days. The survival rate of the thigh flap was thus 88 percent. The model is suitable for use in metabolic, vascular, and immunologic studies of composite free flaps.  相似文献
