摘要 目的:比较急性胰腺炎患者的电子计算机断层扫描(CT)、磁共振成像(MRI)影像学表现及其诊断价值。方法:选择2018年1月-2019年12月我院收治并初步诊断为急性胰腺炎的患者124例,所有患者均同时行CT和MRI检查,并比较两种检查方式诊断急性胰腺炎的影像学表现,以临床最终诊断结果作为参考,比较两种检查方式诊断急性胰腺炎的价值。结果:124例患者,经临床最终诊断为急性胰腺炎96例,28例为非急性胰腺炎,以临床最终诊断结果为"金标准",CT诊断急性胰腺炎的灵敏度、特异度、阳性预测值、阴性预测值和准确度分别为84.38%、75.00%、92.05%、58.33%、82.26%,MRI诊断急性胰腺炎的灵敏度、特异度、阳性预测值、阴性预测值和准确度分别为95.83%、78.57%、93.88%、84.62%、91.94%,MRI诊断急性胰腺炎的灵敏度、阴性预测值和准确度显著高于CT(P<0.05),两种检查方式诊断急性胰腺炎的特异度、阳性预测值比较无统计学差异(P>0.05)。结论:CT和MRI影像学表现有利于急性胰腺炎的诊断,两者对急性胰腺炎诊断均具有较高的灵敏度、特异度和准确度, 但MRI诊断急性胰腺炎的灵敏度、准确度优于CT。  相似文献

摘要 目的:探究CT联合MRI在早期肝癌诊断中的应用价值。方法:选择2016年6月至2019年6月于我院接受诊断治疗的78例已知或疑似肝癌患者,分别对其实施CT及MRI检测,以病灶部位病理学检查结果为金标准(50例确诊为早期肝癌,28例为良性病变),分别评估CT、MRI、CT联合MRI对早期肝癌的诊断价值,将确诊为肝癌的50例患者按照病灶大小区分为直径≤3 cm组(21例)和>3 cm组(29例),对比CT与MRI对不同直径肝癌诊断率。结果:(1)检测发现,CT对早期肝癌诊断一致性为73.08 %,灵敏度为72.00 %,特异度为75.00 %;(2)MRI对早期肝癌诊断一致性为82.05 %,灵敏度为82.00 %,特异度为82.14 %;(3)CT联合MRI检测对早期肝癌诊断一致性为93.59 %,灵敏度为92.00 %,特异度为96.43 %;(4)对比发现,对直径≤3 cm的早期肝癌患者,MRI诊断率明显高于CT(95.24 % vs 76.19 %,P<0.05)。结论:CT及MRI对早期肝癌均具有较好的诊断价值,但联合检测明显优于任一单独检测,同时对病灶直径≤3 cm的早期肝癌患者,MRI诊断准确率更高。  相似文献

摘要 目的:探讨血清白蛋白(AIb)、前白蛋白(PA)、中性粒细胞与淋巴细胞比值(NLR)水平在直肠癌术后发生吻合口瘘评估中的应用。方法:选择2018年6月至2019年12月于我院进行直肠癌手术患者90例患者进行研究,其中38例发生术后吻合口瘘,设为研究组, 52例未发生吻合口瘘作为对照组。分析患者术后血清AIb、PA、NLR水平变化情况,采用受试者工作特征曲线分析血清AIb、PA、NLR对术后发生吻合口瘘的评估作用。结果:研究组血清AIb、PA水平显著低于对照组,NLR水平显著高于对照组,差异显著(P<0.05);造口组术后血清AIb、PA水平显著高于未造口组,NLR水平显著低于未造口组,差异显著(P<0.05);ROC结果显示,血清AIb预测术后吻合口瘘的AUC为0.967,灵敏度为81.25%,特异度为90.14%,截断值为33.06 g/L;血清PA预测术后吻合口瘘的AUC为0.772,灵敏度为80.36%,特异度为89.56%,截断值为119.04 mg/L;血清NLR预测术后吻合口瘘的AUC为0.991,灵敏度为85.62%,特异度为93.23%,截断值为6.86。结论:监测血清AIb、PA、NLR水平有助早期发现直肠癌患者术后吻合口瘘。  相似文献

摘要 目的:探讨右室Tei指数、血清醛固酮水平对慢性阻塞性肺病(COPD)患者发生房颤的预测价值。方法:根据房颤的发生情况,将200例COPD患者分为房颤发生组和无房颤发生组。比较两组的病程、COPD严重程度、血清醛固酮(ALD)水平及右室Tei指数、肺动脉压、右心室横径的差异,分析右室Tei指数和ALD预测房颤发生的ROC曲线下面积、截断值、灵敏度及特异度。结果:房颤发生组病程(8.48±1.3和7.59±1.75)、右心室横径(40.52±2.74和36.27±2.4)、血清ALD(137.64±42.77和98.61±15.39)、右室Tei指数(0.37±0.12和0.31±0.07)、COPD、肺动脉高压的严重程度与无房颤发生组比较差异都有统计学意义(P<0.05)。logistic回归分析结果显示ALD、右室Tei指数、病程、和肺动脉高压程度为影响COPD患者发生房颤的独立影响因素。右室Tei指数预测房颤发生的ROC曲线下面积AUC=0.645,截断值为0.420,灵敏度为38.0%,特异度达到93.33%;ALD预测房颤发生的ROC曲线下面积为0.792,截断值为122.72 pg/mL,灵敏度为66.0%,特异度可达到98.0%。结论:右室Tei指数和血清醛固酮水平可作为慢性阻塞性肺病患者发生房颤的预测参考指标。  相似文献

摘要 目的:探究血浆内皮素配合描记动态脑电图在儿童晕厥诊断中的应用意义。方法:选择2017年1月至2020年1月于我院接受治疗的83例存在晕厥风险儿童,采集其静脉血样进行血浆内皮素水平测定,并实施动态脑电图检测,而后以倾斜试验结果为金标准,分别分析单纯血浆内皮素、单纯动态脑电图以及血浆内皮素+动态脑电图对晕厥的诊断应用意义。结果:(1)83例入组儿童中阳性为68例,阴性为15例,血浆内皮素检测阳性51例,阴性32例,一致性为77.11 %,灵敏度为73.53 %,特异度为93.33 %,阳性预测值为98.04 %, 阴性预测值为43.75 %;(2)动态脑电图诊断一致性为78.31 %,灵敏度为80.88 %,特异度为66.67 %,阳性预测值为91.67 %,阴性预测值为43.48 %;(3)联合检测诊断一致性为93.98 %,灵敏度为94.12 %,特异度为93.33 %,阳性预测值为98.46 %,阴性预测值为77.78 %;(4)检测方式差异性比较发现,联合检测在一致性、灵敏度、阴性预测值方面明显优于血浆内皮素和动态脑电图检测,在特异度方面优于动态脑电图检测(P<0.05)。结论:血浆内皮素联合描记动态脑电图对儿童晕厥具有较好的诊断辅助价值,能够显著提高诊断的一致性、灵敏度和特异度。  相似文献

摘要 目的:探讨心脏磁共振成像(CMRI)诊断原发性醛固酮增多症(PA)患者左室重构的价值。方法:选取2017年10月-2022年12月在辽宁省金秋医院诊断为PA的患者82例作为研究组,另选取同时期内在我院就诊的原发性高血压(EH)患者82例作为对照组。对比研究组和对照组临床资料、CMRI参数、生化及激素指标水平。根据研究组患者有无左室重构分为PA且有左室重构亚组(PA1组,n=38)和PA但无左室重构亚组(PA2组,n=44),对比PA1组和PA2组患者CMRI参数。采用多因素Logistic回归分析 PA患者左室重构的因素,建立回归方程及ROC曲线评价CMRI对PA患者左室重构的诊断效能。结果:研究组患者血清K、肾素活性低于对照组,醛固酮显著高于对照组(P<0.05)。研究组患者EDVi、ESVi、Massi、初始T1 mapping值和ECV均高于对照组,增强后T1 mapping值低于对照组(P<0.05)。多因素Logistic回归分析结果显示,ESVi、Massi、增强后T1 mapping值、ECV均与PA患者左室重构密切关联(P<0.05)。ROC分析结果显示,ESVi、Massi、增强后T1 mapping值、ECV各指标及其联合诊断PA患者左室重构时的AUC分别为0.623、0.677、0.736、0.675、0.779,说明联合诊断效能较高。结论:CMRI可定量评估PA患者的左室重构情况,其诊断效能较高,可为临床治疗、监测进展等提供参考依据。  相似文献

摘要 目的:探究Ki67与MR在宫颈癌根治术后宫颈癌淋巴结转移中评估价值的对比。方法:选择2016年1月至2018年1月于我院接受宫颈癌根治术的151例宫颈癌患者,分别对其实施Ki67检测及MRI检测,以病理学检测结果为金标准,计算两种检测方式对宫颈癌淋巴结转移评估的准确度、敏感度、特异度、阳性预测值及阴性预测值,并进行对比分析。结果:检测评估发现,MRI对宫颈癌淋巴结转移评估准确度为31.79 %,灵敏度为46.75 %,特异度为16.22 %,阳性预测值为36.73 %,阴性预测值为22.64 %。Ki67检测对宫颈癌淋巴结转移评估准确度为42.38 %,灵敏度为52.56 %,特异度为31.51 %,阳性预测值为45.05 %,阴性预测值为38.33 %。两种检测方式对比显示Ki67对宫颈癌淋巴结转移具有更高的诊断准确度。结论:3 相比于MRI检测,Ki67对宫颈癌淋巴结转移具有更高的诊断准确度、特异度和阴性预测值,分析其原因与MRI检测受个体因素影响更大有关。  相似文献

摘要 目的:探讨乳腺癌腋窝淋巴结转移患者应用多普勒超声与CT的诊断价值比较。方法:回顾性分析2017年3月至2019年3月我院接诊的60例经过手术病理证实的乳腺癌患者。比较多普勒超声与128排螺旋CT在乳腺癌腋窝淋巴结转移中的检出率、声像特征比较及两组灵敏度、特异度、准确度。结果:在术后经过病理证实的60例乳腺癌手术患者中,有38例为腋窝淋巴结转移,有22例未腋窝淋巴结转移,在多普勒超声诊断结果对乳腺癌腋窝淋巴结转移诊断中,36例得到确诊,在128排螺旋CT诊断中,30例得到确诊;多普勒超声皮质向心性生长、淋巴结内钙化灶、淋巴结横直径比值及淋巴结边界模糊检出率均显著高于128排螺旋CT检出率,差异显著(P<0.05);将病理结果作为金标准。多普勒超声灵敏度、特异度、准确度均比128排螺旋CT结果高,两组方式比较具有显著差异(P<0.05)。结论:多普勒超声在乳腺癌腋窝淋巴结转移中诊断价值高,可帮助临床提供正确诊断,以选择合适的治疗方案。  相似文献

摘要 目的:研究对比头颅电子计算机断层扫描血管造影(CTA)和头颅磁共振血管成像(MRA)在颅内动脉瘤中诊断价值。方法:回顾性分析医院2019年1月2日~2020年12月15日收治的60例疑似颅内动脉瘤患者资料,所有受试者均开展头颅CTA检查、MRA检查以及数字减影血管造影(DSA)检查。以DSA检查结果为金标准,比较头颅CTA和头颅MRA诊断颅内动脉瘤的效能。此外,对比头颅CTA和头颅MRA诊断不同分级、不同部位颅内动脉瘤的符合率。结果:DAS检查结果显示确诊动脉瘤患者48例,共检出动脉瘤74个,CTA检出动脉瘤65个,MRA检出动脉瘤51个,CTA动脉瘤检出率高于MRA检出率(均P<0.05)。以DSA结果为金标准,CTA诊断颅内动脉瘤的灵敏度、特异度以及准确度分别为94.20%(65/69)、60.00%(3/5)、91.89%(68/74),均高于MRA的82.26%(51/62)、41.67%(5/12)、75.68%(56/74)(均P<0.05)。CTA诊断1级颅内动脉瘤的符合率为13.85%(9/65),高于MRA的7.84%(4/51)(P<0.05)。各种检查方式诊断不同部位颅内动脉瘤的符合率对比均无明显差异(均P>0.05)。结论:CTA诊断颅内动脉瘤的灵敏度、特异度以及准确度均高于MRA,可提高对小直径肿瘤的检出率。然而,MRA具有无辐射、检测费用少、安全性高等优势,临床实际工作中可综合选择合理的检查方式。  相似文献

摘要 目的:探讨QRS时限值(QRS)、QT间期延长(QT)、QTc间期(QTc)及左室射血分数(LVEF)预测心源性猝死的价值分析。方法:选择2018年1月至2019年12月川北医学院附属医院心血管内科治疗的356例心源性猝死患者进行研究,设为病例组,并选择同期体检的健康人200例作为对照组,分析QRS、QT、QTc及LVEF水平变化情况及其预测价值。结果:病例组QRS、QTc水平显著高于对照组,QT、LVEF水平显著低于对照组,差异显著(P<0.05);轻度QRS、QTc显著低于中度、重度患者,QT、LVEF水平显著高于中度、重度患者;中度患者QRS、QTc显著低于重度患者,QT、LVEF水平显著高于重度患者,差异显著(P<0.05);ROC结果显示,QRS预测心源性猝死的AUC为0.989,灵敏度△为84.59%,特异度为87.68%,截断值为115.59ms;QT预测心源性猝死的AUC为0.944,灵敏度85.12%,特异度为88.45%,截断值为21.69ms;QTc预测心源性猝死的AUC为0.984,灵敏度为86.05%,特异度为88.61%,截断值为416.39ms,LVEF预测心源性猝死的AUC为0.997,灵敏度87.15%,特异度为89.05%,截断值为45.63%,(P<0.05)。结论:QRS、QT、QTc及LVEF在心源性猝死患者中检查,可显著提高心源性猝死临床诊断效能。  相似文献

Objective: Adrenal venous sampling (AVS) is the only available method to distinguish bilateral from unilateral primary aldosteronism (PA). AVS has several drawbacks, so it is reasonable to avoid this procedure when the results would not affect clinical management. Our objective was to identify a clinical criterion that can reliably predict nonlateralized AVS as a surrogate for bilateral PA that is not treated surgically.Methods: A retrospective diagnostic cross-sectional study conducted at Slovenian national endocrine referral center included 69 consecutive patients (mean age 56 ± 8 years, 21 females) with PA who underwent AVS. PA was confirmed with the saline infusion test (SIT). AVS was performed sequentially during continuous adrenocorticotrophic hormone (ACTH) infusion. The main outcome measures were variables associated with nonlateralized AVS to derive a clinical prediction rule.Results: Sixty-seven (97%) patients had a successful AVS and were included in the statistical analysis. A total of 39 (58%) patients had nonlateralized AVS. The combined criterion of serum potassium ≥3.5 mmol/L, post-SIT aldosterone <18 ng/dL, and either no or bilateral tumor found on computed tomography (CT) imaging had perfect estimated specificity (and thus 100% positive predictive value) for bilateral PA, saving an estimated 16% of the patients (11/67) from unnecessary AVS. ObjectiveIn patients with primary aldosteronism, adrenal venous sampling (AVS) is performed to determine the presence of unilateral or bilateral adrenal disease. During AVS, verification of catheter positioning within the left adrenal vein (AV) and the right AV by comparison of AV and inferior vena cava (IVC) cortisol levels can be variable. The objective of this study was to determine the utility of AV epinephrine levels in assessing successful AV cannulation.MethodsThis was a single institution, retrospective review of patients who underwent AVS with cosyntropin stimulation for primary aldosteronism between 2009 and 2018. Successful cannulation of the AV was defined by an AV/IVC cortisol ratio selectivity index (SI) ≥3:1. Epinephrine thresholds to predict catheter placement in the AV were determined using logistic regression. The calculated epinephrine thresholds were compared with previously published thresholds.ResultsAVS was performed on 101 consecutive patients and, based on the SI, successful cannulation of the left AV and right AV occurred in 98 (97%) and 91(90%) patients, respectively. The calculated optimal epinephrine threshold to predict AV cannulation was 364 pg/mL (sensitivity, 92.1%; specificity, 94.6%) and the calculated optimal AV/IVC epinephrine ratio threshold was 27.4, (sensitivity, 92.1%; specificity, 91.3%). Among the 14 patients with failed AV cannulation, 3 patients would have been considered to have successful AVS using AV epinephrine levels >364 pg/mL and AV/IVC epinephrine ratio >27.4 thresholds.ConclusionObtaining 2 right AV samples routinely as well as AV and IVC epinephrine levels during AVS could prevent unnecessary repeat AVS in patients with failed AV cannulation based on cortisol-based SI <3:1.  相似文献

Objective: This study investigated the characteristics of the adrenal limbs of primary aldosteronism (PA) patients and evaluated the value of the adrenal limb width measurement for the differentiation of unilateral PA from bilateral PA.Methods: A total of 122 PA patients (93 unilateral PA, ages ranged from 23 to 72 years; 29 bilateral PA, ages ranged from 30 to 68 years) who had undergone successful adrenal venous sampling (AVS) and adrenal gland computed tomography (CT) scan were retrospectively included. The maximum width of each adrenal gland limb (normal area on CT images) was measured, the left adrenal limb width to right adrenal limb width ratio (L/Rw) was calculated, and its potential value in the differentiation of unilateral PA and bilateral PA was analyzed.Results: The mean widths of the left adrenal limbs and the right adrenal limbs were 0.52 ± 0.10 cm and 0.43 ± 0.09 cm in unilateral PA patients, versus 0.52 ± 0.10 cm and 0.49 ± 0.12 cm in bilateral PA patients. The L/Rw ratio was 1.22 ± 0.24 in unilateral PA patients and 1.11 ± 0.23 in bilateral PA patients (P<.05). In the subgroup of PA patients over 55 years of age, compared with AVS, the sensitivity and specificity of the L/Rw ratio at 1.06 for subtype classification were 75% and 82%, respectively.Conclusion: A lower L/Rw ratio, referring to the ratio of the left adrenal limb width to the right adrenal limb width, may be a predictor of bilateral PA, especially in PA patients over 55 years of age.Abbreviations: APA = aldosterone-producing adenoma; AVS = adrenal venous sampling; BAH = bilateral adrenal hyperplasia; BMI = body mass index; CT = computed tomography; L/Rw = ratio of left adrenal limb width to right adrenal limb width; PA = primary aldosteronism  相似文献

Objective: Guidelines recommend withdrawing mineralocorticoid-receptor antagonists (MRAs) for 4 weeks prior to adrenal vein sampling (AVS), but this is not always feasible because of hypertension and hypokalemia. This retrospective study of primary aldosteronism (PA) patients who underwent AVS between 2008 and 2018 assessed the effect of continuing MRA on the AVS procedure.Methods: Clinical data including antihypertensive regimen defined by the World Health Organization Daily Defined Dose (DDD) system were collected for 19 patients with adequate cannulation and lateralization during AVS. Results were compared between 5 patients who continued and 14 patients who discontinued MRA therapy (MRA and non-MRA groups).Results: At diagnosis, plasma renin activity, plasma aldosterone concentration (PAC), potassium (K) doses, and DDD were not significantly different between groups. Aldosterone-renin ratio was significantly higher in the MRA group (median, 375.0; interquartile range &lsqb;IQR], 224.8 to 544.3 vs. 148.7, 118.4 to 192.1; P = .034). No difference was found in lateralization index (median 48.3; IQR, 23.6 to 52.1 vs. 8.7; 4.9 to 20.2; P = .10). Contralateral suppression, defined as aldosterone-cortisol ratio of unaffected adrenal to periphery, trended lower in the MRA group (median, 0.17; IQR, 0.03 to 0.39 vs. 0.51; 0.27 to 1.1; P = .056). All five MRA patients underwent successful adrenalectomy with at least 50% reduction in DDD and PAC and normal K postoperatively. One MRA patient did not lateralize, which was confirmed on repeat AVS, after MRA withdrawal.Conclusion: Continuation of MRA may not interfere with AVS lateralization or affect contralateral adrenal suppression. Continuation of MRA in preparation for AVS may be considered, especially in patients with severe PA, to avoid uncontrolled hypertension and severe hypokalemia.  相似文献

ObjectiveTo describe the challenging case of a 59-year-old male with a deoxycorticosterone (DOC)-producing adrenal adenoma concomitant with an aldosterone-producing microadenoma.MethodsWe measured the patient’s aldosterone and progesterone levels during adrenal venous sampling (AVS). The steroidogenic enzyme expression was studied with in situ hybridization (ISH). Steroids profiles were determined in the peripheral serum obtained before and after the operation, as well as in the main adrenal tumor.ResultsThe patient was diagnosed with primary aldosteronism (PA) based on typical clinical findings. He had an adrenal tumor located at the lower pole of the left adrenal gland. The aldosterone concentration in the adrenal vein proximal to the adrenal tumor was higher than that of the ipsilateral adrenal vein distal to the tumor during the AVS. Progesterone was only elevated in the adrenal vein proximal to the tumor, suggesting that the tumor produced steroids other than aldosterone. The postoperative findings revealed that the main tumor was accompanied by 2 microadenomas. The main adrenal tumor was diagnosed as a DOC-producing adenoma, and one of the microadenomas was diagnosed as aldosterone-producing based on the ISH and the determination of the steroid profiles.ConclusionsConcomitant PA masked the key findings of a DOC-producing tumor; the suppression of aldosterone in this patient. Multiple sampling in the adrenal vein considering the location of the adrenal tumor provided a clue to the diagnosis. Progesterone measurement during AVS is easy and may be useful in diagnosing rare adrenal tumors that produce intermediate products in adrenal steroid biosynthesis. Objective: We prospectively investigated the accuracy of the seated saline suppression test (SSST) in 113 patients with hypertension (including 93 primary aldosteronism [PA] and 20 essential hypertension patients) in the Department of Endocrinology and Metabolism.Methods: Each patient underwent a recumbent saline suppression test (RSST) and SSST. The accuracy of the SSST for a confirmative PA diagnosis and subtype classification was evaluated and compared with the RSST.Results: The area under the receiver operating characteristic (ROC) curve of plasma aldosterone concentration (PAC) for the SSST was significantly greater than that for the RSST (0.945 ± 0.0199 vs. 0.828 ± 0.0404; P<.05). The ROC analysis showed that the optimal PAC cut-off values were 12.94 ng/dL for the SSST (sensitivity 86.02%, specificity 95%; Youden index [YI] 0.810) and 12.04 ng/dL for the RSST (sensitivity 83.15%, specificity 57%; YI 0.401). The optimal PAC cut-off value for classifying aldosterone-producing adenoma and idiopathic hyperaldosteronism was 18.12 ng/dL for the SSST (sensitivity 73.5%, specificity 79.5%). No patients experienced adverse events during the SSST.Conclusion: The SSST is safe and convenient for PA diagnosis. The accuracy of the SSST for a confirmatory diagnosis of PA was better than that of the RSST. 摘要 目的:探讨与分析128排电子计算机断层扫描(Computed Tomography,CT)对股骨头置换术后下肢深静脉血栓形成的诊断价值。方法:2015年1月到2020年7月选择在本院诊治的股骨头置换术后疑似下肢静脉血栓形成患者78例作为研究对象,所有患者都给予128排CT检查,记录影像学特征并判断诊断价值,分析下肢深静脉血栓形成的影响因素。结果:在78例患者中,128排CT判断图像优69例,良9例,优良率为100.0 %。静脉造影判定为术后发生下肢深静脉血栓形成11例(DVT组),发生率为14.1 %,检出病变血管45支。二分类多因素Logistic回归分析显示术中出血量、手术时间、使用激素、年龄是导致股骨头置换术后下肢深静脉血栓形成的重要因素(P<0.05)。DVT组的血容量(cerebral blood volume,BV)与达峰时间(time to peak,TTP)值高于非DVT组(P<0.05),血流量(blood flow, BF)与平均通过时间(mean transit time,MTT)值低于非DVT组(P<0.05)。DVT组的血管狭窄评分低于非DVT组(P<0.05)。128排CT对股骨头置换术后下肢深静脉血栓形成的诊断敏感性与特异性为100.0 %和97.0 %。结论:术中出血量、手术时间、使用激素、年龄是导致股骨头置换术后下肢深静脉血栓形成的重要因素,128排CT能有效检出下肢深静脉血栓形成情况,具有方便、快捷、无创的特点,可为临床诊治提供可靠依据。  相似文献

ABSTRACT: BACKGROUND: The existence of unilateral adrenal hyperplasia (AH) has been considered a rare cause of primary hyperaldosteronism (PA). METHODS: In a prospective study we screened for PA in a non-selected (NSP) and selected hypertensive population (SP), to define the cause of PA. We included 353 consecutive patients with hypertension; age 20 to 88 years, 165 women and 188 men, from a university-based Hypertension and Nephrology Outpatient clinics (123 SP) and two primary care centres, (230 NSP) from the same catch-up area. Serum aldosterone and plasma renin activity (PRA) were measured and the ARR calculated. Verifying diagnostic procedure was performed in patients with both elevated aldosterone and ARR. Patients diagnosed with PA were invited for adrenal venous sampling (AVS) and offered laparoscopic adrenalectomy when AVS found the disease to be unilateral. RESULTS: After screening, 46 patients, 13% of the whole population (22.8% SP and 7.8% NSP) had aldosterone and ARR above the locally defined cut-off limits (0.43 nmol/l and 1.28 respectively). After diagnostic verification, 20 patients (6%) had PA, (14.5% SP and 1.4% NSP). Imaging diagnostic procedures with CT-scans and scintigraphy were inconclusive. AVS, performed in 15 patients verified bilateral disease in 4 and unilateral in 10 patients. One AVS failed. After laparoscopic adrenalectomy, 4 patients were found to have adenoma and 5 unilateral AH. One patient denied operation. CONCLUSION: The prevalence of PA was in agreement with previous studies. The study finds unilateral PA common and unilateral AH as half of those cases. As may be suspected PA is found in much higher frequency in specialised hypertensive units compared to primary care centers. AVS was mandatory in diagnosis of unilateral PA.  相似文献
