This study investigates potential associations between CD36 gene variants and the presence of risk factors in Caucasians with coronary artery disease (CAD) manifested at a young age. The study group consisted of 90 patients; the men were ≤ 50 years old and the women were ≤ 55 years old. Amplicons of exons 4 and 5 including fragments of introns were analyzed by DHPLC. Two polymorphisms were found: IVS3-6 T/C (rs3173798) and IVS4-10 G/A (rs3211892). The C allele of the IVS3-6 T/C polymorphism was associated with higher prevalence of obesity and diabetes, higher hsCRP, lower Lp(a) serum concentrations, and younger age at myocardial infarction. The A allele of the IVS4-10 G/A polymorphism was associated with older age of myocardial infarction and higher white blood cell count. The functional role of CD36 polymorphisms in CAD development needs further research.  相似文献

Seventy heart preparations of persons belonging to different sex and age have been investigated, using a complex of anatomical and histological techniques. The dimensions of the sinoatrial node (SAN) vary with age and depend on various size and form of the heart. The large atrial branch of the right and left coronary arteries supplies mainly the SAN with blood. More seldom the atrial branches of both cardiac arteries, having anastomoses, realize the SAN blood supply. The character of the SAN vascularization depends on branching variations of the atrial vessels. At the right coronary variant the sources of the SAN blood supply are the SAN branch, the right intermediate or right posterior atrial branches, and at the left coronary variant--the anterior left, the posterior left and the intermediate left atrial branches. At the even variant the SAN blood supply sources are the right intermediate and the anterior left atrial or the right posterior and the left posterior atrial branches. The data obtained can be used for comparison with the results of coronography to make a skilled analysis of clinical-roentgenological observations.  相似文献

The effects of spermine (SPM) on the production of Th1 and Th2 cytokines using peripheral blood mononuclear cells (PBMCs) derived from cedar pollinosis subjects were investigated. Th2 cytokines produced by PBMCs stimulated by 10 microM SPM was significantly lower than that in the controls. These findings indicate that blood SPM is an important factor in the Th1/Th2 balance.  相似文献

为研究湖北十堰地区急性冠脉综合征(ACS)患者PAPP-A基因IVS6 +95(C/G)多态性与血浆PAPP-A水平和斑块性质的相关性,随机选择215例ACS患者(包括58例急性心肌梗死(AMI)患者、73例不稳定型心绞痛(UAP)患者和84例稳定型心绞痛(SAP)患者)及142例健康对照者作为研究对象,检测PAPP-A基因IVS6+ 95 (C/G)多态性和血浆PAPP-A浓度,并进行统计学分析.结果显示,AMI组、UAP组和SAP组的PAPP-A浓度具有极显著差异(p<0.01),在稳定性斑块和易损性斑块组间的PAPP-A浓度亦具有极显著差异(p<0.01);AMI组、UAP组和SAP组与对照组间的基因型频率和等位基因频率具有显著差异(p<0.05);在ACS组和对照组中,组内各类基因型人群的血浆PAPP-A浓度的差异不显著(p>0.05).结果表明,PAPP-A基因IVS6+ 95多态性与急性冠脉综合征患者密切相关,血浆PAPP-A浓度与斑块性质相关,能够反映ACS患者斑块的易损性,是ACS患者危险分层的标志物.  相似文献

The possible differential effect of positive inotropic stimulation upon regional myocardial oxygen balance in the two ventricles was investigated during tachycardia and paired electrical stimulation. Isometric contractile force was measured by strain gauge arches; local coronary blood supply was measured by thermistor probes and intracellular NADH redox level was recorded using surface fluorometry. It was found that when contractility was increased by paired stimulation at a basic rate of 140 bpm, the inotropic response was more pronounced in the right ventricle (97.2 +/- 11.5%) than in the left (63.1 +/- 12.6%). Coronary blood supply to the left ventricle increased by 117.8 +/- 30.4% and the corresponding NADH redox level increased by 54.3 +/- 19.9%. When the contractile force was increased to the same extent (64.1 +/- 8.9%) by single stimulation at a rate of 210 bpm, the coronary flow to the left ventricle was increased by only 36.4 +/- 11.0% and the NADH state rose by 67.1 +/- 12.1%. It is concluded that paired stimulation reduced the mechanical limitation to flow during tachycardia, thus allowing coronary blood supply to increase in response to positive inotropic stimulation, thereby preserving a relatively improved oxygen state. It was also observed that the ratio contractile force/blood supply (contraction efficiency) was usually proportional to the NADH redox level (oxygen balance). Nevertheless, variations observed in the force/supply ratio for the left ventricle indicate that the NADH redox level cannot be predicted quantitatively by the force/supply ratio.  相似文献

The method of quantitative evaluation of blood supply of ischemic myocardial focus was suggested, it consists in measuring the venous blood outflow from the ischemic area with the help of ultrasonic technique. In experiments on anaesthetized dogs with coronary artery occlusion the rate of blood flow and dynamics of its changes in the ischemic focus were determined. It was shown that sodium oxybutyrate improves the blood supply of myocardial ischemic area.  相似文献

本文在实验性开胸犬上,用一个微米缩窄器定量造成冠脉左旋支三种狭窄程度,并测量了血液动力学、血气和冠状窦pH、乳酸值。 冠脉轻度狭窄时,左旋支每分血流量(CBF)未下降;而乳酸值增加。临界狭窄时,CBF轻度下降,心肌耗氧量(MVO_2)随之下降而乳酸值增加。重度狭窄时,CBF、MVO_2、心肌供氧/耗氧比值和冠状窦pH值均下降;而氧提取率和乳酸值增加。冠脉狭窄大于75%后,狭窄程度与心肌供氧有明显相关(r=-0.92);而与心肌耗氧呈弱相关(r=-0.58)。  相似文献

Few studies have investigated factors responsible for the O2 demand/supply balance in the right ventricle. Resting right coronary blood flow is lower than left coronary blood flow, which is consistent with the lesser work of the right ventricle. Because right and left coronary artery perfusion pressures are identical, right coronary conductance is less than left coronary conductance, but the signal relating this conductance to the lower right ventricular O2 demand has not been defined. At rest, the left ventricle extracts approximately 75% of the O2 delivered by coronary blood flow, whereas right ventricular O2 extraction is only ~50%. As a result, resting right coronary venous PO2 is approximately 30 mm Hg, whereas left coronary venous PO2 is approximately 20 mm Hg. Right coronary conductance does not sufficiently restrict flow to force the right ventricle to extract the same percentage of O2 as the left ventricle. Endogenous nitric oxide impacts the right ventricular O2 demand/supply balance by increasing the right coronary blood flow at rest and during acute pulmonary hypertension, systemic hypoxia, norepinephrine infusion, and coronary hypoperfusion. The substantial right ventricular O2 extraction reserve is used preferentially during exercise-induced increases in right ventricular myocardial O2 consumption. An augmented, sympathetic-mediated vasoconstrictor tone blunts metabolically mediated dilator mechanisms during exercise and forces the right ventricle to mobilize its O2 extraction reserve, but this tone does not limit resting right coronary flow. During exercise, right coronary vasodilation does not occur until right coronary venous PO2 decreases to approximately 20 mm Hg. The mechanism responsible for right coronary vasodilation at low PO2 has not been delineated. In the poorly autoregulating right coronary circulation, reduced coronary pressure unloads the coronary hydraulic skeleton and reduces right ventricular systolic stiffness. Thus, normal right ventricular external work and O2 demand/supply balance can be maintained during moderate coronary hypoperfusion.  相似文献

The effects of beta-adrenergic blockade upon myocardial blood flow and oxygen balance during exercise were evaluated in eight conscious dogs, instrumented for chronic measurements of coronary blood flow, left ventricular pressure, aortic blood pressure, heart rate, and sampling of arterial and coronary sinus venous blood. The administration of propranolol (1.5 mg/kg iv) produced a decrease in heart rate, peak left ventricular (LV) dP/dt, LV (dP/dt/P, and an increase in LV end-diastolic pressure during exercise. Mean coronary blood flow and myocardial oxygen consumption were lower after propranolol than at the same exercise intensity in control conditions. The oxygen delivery-to-oxygen consumption ratio and the coronary sinus oxygen content were also significantly lower. It is concluded that the relationship between myocardial oxygen supply and demand is modified during exercise after propranolol, so that a given level of myocardial oxygen consumption is achieved with a proportionally lower myocardial blood flow and a higher oxygen extraction.  相似文献

We offer a comprehensive classification of coronary artery anomalies, together with angiographic examples of each entity. Minimal requirements for normality include the following criteria: (1) the dual aortic origin is from right and left coronary ostia; (2) the course of the right coronary artery follows the right atrioventricular groove; (3) the course of the left coronary artery follows the left atrioventricular groove and anterior interventricular groove; (4) the posterior descending branch originates from either the right or left coronary artery; (5) the major coronary branches flow epicardially; and (6) the coronary arteries terminate at the myocardial capillary level. This conception of "normal" coronary arteries has determined the classification of abnormalities presented here. Early and correct diagnosis of anomalies that may compromise the myocardial blood supply is stressed, and possible surgical solutions are offered. Selective coronary angiography is the technique of choice for precise visualization of the coronary artery system.  相似文献

Adenosine and inosine are believed to have cardioprotective effects. However, little is known about their possible role in the metabolic autoregulation of human coronaries and in pathologic conditions with supply/demand imbalance of the heart such as coronary artery disease. Since these low molecular weight nucleosides freely diffuse through the monolayer of the visceral pericardium, adenosine and inosine concentrations in pericardial fluid may well reflect the conditions in cardiac interstitium. The pericardial fluid and systemic venous blood adenosine and inosine concentrations were measured in 98 human subjects undergoing heart surgery for coronary artery disease or valvular heart disease. Adenosine and inosine concentrations were measured by HPLC with UV detection. In subjects with coronary artery disease pericardial fluid nucleoside concentrations were significantly higher than in patients with valvular heart disease (adenosine: 1545 (996-3146) nmol/L [median (25th-75th quartiles)] vs. 738 (390-2527) nmol/L, P<0.01; inosine: 658 (321-1331) nmol/L vs. 347 (159-1037) nmol/L, P<0.05), while in both patient groups pericardial fluid nucleoside concentrations were higher by an order of magnitude than in venous plasma. Our results show the enhanced release of adenosine and inosine by the ischemic myocardium as a marker of supply/demand imbalance and support the hypothesis that these cardiac nucleosides may have an important role in the adaptation of coronary blood flow in human coronary artery disease.  相似文献

摘要 目的:探讨糖尿病(DM)对射血分数保留心力衰竭(HFpEF)患者血生化指标、心脏指标及生活质量的影响。方法:选取2017年1月-2019年5月我院收治的246例HFpEF患者作为研究对象,根据是否并发DM分为DM-HFpEF组(n=98)和NDM-HFpEF组(n=148),比较两组患者基线资料、血生化指标,采用超声心动图检测心功能参数,采用明尼苏达心力衰竭生活质量调查表(MLHFQ)评价患者的生活质量。结果:两组患者体重、收缩压、合并冠心病比例、合并高血压比例相比较,差异有统计学意义(P<0.05);与NDM-HFpEF组患者相比,DM-HFpEF组患者白细胞计数(WBC)、中性粒细胞计数(N)、血肌酐(Scr)、甘油三酯(TG)、空腹血糖(FBG)、餐后两小时血糖(2hPBG)、K+水平升高,血红蛋白(Hb)、高密度脂蛋白胆固醇(HDL-C)水平降低(P<0.05);DM- HFpEF组舒张末期左心室容积指数(LVEDVI)低于NDM-HFpEF组患者,室间隔厚度(IVS)、左心室后壁厚度(PWTD)、E峰、E/e''高于NDM-HFpEF组患者,差异有统计学意义(P<0.05);DM- HFpEF组患者MLHFQ中体力限制、社会限制、情绪、经济维度评分及总分高于NDM-HFpEF组患者,差异有统计学意义(P<0.05)。结论:DM促进了HFpEF患者IVS、PWTD的增厚,降低了心脏舒张功能和患者的生活质量,且明显加重了HFpEF患者血糖血脂的代谢紊乱。  相似文献

The paper presents current principles in designing diagnostic coronary angiography by taking into account of fundamental achievements in endovascular medicine in diagnosing and treating various heart diseases. Analyzing the world and Russian experience gained, the authors present basic qualitative and quantitative requirements for X-ray contrast study of cardiac vessels to enhance the information value of coronary angiography. They consider necessary diagnostic criteria to be achieved during the study to choose a surgical or endovascular treatment: a type and number of diagnostic film projections, X-ray anatomy of coronary vessels. The present-day accumulated data on X-ray anatomic features of coronary vessels in coronary heart disease are classified. Current classifications of coronary circulation, principles in the development of collateral blood supply, main sources of collaterals, and the specific features of the coronary bed in patients with coronary heart disease are also given.  相似文献

The SLC14A1 gene, which encodes the important Kidd blood group antigens, has not been systematically?analyzed at the molecular level in Chinese individuals. In this study, SLC14A1 genetic polymorphism was examined in Chinese individuals with Jk(a+b-), Jk(a+b+), and Jk(a-b+) expression. The Kidd phenotype was determined for 146 specimens using monoclonal anti-Jka and -Jkb antibodies. From these, 87 specimens were Jk(a-b+), 21 were Jk(a+b-), and 38 were Jk(a+b+). According to the Kidd phenotype results, 20 specimens were randomly selected from each group, i.e., Jk(a-b+), Jk(a+b-), and Jk(a+b+), for the molecular analyses of exons 3 to 11 of the SLC14A1 gene. Novel alleles were detected in the SLC14A1 gene, including IVS3-106A, IVS3-99A, exon3 130G, IVS4-299G, IVS4-293G, IVS4+211C, IVS4 +230C, exon6 499A, exon6 588A, IVS7-68T, IVS9+244G, and IVS10-153T, indicating that the locus harbored significant polymorphism. We also showed that IVS4-299, IVS7-68, and IVS10-153 were novel SNPs absolutely associated with exon 8 nt. 838. The minor allele frequencies were all greater than 10% and all SNPs in the Chinese population showed Vel antigen expression on RBC membranes. We identified 12 SNPs in the SLC14A1 gene in the Chinese population, IVS3-106A, IVS3-99A, exon3 130G, IVS4-299G, IVS4-293G, IVS4+211C, IVS4 +230C, exon6 499A, exon6 588A, IVS7-68T, IVS9+244G, and IVS10-153T. Our results also indicated that three novel SNPs produced Jka and Jkb antigens in Chinese individuals.  相似文献

We have previously demonstrated a 40% increase in myocardial blood flow (MBF) during hypercapnia but no significant decrease of MBF during hypocapnia. The present study was undertaken to evaluate if epinephrine infusion, which increases both myocardial oxygen consumption (MVo2) and myocardial performance, might influence the effects of hypocapnia and hypercapnia on MBF. Induction of hypocapnia was performed by hyperventilation in closed-chest dogs anesthetized with pentobarbital. By adding carbon dioxide to the inspiratory gas, normocapnia and hypercapnia were created. Epinephrine infusion (0.8 microgram X kg-1 X min-1) increased MBF and cardiac output (CO) by 90 and 140%, respectively, while MVo2 was increased by 45%. Epinephrine had a direct coronary vasodilating effect in excess of myocardial needs evidenced by increased oxygen content of the coronary sinus blood. During epinephrine infusion, induction of hypocapnia effected no change of MBF, while myocardial oxygen extraction increased significantly. Although oxygen saturation (So2) and Po2 in the coronary sinus blood decreased, these values remained well above those with hypocapnia without epinephrine infusion, thereby excluding impaired oxygen supply to the heart. Hypercapnia induced an increase of MBF by nearly 40% despite the coronary vasodilatation already induced by epinephrine infusion.  相似文献

Data are presented from several campaigns concerning to the residual transport of water, salt and suspended particulate matter (SPM) at different depths and at different longitudinal and cross-sectional locations in the Elbe Estuary, Northern Germany. The intratidal behaviour of SPM is analysed and explained in terms of cyclic settling and resuspension. Superimposed on these local processes are advective horizontal transports of water and SPM in different vertical layers. Tidally averaged horizontal fluxes of salinity and SPM have been calculated at different depths. For the most part, they show upstream directed transport in the bottom layers, while in the upper water column SPM is carried towards the sea. Examples of the influence of vertical salinity gradients on SPM dynamics, of the lateral variation of residual transport of water and SPM, and the effects of a runoff peak are also presented.  相似文献

The arterial blood supply to the sino-atrial (S-A) and atrio-ventricular (A-V) nodes was studied in 63 baboon hearts (Papio cynocephalus and Papio hamadryas). The arterial coronary injection was made with latex neoprene (23 hearts), a latex and lead oxide mixture (30 hearts) and polyester resin (10 hearts) for dissection, radiographic and corrosion cast studies, respectively. The S-A artery is a long recurrent left coronary branch (92.1%) or an ascendant less long right coronary branch (7.9%). The A-V arteries were 2 or 3 small branches of the interventricular posterior artery originating from the right or the left with the same frequency as in man. The percent distribution of the other coronary branches was similar to that observed in man.  相似文献

We genotyped 19 neurofibromatosis type 1 (NF1) families from French Canadians of the Quebec population with four intragenic microsatellites (IVS26-2.3, IVS27AC28.4, IVS27AC33.1, and IVS38GT53.0). Linkage analysis of the four microsatellite markers among the 19 NF1 families indicates that the four microsatellites are strongly linked with NF1 disease (LOD = 2.76-3.64). The four markers are associated (P = 0-0.077) except marker pair IVS26-2.3/IVS27AC33.1 (P = 0.18 or 0.17). However, perhaps due to the high mutation rate of the NF1 gene, no founder effect for NF1 was detected in the Quebec French Canadians.  相似文献
