应用电视胸腔镜进行三切口食管癌根治术的手术配合及护理体会

目的：总结应用电视胸腔镜（video，assistedthoracoscopic surgeryVATS）在三切口食管癌根治术中的手术配合过程。方法：回顾2011年5月至2013年5月共38例中上段食管癌患者应用电视胸腔镜进行三切口食管癌根治术，术中胸部采用微创技术，使用胸腔镜器械进行食管游离和淋巴结清扫，腹部小切口进行胃部游离并制作管状胃，最后颈部切口进行食管胃吻合。结果：顺利完成38例中上段食管癌患者的手术配合，手术护士可以通过电视胸腔镜观察手术进程，熟练默契的配合每个手术步骤，术中出血较少，胸部手术时间30．60min，无一例中转开胸病例。结论：电视胸腔镜手术安全，有效，微创，便于操作，手术时间较短。

Cooperative Matching and Nursing Experience of Thoracoscopic three Incision Esophageal Carcinoma Radical Rresection Applicated with Video-assisted Thoracoscopy

Objective： To summarize the cooperative progress of the video-assisted thoracoscopic surgery （VATS） in the three incision esophageal carcinoma radical esophagectomy during operation with the process. Methods： From May 2011 to May 2013, a total number of 38 cases of esophageal cancer patients by application of thoracoscopic surgery in three incision were analyzed, used video-assisted thoracoscopic resection of esophageal cancer were three incision,the minimally invasive thoracoscopic technique was used for esophageal flee and lymph node dissection In the right chest. Gastric tube was made with, The lymph node using thoracoscope devices for dissociate and esophagus, small abdominal incision were to dissociate and stomach stomach tube production, and finally the gastro-esophageal anastomosis was finished by left neck incision with stomach esophagus anastomosis. Results： All the patients were done smoothly with the expertly cooperation. The nurse could observe the operation progress carefully through the smooth completion of 38 cases of upper middle period of patients with esophageal cancer surgery,the nurse could be observed by video-assisted thoracoscopy. with less bleeding and no conversion to thoracotomy, the good cooperate skilled in each operation steps, would lead less intraoperative bleeding, The average chest operation time was about 30 to 60 rain surgery process, no cases of transit to thoracotomy. Conclusion： Video-assisted thoracoscope surgery might be safe, effective, minimally invasive, and easy to operate and operation time was short, with shorter operation time.
