摘    要:目的:探讨油酸(OA)构建兔急性呼吸窘迫综合征(ARDS)模型3天内的稳定性及血浆炎症因子IL-1、IL-8含量变化的意义。方法:健康新西兰大耳白兔30只随机分为5组,每组6只,实验组(n=24)耳缘静脉注射油酸(0.1mL/kg)建立ARDS模型,对照组(n=6)注射等量生理盐水。分别检测对照组6h和实验组6h、24h、48h、72h(n=6)动脉血PH值、PaO2、PaO2/FiO2、PaCO2、肺湿/干重比值(W/D),ELISA法检测血浆细胞因子IL-1、IL-8含量,HE染色观察肺组织病理学改变。结果:和对照组比较,实验组血中细胞因子IL-1在6h、24h组升高;细胞因子IL-8在6h组、24h组、48h组升高。6h组、24h组、48h组氧合指数〈200mmHg。结论:兔油酸ARDS模型48h内稳定,炎性细胞和IL-1、IL-8可能是导致ARDS的发生和发展主要原因之一。

Abstract:Objective:To investigate the stability and the changes of IL-1,IL-10 in plasma of New Zealand rabbits with acute res-piratory distress syndrome(ARDS) induced by oleic acid(OA) in 3 days.Methods:Thirty New Zealand rabbits were randomly divided into OA experimental groups which were injected with oleic acid(0.1mL/kg) via ear vein and control group which was injected with nor-mal saline(0.1mL/kg).Histopathology,wet weight/dry weight ratio(W/D) of the lung,arterial plasma PH,PaO2,PaCO2 were observed at 6h,24h,48h 72h groups after injection.ELISA method was used to determine plasma IL-1 and IL-8.Results:Compared with control group,the levels of PO2 of experimental groups decreased(p0.01) while the levels of PaCO2 of experimental groups increased(p0.01).Compared with the 72h group,the levels of PO2 of other experimental groups decreased(p0.01) while the levels of PaCO2 of 24h,48h groups increased(p0.01).The wet weight/dry weight ratios(W/D) of the lung increased in 6h,24h,48h groups(p0.01) compared with control group while they decreased in 48h and 72h groups(p0.01) compared with that of the 6h group.The levels of IL-1 in plasma were higher in 6h group(p0.01) and in 24h group(p0.05) than that in control group,while which were lower in 48h and 72h groups(p0.01) than that in 6h group.And the levels of IL-8 in plasma were higher in 6h,24h,48h groups(p0.01) than that in control group while which were lower in 24h,48h groups(p0.05) and in 72h group(p0.01) than that in 6h group.The 6h,24h,48h groups could be diagnosed with ARDS by PaO2/FiO2200mmHg but the 72h group's PaO2/FiO2200mmHg.Conclusion:The ARDS model set up by oleic acid is stable in 48h,and the he inflammation cells and increase of IL-1,IL-8 might play important roles in ARDS.
