内镜辅助鼓膜成形术与显微镜辅助鼓膜成形术的比较分析

为了揭示内窥镜辅助下和显微镜辅助下鼓膜成形术的疗效差异,本研究共纳入70例慢性化脓性中耳炎患者进行研究,其中35例应用内镜辅助鼓膜成形术(观察组),35例应用显微镜辅助鼓膜成形术(对照组),观察两组的手术时间、术中出血量、气骨导差、干耳时间、鼓膜穿孔及并发症。研究结果显示,观察组的手术时间显著低于对照组((34.54±6.34)min vs.(45.25±8.25)min,p=0.022);观察组的术中出血量显著低于对照组((8.87±2.11)mL vs.(15.22±3.07)mL,p=0.003)。两组患者术后6个月的的气骨导差无显著差异((13.67±2.31)vs.(14.25±3.03),p=0.326)。观察组中干耳时间<1个月的患者为22(62.86%)例,对照组为13(37.14%)例,两组差异显著(p=0.031)。两组患者的鼓膜穿孔率无显著差异(p=0.555)。观察组的并发症(8.57%)发生率显著低于对照组(28.57%)(p=0.031)。本研究表明,在治疗慢性化脓性中耳炎过程中,与显微镜下鼓膜成形术相比,应用耳内镜辅助鼓膜成形术可有效减少手术时间和术中出血量,缩短干耳时间,并具有更少的并发症。

Comparative Analysis on Myringoplasty Assisted by Endoscope and Microscope

To reveal the difference in efficacy between endoscopic assisted myringoplasty and microscopic assisted myringoplasty,70 patients with chronic suppurative otitis media were enrolled in the study,of which endoscopic assisted myringoplasty was performed in 35 cases(observation group)and microscopic-assisted myringoplasty was performed in 35 cases(control group).The operation time,bleeding volume,air-bone gap,dry ear time,tympanic membrane perforation and complications were observed in this research.The results showed that the operation time of the observation group was significantly lower than that of the control group((34.54±6.34)min vs.(45.25±8.25)min,p=0.022).The intraoperative blood loss of the observation group was significantly lower than that of the control group((8.87±2.11)mL vs.(15.22±3.07)mL,p=0.003).There was no significant difference in air-bone gap between the two groups at 6 months postoperatively((13.67±2.31)vs.(14.25±3.03),p=0.326).The dry ear time 1 month in the observation group was 22 case(62.86%)and in the control group was 13 case(37.14%).Whereas there was a significant difference between the two groups(p=0.031).There was no significant difference in tympanic perforation rate between the two groups(p=0.555).The incidence of complications in the observation group(8.57%)was significantly lower than in the control group(28.57%)(p=0.031).This study indicated that in the treatment of chronic suppurative otitis media,the use of endoscopic assisted myringoplasty should reduce the operation time and intraoperative blood loss,and shorten the dry ear time,and have less complication compared with microscopic assisted myringoplasty.
