Portal hypertension occurs in approximately 10% of patients with myelofibrosis. Increased portal blood flow secondary to splenomegaly has been proposed to explain its development. In a 60-year-old woman with proven myelofibrosis of 10 years'' duration and gross splenomegaly, portal hypertension developed with esophageal varices and ascites. There was no demonstrable obstruction to portal blood flow. Following splenectomy the ascites and esophageal varices disappeared. Despite the presence of splenic myeloid metaplasia, splenectomy did not impair the patient''s hematologic status. Portal hypertension complicating myelofibrosis has a poor prognosis, so careful attention should be given to its detection. Splenectomy may be preferable to portal-systemic shunting in the management of this complication.  相似文献

Pre-analytical handling may be an important determinant of haematological variables, if analysis is delayed. We investigated the effect of anticoagulants, i.e. tripotassium ethylenediamine-tetraacetic acid (EDTA) and citric acid, theophylline, adenosine, dipyridamole (CTAD), storage time (0.5, 1.5, 3.5, 5.5, 7.5, 25.5 and 27.5 h after blood sampling), and storage temperature (5 degrees C and 20 degrees C) on the variation in haemoglobin (HGB), red blood cell count (RBC), haematocrit (HCT), white blood cell count (WBC), and platelet count (PLT) in minipigs. Medians of HGB, RBC, HCT, WBC and PLT were significantly higher in EDTA tubes than in CTAD tubes due to the dilution effect of the anticoagulant. We found a minor significant increase in HCT after 25.5 h in blood stored at 20 degrees C, and at the same time a minor significant increase in WBC in EDTA tubes stored at 20 degrees C. We found a significant decrease in PLT in blood stored at 5 degrees C, especially in EDTA tubes. Minor variations were also observed in HGB and RBC. Our results indicate that PLT should only be measured in tubes placed at room temperature. If HCT or WBC analyses are to be performed on the day after blood sampling, the samples must be stored in a refrigerator until analysis. Our studies underline that time delay before analysis of haematological variables can cause increased variation, and should therefore be limited as far as possible in order to reduce the number of animals needed to make reliable conclusions.  相似文献

Background

Schistosomiasis mansoni is a chronic liver disease, in which some patients (5–10%) progress to the most severe form, hepatosplenic schistosomiasis. This form is associated with portal hypertension and splenomegaly, and often episodes of gastrointestinal bleeding, even with liver function preserved. Splenectomy is a validated procedure to reduce portal hypertension following digestive bleeding. Here, we evaluate beneficial effects of splenectomy on blood coagulation factors and liver function tests in hepatosplenic schistosomiasis mansoni compared to non-operated patients.

Methodology/Principal Findings

Forty-five patients who had undergone splenectomy surgery were assessed by laboratory analyses and ultrasound examination and compared to a non-operated group (n = 55). Blood samples were obtained for liver function tests, platelet count and prothrombin time. Coagulation factors (II, VII, VIII, IX and X), protein C and antithrombin IIa, plasminogen activator inhibitor-1 were measured by routine photometric, chromogenic or enzyme-linked immunosorbent assays, while hyperfibrinolysis was defined by plasminogen activator inhibitor-1 levels. Both groups had similar age, gender and pattern of periportal fibrosis. Splenectomized patients showed significant reductions in portal vein diameter, alkaline phosphatase and bilirubin levels compared to non-operated patients, while for coagulation factors there were significant improvement in prothrombin, partial thromboplastin times and higher levels of factor VII, VIII, IX, X, protein C and plasminogen activator inhibitor-1.

Conclusion/Significance

This study shows that the decrease of flow pressure in portal circulation after splenectomy restores the capacity of hepatocyte synthesis, especially on the factor VII and protein C levels, and these findings suggest that portal hypertension in patients with hepatosplenic schistosomiasis influences liver functioning and the blood coagulation status.  相似文献

Abstract

Female Wistar-albino rats were given lead acetate (PbAc) for 60 days to investigate the protective effects of L-carnitine (CA) clinically and histopathologically on PbAc-induced tissue damage. Blood samples were obtained from the jugular vein for hemoglobin (HB), hematocrit (HCT), red blood cells (RBC), white blood cells (WBC), platelets (PLT), aspartate aminotransferase (AST), alanine aminotransferase (ALT) and creatinine. PbAc treatment caused a significant decrease in HB, HCT and RBC, a significant increase in WBC, AST, ALT and creatinine compared to controls. Although administration of CA did not reverse HB and HCT values, it reversed both the decrease in RBC and the increase in WBC, AST, ALT and creatinine. After the experimental period, all rats were weighed, then decapitated for pathological examination. Control rat liver, kidney and brain showed normal histological architecture. Lead-induced nephropathic kidneys; degenerative changes, inflammation and portal edema of the liver; and brain neuropil vacuolation, neuronal vacuolation, satellitosis and neuronophagia were observed in experimental groups. All changes were reduced in the PbAc group treated with CA (PbAc + CA). PbAc caused copper/zinc superoxide dismutase (Cu/Zn-SOD) expression in both the hepatocytes and tubular epithelium of the kidney. PbAc + CA exposure caused moderate Cu/Zn-SOD immunoreactivity. While in the brain sections of the PbAc group the degenerative neurons were stained intensely with anti-ubiquitin antibody, PbAc + CA rats showed moderate staining in neurons with anti-ubiquitin antibody. These results show that CA as a food additive reduced the severity of tissue damage caused by PbAc.  相似文献

ABSTRACT

We investigated the effects of quercetin (Q) on some hematological parameters and determined the percentage of alpha-naphthyl acetate esterase (ANAE) positive lymphocytes in rats that had been exposed to cadmium (Cd). Thirty male Wistar albino rats were divided into four groups: control (C), quercetin (Q), cadmium (Cd) and Q + Cd (CdQ). Blood samples were taken to assess erythrocytes (RBC), leukocytes (WBC), hemoglobin levels (Hb), hematocrit values (Hct), platelets (PLT), alpha-naphthyl acetate esterase (ANAE) positive lymphocytes. RBC, Hb, Hct; the number of PLT significantly decreased in the Cd group. To the contrary, these parameters were increased significantly in the CdQ group compared to the Cd group. Although we found a significant increase in total WBC count and neutrophil percentage, the number of lymphocytes decreased in the Cd group compared to the other three groups. Also, the percentage of peripheral blood ANAE positive lymphocytes decreased significantly in the Cd group (p < 0.05). Q exhibits positive effects on some hematological characteristics and the percentage of ANAE positive lymphocyte in cases of acute CD toxicity.  相似文献

Hematological parameters (Ht, Hb, RBC, WBC, PLT), erythrocyte size, and osmotic fragility, differential leukocyte count, ROS production in common carp blood collected on three anticoagulants: heparin (10 IU/mL, Na2EDTA (0.1, 0.5, and 1 mg/mL), and sodium citrate (0.3 mg/mL) were compared. Na2EDTA caused partial blood hemolysis in Ht tubes which made Ht measurement impossible, and resulted in high variability of the results. Both, citrate and Na2EDTA increased sensitivity of red blood cells to hemolysis. Na2EDTA also induced erythrocyte anisocytosis and anisonucleosis. Na2EDTA significantly increased ROS production but no effect of anticoagulants on WBC, PLT or differential leukocyte count was observed. The obtained results show that Na2EDTA should not be used for evaluation of red blood cell parameters and erythrocyte morphology, and for ROS production measurement in common carp. Heparin proved to be the most appropriate anticoagulant to use for this species, although Na2EDTA and sodium citrate may be used for WBC and leukocyte differential count evaluations.  相似文献

Research on hematological disorders relies on suitable animal models. We retrospectively evaluated the use of the hematological parameters hematocrit (HCT), hemoglobin (HGB), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), mean corpuscular volume (MCV), red blood cell count (RBC), white blood cell count (WBC), and platelet count (PLT) in the phenotype-driven Munich N-ethyl-N-nitrosourea (ENU) mouse mutagenesis project as parameters for the generation of novel animal models for human diseases. The analysis was carried out on more than 16,000 G1 and G3 offspring of chemically mutagenized inbred C3H mice to detect dominant and recessive mutations leading to deviations in the levels of the chosen parameters. Identification of animals exhibiting altered values and transmission of the phenotypic deviations to the subsequent generations led to the successful establishment of mutant lines for the parameters MCV, RBC, and PLT. Analysis of the causative mutation was started in selected lines, thereby revealing a novel mutation in the transferrin receptor gene (Tfrc) in one line. Thus, novel phenotype-driven mouse models were established to analyze the genetic components of hematological disorders.  相似文献

以3个品种(长白猪、大白猪、松辽黑猪)16个公猪家系共计368头仔猪组成资源群体,在猪2、7和8号染色体上共选取35个微卫星标记,采用基于线性混合模型的方差组分分析方法,对影响与猪白细胞、红细胞和血小板相关的共计18项血常规指标的数量性状基因座(quantitative trait loci,QTL)进行了检测.通过似然比检验,并以自由度为2的卡方分布作为检验统计量的分布,共发现22个在P〈5%水平下显著的QTL,其中在2号染色体上有9个,分别影响白细胞总数、中性粒细胞数、平均红细胞体积、血红蛋白含量、平均红细胞血红蛋白浓度、血小板总数、平均血小板体积、血小板分布宽度和血小板压积,在7号染色体有7个,分别影响白细胞总数、中性粒细胞数、平均红细胞血红蛋白浓度、血红蛋白含量、血小板总数、平均红细胞体积和红细胞分布宽度变异,在8号染色体上有6个,分别影响中性粒细胞百分比、淋巴细胞百分比、平均红细胞血红蛋白浓度、血小板总数、血小板压积和平均红细胞体积.为尽可能地避免由于多重检验所造成的假阳性率的升高,我们采用了控制假检出率(false discovery rate,FDR)的方法来对这22个QTL进行进一步检验,发现有14个达到FDR〈5%显著水平,其中又有9个达到FDR〈1%显著水平.  相似文献

The authors present their own comments on the effect of Warren shunt on hypersplenism. Hypersplenism is frequent complication in portal hypertension. The authors estimate its frequency on 64.7% of patients. Spleen size, thrombocyte count, WBC count, and esophageal endoscopy have been carried out prior to and following distal spleno-renal shunt. Surgery produced favourable effect in 70% of patients, i.e. a decrease of hypersplenism symptoms. An increase in blood platelet count may serve as a screening marker of anastomosis patency.  相似文献

The time course of hemorheological alterations was investigated after heavy anaerobic exercise in untrained male human subjects. The Wingate protocol was performed by each subject, and blood lactate, red blood cell (RBC) deformability and aggregation, white blood cell (WBC) activation, and several hematological parameters were investigated during 24 h after the exercise and compared with preexercise values. Compared with the preexercise value, blood lactate level was found to be approximately 10-fold higher immediately after the exercise. There was a transient, significant increment of RBC and WBC counts immediately after exercise that was followed by a decrement of RBC count. There was a second increase of WBC count, accompanied with increased percentages of granulocytes and granulocyte activation, starting 45 min after exercise. RBC deformability was found to be impaired immediately after exercise and remained reduced for at least 12 h; RBC aggregation was also found to be decreased after exercise, with the onset of this decrease delayed by 30 min. The results of this study indicate that a single bout of heavy anaerobic exercise may induce significant hemorheological deterioration lasting for up to 12 h and thus suggest the need to consider such effects in individuals with impaired cardiovascular function.  相似文献

目的:探讨脾切除及贲门周围血管离断术对肝硬化门静脉高压患者肝脏血流动力学的影响,并分析患者术后门静脉血栓形成的危险因素。方法:选择2016年1月-2017年12月在我院进行脾切除及贲门周围血管离断术的96例肝硬化门静脉高压患者,于术前、术后1d、3d、7d采用彩色多普勒超声对患者的肝脏血流动力学指标进行动态监测。统计术后7d内患者门静脉血栓的发生率,并将患者分为血栓组(n=28)和无血栓组(n=68),对两组患者的一般资料、手术指标、彩色多普勒超声监测指标等进行单因素分析,并采用Logistic多因素回归分析门静脉血栓形成的危险因素。结果:患者在术前、术后1d、3d、7d时的门静脉内径、最大流速、血流量呈逐渐降低的趋势,肝动脉内径、最大流速、血流量呈逐渐升高的趋势,且各时间点间两两比较差异有统计学意义(P0.05)。术后7d内有28例患者出现门静脉血栓,发生率为29.17%。血栓组和无血栓组患者在性别、年龄、体质量指数、手术时间、术前门静脉流速比较差异无统计学意义(P0.05);血栓组患者Child-Pugh分级为B级比例、术中出血量、脾质量、腹水量、术前门静脉内径均高于无血栓组,术后门静脉内径、术后门静脉流速均低于无血栓组(P0.05)。经Logistic多因素回归分析显示,患者术后门静脉内径、术后门静脉流速是门静脉血栓形成的危险因素(P0.05)。结论:行脾切除及贲门周围血管离断术的肝硬化门静脉高压患者术后进行肝脏血流动力学监测,有助于患者术后的疗效判断,且术后门静脉内径、术后门静脉流速是门静脉血栓形成的危险因素。  相似文献

摘要 目的:探究血浆置换及血小板输注治疗特发性血小板减少性紫癜疗效。方法:选择2016年2月至2019年1月于我院接受治疗的60例特发性血小板减少性紫癜患者为研究对象,按照其选择治疗方式的差异将其分为血小板输注组(20例)及血浆置换(Plasma exchange,PE)组(40例),对比两组患者治疗有效率、治疗前后血细胞计数变化情况以及治疗中各类不良反应发生情况。结果:血小板输注组患者治疗显效数10例,有效数6例,总有效率80.00 %,PE组患者治疗显效数27例,有效数12例,治疗总有效率97.50 %,PE组治疗总有效率高于血小板输注组(P<0.05)。与治疗前比较,PE组患者的PLT、RBC计数和Hb水平出现了明显的升高,WBC计数出现明显的下降(P<0.05),血小板输注组PLT、RBC计数和Hb水平也出现明显升高,WBC计数水平出现下降(P<0.05),但组间比较显示治疗后PE组患者上述指标均优于血小板输注组(P<0.05)。血小板输注组患者不良反应总发生人数为4人,不良反应总发生率为20.00 %,PE组总不良反应发生人数3人,不良反应总发生率为7.50 %,PE组不良反应总发生率明显低于血小板输注组(P<0.05)。结论:血血浆置换及血小板输注治疗均对特发性血小板减少性紫癜具有较好的治疗效果,能够显著改善患者血细胞计数异常情况,但血浆置换治疗安全性更高。  相似文献

The aim of this work was to assess changes of morphological parameters in the blood of rats after oral (po) administration of aluminum (Al), in relation to the time and the administered dose. The experiment was performed on female Wistar rats. The animals were administered aluminum chloride (100 mg Al/kg) daily for 21 d. Morphological assays: red blood cells (RBC), hemoglobin (HGB), hematocrit (HCT), iron serum concentration (Fe), MCH, MCHC, absolute corrected reticulocyte count (ACRC), white blood cells (WBC), and platelet count (PLT) were estimated on d 3, 7, 14, and 21, both in the control group and in intoxicated rats. After wk 1 of aluminum administration we observed a decrease of RBC, HCT, HGB and serum iron concentration in the blood of rats. The increase of the platelet count was observed earlier than changes in other parameters. Investigation has proved that the exposure of rats to aluminum administered orally results in normocytic anemia.  相似文献

Hepatic encephalopathy (HE) is a neuropsychiatric state potentially complicating cirrhosis following the accumulation of toxic compounds that cross the blood–brain barrier and affect brain function; the compounds may undergo alveolar gas exchange and be partially excreted by exhalation. Thus breath analysis as a non-invasive means of diagnosing HE, cirrhosis and harmful drinking was investigated in a pilot study. One litre samples of breath were collected from patients with alcohol-related cirrhosis (n = 34) with HE (n = 11) and without HE (n = 23), non-alcoholic cirrhosis without HE (n = 13), harmful drinkers without cirrhosis (n = 7), and healthy controls (n = 15) in a hospital setting. Breath compounds trapped on adsorbent tubes were released via thermal desorption and analysed by gas chromatography mass spectrometry for separation and detection. Multivariate discriminant analysis was used to identify volatile organic compounds to differentiate patients according to disease status and build models for disease classification. HE was correctly identified in 90.9 % of alcoholic cirrhotic patients and liver cirrhosis in 100 % of alcoholic patients. In patients without clinical HE, alcohol was correctly predicted as the cause of cirrhosis in 78.3 % of patients and non-alcoholic causes of cirrhosis were correctly determined in 69.2 %. Non-alcoholic cirrhosis, alcoholic cirrhosis, and harmful drinking could be discriminated from healthy controls with a sensitivity of 92.3, 97.1 and 100 %, respectively. Breath volatile analysis has the potential to aid the diagnosis of HE and a range of liver disorders.  相似文献

Background

Spleen enlargement is often detected in patients with liver cirrhosis, but the precise pathogenetic mechanisms behind the phenomenon have not been clearly elucidated. We investigated the pathogenetic mechanisms of splenomegaly in both portal hypertensive patients and rats, and tried to identify the possible therapy for this disease.

Methods

Spleen samples were collected from portal hypertensive patients after splenectomy. Rat models of portal hypertension were induced by common bile duct ligation and partial portal vein ligation. Spleen samples from patients and rats were used to study the characteristics of splenomegaly by histological, immunohistochemical, and western blot analyses. Rapamycin or vehicle was administered to rats to determine the contribution of mTOR signaling pathway in the development of splenomegaly.

Results

We found that not only spleen congestion, but also increasing angiogenesis, fibrogenesis, inflammation and proliferation of splenic lymphoid tissue contributed to the development of splenomegaly in portal hypertensive patients and rats. Intriguingly, splenomegaly developed time-dependently in portal hypertensive rat that accompanied with progressive activation of mTOR signaling pathway. mTOR blockade by rapamycin profoundly ameliorated splenomegaly by limiting lymphocytes proliferation, angiogenesis, fibrogenesis and inflammation as well as decreasing portal pressure.

Conclusions

This study provides compelling evidence indicating that mTOR signaling activation pathway plays a key role in the pathogenesis of splenomegaly in both portal hypertensive patients and rats. Therapeutic intervention targeting mTOR could be a promising strategy for patients with portal hypertension and splenomegaly.  相似文献

The use of apheresis technique to collect platelets has rapidly increased in recent years. With an increased demand for plateletpheresis, higher donation frequencies are now observed. The aim of this study is to investigate changes in complete blood count values after frequent multiple plateletpheresis. A total of seventy-four blood donors were selected, from which complete blood count values in the first and the last screening were taken. There were fifty-four high frequency donors(13.0±2.6 plateletpheresis/year) and twenty low frequency donors(6.6±0.5 plateletpheresis/year). The results showed that complete blood count values at the first and last screening after plateletpheresis of both groups were within the normal range, and changes in their WBC, RBC and PLT values were not statistically significant(P>0.05, P>0.05, and P>0.05, respectively). The study suggests that frequent multiple plateletpheresis has no effects on complete blood count, and no adverse effects on blood donors.  相似文献

目的:观察复方浙贝颗粒(CZBG)对阿霉素(ADM)导致小鼠淋巴细胞白血病细胞(P388)移植瘤小鼠外周血象下降的影响。方法:将70只成功造模的昆明小鼠随机分为生理盐水对照、ADM常规剂量、CZBG、CZBG联合ADM大剂量、CZBG联合ADM常规剂量、CZBG联合ADM小剂量、CZBG联合ADM极低剂量7组,每组各10只小鼠。分别于给药前、实验第7天和实验第14天,从小鼠尾静脉取血20μl,抗凝处理后用日本SWSMEX公司SF3000型血液分析仪检测外周血象。结果:给药前各组WBC、R13C(除CZBG联合ADM极低剂量组)、Hgb(除生理盐水对照组)、PLT数值比较,无统计学意义(P〉0.05);实验第7天,大部分组WBC、RBC、Hgb、PLT数值有升高趋势;实验第14天,大部分组WBC、RBC、Hgb、PLT数值有降低趋势,与给药前比较无明显变化。结论:CZBG具有一定的舒缓ADM导致模型小鼠外周血象下降效应,其舒缓程度亦与ADM用药剂量相关。  相似文献

目的:探讨门静脉高压症脾切断流术后门静脉系统血栓形成的相关原因。方法:回顾性分析2010年4月-2011年12月我科450例因肝硬化门静脉高压症行脾切断流术患者的临床资料,应用超声多普勒检测手术前后门静脉血流速度、门静脉直径及脾静脉、肠系膜上静脉、门静脉血栓情况,用Logistic回归分析术前肝功能Child-Pugh分级、门静脉直径、门静脉血流速度、脾脏的质量及术后血小板数量与门静脉系统血栓形成的关系。结果:术前门静脉系统有血栓患者75例,占16.7%。术后门静脉血栓再形成率52.9%。Logistic单因素分析提示门静脉系统血栓形成与门静脉内径、门静脉血流速度、脾脏质量、血清总胆红素、术后血小板数量有关。多因素分析发现门静脉系统血栓形成与门静脉内径、门静脉血流速度、脾脏质量有关,而与血清总胆红素、术后血小板数量无关。结论:肝硬化门静脉高压症脾切除术后门静脉系统血栓形成与门静脉内径、门静脉血流速度、脾脏质量有关。  相似文献

目的:探讨部分性脾动脉栓塞术(PSE)治疗原发性肝癌合并脾功能亢进的临床疗效及安全性。方法:选择2008年5月至2013年4月我科收治的160例原发性肝癌合并有脾功亢进患者为研究对象,患者在行肝动脉化疗栓塞术(TACE)的同时进行PSE,PSE栓塞面积在30%~60%之间,观察治疗后3天、7天、1个月、3个月、6个月患者白细胞(WBC)计数和血小板(PLT)计数的动态变化情况。结果:以外周血白细胞计数达到4.0~13×109/L、血小板达计数到50×109/L为治疗有效,160例患者术后3个月时外周血白细胞治疗有效为151例(94.38%),血小板治疗有效为155例(96.88%),术后6个月时白细胞治疗有效136例(85%),血小板治疗有效125例(78.13%)。所有患者术后6个月均未发生严重并发症。结论:PSE联合TACE治疗原发性肝癌合并有脾功亢进患者具有良好的疗效,且安全性较高。  相似文献

目的:对肝硬化门静脉高压症大鼠行脾切除手术,观察术后大鼠免疫系统功能的变化情况,探讨脾切除术对免疫系统的影响,为临床治疗提供理论基础。方法:选取SD大鼠45只建立PHT模型,随机分为PHT组、脾全切组及部分脾切除组,每组各15只。另选取10只健康大鼠为对照组。分别对四组大鼠行不同的脾切除方案,术后检测四组大鼠的血常规、免疫球蛋白含量、T淋巴细胞亚群的变化及组织病理学染色结果等,分析各指标的意义。结果:四组大鼠血常规检测结果显示,PHT组大鼠WBC高于对照组,PHTTS组高于PHT组,PHTSS组高于PHTTS组,差异显著且具有统计学意义(P0.05)。四组大鼠血浆中IgG含量差异显著(P0.05);IgA、IgM含量无显著性差异(P0.05)。四组大鼠CD4+、CD8+含量差异显著(P0.05);CD29+含量无显著性差异(P0.05)。结论:脾脏直接参于细胞介导免疫调节,脾切除可降低门脉压力,消除脾亢,有利于肝硬化门脉高压症的缓解。  相似文献
