广州市花都区789例人乳头瘤病毒感染状况及其基因型研究

摘    要:目的 对广州市花都区789例疑似人乳头瘤病毒( HPV)感染的妇女进行调查,研究HPV感染状况和基因亚型分布情况.方法 采用荧光定量聚合酶链式反应(Fluorescence quantitative PCR,FQ-PCR)对789例疑似HPV感染患者的宫颈分泌物进行21种人乳头瘤病毒基因亚型的检测.结果 (1)789例标本中,HPV阳性占48.42%(382/789),共检出10种基因型,其中检出单一低危型(HPV6、11、43和44型)279例,占感染者73.04%;单一高危型( HPV16、18、33、52、53、58)68例,占感染者17.80%;多重感染35例,占感染者9.16%.低危型以HPV6、11为主,高危型以HPV16、18为主.HPV6为主要致病基因型,检出率为34.0% (P <0.01);(2)各年龄段间感染率差异无统计学意义(P>0.05).结论 花都区HPV感染率较高,且多为单一轻危型感染,以HPV6为主要感染亚型,HPV基因型分布有助于确定宫颈癌发生的高危因素及筛查高危人群,对本地区HPV疫苗开发、应用具有重大意义.

Abstract:Objective To investigate the status of Human Papillomavirus(HPV) infection and its genotypes distribution in 789 women suspected to have HPV infection in Huadu district.Method 789 specimens were detected for HPV genotypes by using Fluorescence quantitative PCR(FQ-PCR).Result(1) 382 cases were HPV with 10 subtypes;the positive rate was 35.36%.279 cases belonged to the single low-risk HPV genotype(HPV6,11,43 and 44) which accounted for 73.04% of HPV positive cases;68 cases were the single high-risk type(HPV16,18,33,52,53 and 58),accounted for 17.80%;35 cases were mixed infection of HPV and the ratio was 9.16%.HPV6 subtype ranked the first in all types(P<0.01),followed by HPV11.HPV16 and HPV18 were the most common HPV infection high-risk types.(2) The infection rates between various age groups showed no significant difference.Conclusion HPV positive rate is high in Huadu district,with the single genotype being the most common HPV infection type,especially HPV6.HPV genotype distribution can help to determine the risk factors of cervical cancer and screen high-risk populations.It is full of great significance for the local HPV vaccine development and application.
