Background

Many countries face severe scale-up barriers toward achievement of MDGs. We ascertained motivational and experiential dimensions of participation in a novel, rapid, “diagonal” Integrated Prevention Campaign (IPC) in rural Kenya that provided prevention goods and services to 47,000 people within one week, aimed at rapidly moving the region toward MDG achievement. Specifically, the IPC provided interventions and commodities targeting disease burden reduction in HIV/AIDS, malaria, and water-borne illness.

Methods

Qualitative in-depth interviews (IDI) were conducted with 34 people (18 living with HIV/AIDS and 16 not HIV-infected) randomly selected from IPC attendees consenting to participate. Interviews were examined for themes and patterns to elucidate participant experience and motivation with IPC.

Findings

Participants report being primarily motivated to attend IPC to learn of their HIV status (through voluntary counseling and testing), and with receipt of prevention commodities (bednets, water filters, and condoms) providing further incentive. Participants reported that they were satisfied with the IPC experience and offered suggestions to improve future campaigns.

Interpretation

Learning their HIV status motivated participants along with the incentive of a wider set of commodities that were rapidly deployed through IPC in this challenging region. The critical role of wanting to know their HIV status combined with commodity incentives may offer a new model for rapid scaled-up of prevention strategies that are wider in scope in rural Africa.  相似文献

An outbreak of canine distemper in a kennel of German shepherds in the province of Bari is reported. Six 42-day-old pups developed typical signs of canine distemper (fever, conjunctivitis, respiratory distress and enteritis) and died within 7-10 days. Neurological symptoms were observed only in one pup. Four additional pups, which had shown no sign of illness, were separated and vaccinated, but two of these developed a severe, fatal nervous form 15 days later. Post-mortem examination, carried out on two pups which died without neurological signs, showed pneumonia and enteritis, more severe in one of the two examined pups. Smears from the brain and the conjunctiva of both dogs tested positive for canine distemper virus (CDV) by an immunofluorescent assay, confirmed by the identification of viral RNA using RT-PCR. Bordetella bronchiseptica and a canine adenovirus strain, characterized as canine adenovirus type 2 by a differential PCR assay, were isolated from the lungs of the pup showing the most pronounced lesions. Furthermore, canine coronavirus was detected by PCR in the intestinal content of this pup, suggesting a multifactorial aetiology of the outbreak.  相似文献

Electrospray ionization mass spectrometric peptide mapping is demonstrated to be a useful new technique for protein structure analysis. The procedure involves the digestion of the protein with trypsin and subsequent analysis of the total unfractionated digest by electrospray ionization mass spectrometry. The utility of the technique for investigating protein structure is illustrated by a peptide mapping analysis of human apolipoprotein AI (Mr = 28 kDa). The technique is rapid, sensitive, and requires no prior separation of the peptides. The discrimination effects observed in other mass spectrometric methods are less important in the present procedure.  相似文献

This comparison of ethnic relations in two countries, Trinidad and Tobago, and Guyana, supplements the research findings of synchronic studies of "the social construction of race" by offering a historically based framework to understand particular and local instances of ethnic relations. Drawing on a long historical study of Trinidad and Tobago, and Guyana, I argue that the institutional sources of definition of key "ethnicities" have shifted through the centuries. "Ethnicities" have been successively defined by the institutions of capital, state and community. While these institutions have overlapped in time they are not equally important at a given moment in the matter of defining "ethnicity". The content of the definitions has also varied significantly. At present political communities and the family are the major social institutions that determine "ethnic" content.  相似文献

An understanding of urban aquatic environments as mosquito larval habitats is necessary to prioritize sites for surveillance and control of arbovirus vectors in urban areas. Natural and artificial water bodies at ground level that may be larval mosquito habitats in Córdoba city, Argentina were surveyed. Data on the characteristics of aquatic sites and the presence and abundance of mosquito larvae and pupae were collected in the summer of 2006, coinciding with the first report of human WNV and following an outbreak of St. Louis encephalitis in 2005. Eight species in the genera Aedes, Culex, and Mansonia were identified. At 64.2% (34 of 53) of the sites, only one species was collected, while 3.8% (2 of 53) had three associated species, the highest richness found per site. Culex quinquefasciatus represented over 99% (out of 32,729) of the specimens. It was also the most widely distributed and detected under diverse habitat conditions. Although puddles and semi‐permanent pools harbored a greater number of species, drainages and channels may be more relevant as risk factors from an epidemiological point of view because they showed the highest larval densities, mainly of Cx. quinquefasciatus (vector of SLE and WNV). Also, higher densities of this species were associated with stormwater runoff and sewage water, thus water management systems should be targeted and closely monitored for mosquito control purposes.  相似文献

The genetic composition of a hybrid zone can provide insight into the evolution of diversification in plants. We carried out morphological and amplified fragment length polymorphism analyses to investigate the genetic composition of a hybrid zone between two violets, Viola bissetii Hemsl. and Viola rossii Maxim. Our aim was to clarify the formation and maintenance of hybrids between these Viola species. We found that most hybrid individuals (V. bissetii × V. rossii) were of the F1 generation, with a few of the F2 generation. We found no backcrosses. The scarcity of post‐F1 hybrids indicates that a species barrier is established between the parental species. The F1‐dominated hybrid zone occupied only a narrow, intermediate ecotone between the parental habitats, suggesting that selection by environmental factors against hybrids may help to maintain the current conditions in this hybrid zone.  相似文献

Objectives: The accuracy of cone-beam technique, cone-beam computed tomography (CBCT), compared with that of the multislice spiral CT (MSCT), for image-based linear measurements of midpalatal suture was assessed.

Material and Methods: Two measurements were performed by one investigator on the dry skull by using one digital caliper and in the axial cuts by using software. A 2D object-based image registration process was applied to determine the best affine transformation that maps a 2D input image (CBCT) in a reference image (MSCT).

Results: The value of the intraclass correlation coefficient was approximately 0.9%. The results suggest that differences between two scanners did not exist (observation 1, p = 0.964 and observation 2, p = 0.795). With regard to the dry skull and the image, the significance probabilities equaled zero (observation 1, p = 0.002 and observation 2, p = 0.004), therefore, indicating significant differences.

Conclusion: Measurements acquired in the images were similar and these findings contribute to stimulate the use of CBCT for evaluation of the maxillary expansion procedure.  相似文献

Circoviruses are among the smallest and simplest of all viruses, but they are relatively poorly characterized. Here, we intensively sampled two sympatric parrot populations from Mauritius over a period of 11 years and screened for the circovirus Beak and feather disease virus (BFDV). During the sampling period, a severe outbreak of psittacine beak and feather disease, which is caused by BFDV, occurred in Echo parakeets. Consequently, this data set presents an ideal system for studying the evolution of a pathogen in a natural population and to understand the adaptive changes that cause outbreaks. Unexpectedly, we discovered that the outbreak was most likely caused by changes in functionally important regions of the normally conserved replication-associated protein gene and not the immunogenic capsid. Moreover, these mutations were completely fixed in the Echo parakeet host population very shortly after the outbreak. Several capsid alleles were linked to the replication-associated protein outbreak allele, suggesting that whereas the key changes occurred in the latter, the scope of the outbreak and the selective sweep may have been influenced by positive selection in the capsid. We found evidence for viral transmission between the two host populations though evidence for the invasive species as the source of the outbreak was equivocal. Finally, the high evolutionary rate that we estimated shows how rapidly new variation can arise in BFDV and is consistent with recent results from other small single-stranded DNA viruses.  相似文献

BackgroundOver 240 million people are infected with schistosomiasis, the majority in sub-Saharan Africa. In Uganda, high infection rates exist in communities on the shores of Lake Victoria. Praziquantel mass drug administration (MDA) delivered by village health teams is the mainstay of schistosomiasis control. However, treatment uptake remains suboptimal, with many people unaware of treatment or thinking it is only for children. Furthermore, people are often rapidly reinfected post-treatment due to continued exposure. In three Schistosoma mansoni high endemicity lake-shore communities in Mayuge district, Eastern Uganda, we investigated the sources of schistosomiasis information, remembered content of information, and the perception of information and related practices towards the control of schistosomiasis.Methods and principal findingsData were collected from September 2017 to March 2018 using a rapid ethnographic assessment that included transect walks, observations, individual in-depth interviews and focus group discussions. Data were analysed thematically using iterative categorisation. We found that the main sources of schistosomiasis information included health workers at government facilities, village health teams, teachers, and radio programmes produced by the Ministry of Health. These messages described the symptoms of schistosomiasis, but did not mention the side effects of praziquantel treatment. Despite this messaging, the main cause of the disease and transmission was unclear to most participants. The translation of schistosomiasis on the radio into the local language ‘ekidada’—meaning swollen stomach—increased, rather than reduced, confusion about the cause(s) of schistosomiasis, due to believed links between ekidada and witchcraft, and prompted a reluctance to engage with treatment or preventative efforts.Conclusion and significanceThis study highlights gaps in schistosomiasis messaging. We recommend MDA is complemented by effective, evidence-based messaging on schistosomiasis transmission, prevention, and treatment, that is sensitive to local language and context issues, resulting in clear, concise, and consistent messages, to increase effectiveness.  相似文献

Water voles have suffered large population declines in the United Kingdom due to habitat degradation and predation by invasive American mink. Habitat restoration of floodplain wetlands could help to reverse this decline, but the detailed habitat preferences of water voles in these environments have not been well studied, and the impacts of restoration practices on water vole populations are not known. This study investigated the habitat preferences of water voles in a reconnected lowland river floodplain. The results show that water voles preferred wider water bodies, and taller and more diverse vegetation. The impact of flooding on water voles was also investigated by comparing their occurrence between two survey periods which were separated by large flood events, and by comparing distribution patterns before and after restoration. Contrary to previous reports, there was no observed negative impact of flood events on water vole distribution, which has slightly expanded since the floodplain was reconnected to the river in 2009. Overall this study demonstrates that restored wetlands can provide suitable habitat for water voles, and provides guidance on some of the factors which should be considered when designing floodplains for water vole conservation.  相似文献

We propose a method to analyse the 2009 outbreak in the region of Botucatu in the state of São Paulo (SP), Brazil, when 28 yellow fever (YF) cases were confirmed, including 11 deaths. At the time of the outbreak, the Secretary of Health of the State of São Paulo vaccinated one million people, causing the death of five individuals, an unprecedented number of YF vaccine-induced fatalities. We apply a mathematical model described previously to optimise the proportion of people who should be vaccinated to minimise the total number of deaths. The model was used to calculate the optimum proportion that should be vaccinated in the remaining, vaccine-free regions of SP, considering the risk of vaccine-induced fatalities and the risk of YF outbreaks in these regions.  相似文献

The end‐Triassic mass extinction (c. 201.6 Ma) was one of the five largest mass‐extinction events in the history of animal life. It was also associated with a dramatic, long‐lasting change in sedimentation style along the margins of the Tethys Ocean, from generally organic‐matter‐poor sediments during the Triassic to generally organic‐matter‐rich black shales during the Jurassic. New core material from Germany provides biomarker evidence of persistent photic‐zone euxinia during the Hettangian, the onset of which is associated with a series of both negative and positive carbon isotope excursions. Combined inorganic and organic geochemical and micropalaeontological analyses reveal strong similarities between the Hettangian and the better‐known Toarcian anoxic event. These events appear to be the most clearly expressed events within a series of anoxic episodes that also include poorly studied black shale intervals during the Sinemurian and Pliensbachian. Both the Hettangian and Toarcian events are marked by important changes in phytoplankton assemblages from chromophyte‐ to chlorophyte‐dominated assemblages within the European Epicontinental Seaway. Phytoplankton changes occurred in association with the establishment of photic‐zone euxinia, driven by a general increase in salinity stratification and warming of surface waters. For both events, the causes of large negative carbon isotope excursions remain incompletely understood; evidence exists for both variation in the δ13C of atmospheric CO2 and variation in the sources of organic carbon. Regardless of the causes of δ13C variability, long‐term ocean anoxia during the Early Jurassic can be attributed to greenhouse warming and increased nutrient delivery to the oceans triggered by flood basalt volcanism.  相似文献

In Salicornia, morphology does not provide reliable diagnostic characters supporting the true extent of evolutionary divergence in the genus, and species concepts have been challenged by molecular analyses. Here, we report the results of an analysis of 91 accessions of the S. meyeriana complex from South Africa and Namibia using the measurements of 38 morphological traits and external transcribed spacer (ETS) sequence data. Morphological data were analysed using discriminant analysis, principal coordinate analysis and nonmetric multidimensional scaling. Phylogenetic divergence was compared with the geographical and ecological diversity of the sampled populations. Tree topology corresponds to geography and ecology, but not to morphology. Most clades have distinct distribution areas and ecological profiles related to tidal, supratidal or inland saline habitats. Salicornia probably diversified in habitats that have experienced regular fragmentation by marine transgression/regression cycles during the Pleistocene. We suggest that this radiation produced young, but genetically, geographically and ecologically well‐defined lineages. The lack of morphological signal reveals the existence of cryptic species in Salicornia and demonstrates the necessity of using molecular data to define taxa in this genus. We propose the recognition of two subspecies in the S. meyeriana complex: S. meyeriana subsp. meyeriana and S. meyeriana subsp. knysnaensis. © 2013 The Linnean Society of London, Botanical Journal of the Linnean Society, 2013, 172 , 175–186.  相似文献

Accurate quantification of pure peptides and proteins is essential for biotechnology, clinical chemistry, proteomics, and systems biology. The reference method to quantify peptides and proteins is amino acid analysis (AAA). This consists of an acidic hydrolysis followed by chromatographic separation and spectrophotometric detection of amino acids. Although widely used, this method displays some limitations, in particular the need for large amounts of starting material. Driven by the need to quantify isotope-dilution standards used for absolute quantitative proteomics, particularly stable isotope-labeled (SIL) peptides and PSAQ proteins, we developed a new AAA assay (AAA-MS). This method requires neither derivatization nor chromatographic separation of amino acids. It is based on rapid microwave-assisted acidic hydrolysis followed by high-resolution mass spectrometry analysis of amino acids. Quantification is performed by comparing MS signals from labeled amino acids (SIL peptide- and PSAQ-derived) with those of unlabeled amino acids originating from co-hydrolyzed NIST standard reference materials. For both SIL peptides and PSAQ standards, AAA-MS quantification results were consistent with classical AAA measurements. Compared to AAA assay, AAA-MS was much faster and was 100-fold more sensitive for peptide and protein quantification. Finally, thanks to the development of a labeled protein standard, we also extended AAA-MS analysis to the quantification of unlabeled proteins.  相似文献
