不同剂量舒芬太尼用于开腹手术后静脉自控镇痛的效果比较

目的比较开腹手术后不同剂量舒芬太尼用于自控静脉镇痛的效果,择出最佳剂量,为开腹手术病人术后静脉自控镇痛(PCIA)用药提供参考。方法将择期开腹手术并要求术后行PCIA的病人100例随机分为A、B、C、D组4组,每组25例,术后分别以1.5、2.0、2.5、3.0μg/kg舒芬太尼加甲氧氯普胺20mg,用0.9%氯化钠注射液稀释至150ml,背景剂量3ml/h,PCA剂量3ml/次,负荷剂量为0ml,锁定时间20min。分别于术后6h(T1)、12h(T2)、24h(T3)、36h(T4)进行痛疼评分,并对各组生命体征、不良反应等进行比较。结果 4组患者术后生命体征改变和不良反应发生率比较,差异均无统计学意义(P0.05)。D组患者的术后镇痛效果明显高于A、B组(P0.05),而且PCA按压次数最少。结论舒芬太尼用于PCIA的最佳配药剂量为3.0μg/kg。

Patient-controlled intravenous analgesia with different dose sufentanil after abdominal surgery：a comparative study

Objective To compare the clinical efficacy of different dose sufentanil for patient‐controlled intrave‐nous analgesia （PCIA） after abdominal surgery ,for getting the optimum dose . Methods 100 undergoing abdom‐inal surgery patients who ask PCIA after the surgery were randomly divided into 4 group （n=25） ,group A ,B ,C , D use 1.5 ,2.0 ,2.5 ,3.0 μg/kg sufentanil respectively ,mixing 0.9% sodium chloride injection up to 150 ml .PCIA was administered with background infusion of 3 ml/h ,bolus injection of 3 ml and lockout time of 20 min .The anal‐gesic effect ,untoward reactions and vital signs were recorded at 6 h（T1 ） ,12 h（T2 ） ,24 h（T3 ） ,36 h（T4 ） after the administration respectively . Results The analgesic effect of group D was obviously superior than other group（P〈0.05） ,and the PCA times of group D was obviously less than other group （P〈0.05） .There was no significant difference on the change of vital signs and the untoward reactions among 4 groups（P〉0.05） . Conclusion The optimum dose of PCIA with sufentanil is 3.0 μg/kg .
