BACKGROUND:Women generally have longer life expectancy than men but have higher levels of disability and morbidity. Few studies have identified factors that explain higher mortality in men. The aim of this study was to identify potential factors contributing to sex differences in mortality at older age and to investigate variation across countries.METHODS:This study included participants age ≥ 50 yr from 28 countries in 12 cohort studies of the Ageing Trajectories of Health: Longitudinal Opportunities and Synergies (ATHLOS) consortium. Using a 2-step individual participant data meta-analysis framework, we applied Cox proportional hazards modelling to investigate the association between sex and mortality across different countries. We included socioeconomic (education, wealth), lifestyle (smoking, alcohol consumption), social (marital status, living alone) and health factors (cardiovascular disease, diabetes, mental disorders) as covariates or interaction terms with sex to test whether these factors contributed to the mortality gap between men and women.RESULTS:The study included 179 044 individuals. Men had 60% higher mortality risk than women after adjustment for age (pooled hazard ratio [HR] 1.6; 95% confidence interval 1.5–1.7), yet the effect sizes varied across countries (I2 = 71.5%, HR range 1.1–2.4). Only smoking and cardiovascular diseases substantially attenuated the effect size (by about 22%).INTERPRETATION:Lifestyle and health factors may partially account for excess mortality in men compared with women, but residual variation remains unaccounted for. Variation in the effect sizes across countries may indicate contextual factors contributing to gender inequality in specific settings.

Life expectancy has increased over the last 6 decades in many societies around the world.1 Women generally have longer life expectancy than men, yet have higher levels of disability and morbidity.2,3 Male:female mortality ratios increased from the beginning of the 19th century and slightly decreased over the last 3 decades.4,5 It has been suggested that the biological differences between the sexes, including genetics and hormones, provide stronger resilience to disadvantageous situations for women than men.6 However, biological sex is related to gender, a construct that also incorporates cultural and social differences between men and women. Although some studies suggest that the recent reduction in the male:female mortality ratio is likely a result of improvements in men’s health, lifestyle or occupational environments, others attribute it to women’s changing societal roles and increasing mortality from diseases such as lung cancer, which have traditionally affected mostly men.3,79 Many studies have examined the potential impact of social, behavioural and biological factors on sex differences in mortality,10,11 but few have been able to investigate potential variation across countries. Different cultural traditions, historical contexts, and economic and societal development may influence gender experiences in different countries, and thus variably affect the health status of men and women.We aimed to identify factors that may explain the difference in mortality risk between men and women at older age and to investigate potential variation across countries, using the harmonized data set of 12 cohort studies from the Ageing Trajectories of Health: Longitudinal Opportunities and Synergies (ATHLOS) consortium.12  相似文献

In a comparative study of neophilia, innovation and social attentiveness we exposed individuals in seven callitrichid species, from three genera, to novel extractive foraging tasks. The results revealed consistently shorter response latencies, higher levels of successful and unsuccessful manipulation, and greater attentiveness to the task and to conspecifics inLeontopithecus (lion tamarins) than in both Saguinus (tamarins) and Callithrix (marmosets). This is consistent with the hypothesis that species dependent upon manipulative and explorative foraging tend to be less neophobic and more innovative than other species. Furthermore, Callithrix appeared to be less neophobic than Saguinus; ifCallithrix is regarded as the greater specialist, this result is inconsistent with the hypothesis that neophobia is associated with foraging specialization. We consider the relevance of our findings to taxonomic relationships, and to technical and Machiavellian intelligence hypotheses and discuss the implications for captive breeding and reintroduction strategies.Copyright 2003 Published by Elsevier Science Ltd on behalf of The Association for the Study of Animal Behaviour.   相似文献

The longitudinal pattern of reproductive senescence was described in individual female rats from 4 to 18-22 mo of age. There were two subgroups of rats with different patterns of aging. The Constant Estrus (CE) subgroup progressed through regular cycles, irregular cycles, and constant estrus, followed by a return to irregular cycles, and then persistent diestrus. In contrast, the Irregular subgroup skipped constant estrus, maintaining irregular cycles until they entered persistent diestrus. In both subgroups, irregular cycles were a transition between the major reproductive states, although the type of transition was different in each subgroup. In the CE subgroup, the transition was gradual, continuous, and began with the onset of irregular cycles. In contrast, in the Irregular subgroup, the transition did not begin until the end of irregular cycles, suggesting that the process of aging was delayed. Most rats entered constant lordosis, a state characterized by a strong lordosis reflex that could be elicited by manual palpation on each day. The CE subgroup maintained the state once they entered it, whereas the Irregular subgroup intermittently returned to a lordosis reflex intensity characteristic of young rats. In addition, in the CE subgroup, but not the Irregular subgroup, changes in lordosis reflex intensity during aging were coupled to changes in the proportion of estrogenized vaginal smears during the cycle.  相似文献

Abstract

This paper examines the correlates of the relationship of age at first birth to completed parity, using data from a fifteen‐year longitudinal study. Particular emphasis is given to measures of orientation toward family roles which have been singled out in previous studies as a likely causal factor not yet examined. Women who had an early first birth went on to have larger families than women who postponed childbearing longer. Demographic factors, including premarital pregnancies, unwanted births, and fecundity differentials, did not appear to account for the observed relationship. The hypothesis that early socialization toward traditional female roles might account for the higher fertility of women who began childbearing early was not confirmed. Women with a first birth at a young age were not characterized by more traditional sex‐role behavior or attitudes nor did they express higher initial fertility preferences. They did more often increase their fertility preferences over the inter‐survey period.  相似文献

OBJECTIVES--To determine the nature and extent of psychosocial problems in epilepsy and their associations. DESIGN--A postal survey was used drawing data from prospective consecutive cases. Data on demographic, medical, and social backgrounds were collected. A specially designed, validated attitude questionnaire examined 14 areas of psychosocial adjustment to epilepsy. SETTING--124 primary care general practices. SUBJECTS--Adults (aged over 17) with a recent diagnosis of epilepsy (within previous 36 months). They were registered with the national general practice study of epilepsy and had a confirmed diagnosis according to the usual criteria. 216 subjects were approached for the survey by their general practitioners; 192 returned questionnaires. RESULTS--Problems in at least one area were experienced by 175 (91%) of the 192 subjects. Problems were generally mild, which contrasts strongly with findings in chronic cases. The areas of greatest concern were fear of seizures (80% of cases; 72% moderate or severe) and fear of stigma in employment (69% of cases; 40% moderate or severe). A highly significant relation was found between psychosocial effects and the frequency and recency of seizures. CONCLUSIONS--In the early stages of epilepsy psychosocial effects are closely related to the severity of the medical condition, suggesting that the argument for the stigmatising effect of the diagnosis by itself is less important than previously thought. The findings also suggest that problems may evolve as the condition becomes chronic.  相似文献

Participants in two Tay-Sachs screening programs were generally satisifed with the organization of the tests and the results. There was no evidence of adverse impact on reproductive plans or interpersonal relations, and the respondents professed to believe in the value of screening. While the carriers discussed their condition freely with others and were no less favorable to the idea of screening than the noncarriers, about one-half of their number expressed discomfort in being told they were heterozygotes. These feelings were allayed by counseling, but there was evidence of some residual unease. It is suggested that this anxiety would be less prominent and more easily reduced if screening were done under conditions of ordinary primary medical care rather than outside the conventional system.  相似文献

Although xerostomia in older adults has received substantial research attention, there have been few longitudinal studies of non-patient populations. Consequently, little is known about the incidence or course of this condition among this group. This paper reports the results of a longitudinal study designed to address these issues. In 1989, data on xerostomia were collected from 907 randomly-selected community dwelling adults aged 50 years and over. Three years later, 611 (71% of those presumed to be alive) were followed-up and data on xerostomia collected again. At baseline, 15.5% of these 611 subjects reported xerostomia, while at follow-up this had risen to 29.5%. The majority of the latter (115/180) were incident cases, reporting xerostomia only at follow-up, while the remainder were chronic cases, reporting xerostomia at both baseline and follow-up. A crude estimate of the three-year incidence rate was 22.5%. In a logistic regression analysis, three baseline variables were associated with incidence; older subjects, those with one or more chronic medical conditions and those reporting their general health as poor were more likely to develop xerostomia. In a similar analysis, age was the only variable associated with chronicity, with older subjects more likely to be chronic cases. The data also suggest that the onset of xerostomia was associated with an increase in other oral symptoms and problems with eating, communication and social interaction.  相似文献

Testosterone (T) has been found to have a stimulating effect on aggressive behavior in a wide range of vertebrate species. There is also some evidence of a positive relationship in humans, albeit less consistently. In the present study we investigated the relationship between T and aggression, dominance and delinquency over time, covering a period from early adolescence to adulthood. From a large population-based sample (n = 1.161) a subgroup of 96 boys was selected whose behavior had been assessed repeatedly by different informants from age 12 to 21 years, and who had provided multiple T samples over these years of assessment. On the whole, a decrease in aggressive and delinquent behavior was observed in a period in which T rises dramatically. Boys who developed a criminal record, had higher T levels at age 16. In addition, positive associations were observed between T and proactive and reactive aggression and self-reported delinquent behavior. Over the pubertal years different forms of aggressive and delinquent behavior were positively related to T, which may indicate that specific positive links are dependent on the social setting in which this relationship is assessed.  相似文献

Forty consecutive healthy pregnant women aged 17–38 yr who attended the antenatal clinic of the Department of Obstetrics and Gynecology, Ankara University in their first trimester participated in the study. The pregnant women were followed up longitudinally until the end of their pregnancy. Forty healthy age-matched nonpregnant women were used as a control group. Each pregnant woman was interviewed and a special questionnaire recording dietary history (3-d recall) and socioeconomic status (SES) was used. Birth weight, height, and head circumference of the newborn were measured and a complete physical examination was done for each neonate by the same observer. Blood samples were obtained at each trimester and zinc determinations were made using flame atomic absorption spectrophotometer. The results of plasma Zn measurements were available in 39 pregnant women. There were 23 women of low SES (mean plasma Zn level: 59.0 ± 6.9 μg/dL) and 16 of high SES (mean plasma Zn: 70.3 ± 5.2 μg/dL). The difference between the mean plasma Zn levels of these two groups was significant (p<0.001). The nutritional status in our study appeared to be an important factor responsible for low plasma Zn levels during pregnancy. However, we did not find any correlation between plasma Zn levels and anthropometric parameters of the newborn and pregnancy outcome. Further studies using larger sample sizes are needed to clarify the role of plasma Zn levels on maternal features and fetal outcomes in Turkey.  相似文献

Objectives To assess adult socioeconomic, educational, social, and psychological outcomes of childhood obesity by using nationally representative data.Design 1970 British birth cohort.Participants 16 567 babies born in Great Britain 5-11 April 1970 and followed up at 5, 10, and 29-30 years.Main outcome measures Obesity at age 10 and 30 years. Self reported socioeconomic, educational, psychological, and social outcomes at 30 years. Odds ratios were calculated for the risk of each adult outcome associated with obesity in childhood only, obesity in adulthood only, and persistent child and adult obesity, compared with those obese at neither period.Results Of the 8490 participants with data on body mass index at 10 and 30 years, 4.3% were obese at 10 years and 16.3% at 30 years. Obesity in childhood only was not associated with adult social class, income, years of schooling, educational attainment, relationships, or psychological morbidity in either sex after adjustment for confounding factors. Persistent obesity was not associated with any adverse adult outcomes in men, though it was associated among women with a higher risk of never having been gainfully employed (odds ratio 1.9, 95% confidence interval 1.1 to 3.3) and not having a current partner (2.0, 1.3 to 3.3).Conclusions Obesity limited to childhood has little impact on adult outcomes. Persistent obesity in women is associated with poorer employment and relationship outcomes. Efforts to reduce the socioeconomic and psychosocial burden of obesity in adult life should focus on prevention of the persistence of obesity from childhood into adulthood.  相似文献

The relation between racism, identity and well-being has stimulated political debate as well as psychological analysis. Research is reported that explores the relation among young people of white (n=97) and minority (n=174) ethnicity. On first assessment, minority ethnic participants reported: (1) more frequent experiences of racism and discrimination than white participants; and (2) higher levels of ethnic identity and equivalent levels of national identity. Three years later, depressive symptomatology was highest among minority ethnic participants, and (for this group only) positively associated with racism and discrimination reported at first assessment, and negatively associated with national identity.  相似文献

Background

The aim of this study was to investigate whether insomnia and obstructive sleep apnea (OSA) were predictors of hospitalizations or emergency department visits during two years following the Sao Paulo Epidemiologic Sleep Study (EPISONO) sample.

Methods and Findings

All participants (n = 1,101) who underwent a baseline evaluation between July and December 2007 were contacted in December 2009 and asked to fill out a questionnaire about body weight changes, number of hospitalizations and visits to the emergency department. Participants lost during the follow-up period represented 3.2% (n = 35) and 7 subjects had died. Hospitalizations were reported by 116 volunteers (10.5%) and emergency department visits were reported by 136 participants (12.4%). The average body mass index (BMI) did not vary significantly between the first and the second assessment [26.7(95%CI:26.3–27.1) vs. 26.9(26.5–27.4) kg/m2]. After adjusting for confounders, a multiple logistic regression model revealed that female gender [1.4(1.0–1.9)], age ≥40 years, insomnia diagnosed according to the DSM-IV criteria [1.6(1.0–2.6)], and apnea-hypopnea index ≥15 [1.5(1.0–2.2)] were predictors of hospitalizations and/or demand for emergency services.

Conclusion

Our study of a probabilistic sample of the Sao Paulo inhabitants shows that over a period of two years, insomnia and OSA were both associated with health impairment. Considering the high prevalence and public health burden of sleep disorders, the consequences of untreated disease for both the individual and society are undeniable and should be addressed.  相似文献

After implementation of programmes for active immunization against Haemophilus influenzae b, Streptococcus pneumoniae and Neisseria meningitidis became the most common agents of bacterial meningitis in childhood. Over a 9-year period, children showing clinical and laboratory findings of meningitis on the basis of their positive cultures of blood or cerebro-spinal fluid (CSF) for S. pneumoniae were enrolled. Predisposing conditions, clinical and laboratory findings, and microbiological and imaging studies were considered. Meningitis-related death or neurological sequelae defined an unfavourable outcome. Sixty-four patients met the inclusion criteria. Thirty-one (48%) children had predisposing conditions to pneumococcal meningitis. Fever and neck stiffness were the main symptoms; 14 patients (22%) reported seizures before admission. Twenty-one patients required treatment in the intensive care unit (ICU). Streptococcus pneumoniae strains were penicillin susceptible in 54 cases (84%). Forty-eight children (75%) showed complete recovery. Two patients (3%) died, and 14 (22%) had sequelae. Patients with a low CSF cell count, low neutrophils, early admission to ICU or infection by penicillin-nonsusceptible strains of S. pneumoniae had an unfavourable outcome more frequently. Low blood neutrophils, low CSF cell count, early admission to ICU and infection by penicillin-nonsusceptible strains are the main factors predicting an unfavourable outcome in children with pneumococcal meningitis.  相似文献

Migration is an energetically expensive and hazardous stage of the annual cycle of non‐resident avian species, and requires certain morphological adaptations. Wing shape is one of the morphological traits that is expected to be evolutionarily shaped by migration. Aerodynamic theory predicts that long‐distance migrants should have more pointed wings with distal primaries relatively longer than proximal primaries, an arrangement that minimizes induced drag and wing inertia, but this prediction has mostly been tested in passerine species. We applied the comparative method of phylogenetically independent contrasts to assess convergent evolution between wing shape and migration within shorebirds. We confirmed the assumption that long‐distance migrants have less rounded wings than species migrating shorter distances. Furthermore, wing roundedness negatively correlates with fat load and mean distance of migratory flights, the basic components of migration strategies. After controlling for interspecific differences in body size, we found no support for a link between wing length and migration, indicating that wing shape is a more important predictor of shorebird migratory behaviour than wing length. The results suggest that total migration distance and migratory strategy may simultaneously act on the evolution of wing shape in shorebirds, and possibly in other avian species.  相似文献

The purpose of this study was to establish the reproducibility of a series of anthropometric measures performed twice during one week during a three year period in boys and girls. The subjects of this investigation were 39 children (21 boys and 18 girls), 9-10 year of age at the beginning of the study. Children were measured three times with one year interval. Children were classified by Tanner stage 1-2 during the first measurements, stage 1-3 during the second measurements and stage 1-4 during the third measurements. Body height and weight were measured and BMI calculated. All anthropometric parameters were measured according to the protocol recommended by the International Society for the Advancement of Kinanthropometry (Norton & Olds 1996). Nine skinfolds, 13 girths, eight lengths and eight breadths/lengths were measured. The reproducibility of body height (r = 0.995-0.999), body weight (r = 0.990-0.999) and BMI (r = 0.969-0.999) was very high in boys and girls. The intraclass correlations (ICC), technical errors (TE) and coefficients of variation (CV) were quite different depending on the measurement site of the skinfold thickness. It was surprising that the ICCs were highest and TEs and CVs were lowest during the second year of the measurement. The computed ICC was high, and TE and CV values were quite similar and relatively low in girth, length and breadth/length measurements. It was concluded that the reproducibility of girths, lengths and breadths/lengths in children is very high and the reproducibility of skinfolds is high. Specifically, the reproducibility is very high immediately before puberty in boys and girls.  相似文献
