ABSTRACT

The signing of the Pets Evacuation and Transportation Act into law will require states to include companion and service animals in their disaster response plans. Consequently, animal stakeholders across the United States are or will be involved in assessing their response capabilities as a part of developing or improving plans to evacuate and house animals. This paper reports on a mock disaster response exercise involving the evacuation of an urban animal shelter. A simulated emergency provided the opportunity to test the shelter's disaster evacuation capabilities and to illuminate issues that animal stakeholders should address when creating and refining emergency response plans. The participants successfully evacuated all animals from the building in good time, but the exercise highlighted two pitfalls. The first concerns the use of volunteers who have no training in disaster response but who nevertheless want to help. In situations involving animals, large numbers of well-meaning but untrained people will volunteer, and animal stakeholders should anticipate and manage their arrival. The second concerns the inevitable presence of the media and the need for a single point of contact for information about the incident.  相似文献

Background

The concept of disaster surge has arisen in recent years to describe the phenomenon of severely increased demands on healthcare systems resulting from catastrophic mass casualty events (MCEs) such as natural disasters and terrorist attacks. The major challenge in dealing with a disaster surge is the efficient triage and utilization of the healthcare resources appropriate to the magnitude and character of the affected population in terms of its demographics and the types of injuries that have been sustained.

Results

In this paper a deterministic population kinetics model is used to predict the effect of the availability of a pediatric trauma center (PTC) upon the response to an arbitrary disaster surge as a function of the rates of pediatric patients' admission to adult and pediatric centers and the corresponding discharge rates of these centers. We find that adding a hypothetical pediatric trauma center to the response documented in an historical example (the Israeli Defense Forces field hospital that responded to the Haiti earthquake of 2010) would have allowed for a significant increase in the overall rate of admission of the pediatric surge cohort. This would have reduced the time to treatment in this example by approximately half. The time needed to completely treat all children affected by the disaster would have decreased by slightly more than a third, with the caveat that the PTC would have to have been approximately as fast as the adult center in discharging its patients. Lastly, if disaster death rates from other events reported in the literature are included in the model, availability of a PTC would result in a relative mortality risk reduction of 37%.

Conclusions

Our model provides a mathematical justification for aggressive inclusion of PTCs in planning for disasters by public health agencies.  相似文献

ABSTRACT

The establishment and use of community-based natural resource management (CBNRM) groups is now the established route towards conservation in large parts of the Pacific, especially in Fiji. One of the main strengths of CBNRM that is often mentioned is its adaptability to different contexts and to changing objectives among stakeholders, an aspect thoroughly tested by events such as ‘natural’ disasters. This paper is based on 10 months of fieldwork in Kubulau district in Fiji before, during and after Cyclone Winston. It provides an ethnographic account of how this ‘natural’ disaster affected a community and the local CBNRM group. It looks at how this event was experienced and managed by the group, and how the group’s involvement in disaster assistance shifted its role both within and outside of the community, as well as changing its future agenda during the months that followed. We can see in the aftermath of disaster a shift within the group away from the original primary focus on conservation and towards development. The paper provides an account of how the disaster seemingly strengthened the group and the ‘community’ in terms of organisation, knowledge and motivation, but also how it challenged previous norms and exposed weaknesses that are arguably inherent within CBNRM itself.  相似文献

Results of a cross-sectional study to determine the level of emergency and disaster response planning at Ohio nonhuman animal shelters and the role Ohio agencies have in emergency and disaster response planning in their communities indicated a lack of preparedness coupled with underutilization of the agencies as a resource. A total of 115 agencies (68%) responded to a standardized survey mailed to 170 Ohio agencies. Most (68%) agencies agreed that emergency and disaster response planning was important to their organization, although only 13% of agencies had completed a written emergency and disaster response plan. The majority (80%) of agencies indicated they would provide critical resources in an emergency or disaster in their community. Only 38 (33%) of the responding agencies were aware of the PETS Act of 2006. Although many agencies indicated the importance of an emergency and disaster plan, there may be insufficient resources, including time and proper training, available to ensure plans are developed. Improved coordination among veterinarians, local veterinary medical associations, emergency preparedness agencies, and animal shelters would enhance the relief efforts in a crisis.  相似文献

Introduction

There is evidence for beneficial effects of acute and long-term exercise interventions on several forms of memory, including procedural motor learning. In the present study we examined how performing a single bout of continuous moderate intensity aerobic exercise would impact motor skill acquisition and retention in young healthy adults, compared to a period of rest. We hypothesized that exercise would improve motor skill acquisition and retention, compared to motor practice alone.

Materials and Methods

Sixteen healthy adults completed sessions of aerobic exercise or seated rest that were immediately followed by practice of a novel motor task (practice). Exercise consisted of 30 minutes of continuous cycling at 60% peak O2 uptake. Twenty-four hours after practice, we assessed motor learning with a no-exercise retention test (retention). We also quantified changes in offline motor memory consolidation, which occurred between practice and retention (offline). Tracking error was separated into indices of temporal precision and spatial accuracy.

Results

There were no differences between conditions in the timing of movements during practice (p = 0.066), at retention (p = 0.761), or offline (p = 0.966). However, the exercise condition enabled participants to maintain spatial accuracy during practice (p = 0.477); whereas, following rest performance diminished (p = 0.050). There were no significant differences between conditions at retention (p = 0.532) or offline (p = 0.246).

Discussion

An acute bout of moderate-intensity aerobic exercise facilitated the maintenance of motor performance during skill acquisition, but did not influence motor learning. Given past work showing that pairing high intensity exercise with skilled motor practice benefits learning, it seems plausible that intensity is a key modulator of the effects of acute aerobic exercise on changes in complex motor behavior. Further work is necessary to establish a dose-response relationship between aerobic exercise and motor learning.  相似文献

The potential for a novel influenza virus to cause a pandemic represents a significant threat to global health. Planning for pandemic flu, as compared to planning for other types of hazards, presents some unique challenges to businesses, communities, and education institutions. To identify and address the challenges that may be faced by major metropolitan universities during a flu pandemic, a tabletop exercise was developed, offered, and evaluated. Its purpose was to assess existing University of Washington (UW) plans and policies for responding to an influenza pandemic. On May 31, 2006, more than 50 participants, including UW administrators and unit leaders and a number of key external partners, participated in a tabletop exercise designed to simulate all phases of an influenza pandemic. This exercise revealed existing gaps in university pandemic influenza plans and policies, including issues related to isolation and quarantine, continuity of operations, disaster mental health services, integration of volunteers into a disaster response, tracking travel of university students and personnel, communication problems, and ways to meet the needs of resident and foreign students and faculty during an outbreak. Policy and planning recommendations are offered that address each of these challenges faced by UW as well as other major research universities and colleges.  相似文献

This paper outlines some of the ways Mengen speakers have positioned Ulawun volcano, its inhabitants and outputs as beneficial features of daily life. By way of contrast, volcanology and more recent disaster planning discourses in Papua New Guinea, have often positioned Ulawun as a hazard that creates potential dangers and risks. The paper explores how these two distinct orientations to Ulawun’s past and future influence the possibilities of Ulawun’s beneficial and dangerous outcomes. We emphasise how Ulawun’s anticipated future has been articulated into forms of disaster planning involving new modes of action, accountability and world making.  相似文献

Cardiovascular disease (CVD) and CVD risk factors are highly heritable, and numerous lines of evidence indicate they have a strong genetic basis. While there is nothing known about the interactive effects of genetics and exercise training on CVD itself, there is at least some literature addressing their interactive effect on CVD risk factors. There is some evidence indicating that CVD risk factor responses to exercise training are also heritable and, thus, may have a genetic basis. While roughly 100 studies have reported significant effects of genetic variants on CVD risk factor responses to exercise training, no definitive conclusions can be generated at the present time, because of the lack of consistent and replicated results and the small sample sizes evident in most studies. There is some evidence supporting "possible" candidate genes that may affect these responses to exercise training: APO E and CETP for plasma lipoprotein-lipid profiles; eNOS, ACE, EDN1, and GNB3 for blood pressure; PPARG for type 2 diabetes phenotypes; and FTO and BAR genes for obesity-related phenotypes. However, while genotyping technologies and statistical methods are advancing rapidly, the primary limitation in this field is the need to generate what in terms of exercise intervention studies would be almost incomprehensible sample sizes. Most recent diabetes, obesity, and blood pressure genetic studies have utilized populations of 10,000-250,000 subjects, which result in the necessary statistical power to detect the magnitude of effects that would probably be expected for the impact of an individual gene on CVD risk factor responses to exercise training. Thus at this time it is difficult to see how this field will advance in the future to the point where robust, consistent, and replicated data are available to address these issues. However, the results of recent large-scale genomewide association studies for baseline CVD risk factors may drive future hypothesis-driven exercise training intervention studies in smaller populations addressing the impact of specific genetic variants on well-defined physiological phenotypes.  相似文献

In a national disaster, the medical profession would lose physicians and auxiliary personnel and would need assistance. Canada''s 22,000 physicians and 85,000 nurses are located for the most part in potential target areas. Survivors among Canada''s 6396 dentists could supply 30% reinforcement. The dentist''s training, his manual dexterity and experience acquired in the management of hemorrhage, shock, débridement, suturing, reduction and immobilization of fractures, and control of pain and infection would be valuable. Additional functions he could perform would be first-aid, including but not limited to artificial respiration, early management of chest wounds, preparation of casualties for movement, and assistance in general surgical procedures. Dentists with special training in anesthesia, oral surgery or public health could be of particular value in relieving anesthetists, surgeons, radiologists and public health officers of some of their duties. Joint training of physicians and dentists in mass casualty care could increase the efficiency of the team work in disaster and is being considered by many medical and dental faculties.  相似文献

The urgency of the crisis following a nuclear attack staggers the imagination. We would have thousands or millions of survivors making a desperate struggle to survive. Safe water supplies and waste-disposal systems would be gone. In some areas, there would be little or no food or shelter.Yet California has already manned a medical arsenal that is second to none in the United States. We have stored 115 emergency hospitals at strategic points, and through the county medical associations we have appointed cadres including physicians, nurses and technicians. Plans have been made for workers who will assist in setting up the hospitals and first aid stations.In our future operations we will continue to place strong emphasis on the medical phase of our program of disaster care.The program would be just as essential in the event of major natural disaster as nuclear war.Our objective is a simple one. When disasters strike, companion animals (pets) matter. Emergency planning for them is a key aspect of disaster preparedness, especially considering that people may delay evacuation out of concern for their pets. Temporary boarding options for pets are important; however, caregivers (owners) must ultimately return to permanent housing. Surprisingly little attention has been paid to housing recovery in the disaster literature on pet ownership, and no studies have examined the potential for increased vulnerability among tenants with pets. This study analyzed online rental listings in a city that was severely flooded in 2013. In the following year, demand for pet-friendly rental housing outweighed supply. Landlords frequently stipulated restrictions on the allowable sizes, species, or breeds of pets. Dogs were often banned outright. To keep their pets, prospective tenants needed to exercise flexibility in location and pay higher surcharges. The implications of housing insecurity for tenants with pets have broad relevance, not just in disaster circumstances. Giving up a companion animal to secure housing can negatively impact resilience, whereas living in unsafe environments to avoid pet relinquishment may increase vulnerability.  相似文献

AimsIn this study, we investigated whether short-term exercise, known to promote hippocampal brain-derived neurotrophic factor (BDNF) expression, would also enhance activity in the Porsolt forced swim test (FST), a model for assessing antidepressant efficacy. We also wished to determine whether exercise combined with antidepressants would be more effective at modifying behavior in the FST than either intervention alone. In parallel with this, we also expected that these interventions would preserve post-stress levels of BDNF, and that antidepressants designed to selectively enhance noradrenergic or serotonergic neurotransmission (reboxetine or citalopram, respectively) would have differential effects on behavior and BDNF expression.Main methodsMale Sprague-Dawley rats were treated with exercise (voluntary wheel running), reboxetine, citalopram, or the combination of exercise and each antidepressant, for 1 week. At the end of this period, a subset of animals from each treatment group underwent the FST. Post-stress levels of hippocampal BDNF mRNA were then quantified via in situ hybridization.Key findingsOur results indicate that while both exercise and antidepressant treatment preserved post-stress levels of hippocampal BDNF mRNA, each intervention led to a unique behavioral profile in the FST. We found that antidepressant treatment increased swimming time in the FST, but that exercise decreased swimming time. While the combination of reboxetine-plus-exercise led to an increase in climbing and diving, citalopram-plus-exercise reduced these behaviors.SignificanceIt is possible that active behaviors during the FST, though specific to antidepressant medications, may not reflect increased hippocampal BDNF expression or other survival- associated benefits.  相似文献

Abstract

Not many rigorous researches have studied on how ecosystems have interacted with the occurrence of natural disaster. This article aims to evaluate the role of ecosystems in the field of natural disaster management with the ultimate goal of reducing its various impacts. Qualitative content analysis has been utilized as the principal methodology. Additionally, the ecosystem low-awareness approach has been compared with the ecosystem integration approach with respect to international institutions, national governments, business corporations, non-government organizations, and local communities. The most significant finding is that the field needs to transform its ecosystem low-awareness approach into the integration approach while addressing international networks, scientific research, holistic thinking, sustainability, and education. As a contribution, this article presents a more comprehensive evaluation of the issue of ecosystem regarding natural disaster management (as a common frame) than previous studies.  相似文献

The purpose of the study was to determine whether exercise would activate JNK in the heart and whether chronic exercise training would alter the response. Untrained rats were familiarized with the treadmill and assigned to one of four groups: low intensity (LI), 10 min, 0%, 15 m/min; medium intensity (MI), 10 min, 0%, 33 m/min; high intensity (HI), 10 min, 25%, 33 m/min; long duration (LD), 30 min, 0%, 15 m/min. Another cohort of rats was subjected to a progressive 6 wk high-intensity training protocol that produced a 12% increase in heart mass. In untrained rats, JNK activity was LI: 1.5 (fold nonrun control), MI: 2.0, HI: 2.5, LD: 1.25 immediately after a single bout of exercise. In trained rats, no activation of JNK above baseline was detected after either a 10-min or 1-h bout of exercise. We concluded that treadmill exercise activates JNK in the rat heart in an intensity-dependent manner and that chronic training abrogates the myocardial JNK response to a bout of exercise.  相似文献

It is presently unclear how the fast and slow components of pulmonary oxygen uptake (VO(2)) kinetics would be altered by body posture during heavy exercise [i.e., above the lactate threshold (LT)]. Nine subjects performed transitions from unloaded cycling to work rates representing moderate (below the estimated LT) and heavy exercise (VO(2) equal to 50% of the difference between LT and peak VO(2)) under conditions of upright and supine positions. During moderate exercise, the steady-state increase in VO(2) was similar in the two positions, but VO(2) kinetics were slower in the supine position. During heavy exercise, the rate of adjustment of VO(2) to the 6-min value was also slower in the supine position but was characterized by a significant reduction in the amplitude of the fast component of VO(2), without a significant slowing of the phase 2 time constant. However, the amplitude of the slow component was significantly increased, such that the end-exercise VO(2) was the same in the two positions. The changes in VO(2) kinetics for the supine vs. upright position were paralleled by a blunted response of heart rate at 2 min into exercise during supine compared with upright heavy exercise. Thus the supine position was associated with not only a greater amplitude of the slow component for VO(2) but also, concomitantly, with a reduced amplitude of the fast component; this latter effect may be due, at least in part, to an attenuated early rise in heart rate in the supine position.  相似文献

以南昌市主城区的公园绿地现状图为基本信息源,借助GIS技术,建立公园绿地空间数据库;利用FRAGSTATS景观格局软件计算相应的景观指数.在对现有的南昌市公园绿地景观格局进行定量分析的基础上,提出优化策略与方案,并对优化后的景观格局进行评价. 结果表明: 现有公园绿地的斑块空间分布不均衡,且面积差异较大,尤其在人口密集区,公园绿地斑块数量偏少,有效避灾面积明显偏低;通过大幅度增加斑块数、斑块面积,提高斑块间的连通性,并适当提高景观破碎化指数,可以优化南昌市公园绿地系统的“点-线-面”格局,使空间分布更趋合理,有效增强其生物多样性保护与防灾避险功能;优化后的公园绿地的斑块多样性指数、均匀度指数均有大幅度提高,聚集度指数也明显升高,且相应降低了优势度指数,景观多样性更为丰富.  相似文献

ABSTRACT

Providing emergency relief to the victims of natural disasters is a hugely complex process fraught with many challenging aspects: multiple uncertainties, little reliable information, scarcity of resources, a variety of involved entities, and so on. Nowadays there is a lot of information that could be used to improve decision-making in disaster management, but usually it is not available at the right moment, in the right way, or it is partially known or vague. In this article we analyze the decision-making process for disaster management from the general view of intelligent decision-making to the specific characteristics of this context. This specificity deals with a new kind of logistics, and it is shown how this humanitarian logistics, specifically designed with the aim of alleviating suffering of vulnerable people, is a growing new research area to develop new decision aid models for disaster management, identifying new and relevant differences with other types of logistics. To illustrate these claims, two models are introduced, one for assessment of consequences in the earlier stage after a disaster (focused on the unknown, one of the main characteristics in disaster management), and another one for last mile distribution of humanitarian aid (focused on the multicriteria nature of decision-making on disaster management).  相似文献

The effects of exercise on adiponectin levels have been reported to be variable and may be attributable to an interaction between environmental and genetic factors. The single nucleotide polymorphisms (SNP) 45 (T > G) and SNP276 (G > T) of the adiponectin gene are associated with metabolic risk factors including adiponectin levels. We examined whether SNP45 and SNP276 would differentially influence the effect of exercise training in middle-aged women with uncomplicated obesity. We conducted a prospective study in the general community that included 90 Korean women (age 47.0 ± 5.1 years) with uncomplicated obesity. The intervention was aerobic exercise training for 3 months. Body composition, adiponectin levels, and other metabolic risk factors were measured. Prior to exercise training, only body weight differed among the SNP276 genotypes. Exercise training improved body composition, systolic blood pressure, maximal oxygen consumption, high-density lipoprotein cholesterol, and leptin levels. In addition, exercise improved adiponectin levels irrespective of weight gain or loss. However, after adjustments for age, BMI, body fat (%), and waist circumference, no differences were found in obesity-related characteristics (e.g., adiponectin) following exercise training among the SNP45 and the 276 genotypes. Our findings suggest that aerobic exercise affects adiponectin levels regardless of weight loss and this effect would not be influenced by SNP45 and SNP276 in the adiponectin gene.  相似文献

Exercise training is known to induce a molecular adaptation process involving inflammatory responses. However any time-of-day effect of exercise on inflammatory responses remains unknown. The aim of the present study was to investigate whether acute bouts of intense exercise performed at different times of the day would affect the release Interleukin-6 (IL-6), one of the most abundant cytokines in mammalian endocrine response to exercise. Cortisol levels were measured as a confirmation of correct timing of exercise and to determine any impact it may have on the cytokine release. Twelve healthy male participants carried out 30 min of intense exercise (3 sets of 8-12 repetitions for 4 resistance exercises at 70% of 1RM) in morning (08:15-09:00 h), and evening (18:15-19:00 h) sessions. An 8h fasting period was required before each exercise session. Blood samples were taken immediately pre and post each exercise sessions to determine IL-6 and cortisol levels. Our data show that whilst the training group showed no post-exercise changes in serum_IL-6 levels (P>0.05), the control group on the other hand showed significant time-of-day modifications in serum_IL-6 levels (P=0.008). Moreover, a significant interaction between intervention phase (pre-post training, AM vs. PM) and group (Exercise vs. Control) is evidenced in terms of serum_IL-6 levels (P=0.014). This interaction however was nullified when the between group differences at baseline were partialled out in a covariate analysis (P>0.05). We also found that the main effect of experimental phase on Cortisol was present in both the trained (P=0.004) and control groups (p<0.001) with no significant interaction (P>0.05). Based on the current data, we would propose that exercise and/or time-of-day would not interfere with clinical endocrine profiling of IL-6 in a population.  相似文献
