瑞倍三联1周疗法根除幽门螺杆菌的临床研究   总被引:2,自引:0,他引:2  
目的 :观察枸橼酸铋雷尼替丁 (RanitidineBismuthCitrate ,RBC瑞倍 )为主的 1周三联疗法的幽门螺杆菌 (Helicobaterpylori,Hp)根除疗效及安全性。方法 :随机将 10 0例Hp阳性患者分为瑞倍治疗组 (A组 )与奥美拉唑三联疗法组 (B组 ) ,疗程 1周 ,14 C 尿素呼气试验及粪抗原检测判断Hp根除效果。结果 :根据意图治疗 (ITT)分析Hp根除率分别为A组 84 0 %及B组 78 0 %。根据试验方案分析 (PP)Hp根除率分别为A组 87 5 %及B组 83 0 %。A组副反应发生率 12 5 % ,B组为 6 4 %。两组变化均无统计学意义 (P >0 0 5 )。结论 ;瑞倍为主短程三联方案的Hp根除疗效与奥美拉唑为主的短程疗效相当 ,副反应发生相似  相似文献

目的:探讨三联疗法与胃复春联合治疗幽门螺杆菌(Hp)阳性胃溃疡的临床疗效。方法:选择2011年11月到2014年11月在我院就诊的240例Hp阳性胃溃疡患者,随机分为实验组(n=120)和对照组(n=120)。对照组采用三联疗法,实验组在对照组基础上加服胃复春。比较两组患者的临床疗效、Hp根除率、胃泌素、胃动素水平。结果:实验组患者的总有效率和Hp根除率均明显高于对照组,具有显著性差异(均P0.05)。实验组治疗后胃泌素水平低于治疗前,亦低于对照组治疗后,具有显著性差异(均P0.05),胃动素变化无显著性差异。结论:三联疗法联合胃复春治疗Hp阳性胃溃疡能有效根除Hp,降低胃泌素水平,治疗效果良好。  相似文献

联和用药治疗幽门螺杆菌相关性消化性溃疡的疗效比较   总被引:2,自引:1,他引:1  
观察90例幽门螺杆菌(Hp)感染的消化性溃疡者随机分成A、B、C三组治疗,A组:丽珠胃三联-枸椽铋钾、克拉霉素、替硝唑;B组奥美拉唑、阿莫西林、甲硝唑、C组:奥美拉唑、克拉霉素、替硝唑;疗效均为一周。用药前及停药4周后作内镜。尿毒酶检测及病理学检测后二者Hp均阳性者定为Hp感染,均为阴性定为Hp根除,A、B、C三组Hp根除率分为别为87.5、85.2、87.0;溃疡治疗总有百分率分别为93.8、92.5、96.7。统计学上无显著差异。均有轻微副反应,停药后消失。结果表明丽珠胃三联-周治疗方案具有药物剂量小,疗效程、疗效高、价格低等特点,是较为理想的根除、治疗Hp相关消化性溃疡的药物。  相似文献

目的:观察奥美拉联用呋喃唑酮及阿莫西林治疗幽门螺杆菌(Hp)阳性十二指肠溃疡的临床疗效。方法:对45例经胃镜活检证实的Hp阳性十二指肠溃疡应用奥美拉唑20mg Bid 呋喃唑酮0.2g Bid 阿莫西林1.0g Bid,疗程1w。结果:45例病例中,42例愈合,40例根除,7d疼痛缓解率为87%。结论:呋喃唑酮三联疗法能有效地根除Hp,且不良反应少,疗效高,病人依从性好,价格适中,是较理想的方案,值得进一步尝试。  相似文献

目的:比较丽珠胃三联不同疗程对胃溃疡的疗效,为临床合理使用丽珠胃三联提供依据.方法:150例患者随机分为三组:7天治疗组、14天治疗组和21天治疗组三组,分别给予丽珠胃三联治疗7、14、21天,每日2次.疗程结束后,停药1月复查胃镜和HP检查,以判断溃疡愈合和Hp的根除情况.结果:经丽珠胃三联治疗7、14、21天后,Hp得到有效根除,胃溃疡愈合率大幅提升,其中7天治疗组Hp的根除率达81.39%,溃疡愈合率达79.07%,14天治疗组和21天治疗组Hp的根除率和胃溃疡愈合率明显高于7天治疗组(p<0.05),14天治疗组与21天治疗组无明显差异.结论:三组方案均能有效治疗胃溃疡,并能有效地根除幽门螺杆菌.14天疗程方案为更科学、有效的治疗方案.  相似文献

目的观察兰索拉唑联合磷酸铝凝胶治疗幽门螺杆菌(Hp)阳性胃溃疡(GU)的临床疗效。方法选择2017年1~12月我院治疗的110例Hp阳性GU患者为研究对象,按随机数字表法分为两组各55例。对照组采取兰索拉唑治疗,观察组联合应用磷酸铝凝胶治疗,观察比较两组患者的临床疗效、Hp根除率、不良反应情况以及治疗前后胃泌素(GAS)、金属蛋白酶组织抑制剂-1(TIMP-1)、基质金属蛋白酶-9(MMP-9)水平变化情况。结果与对照组Hp根除率(72.73%)、总有效率(74.55%)相比,观察组Hp根除率(90.91%)、总有效率(94.55%)相对较高,差异有统计学意义(P0.05)。治疗前,两组患者的GAS、TIMP-1、MMP-9水平比较,差异无统计学意义(P0.05);治疗后,与对照组GAS[(161.23±15.94)pg/ml]、TIMP-1[(1.49±0.34)ng/L、[MMP-9[(96.78±14.47)g/L]水平比较,观察组[GAS(123.08±13.42)pg/ml、TIMP-1(0.61±0.11)ng/L、MMP-9(76.61±12.28)g/L]相对较低,差异有统计学意义(P0.05)。两组不良反应发生率比较,差异无统计学意义(P0.05)。结论兰索拉唑、磷酸铝凝胶联合治疗Hp阳性GU的安全性高,且疗效确切,有利促进症状缓解,增强Hp根除效果,降低GAS、TIMP-1、MMP-9水平。  相似文献

目的:探究胃炎康方5号联合西医治疗慢性萎缩性胃炎的临床疗效。方法:选取在我院接受治疗的慢性萎缩性胃炎患者94例,分为对照组和实验组,对照组患者采用常规的胃三联治疗方法,实验组患者在对照组患者的治疗方法之上再加上胃炎康方5号治疗,观察分析和对比两组患者的治疗效果以及Hp根除率(幽门螺旋杆菌根除率)。结果:对照组患者治疗的总有效率为46.81%,Hp根除率为82.98%,实验组患者治疗的总有效率为80.85%,根除率为93.62%,两组结果相比较,差异显著,有统计学意义(P0.05)。结论:胃炎康方5号联合西药治疗慢性萎缩性胃炎的临床疗效显著,值得临床推广。  相似文献

目的:通过与短程三联方法比较,观察香连片联合三联疗法根除十二指肠溃疡患者幽门螺杆菌(Hp)感染的根除率和治疗十二指肠溃疡的疗效.方法:120例经胃镜检查确诊为十二指肠溃疡活动期并经快速尿素酶试验和血清杭Hp杭体(ELASE)或病理学检查确定为Hp阳性的患者分为4组:OACX组(奥美拉唑20mg+阿莫西林1000mg+克拉霉素250mg+香连片5片)、OMCX组(奥美拉唑20mg+甲硝唑400mg+克拉霉素250mg+香连片5片),OAC组(奥美拉唑20mg+阿莫西林1OOOmg+克拉霉素250mg)和OMC组(奥美拉唑20mg+甲硝唑400mg+克拉霉素250mg),每日2次,连续7天.然后单独奥美拉唑20mg,每日1次,连续28天,并于用药结束后第28-56天内作13C-尿素呼气试验判断Hp根除率的效果.同时在用药后第1、2、3、7、21和35天对患者的上腹痛、反酸以及上腹烧灼感等症状进行评估.结果:108例患者完成全部治疗方案,12例失访.Hp根除率检查结果显示OACX组的Hp根除率为92.9%(26/28)OMCX组为85.7%(24/28),OAC组为66.7%(18/27)和OMC组为60.0%(15/25).4组间差异有显著性(P<0.01).症状改善情况:4组从用药第1天起均能有效改善患者的上腹痛、反酸、上腹烧灼感等症状,但是,OACX组和OMCX组的总得分下降幅度均显著高于OAC和OMC组(P<0.05-0.01).结论:OACX组和OMCX组对Hp具有相当高的根除率OACX组和OMCX组方案是适合我国国情的根除Hp行之有效的方案,而且,缓解十二指肠溃疡患者的症状,OACX组和OMCX组亦为优.  相似文献

摘要 目的:探讨胃苏颗粒联合四联疗法对幽门螺杆菌(Hp)相关性消化性溃疡患者血清炎性因子、胃肠激素及生活质量的影响。方法:选取2018年3月~2019年12月期间我院收治的Hp阳性消化性溃疡患者124例,根据奇偶数字法将患者分为对照组、联合组,各62例。对照组给予四联疗法,联合组给予胃苏颗粒联合四联疗法,疗程均为4周。对比两组疗效、Hp根除率、炎性因子水平、胃肠激素水平、生活质量及不良反应。结果:联合组Hp根除率、临床总有效率高于对照组(P<0.05)。联合组治疗4周后SF-36量表各维度评分较对照组更高(P<0.05)。联合组治疗4周后血清降钙素原(PCT)、白介素-6(IL-6)、超敏C反应蛋白(hs-CRP)水平低于对照组(P<0.05)。联合组治疗4周后胃泌素(GAS)、胃动素(MTL)低于对照组(P<0.05)。两组不良反应发生率对比差异无统计学意义(P>0.05)。结论:胃苏颗粒联合四联疗法治疗Hp相关性消化性溃疡患者,可有效改善患者体内血清炎性因子及胃肠激素水平,并提高其生活质量和Hp根除率,疗效优于单纯四联疗法。  相似文献

The biological functions of Hp can be related to its ability to bind haemoglobin or to modulate immune response. Hp is expressed at a high level in lung cells, yet its protective role(s) in the lung is not known. Using transgenic mice overexpressing Hp, we demonstrated that Hp can reduce blood-induced lung injury. Haptoglobin (Hp) prevents the hemoglobin driven generation of hydroxyl radicals and lipid peroxides. Hp can reduce the neutrophil respiratory burst and is an antioxidative molecule in its own right. We aimed to evaluate Hp concentrations, oxidative stress and antioxidative capacity in blood during weaning and to characterise potential relationships between these parameters. Two batches of 10 piglets each (2 trials) weaned at the age of 27-30 days were fed a starter feed mix ad libitum. Blood samples were taken 1 week before weaning and at weekly intervals thereafter. Oxidative stress was monitored via the D-ROM system, antioxidative capacity was measured with the TEAC assay and Hp concentrations were measured by ELISA. Neutrophil phagocytic activity and oxidative burst were examined via flow-cytometry. Body weights were recorded weekly. Hp concentrations were increased in both trials post-weaning (P < 0.01); oxidative stress and oxidative burst were elevated in trial I (P < 0.005). In trial I, Hp and ROM values returned to baseline levels at 6 weeks post-weaning. The piglets in trial II showed respiratory symptoms and maintained elevated Hp concentrations. ROM values and Hp were related (r = 0.58; P < 0.01). Hp and body weight gain were inversely related post-weaning.  相似文献

The haptoglobin (Hp) polymorphism is investigated in 11 African groups living in an area from the Algerian Sahara to Central Africa. More than 4,000 samples were examined. In the Saharian samples, the Hp1 gene frequency is higher than in any other African group. From north to south, a decrease in the Hp1 gene frequency is observed; in the Pygmy sample only, this frequency is lower than the frequency of the Hp2 gene. By means of a sensitive radioimmunoelectrophoresis, the presence of a residual Hp in Hp O sera in which the Hp polymorphism can also be determined can be revealed. Absence of Hp 1-1 and significant excess of Hp 2-2 individuals were observed. More Hp 2-1M phenotypes were detected in the Hp O population than in the non-Hp O population examined. In the Hp O samples, the influence of the phenotype distribution on the Hp gene frequencies is discussed. The heavy polymers of the Hp related to the presence of the alpha 2 chain (Hp2 gene product) are involved only in the biological mechanisms responsible for the presence of Hp O and Hp 2-1 M phenotypes among African groups.  相似文献

Haptoglobin (Hp) is produced as an acute phase reactant during inflammation, infection, malignant diseases, and several cancers. In proteomics analysis using human blood samples, the Hp peptide levels were about 3-fold higher in lung cancer patients versus normal individuals. This study is aimed at analyzing the elevation of which chain of Hp is closely related to lung cancers and can be a serum biomarker for lung cancers. In Western blot (WB) analysis, we found that the Hp β chain can be a better diagnostic biomarker for lung cancers. In the result of the Hp β chain ELISA developed by us, the concentrations of the Hp β chain in the sera increased about 4-fold in 190 lung adenocarcinoma patients versus 190 healthy controls (8.0 ± 3.8 μg ml(-1)vs. 1.9 ± 1.2 μg ml(-1)). ELISA data showed that the serum levels of the Hp β chain in breast cancer (1.5 ± 0.5 μg ml(-1)) and hepatocellular carcinoma (HCC) (1.4 ± 1.0 μg ml(-1)) patients remained similar to those of healthy controls. Compared to lung adenocarcinoma, the Hp β chain levels in the plasma of patients with other respiratory diseases such as tuberculosis (TBC), idiopathic pulmonary fibrosis (IPF) and bronchial asthma (BA) were closer to those of healthy controls. Our data suggest that an increase of the Hp β chain can be a potential serum biomarker for lung cancers.  相似文献

Gastric functions can be understood only in the context of a network including the brain gut axis, neuro-endocrine and paracrine mechanisms and growth factors. These host factors including parietal cell sensitivity (PCS) may well interact with an important environmental factor, Helicobacter pylori (Hp), and help to explain its actions. The aim of this study was to investigate PCS related to Hp status and duodenal ulcer (DU). PCS was assessed by constructing dose-response curves after pentagastrin and calculating the D50. Five groups of patients were studied: I) active DU, Hp pos. (8); II) history of DU, Hp pos. (8); III) asymptomatic Hp pos. (8); IV) asymptomatic Hp neg. (10); V) DU on maintenance H2 blocker therapy, Hp pos. (20). PCS was repeated after Hp eradication. PCS was lowest in group IV, and in Hp pos. groups, was significantly higher, with insignificant differences among them, irrespective of DU. PCS declined significantly after Hp eradication. Group V showed an insignificant decline in PCS during treatment, not preventing recurrence. A higher PCS in Hp infection irrespective of DU, declining after eradication, suggests that this may be a reversible epiphenomena related to Hp infection. This may offer an explanation as to why DU develops only in some subjects with Hp, suggesting the importance of the host in the pathogenesis of DU.  相似文献

为了探讨胃癌中幽门螺旋杆菌(Hp)感染和Sonic Hedgehog(Shh)、C-myc表达,它们之间的相关性以及胃癌发生的可能机制,采用免疫组化法检测89例胃癌组织及20例正常胃上皮组织中Shh及C-myc的表达。并采用快速尿素酶试验,组织病理学检测两种方法检查Hp。实验结果显示,胃癌组织Shh的表达要明显高于正常上皮组织,二者之间有显著差异(P<0.05);胃癌组织C-myc的表达水平也高于正常胃上皮组织,二者之间有显著差异(P<0.05);Hp阳性的C-myc阳性表达率明显高于Hp阴性,二者之间有显著差异(P<0.05);Shh表达阳性率在Hp阳性和阴性胃癌中无显著差异(P>0.05)。结果提示,胃癌的发生与癌基因Shh及C-myc的过度表达有关,Hp感染的致癌机制中可能有癌基因C-myc参与。  相似文献

The change in glycosylation of the two acute-phase proteins, alpha-1-proteinase inhibitor (API) and haptoglobin (Hp), in progressive ovarian cancer is different. This has been shown by monosaccharide analysis and lectin-binding studies of proteins purified from serum. In the glycan chains of API, there is decreased branching (more biantennary chains), less branches ending in alpha 2-3 sialic acid, more branches ending in alpha 2-6 sialic acid and more fucose, probably linked alpha 1-6 to the core region. On the other hand, Hp shows increased branching (more triantennary chains), more branches ending in alpha 2-3 sialic acid, less branches ending in alpha 2-6 sialic acid, and more fucose, probably in the alpha 1-3 linkage at the end of the chains. This is surprising because API and Hp are thought to be glycosylated by a common pathway in the liver. We have also shown that the fucose-specific lectin,lotus tetragonolobus, extracts abnormal forms of both Hp and API in ovarian cancer, but the expression of this Hp is related to tumour burden and the expression of this API is related to lack of response to therapy. It is suggested that this difference in the behaviour of API and Hp in ovarian cancer may be associated with the different changes in their glycosylation. Of the many mechanisms that could explain these findings, a likely one is that a pathological process is removing API with triantennary chains from the circulation. In addition to their normal roles (API-enzyme inhibitor and Hp-transport protein) these proteins are reported to have many other effects in biological systems, such as immunosuppression. As correct glycosylation of API and Hp is required for their normal stability/activity, changes in glycosylation could affect their functions in ovarian cancer and these modifications could alter the course of the disease.  相似文献

1. The cellular uptake and the efficiency in sensitizing cells to photoinactivation were determined for hematoporphyrin (Hp) diphenyl ether, Hp dicyclohexyl ether and Hp dihexyl ether. 2. The phenyl diether was taken up by the cells to the same degree as was the clinically used porphyrin preparation photofrin II, while the dihexyl and notably the dicyclohexyl ether were taken up 3-4 times better. 3. Furthermore, the quantum yields for photoinactivation of cells were similar for the three diethers and twice as large as that for photofrin II. 4. Fluorescence- and absorption spectroscopy indicate that these findings are related to the fact that photofrin II is much more aggregated in the cells than are the three Hp diethers. 5. When cells loaded with the porphyrins are incubated with porphyrin-free medium containing serum a certain percentage of the cell-bound drug is removed: 14% for photofrin II, 28% for Hp diphenyl ether, 50% for Hp dicyclohexyl ether and 20% for Hp dihexyl ether. 6. With respect to cell uptake and retention of the dyes, the data did not show any uniform relationship to the polarity of the drugs, in contrast to what has been found earlier for Hp diethers of linear hydrocarbons.  相似文献
