An original social-ecological concept of the epidemic process has been constructed on the basis of using social ecology, systemic approach and the basic principles of cybernetics. According to this concept, the epidemic process is regarded as a biosocial, hierarchic, integral system providing for the reproduction of the species of human parasites. At a higher level of organization, the epidemic process is an epidemiological social-ecological system consisting of two interacting subsystems: the biological (epidemiological ecosystem) and the social (social and economic conditions of life of the society) subsystems where the biological subsystem plays the role of the governed object and the social acts as the internal regulator of these interactions. On the basis of this concept a rational structure of the system of epidemiological surveillance over infectious diseases has been proposed according to which each level of the structure of the epidemic process should be subject to adequate monitoring.  相似文献

The data obtained in the study of an explosive outbreak of acute respiratory diseases, tonsillitis and scarlet fever in one of schools in Moscow have made it possible to exclude the alimentary mechanism of its development and to demonstrate the role of the droplet mechanism of transmission in the appearance of its outbreak. The epidemiological analysis of the outbreak has permitted the formulation of the hypothesis on the conditions of the formation and spread of the epidemic variant of the infective agent; this hypothesis corresponds to the available data in literature on the qualitative changes of the infective agent in the course of the epidemic process. The study has shown that the prophylaxis of the explosive outbreaks of respiratory streptococcal infections must be ensured by the system of epidemiological surveillance with timely intervention into the epidemiological process at its early stages.  相似文献

A retrospective epidemiologic analysis of cases diagnosed as hepatitis A (HA) has been made in territories characterized by high intensity (4 towns in Central Asia) and low intensity (Novomoskovsk, Tula Province) of the epidemic process development. Morbidity structures for different age and social groups of the population, as well as the morbidity time course, both annual and over many years, were analyzed over 1973-1986. Specific features in the development of the epidemic process in HA and hepatitis E (HE), formerly called hepatitis non-A, non-B with the fecal/oral mechanism of the infection transmission, were studied. Twelve epidemiological differential diagnostic signs of these two infections were formulated, classified, and validated. Contribution of centralized water supply and sewage systems to the development of HE epidemic process and the regulating role of infectious immunological mechanisms in the development of HA epidemic process were demonstrated.  相似文献

The signs necessary for the prognostication of the development of the epidemic process have been formulated on the basis of the epidemiological analysis of materials obtained in the process of trials of the system for the surveillance of meningococcal infection with the use of previously established characteristics. The data on the prevalence of meningococcal infection among different age groups and on the seasonal distribution of the infection as well as on the serogroups of meningococci isolated from patients, have been shown to be of importance for prognostication.  相似文献

The use of complex typing of Sh. sonnei by the colicinogenic and enzymatic properties in epidemiological analysis of dysentery caused by Sh. sonnei, apart from establishment of epidemiological connections in the foci, offered a possibility of introducing significant corrections in the determination of the sizes and duration of existence of the epidemic foci, as well as in detection of the sources and factors of transmission of the infection established in epidemiological examination. Typing of Sh. sonnei also aided in ascertaining that, apart from general regularities in the individual territories, the epidemic process in individual groups of the population and in different seasons of the year could also course autonomously.  相似文献

The socio-ecological concept in epidemiology, formulated by the author in his earlier works and regarding the epidemic process as a biosocial hierarchical, integral system, can serve as a theoretical basis for the epidemiological surveillance of infectious and parasitic diseases. In accordance with the structure of the epidemic process, the system of epidemiological surveillance is subdivided into the following monitoring levels: molecular, microbiological, clinical, biocenological, statistical and sociological. The functions of epidemiological surveillance should be performed by the existing and newly organized All-Union Centers for the corresponding nosological forms.  相似文献

The epidemiological analysis of infectious morbidity for recent years has been made and the main nosological forms existing in Angola (malaria, tuberculosis, lepra, African trypanosomiasis, plague, intestinal diseases, etc.) have been briefly characterized on the basis of primary medical reports and the data provided by the literature and experimental work. This analysis creates the necessary prerequisites which enable the local public health organs to determine the regularities of the epidemic process, thus making it possible to take rational prophylactic measures and to organize proper epidemiological supervision.  相似文献

The data on the spread of HIV infection in Irkutsk and Irkutsk Province, starting from March 9, 1999, are presented. The simultaneous appearance of 17 multiple morbidity foci of HIV infection, involving 2,227 persons into the epidemic, within the period of 8 months was noted. The age structure of the epidemic process was represented by 1,846 adults aged 18-50 years (83%), 256 adolescents aged 16-17 years (12.9%), 78 children aged 10-15 years (3.5%) and 17 persons of other ages (0.53%). The leading role in the age structure of the epidemic process belonged to young people aged 10-29 years (90.4% of cases). The epidemiological analysis made it possible to establish the fact that the epidemic was linked with the intravenous injection of heroin. The viral contamination of the drug is not excluded also in the process of its preparation for injection.  相似文献

The specific features of the epidemiology of meningococcal infection in organized groups have been established after comprehensive (epidemiological, microbiological and immunological) studies, evaluated from the viewpoint of the law of the autoregulation of the epidemic process. The studies have revealed the phenomenon of the adaptive biological variability of meningococci with their prolonged reservation due to the transformation of typical bacterial forms of meningococci characterized by strict serogroup specificity and typical fermentative activity into serologically and biochemically undifferentiated forms, capable of prolonged persistence in human populations. The model demonstrating the development of the epidemic process in meningococcal infection in an organized group has been worked out.  相似文献

An epidemic outbreak of Sonne dysentery has been studied. The data of epidemiological monitoring before and after the epidemic have been analyzed. The real prognostication value of controlling the biological properties of Shigella sonnei and the increase of their intrapopulation heterogeneity at the period of the activation of the epidemic process of Sonne dysentery has been established.  相似文献

The authors present characteristics of meningococcus infection epidemic process in case of sporadic cases and under epidemic conditions (1965--1976). A scheme of epidemiological analysis suggested by the authors permitted to differentiate and to record the incidence of various clinical forms of meningococcus infection, to present data on the age, seasonal characteristics, focality, etc. Comparison of intensive morbidity indices for 10 years, both at the individual administration territories and in the Republic as a whole demonstrated morbidity level of 1.5--2.0 to be one of prognostic signs of the beginning epidemic. The main features differentiating the sporadic and epidemic morbidity periods were revealed. The presence of group diseases, a greater percentage of children among those who fell ill, and marked signs of seasonality and territorial difference characterized the period of rise caused by meningococcus of serological group A.  相似文献

The work demonstrates the main approaches to the use of the methods of multidimensional analysis for the creation of a hypothesis on the mechanism of the epidemiological process of dysentery in organized groups. The main risk factors have been established, and their role in the formation of annual, all-the-year-round and seasonal dysentery morbidity has been quantitatively evaluated. The results of analysis show the existence of diverse variants of the alimentary route of the transmission of infection, maintaining the epidemic process of dysentery, and the necessity of differentiating measures for the prophylaxis of all-the-year-round and seasonal morbidity.  相似文献

During the chronic water epidemic of typhoid fever in Tajikistan in 1996-1997 specific features of the epidemic process dynamics in groups of servicemen were studied in several cities. The infective agent was proved to be transmitted by the alimentary route and through everyday contacts, the water route of transmission playing the most important role. The early clinical and epidemiological signs of the water outbreak of typhoid fever and the risk factors were established. A new epidemiological concept (socio-ecological) has been formulated on the basis of the principles of the theory of systems and the theory of information. In accordance with this concept, the epidemic process is organized on the same principle as living matter, and the stability of this process at all levels of its organization is ensured by the processes of self-regulation. The conditions of the life of human society have been shown to be organically incorporated into the structure of the epidemic process as a regulating subsystem on the socio-ecological level.  相似文献

Retrospective analysis of the epidemic of HIV infection in Moscow allowed to mark out two periods: the first--from 1987 to 1993 and the second one--from 1994 to 2000. The characteristic feature of the first period of the epidemic was the sexual transmission of the agent mainly among homosexuals; the most affected group were persons aged 20-39 years and the number of AIDS patient decreased with a simultaneous growth in lethality. During the second stage of the HIV infection epidemic changes in the prevailing transmission routes of the agent occurred: injection drug addicts took the leading role. Changes in the ratio of HIV-infected men and women took place along with active involvement of persons aged 13-18 years into the epidemic process. The established changes in the epidemiological situation require corrections in the strategy and tactics of epidemic control and prophylactic measures.  相似文献

The previously established zonal character of the prevalence of viral hepatitis A in the Ukraine is a stable epidemiological regularity observed for more than 30 years (1952-1985). Zonal differences in the dynamics of the epidemic process of viral hepatitis A were established. These differences became particularly hepatitis manifest in the years of periodic rises in the morbidity rate which is also irregular within zonal and regional boundaries. The main typological variants of the dynamics of the epidemic process ("urban" and "rural") were defined. Typological approach appears to be useful in the provision of information necessary for the proper functioning of epidemiological surveillance and for taking measures aimed at epidemics control.  相似文献

The present article substantiates the necessity of defining more accurately the notion "epidemic process" and using this term only in reference to anthroponotic diseases. Proceeding from the philosophical concept of the qualitative definiteness of material objects and phenomena, the author proposes to distinguish the epidemic process in anthroponoses from mass spread of zoonotic diseases among humans on the basis of using the systemic qualities of the epidemic process, and in particular the principle of the dual character of the qualitative definiteness of objects and phenomena as the distinctive feature of the epidemic process of anthroponoses. Distinction, based on this principle, between anthroponotic diseases and zoonotic diseases in groups of humans must be reflected in the definitions of these phenomena. The application of the term "epidemic process" only in reference to anthroponotic diseases is proposed, while zoonotic diseases in groups of humans should be considered as epidemic manifestations of zoonotic infections.  相似文献
