Reconstruction of the pharyngoesophagus with free jejunal transfer is a major challenge when recipient neck vessels are absent because of previous surgery or irradiation. In such instances, jejunal transfer using a muscle flap as a "vascular carrier" may be a problem-solving alternative. Pretransfer vascularization of the jejunum is achieved by wrapping the muscle flap around the small bowel segment. After a short staging period, the mesenteric pedicle is divided and the bowel segment is transferred up to the neck based on its new blood supply. The objectives of this study were to develop an animal model for prefabricating independently revascularized jejunal segments using the rectus abdominis muscle flap and to determine the minimal time required for independent bowel survival. Twenty-four mature (500-g to 700-g) rats were divided into six experimental groups of four animals each. In each animal, a 1.5-cm segment of proximal jejunum was isolated on two jejunal arteries and wrapped with a superior pedicled rectus abdominis muscle flap. To determine the time of neovascular takeover, the mesenteric pedicles were ligated on postoperative day 2 (group I), day 3 (group II), day 4 (group III), day 5 (group IV), day 6 (group V), and day 7 (group VI). At the time of pedicle ligation, the composite flap was transposed to a new subcutaneous position. Viability of bowel was assessed according to gross appearance and histologic examination 48 hours after transfer. Complete survival of revascularized jejunum in 11 of 12 animals was obtained after pedicle ligation on postoperative day 5 and beyond (p < 0.0001, Fisher's exact test). These bowel segments demonstrated luminal patency, intact pink mucosa, mucus production, and visible peristalsis. Histologic examination showed healthy intestinal epithelium and tissue integration along the serosa-muscle interphase. In contrast, pedicle ligation on day 4 and earlier resulted in varying degrees of bowel necrosis characterized by flattening or ulceration of mucosa (day 4), mucosal sloughing and necrosis of mural musculature (day 3), and complete loss of bowel architecture with lumen obliteration (day 2). These findings suggest that jejunal segments may be independently revascularized with the rectus abdominis muscle flap in the rat model. Complete survival and gross normal bowel function may be obtained without mesenteric perfusion after a minimal time of 5 days.  相似文献

Enteric free flaps have proven to be useful for reconstructing the cervical esophagus. Although jejunum is favored, the rationale for this is not at all clear. We have postulated that resistance to warm ischemia varies in different regions of the gut. An experiment was carried out in 10 mongrel dogs in which 10-cm segments of proximal, middle, and distal small bowel were isolated on single vascular pedicles. In each portion of the gut there were three segments: a control, a segment subjected to 60 minutes of warm ischemia, and a segment subjected to 120 minutes of warm ischemia. The following day each animal was reexplored, and the viability of bowel segments was assessed visually and with fluorescein. All control segments were viable at 24 hours. Twenty segments were subjected to 1 hour of warm ischemia, and all but two were viable. Nineteen gut segments were subjected to 2 hours of warm ischemia. Seven of eight proximal segments were viable, two of five midsegments were viable, and zero of six distal segments were viable. Survival in the distal portion compared to the proximal portion was significantly less (p less than 0.01). It appears from this study that isolated distal small bowel segments are less resistant to warm ischemia than proximal segments.  相似文献

Vascular anatomy of the forearm muscles: a study of 50 dissections   总被引:1,自引:0,他引:1  
This anatomic study is based on 50 adult cadaver upper extremities. The general disposition of the forearm arteries and muscles and the main anatomic variations encountered are specified. Constant existence of an "anterior oblique artery" satellite of the pronator teres was established. The median nerve artery was principally dedicated to the flexor digitorum superficialis and participated appreciably in the constitution of palmar arches in only one case. A supernumerary intermedial radial muscle was found only in two cases. The abductor pollicis longus and extensor pollicis brevis appeared as a single muscular and vascular unit in 84 percent of cases. All the arteries destined for muscles were reckoned whatever their caliber might be. Despite its limitations, this study confirms the very great number of the forearm muscular pedicles. Each forearm contained an average of 264 muscular vascular pedicles. The systematization of the origins and destinations of the 13,158 muscular pedicles is described in a numbered manner for each of the 20 normal forearm muscles and for each of the 12 studied arterial segments. The pronator teres was likely to be supplied by all the anterior arteries of the upper limb. The flexor carpi radialis had one or two dominant pedicles originated from the recurrens ulnaris anterior, recurrens ulnaris, or ulnaris-interossea communis arteries, and many transversal branches originated from the radial artery. The flexor carpi ulnaris was supplied in its proximal third by the recurrens ulnaris posterior artery and in its distal two-thirds by many branches of ulnar artery.(ABSTRACT TRUNCATED AT 250 WORDS)  相似文献

The sublingua of Microcebus murinus was studied gross anatomically and light microscopically. The apex of the sublingua ends in two lobe-like projections separated by a distinct median cleft. The lobes exhibit at their oral, free margin tiny processes, but no "comb"-like specializations of intensely keratinized mucosa. At the ventral surface of the sublingua three longitudinally oriented keels or ridges are found; the mucosa of the keels is strongly keratinized, so that these structures maximize the rigidity of the undertongue. In none of the specimens examined was any trace of sublingual cartilage found; in the sublingua of Cheirogaleus (only one individual examined), the nearest relative of Microcebus, cartilage was also absent. The absence of sublingual cartilage and the incidental, irregular occurrence of cartilaginous elements in the sublingua of Tarsius and Tupaia indicate that the sublingual cartilaginous skeleton is not a derivative of the hyoid arch. It is a newly developed supporting structure which does not regularly occur. A sublingua occurs only in prosimians and Tarsius; whether the fimbria linguae of the Hominoidea is the homologue of a sublingua is still disputable. The exclusive occurrence of a sublingua in prosimians and Tarsius indicates a phylogenetic relationship between these two groups and Tupaia. The "Tupaia-problem" concerns the origin of primates and from this point it derives its general importance. At present we are far from a final solution; a definite answer cannot be expected before sufficient fossil records are available which document conclusively the historical paths of the origin of primates.  相似文献

Despite high bacterial colonization and frequent allergen contact, acute inflammatory and allergic reactions are rarely seen in the oral mucosa. Therefore we assert that immune tolerance predominates at this site and antigen presenting cells, such as dendritic cells and different T cell subtypes, serve as key players in oral mucosal tolerance induction. In this article we describe the mechanisms that lead to tolerance induced in the oral mucosa and how they differ from tolerance induced in the lower gastrointestinal tract. Furthermore we discuss ways in which novel nonparenteral approaches for immune intervention, such as allergen-specific immunotherapy applied by way of the sublingual route, might be improved to target the tolerogenic properties of the sophisticated oral mucosal immune network.  相似文献

Secondary growth in the stem of Dolichos lablab is achieved by the formation of eccentric successive rings of vascular bundles. The stem is composed of parenchymatous ground tissue and xylem and phloem confined to portions of small cambial segments. However, development of new cambial segments can be observed from the obliterating ray parenchyma, the outermost phloem parenchyma and the secondary cortical parenchyma. Initially cambium develops as small segments, which latter become joined to form a complete cylinder of vascular cambium. Each cambial ring is functionally divided into two distinct regions. The one segment of cambium produces thick-walled lignified xylem derivatives in centripetal direction and phloem elements centrifugally. The other segment produces only thin-walled parenchyma on both xylem and phloem side. In mature stems, some of the axial parenchyma embedded deep inside the xylem acquires meristematic activity and leads to the formation of thick-walled xylem derivatives centrifugally and phloem elements centripetally. The secondary xylem comprises vessel elements, tracheids, fibres and axial parenchyma. Rays are uni-multiseriate in the region of cambium that produces xylem and phloem derivatives, while in some of the regions of cambium large multiseriate, compound, aggregate and polycentric rays can be noticed.  相似文献

滇重楼[Paris polyphylla var.yunnanensis(Fr.)Hand.-Muzz.]根茎的各部位切段都能繁殖出新植株。这些后代的四年生根茎,其增长速度比同龄实生苗快3—6倍。但切段间表现出较大的差异性:带顶芽切段能于种植的第一年成苗,并开花、结实,是根茎中生活力最强的部位,但其地上茎一般为单轴;中间和末端的切段,它们的出苗情况和有性繁殖相似,第一年只有极少数出苗,第二年才大量出土成苗,但不开花,地上茎出现较多的多轴现象。后代根茎和地上茎的主要成份与野生滇重楼基本相同。  相似文献

Light microscopic sections of the adult opossum (Didelphis virginiana) spleen were observed to lack venous sinuses; this primitive mammalian spleen may be classified as non-sinusal in nature. In the spleen of the opossum, the capillary segments of the penicillar arteries lacked ellipsoid sheaths characteristic of certain mammalian spleens. Separating the lymphoid nodules from the surrounding red pulp was a distinct band of vascular tissue, the marginal zone. Arising from the central artery within the lymphoid nodule, vessels of capillary dimension were observed to terminate within the marginal zone and the area between lymphoid nodule and marginal zone. In addition to the vascular channels established by the terminal arterial vessels within the red pulp, the system of vessels within the marginal zone has been implicated as an important intermediate vascular channel within the spleen.  相似文献

Arteriovenous anastomoses in the rabbit ear were examined with scanning electron microscopy to elucidate the structural differentiation of the media of the shunt. Arterial, intermediate, and venous segments in the shunt and two layers of the media in the intermediate segment were differentiated based on cell shape and cell organization. In the arterial segment, smooth muscle cells were spindle-shaped, either elongated or short, with a few branches, and were arranged circularly or diagonally with respect to the vessel's long axis. There were also stellate muscle cells with radiating processes. In the intermediate segment, the smooth muscle cells of the outer layer of the media were also arranged circularly and resembled the elongated cells in the arterial segments, but they were more irregular in shape and had more processes than those of the arterial segment. The epithelioid cells of the inner layer of the media were oval or polygonal and oriented irregularly with respect to the vessel's long axis, clustering to form longitudinal plicae. The smooth muscle cells of the venous segment were flat with many lateral processes and formed a thin, discontinuous layer. The smooth muscle cells in the arterial segment and those of the outer layer of the intermediate segment exhibited a highly rugged surface texture, indicating their strong contractility; the epithelioid cells and the smooth muscle cells in the venous segment exhibited a generally smooth surface, indicating less contractility. The intermediate segments were supplied with a dense nerve plexus. The intermediate segments, therefore, may be actively involved in the regulation of blood flow under neuronal influence.  相似文献

Reserpine induces an appreciable reduction of the calcium content in the segments of the distal aorta and rabbit femoral arteries incubated in a saline. These changes develop simultaneously to those in the reaction pattern of the vascular segment to direct electric stimulation (decline of constrictor responses and their subsequent replacement by relaxation). It is postulated that the decreased calcium content in the vascular wall may be one of the causes of altered vascular reactivity.  相似文献

Previous studies revealed novel genetic changes in the duodenal mucosa of iron-deprived rats during postnatal development. These observations are now extended to compare the genetic response to iron deficiency in the duodenum versus jejunum of 12-wk-old rats. cRNA samples were prepared from the duodenal and jejunal mucosa of three groups each of control and iron-deficient rats and hybridized with RAE 230A and 230B gene chips (Affymetrix). Stringent data reduction strategies were employed. Results showed that several genes were similarly induced in both gut segments, including DMT1, Dcytb, transferrin receptor 1, heme oxygenase 1, metallothionein, the Menkes copper ATPase (ATP7A), tripartitie motif protein 27, and the sodium-dependent vitamin C transporter. However, a subset of genes showed regulation in only one or the other gut segment. In duodenum only, gastrokine 1, trefoil factor 1 and claudin 2 were induced by iron-deficiency. Other genes previously identified were only regulated in the duodenum. Overall, these studies demonstrate similarities and distinct differences in the genetic response to iron deprivation in the duodenum versus jejunum and provide evidence that more distal gut segments also may play a role in increasing iron absorption in iron-deficiency anemia.  相似文献

Energy-dependent accumulation of glycine and glycyl-L-valine within the small intestinal mucosa in a chicken model of in vitro local oxygenation of the small intestinal preparation was studied. It has been shown that the most effective bilateral oxygenation significantly increase accumulation of glycyl-L-valine in the proximal segment as compared to that under oxygenation only from serosal surface both in the fed and 24-hour fasted chickens, whereas in other segments these differences was less apparent. This may be due to increased H+/ peptide cotransporter expression in the proximal segment. Thus the bilateral oxygenation probably may turn on an additional amount of already existing (but non-functional during serosal oxygenation) H+/ peptide co-transporters. Moreover, low glycine transporter expression may be the reason why supplemental oxygen (bilateral oxygenation) has no effect on glycine accumulation in the distal segment of fed chickens. A 48-hour fasting decreases glycyl-L-valine accumulation in the proximal (and medial) segments, possibly due to progressive decrease in villus height. It is concluded that: a) the accumulation rate of glycine was greater when presented as the glycyl-L-valine than when presented as the equivalent amount of free amino acid; b) the rates of accumulation of glycyl-L-valine are highest in the proximal segment, decrease in the medial segment and are the lowest in the distal segment; c) the serosal oxygenation is less effective than the mucosal and bilateral oxygenation, which markedly stimulates accumulation of nutrients in the intestinal mucosa; d) a 24-hour fasting increases glycyl-L-valine accumulation in the proximal segment only, while glycine uptake was increased in the distal segment.  相似文献

This paper deals with responses to concept elements for grapefruit juice using hybrid conjoint analysis. Sensory-based concept elements perform as well (and as poorly) as do nonsensory-based concept elements. Concept-response segmentation analysis revealed three distinct groups, oriented to sweet taste, nonsweet taste, and color/appearance, respectively. Response patterns of segments are not polarized. What one segment likes most the other segment may simply accept to a moderate degree. This pattern differs from what typically happens with actual products. The segments differ in the way that they process the information in a concept, based upon response time as a separate dependent measure. Sensory-based concept elements may act as gatekeepers to assure respondents that the product is acceptable, and thus may shorten the search for additional information. In contrast, for products and sensory-preference segmentation, sensory attributes typically act as Drivers of LikingTM, rather than as gatekeepers.  相似文献

Saliva is armed with various defense mechanisms, such as the immunological and enzymatic defense systems. In addition, saliva has the ability to protect the mucosa against mechanical insults and to promote its healing via the activity of epidermal growth factor. However, another defense mechanism, the antioxidant system, exists in saliva and seems to be of paramount importance. The most interesting finding of the present study was the demonstration of the existence of much higher concentrations of the various salivary molecular and enzymatic antioxidant parameters in the parotid saliva compared with the submandibular/sublingual saliva. For example, peroxidase, superoxide dismutase, uric acid, and total antioxidant status were higher in resting parotid saliva compared with resting submandibular/sublingual saliva by 2405, 235, 245, and 147%, respectively. Another important finding was the distinction between the salivary antioxidant system and the immunological and enzymatic protective systems, as represented by the salivary concentrations of secretory IgA and lysozyme, respectively. These findings suggest that the profound antioxidant capacity of saliva secreted from parotid glands is related either to the different physiological demands related to eating (parotid predominance), to oral integrity maintenance (submandibular/sublingual predominance), or to the high content of deleterious redox-active transitional metal ions present in parotid saliva. This also may signify that our oral cavity environment is only partially protected against oxidative stress during most of the day and night.  相似文献

Models of arterial trees are generated by the algorithm of Constrained Constructive Optimization (CCO). Straight cylindrical, binary branching tubes are arranged in an optimized fashion so as to convey blood to the terminal sites of the tree, which are distributed over a predefined area, representing the tissue to be perfused. All terminal segments supply equal flows at a unique terminal pressure, and the radii of parent and daughter segments are related via a bifurcation law. The connective structure and geometry of the model are optimized according to a target function such as total intravascular volume. The shear rate between blood and the vessel walls is computed in each segment and a new method is presented for rescaling a given CCO tree to a desired value of shear rate in the root segment. The effect of viscosity varying with shear rate is evaluated and a new method is presented for rescaling a CCO-tree segment by segment to consistent values of radii and variable viscosity. Shear stress is evaluated for its deviation from being proportional to shear rate and then subjected to various types of analyses. Usually both, shear stress and its variability, are found to be larger in the smaller than in the larger segments of the CCO-model trees. However, it is shown how the shear-stress distribution can be reshuffled between small and large segments when rescaling a CCO tree to obey a different bifurcation law, while its whole geometry remains unchanged and all boundary conditions remain fulfilled. The selection of optimization target is found to drastically affect shear-stress variability within bifurcations, which reaches a distinct minimum if the model is optimized according to intravascular volume. Finally, a rank-analysis of shear stress within each bifurcation shows that only two out of six possible rank patterns actually occur: the parent segment always experiences medium shear stress while minimum shear stress resides mostly in the larger, less frequently in the smaller daughter.  相似文献

Summary In interphase nuclei of leukocytes and oral mucosa cells of normal human males and f males, two types of heterochromatin can he distinguished according to their location in the nucleus. Firstly, nucleolus-associated heterochromatin which consists of one large mass of autosomal segments surrounding the nucleolus, or several large masses if there appears to be more than one nucleolus in the same nucleus. Secondly, scattered heterochromatin composed of a large number of positively heteropycnotic bodies scattered throughout the nucleus and not directly associated with the nucleolus. The correspondence of this type of heterochromatin with chromosome segments is obtained at late prophase where several positively heteropycnotic regions belonging to the autosomes are found scattered throughout the nucleus.In human females sex-chromatin is present in addition to these two types. In leukocytes the sex-chromatin cannot be easily identified due to the large size and number of the scattered heterochromatic bodies, but in oral mucosa cells such a distinction is more easily achieved due to the smaller amount of autosomal heterochromatin.Nucleolus-associated and scattered heterochromatin from leukocytes of both sexes synthesized their DNA at a different period of time from the euchromatin. The asynchrony of replication observed in the heterochromatin at interphase is in agreement with the asynchrony between autosomes and within autosomes described by many authors at metaphase. This does not mean, however, that every segment or chromosome found replicating asynchronously at metaphase contains necessarily heterochromatin.Dedicated to Professor H. Using optimized computer models of arterial trees we demonstrate that flow heterogeneity is a necessary consequence of a uniform shear stress distribution. Model trees are generated and optimized under different modes of boundary conditions. In one mode flow is delivered to the tissue as homogeneously as possible. Although this primary goal can be achieved, resulting shear stresses between blood and the vessel walls show very large spread. In a second mode, models are optimized under the condition of uniform shear stress in all segments which in turn renders flow distribution heterogeneous. Both homogeneous perfusion and uniform shear stress are desirable goals in real arterial trees but each of these goals can only be approached at the expense of the other. While the present paper refers only to optimized models, we assume that this dual relation between the heterogeneities in flow and shear stress may represent a more general principle of vascular systems.  相似文献

The presence of adrenergic innervation was investigated in four different vascular segments of the neotenic tiger salamander, Ambystoma tigrinum, by histofluorescent staining for catecholamines. The segments were the respiratory section of the gill, the branchial shunt vessels, a vascular plexus in the pulmonary artery, and the dorsal aorta. No adrenergic fibers were detected in the respiratory section of the gill or the pulmonary arterial plexus. In contrast, the branchial shunt vessels contained both adrenergic varicosities and catecholamine-containing cell bodies. These cells resemble Type I cells of the mammalian carotid body and amphibian carotid labyrinth. Adrenergic innervation of the dorsal aorta was sparse and restricted to the adventitia. The results suggest that adrenergic nerves may directly regulate blood flow in the gill, and thus gas exchange, by controlling vascular resistance of the branchial shunts. The contractile state of the dorsal aorta may also be under adrenergic control. In addition, it is suggested that the adrenergic cells of the branchial shunts may serve a receptor function in being sensitive to arterial blood gases.  相似文献

Anterior head segmentation is governed by different regulatory mechanisms than those that control trunk segmentation in Drosophila. For segment polarity genes, both initial mode of activation as well as cross-regulatory interactions among them differ from the typical genetic circuitry in the trunk and are unique for each of the procephalic segments. In order to better understand the segment-specific gene network responsible for the procephalic expression of the earliest active segment polarity genes wingless and hedgehog, we started to identify and analyze cis-regulatory DNA elements of these genes. For hedgehog, we could identify a cis-regulatory element, ic-CRE, that mediates expression specifically in the posterior part of the intercalary segment and requires promoter-specific interaction for its function. The intercalary stripe is the last part of the metameric hedgehog expression pattern that appears during embryonic development, which probably reflects the late and distinct establishment of this segment. The identification of a cis-regulatory element that is specific for one head segment supports the mutant-based observation that the expression of segment polarity genes is governed by a unique gene network in each of the procephalic segments. This provides further indication that the anterior-most head segments represent primary segments, which are set up independently, in contrast to the secondary segments of the trunk, which resemble true repetitive units.  相似文献
