Clinical progress in the field of liver transplantation has been largely supported by animal models1,2. Since the publication of the first orthotopic rat liver transplantation in 1979 by Kamada et al.3, this model has remained the gold standard despite various proposed alternative techniques4. Nevertheless, its broader use is limited by its steep learning curve5.In this video paper, we show a simple and easy-to-establish revision of Kamada''s two-cuff technique. The suprahepatic vena cava anastomosis is performed manually with a running suture, and the vena porta and infrahepatic vena cava anastomoses are performed utilizing a quick-linker cuff system6. Mouse renal transplantation is a technically challenging procedure. Although the first kidney transplants in mice were performed over 34 years ago and refined some years later, the classical techniques of mouse renal transplantation required clamping both vena cava and aorta simultaneously and carry out suture anastomoses of the renal artery and vein in a heterotopic position. In our laboratory, we have successfully developed mouse orthotopic kidney transplantation for the first time, using a rapid “cuffed” renal vein technique for vessel anastomosis, wherein the donor’s renal vein was inserted through an intravenous catheter, folded back and tied. During grafting, the cuffed renal vein was directly inserted into the recipient’s renal vein without the need for the clamping vena cava and suturing of renal vein. This technique allowed for the exact transplantation of the kidney into the original position, compared to the classical technique, and has significantly shortened the clamping time due to a quicker and precise anastomosis of renal vein as described. This also allowed for a quicker recovery of the lower extremity activity, reduction in myoglobinuria with resultant kidney graft survival of 88.9%. Thus we believe that the cuffed renal vein technique simplifies microvascular anastomoses and affords important additional benefits.  相似文献

In this study, we analyzed causes of abdominal hemorrhage after reduced-size liver transplantation in rat. Healthy SD rats (weight range of 260–280 g) underwent liver transplantation, the donors were female and the recipients were male rats. The recipients were, on average, by <10 g heavier than the donors. All operations on donor rats were performed by the same person using unaided eyesight. Operations on recipient rats were performed by two persons using unaided eyesight. About 270 rats received reduced-size liver transplantation of which 44 died because of intra-abdominal hemorrhages. The distribution of abdominal hemorrhage sites was as follows: 28 cases with anastomotic hemorrhages of the inferior vena cava of the superior liver, 9 cases with subcapsular hemorrhage, 9 cases with tied hemorrhages from the left lateral lobe, 7 cases with hemorrhages from the papillary lobe, 7 hemorrhages from the triangular lobe, 5 hemorrhages of the right suprarenal vein and lumbar veins, 4 hemorrhages caused by mechanical injury, 4 cuff hemorrhages of the portal vein and the inferior vein cava of the inferior liver, 8 anastomotic hemorrhages of the inferior vena cava of the superior liver and the tied hemorrhage of the left outboard lobe, and 5 hemorrhages of the two tied points of the reduced-size liver. Ten hemorrhages were stopped by suture or/and ligature, and 6 were stopped by washing with or soaking in hot water. Results of this study will be helpful to raise success rates of reduced-size liver transplantation.  相似文献

Mice are often used as heart transplant donors and recipients in studies of transplant immunology due to the wide range of transgenic mice and reagents available. A difficulty is presented due to the small size of the animal and the considerable technical challenges of the microsurgery involved in heart transplantation. In particular, a high rate of technical failure early after transplantation may result from recipient death and post-operative complications such as hind limb paralysis or a non-beating heart. Here, the complete technique for heterotopic mouse heart transplantation is demonstrated, involving harvesting the donor heart and its subsequent implantation into a recipient mouse. The donor heart is harvested immediately following in situ perfusion with cold heparinized saline and transection of the ascending aorta and pulmonary artery. The recipient operation involves preparation of the abdominal aorta and inferior vena cava (IVC), followed by end-to-side anastomosis of the donor aorta with the recipient aorta using a single running 10-0 microsuture and a similar anastomosis of the donor pulmonary artery with the recipient IVC. Following the operation the animal is injected with 0.6 ml normal saline subcutaneously and allowed to recover on a 37 °C heating pad. The results from 227 mouse heart transplants are summarized with a success rate at 48 hr of 86.8%. Of the 13.2% failures within 48 hr, 5 (2.2%) experienced hind limb paralysis, 10 (4.4%) had a non-beating heart due to graft ischemic injury and/or thrombosis, while 15 (6.6%) died within 48 hr.  相似文献

Since 1990, the development of tacrolimus-based immunosuppression and improved surgical techniques, the increased array of potent immunosuppressive medications, infection prophylaxis, and suitable patient selection helped improve actuarial graft and patient survival rates for all types of intestine transplantation. Patients with irreversible intestinal failure and complications of parenteral nutrition should now be routinely considered for small intestine transplantation. However, Survival rates for small intestinal transplantation have been slow to improve compares increasingly favorably with renal, liver, heart and lung. The small bowel transplantation is still unsatisfactory compared with other organs. Further progress may depend on better understanding of immunology and physiology of the graft and can be greatly facilitated by animal models. A wider use of mouse small bowel transplantation model is needed in the study of immunology and physiology of the transplantation gut as well as efficient methods in diagnosing early rejection. However, this model is limited to use because the techniques involved is an extremely technically challenging. We have developed a modified technique. When making anastomosis of portal vein and inferior vena cava, two stay sutures are made at the proximal apex and distal apex of the recipient s inferior vena cava with the donor s portal vein. The left wall of the inferior vena cava and donor s portal vein is closed with continuing sutures in the inside of the inferior vena cava after, after one knot with the proximal apex stay suture the right wall of the inferior vena cava and the donor s portal vein are closed with continuing sutures outside the inferior vena cave with 10-0 sutures. This method is easier to perform because anastomosis is made just on the one side of the inferior vena cava and 10-0 sutures is the right size to avoid bleeding and thrombosis. The mouse heterotopic heart transplantation has been used widely since it was introduced by Drs. Corry and Russell in 1973. It is particularly valuable for studying rejection and immune response now that newer transgenic and gene knockout mice are available, and a large number of immunologic reagents have been developed. The heart transplant model is less stringent than the skin transplant models, although technically more challenging. We have developed a modified technique and have completed over 1000 successful cases of heterotopic heart transplantation in mice. When making anastomosis of the ascending aorta and abdominal aorta, two stay sutures are placed at the proximal and distal apexes of recipient abdominal aorta with the donor s ascending aorta, then using 11-0 suture for anastomosis on both side of aorta with continuing sutures. The stay sutures make the anastomosis easier and 11-0 is an ideal suture size to avoid bleeding and thrombosis.When making anastomosis of pulmonary artery and inferior vena cava, two stay sutures are made at the proximal apex and distal apex of the recipient s inferior vena cava with the donor s pulmonary artery. The left wall of the inferior vena cava and donor s pulmonary artery is closed with continuing sutures in the inside of the inferior vena cava after, one knot with the proximal apex stay suture the right wall of the inferior vena cava and the donor s pulmonary artery are closed with continuing sutures outside the inferior vena cave with 10-0 sutures. This method is easier to perform because anastomosis is made just on the one side of the inferior vena cava and 10-0 sutures is the right size to avoid bleeding and thrombosis. There is an emergent need for improving the microsurgical technique of variant arterial anastomosis to reduce the often seen surgery-related complications. We describe in this article our experience in improving this technique, in 73 living donor liver grafts (64 right lobes, 9 left lobes) in patients with end-stage liver disease during living donor liver transplantation. The hepatic arteries were evaluated preoperatively with computed tomography and magnetic resonance angiography. In this series, 13 grafts (17.80 %) with variant hepatic artery were conducted arterioplasty on a back-table under a loupe or a high-power microscope, which included one recipient in situ interposition vessel graft of recipient proper hepatic artery for artery reconstruction. The back-table reconstruction time was 16 ± 5.6 min. No arterial thrombosis was found in these cases during the 6-month postoperative follow-up. On the basis of our experience, we suggest that back-table microsurgical plasty for graft with arterial variation should be applied to minimize operative difficulties and to avoid arterial complications in living donor liver transplantation.  相似文献

目的建立一种手术难度低,成功率高的大鼠肾移植模型。方法 Wistar大鼠作供体,SD大鼠作受体,将供体腔静脉与受体肾静脉端端吻合,供体腹主动脉与受体腹主动脉端侧吻合,供体膀胱瓣与受体膀胱吻合。根据血管吻合时应用硬膜外导管与否,将受体分为有支架组和无支架组两组。结果有支架组共进行肾移植30次,成活26只;无支架组共进行肾移植20次,成活10只。有支架组的成活率86.7%(26/30)较无支架组50.0%(10/20)明显提高(P〈0.05),血管吻合总时间(22±2)min较无支架组(32±2)min明显缩短(P〈0.05)。结论硬膜外导管应用于大鼠肾移植血管吻合,降低了手术难度,减少了吻合口出血,提高了手术成功率。  相似文献

A chronic arteriovenous (AV) blood access shunt has been developed in baboons. It is composed of silicone rubber tubing extensions bonded to vascular graft polytetrafluoroethylene (ePTFE) cuffs, for creating anastomoses with the distal aorta and inferior vena cava. Following surgical implantation, shunts remain patent for months (X = 9 ± 2.5 months) and provides long-term ex vivo access to non-anticoagulated blood (Mean Blood Flow = 312 ± 69 mL/min).  相似文献

Today successful kidney transplantation procedures, techniques and immunosuppression protocols are a consequence of extensive research on animal models. During every transplantation surgery there are two crucial points for the success of the entire procedure: vascular (arterial end venous) and ureteral or ureterovesical anastomosis. Renal artery and vein of the donor kidney can be anastomosed end-to-side to the abdominal aorta and vena cava of the recipient (heterotopic transplantation), or end-to-end to the remains of renal artery and vain of the recipient (orthotopic transplantation) after nephrectomy. The ureter can be anastomosed also end-to-end or we can connect it directly to the urinary bladder (ureterocystoneostomy). The aim of this study was to elucidate which technique has better results according to: animal survival, reperfusion and perfusion of the transplanted kidney, elimination of the urine from the transplanted kidney and procedure costs. The study included 240 (120 donors and 120 recipients) male Wistar rats (3 months old; weight 250-300 g Our results are clearly showing that the end-to-end vascular anastomosis, and Paquins ureterovesical anastomosis have better results in transplanted rat kidneys survival and urine drainage compared to end-to-side vascular anastomosis and end-to-end ureteral anastomosis. Based on our experience we can conclude that described methods of end-to-end vascular anastomosis and Paquins ureterovesical anastomosis are less technically demanding and have a shorter learning curve. Therefore, we can recommend the use of described methods in kidney transplantation related researches.  相似文献

To test for an association with risk for restenosis after coronary stent placement, the TNF-α and IL-10 polymorphisms were analyzed by 5′ exonuclease TaqMan assays in 162 patients who initially underwent coronary stenting. Analysis of basal and procedure coronary angiographies revealed a higher proportion of restenosis in lesions treated with bare metal stents compared with those treated with drug-eluting stents (P < 0.001). Distribution of TNF-α genotypes was similar in patients with and without restenosis. The IL-10 polymorphisms showed a moderate protective trend of the ?819 TT genotype against restenosis when the lesions were analyzed (P = 0.071, OR = 0.471). Multivariate analysis confirmed a protective role for drug-eluting stents (P < 0.001, OR = 0.199) and the ?819 TT genotype (P = 0.037, OR = 0.391). These results suggest the IL-10 ?819 TT genotype has a protective role against in-stent restenosis.  相似文献

The objective of the study is to explore change and significance of IL-8, IL-4 and IL-10 in the pathogenesis of terminal Ileitis in SD rat. 60 male SD rats were divided into model group, suture group, and control group equally. The rats subjected to ileum-cecum side-to-side anastomosis in terminal ileum in model group, suture in terminal ileum in suture group, and the control group accepted no special treatment. The terminal ileum tissue which was 1–3 cm from anastomotic stoma was collected at 2 and 8 weeks after surgery in each group. The pathological slice was observed under microscope, and PCR was applied to detect the expression of IL-4, IL-8, and IL-10 at different times. Pathological result showed that neutrophils significantly increased in model group and suture group at 2nd week, showing acute inflammatory reaction; model group showed chronic inflammation at 8th week. The change of IL-8, IL-4, and IL-10 expression level at 2 weeks after surgery: The IL-8 expression level of SD rat terminal ileum tissue in model group was significantly higher than in control and suture groups (P < 0.01), and it was higher in suture group compared to control group (P < 0.01); the expression level of IL-4 in control group was higher than model and suture groups (P < 0.05); there was no statistical significance between model group and suture group (P = 0.363); the expression level of IL-10 in control group was higher than in model and suture groups (P < 0.01), and it was higher in suture group compared to model group (P < 0.01). The change of IL-8, IL-4, IL-10 expression level at 8 weeks after surgery: The expression level of IL-8 significantly decreased in model group, and there was no significantly difference between three groups (P > 0.05); the expression level of IL-4 was higher in model group and suture group compared to 2nd week; there was no significance between three groups (P < 0.05); the expression of IL-10 was higher in model group compared to 2nd week (P < 0.01), it was lower than control group and suture group (P < 0.01); there was no significant difference between suture group and control group (P > 0.05). The chronic terminal ileum model could be successfully established by ileum-cecum side-to-side anastomosis in terminal ileum in SD rats; IL-8 can induce the inflammatory reaction in terminal ileitis and chemokines aggregation and mediate inflammatory reaction by mediating other inflammatory factors; as a proinflammatory cytokine, IL-8 can inhibit IL-10; IL-10 and IL-4 can inhibit the inflammatory reaction of terminal ileum.  相似文献

We have developed a technique for harvesting and transplanting two kidneys from one donor rat. Our method involves end-to-end anastomosis of the renal artery and the renal vein. Because of the short distance between the caudal vena cava and the right kidney, renal transplantation is not feasible on the right side of the recipient. The right donor kidney was therefore transplanted to the left side. Graft morphology was assessed on paraffin sections. We observed no differences between conventionally transplanted kidneys (n = 116) and right kidneys transplanted according to the method described (n = 40). In conclusion, our technique is simple, saves time and reduces the number of donor rats.  相似文献

Portacaval shunt operations were done in 15 of 18 patients who were treated surgically for portal hypertension. In eight cases the shunt was established by splenorenal anastomosis; in seven by anastomosis of the portal vein to the side of the inferior vena cava. Of the eight patients with splenorenal shunt, two are well, four are considered improved, and two have died. Of the seven in whom the portal vein was joined to the vena cava, two are improved, one is unimproved, and four have died.  相似文献

A cannulation system with fixation by a metal cuff around the tail was used for blood sampling. The cannula was guided subcutaneously and positioned in the vena cava after entering the body via the femoral vein. Histopathology was performed after long-term cannulation of up to 35 and 45 days. The presence of the cannula in the vena cava induced endothelial hypertrophy and hyperplasia accompanied by stromal hypertrophy. The endothelial activation was not limited to the vena cava but was also observed in both the cannulated vena iliaca and the contralateral control vena iliaca, the latter showing only minor alterations. In the lung, thrombi were noted in the larger lung arteries; and foreign body emboli, probably situated in the alveolar septi, could be detected occasionally. Inflammatory reactions in the tail at the site of cuff fixation consisted of a mixture of acute and chronic inflammatory responses. The chronic inflammation extended into the tail muscles, as shown by the presence of fibrous tissue associated with muscle degeneration. In conclusion, prolonged venous cannulation in rats resulted in local alterations in the veins, small emboli in the lungs and a moderate to marked inflammation in the tail. However, the procedure itself was well tolerated by the animals.  相似文献

In 20 mongrel dogs after a preliminary stenosis of the portal vein, formation of the spleno-renal or mesenteric-caval anastomosis has been performed. For revealing the vessels vital portography, injection of roentgenopaque masses and morphometry have been carried out. The spleno-renal anastomosis ensures a selective outflow of blood from the esophago-gastro-splenic zone. The collaterals, developing as a result of the porto-caval anastomoses with the anterior vena cava, and the collaterals, developing as a results of the porto-portal anastomoses, disappear. The mesenteric-caval anastomoses favourably affects the intestinal hemodynamics. The bypasses of the blood stream, developed between the portal vein and posterior vena cava reduce predominantly. The decompressive angioanastomoses result in local hypertension in the posterior vena cava system.  相似文献

PURPOSE: To evaluate the efficacy and safety of antiangiogenic agents (sorafenib and sunitinib) as postoperative adjuvant therapy in patients with nonmetastatic renal cell carcinoma (RCC) and venous tumor thrombus (VTT). MATERIAL AND METHODS: From March 2006 to January 2016, 147 patients who met the inclusion criteria were enrolled; 27 patients received sorafenib, and 17 patients received sunitinib. After radical nephrectomy and thrombectomy, the duration of maintenance targeted medication treatment was approximately 1 year. The primary objective was to compare disease-free survival (DFS) between each experimental group and control. Secondary end points included overall survival (OS) and toxic effects. RESULTS: The three groups were well balanced in terms of age, body mass index, gender, performance status, medical history, American Society of Anesthesiologists score, surgical approach, and tumor side and size. However, more patients receiving adjuvant therapy had inferior vena cava tumor thrombus. DFS and OS did not differ significantly between groups (P = .459 and .871, respectively). After adjusting for potential confounding factors, results of multivariate analysis proved that postoperative adjuvant therapy was not an independent factor for predicting DFS and OS (P > .05 for both). The subgroup analyses for inferior vena cava tumor thrombus found similar results. The common adverse events were hand-foot syndrome, diarrhea, fatigue, and neutropenia. The adverse effects were mild in both groups, and the incidence was not significantly different between sorafenib and sunitinib. CONCLUSIONS: Adjuvant treatment postoperatively with sorafenib or sunitinib showed no survival benefit relative to control for patients with nonmetastatic RCC and VTT in a prospective cohort study.  相似文献

Background

Sleeve anastomosis is the most common technique used to rearterialize orthotopic liver transplants (OLT). However, this technique has a number of disadvantages, including difficulty of performance of the technique visually unaided. We herein describe a novel rearterialized OLT model in the rat.

Materials and Methods

Forty-six male Sprague Dawley rats (300–400 g) were used as donors and recipients. Based on Kamada’s cuff technique, the new model involved performing a modified “sleeve” anastomosis between the celiac trunk of the donor and common hepatic artery of the recipient to reconstruct blood flow to the hepatic artery. An additional ten male Sprague Dawley rats underwent liver transplantation without artery reconstruction. Liver grafts were retrieved from the two groups and histological examination was performed following surgery.

Results

Total mean operating times were ~42 minutes for the donor liver extraction and 57 minutes for the recipient transplantation. Graft preparation took an additional 15 minutes and the time to fix the arterial bracket was ~3 minutes. During transplantation, the anhepatic phase lasted 18 ± 2.5 min and the artery reconstruction only required ~3 minutes. The patency rate was 94.44% and the 4-week survival rate was 90%. Histology indicated obvious fibrosis in the liver grafts without artery reconstruction, while normal histology was observed in the arterialized graft.

Conclusions

This new method allows for the surgical procedure to be performed visually unaided with good survival and patency rates and represents an alternative model investigating OLT in rats.  相似文献
