Reduced range of motion, prosthetic impingement, and joint dislocation can all result from misalignment of the acetabular component (i.e. cup alignment) in patients undergoing total hip arthroplasty. Most methods for acetabular component alignment are designed to provide 45-50 degrees abduction and 15-25 degrees of operative anteversion (also known as flexion) with respect to the anterior pelvic plane coordinate system. Yet in most cases, this coordinate system is not assigned properly, due to differences in patient anatomy and improper positioning in the operating room. This misalignment can result in an error in the cup alignment, which can cause the above-mentioned consequences. This work presents a complete mathematical formulation for the analysis of the inaccuracies related to the anterior pelvic plane axes (APPA) definition and their effect on final cup orientation. We do this by introducing a method taken from Kinematics of Mechanisms, and by representing the errors in the APPA as three concurrent axes of rotation, followed by the version and abduction rotations which are defined relative to the previous rotations. We also present a sensitivity analysis of the results by introducing differential changes between sequential coordinate frames, which simulates the errors in the APPA and their effect on cup orientation. Finally, we demonstrate a computational method which provides corrected version and abduction angles to achieve the desired cup orientation, given that the actual measurement errors are known.  相似文献

Background: Ceramic surfaces offer excellent tribological properties and for this reason are frequently used in younger patients with osteoarthritis of the hip. However, they may fracture easily owing to low tolerance to tensile and impact stresses in particular. We report a case of early fracture of a currently available ceramic liner in a 44-year-old woman with revision total hip arthroplasty. Methods and Results: Scanning electron microscopy and macroscopic fractographic examination were done to elucidate the cause of failure. These showed the fracture started most probably at the inner surface of the cup. Several mechanisms of fracture initiation are discussed in the paper. Conclusion: This is the first report on early fracture of the Plasma cup ceramic liner in white man. The fracture occurred intraoperatively or early postoperatively while the diagnosis was determined much later despite the patient had experienced sensations of subluxations long time before.  相似文献

An uncemented implanted acetabular cup must ensure primary stability and postoperative osteointegration. For both conditions the immediate postoperative contact between bone and the structured surface of the cup is of decisive importance. Two different types of acetabular cups manufactured by ESKA (standard model and Kapuziner model) were implanted in the innominate bones of elderly deceased persons. After plastination, thin serial sections were obtained. On the basis of X-rays of these sections, contact between bone and the reticulate surface of the cups was quantified. The quality of the contact was analysed densitometrically.  相似文献

The use of a perforated, titanium funicular shell to support the proximal femoral cortex in total hip arthroplasty was evaluated with the aid of both analytical and numerical techniques. The principal interactions between the femoral cortex, the metal shell, the implant stem and the acrylic bone cement were modeled using beam on elastic foundations theory and two-dimensional elasticity theory. Subsequent formulation of this model as a nonlinear design optimization problem enabled the determination of the dimensions of the implant and reinforcing shell which minimized an objective function based on a simplified material failure criterion. Two cases were examined, each with two cervico-diaphyseal angles: case A: with a rigid contact between a proximal prosthesis collar and the calcar femorale and case B: no collar contact (a collarless prosthesis or post-operative loosening). Case A achieved an optimal solution at a stem diameter 11-23 percent of the cortex inner diameter, a stem length to diameter ratio of 12-40, shell diameter 22-53 percent and thickness 0.2-7.2 percent of the cortex inner diameter and thickness, respectively. Case B achieved an optimal solution at a stem diameter 67-92 percent of the cortex inner diameter, length to diameter ratio of 4-6, and no shell. In case A the collar support makes the type of internal fixation unimportant, while in the more realistic case B, the shell is not recommended.  相似文献

A finite elements model was developed in order to evaluate the combined influence of the head lateral microseparation and the cup abduction angle on the contact pressure in Ceramic-on-Ceramic Total Hip Arthroplasty. The model's parameters were those used on the Leeds II hip simulator. A 32 mm head diameter and a 30 μm radial clearance was used. The cup was positioned with an abduction angle ranging from 45° to 90°. The medio-lateral microseparation varied from 0 to 500 μm. A load of 2500 N was applied through the head centre. For 45° abduction angle, edge loading appeared above a medial-lateral separation of 30 μm. Complete edge loading was obtained for a 60 μm medial-lateral separation. Under edge loading conditions, the contact area was found to be elliptical. For 45° abduction angle, as the head lateral separation increased, the maximal contact pressure increased from 66 MPa and converged to an asymptotic value of 205 MPa. Both cup abduction and lateral microseparation displacement induced a large increase in the stresses in Ceramic-on-Ceramic THA. However, this increase in contact pressure induced by higher abduction angle, became negligible as the lateral separation increased.  相似文献

Nascent phagosomes, which are derived from the plasma membrane, acquire microbicidal properties through multiple fusion and fission events collectively known as maturation. Here we show that remodeling of the phagosomal membrane is apparent even before sealing, particularly when large particles are ingested. Fluorescent probes targeted to the plasma membrane are cleared from the region lining the particle before engulfment is completed. Extensive clearance was noted for components of the inner as well as outer monolayer of the plasmalemma. Segregation of lipid microdomains was ruled out as the mechanism underlying membrane remodeling, because markers residing in rafts and those that are excluded were similarly depleted. Selective endocytosis was also ruled out. Instead, several lines of evidence indicate that endomembranes inserted by exocytosis at sites of ingestion displace the original membrane constituents from the base of the phagosomal cup. The Fcgamma receptors that trigger phagocytosis remain associated with their ligands. By contrast, Src-family kinases that are the immediate effectors of receptor activation are flushed away from the cup by the incoming membranes. Together with the depletion of phosphoinositides required for signal transduction, the disengagement of receptors from their effectors by bulk membrane remodeling provides a novel means to terminate receptor signaling.  相似文献

A sixth order nonlinear model for horizontal head rotations in humans is analyzed using an extended parameter sensitivity analysis and a global optimization algorithm. The sensitivity analysis is used in both the direct sense, as a model fitting tool, and in the indirect sense, as a guide to experimental design. Resolution is defined in terms of the sensitivity table, and is used to interpret the sensitivity results. Using sensitivity analyses, the head and eye movement systems are compared and contrasted. Controller signal parameters are the most influential. Their variations and effects on head movement trajectories and accelerations are investigated, and the conclusions are compared with clinical neurological findings. Particle swarm optimization (PSO) is a population-based, stochastic search algorithm inspired by the flocking behaviour of birds. The PSO algorithm has been shown to be rather sensitive to its control parameters, and thus, performance may be greatly improved by employing appropriately tuned parameters. However, parameter tuning is typically a time-intensive empirical process. Furthermore, a priori parameter tuning makes the implicit assumption that the optimal parameters of the PSO algorithm are not time-dependent. To address these issues, self-adaptive particle swarm optimization (SAPSO) algorithms adapt their control parameters throughout execution. While there is a wide variety of such SAPSO algorithms in the literature, their behaviours are not well understood. Specifically, it is unknown whether these SAPSO algorithms will even exhibit convergent behaviour. This paper addresses this lack of understanding by investigating the convergence behaviours of 18 SAPSO algorithms both analytically and empirically. This paper also empirically examines whether the adapted parameters reach a stable point and whether the final parameter values adhere to a well-known convergence criterion. The results depict a grim state for SAPSO algorithms; over half of the SAPSO algorithms exhibit divergent behaviour while many others prematurely converge.  相似文献

A method has been developed for quantifying movement and wear of the acetabular component (cup) of total hip replacements (THR) from routine postoperative and review radiographs. The method uses both interactive and automatic computer image analysis techniques. Dimensions of the prosthesis are used to scale the measurements and so overcom variation in radiographic alignment. The application of the method is illustrated by retrospective investigations of cup migration and wear using review radiographs taken over a follow-up of at least 12 years.  相似文献

Conventional laser scanning microscopy for multiple fluorescent stains can be a useful tool if the problems of autofluorescence and cross-talk are eliminated. The technique of spectral imaging was employed to unmix five different fluorophores - ranging in emission from 435 to 665 nm - applied to a Pseudomonas aeruginosa biofilm with overlapping spectra and which was not possible using traditional channel mode operation. Using lambda scanning and linear unmixing, the five fluorophores could be distinguished with regions of differentiation apparent.  相似文献

Abstract:Cladonia monomorpha is recognized as a distinct species in the Cladonia pyxidata group. It is characterized by thallus lobes with narrowly recurved margins, by the presence of discoid, bullate plates on the podetial surface and by long and sometimes branched proliferations of the scyphus margins supporting the apothecium discs. It is described from the Netherlands, where it occurs in acid inland sand dune areas with the highest terrestrial lichen diversity. Realistic stirring is an important concern when using benthic chambers to measure processes at the sediment-water column interface. A simple whirling cup rotor device that is driven by external flow and stirs benthic chambers effectively is described here. Flow tank studies and field studies demonstrated that this device homogenizes benthic chambers rapidly and responds quickly and linearly to external flow conditions. The device is simple to build and use and avoids many of the disadvantages of conventional stirring motors and pumps.  相似文献

In this investigation, a gas chromatography/mass spectrometry (GC/MS)-based metabolomic protocol for adherent cell cultures was developed using statistical design of experiments. Cell disruption, metabolite extraction, and the GC/MS settings were optimized aiming at a gentle, unbiased, sensitive, and high-throughput metabolomic protocol. Due to the heterogeneity of the metabolome and the inherent selectivity of all analytical techniques, development of unbiased protocols is highly complex. Changing one parameter of the protocol may change the response of many groups of metabolites. In this investigation, statistical design of experiments and multivariate analysis also allowed such interaction effects to be taken into account. The protocol was validated with respect to linear range, precision, and limit of detection in a clonal rat insulinoma cell line (INS-1 832/13). The protocol allowed high-throughput profiling of metabolites covering the major metabolic pathways. The majority of metabolites displayed a linear range from a single well in a 96-well plate up to a 10 cm culture dish. The method allowed a total of 47 analyses to be performed in 24 h.  相似文献
