腹腔镜阑尾切除术与开腹阑尾切除术在阑尾炎治疗中的应用效果比较

目的:对比腹腔镜阑尾切除术(LA)与传统开腹阑尾切除术(OA)治疗阑尾炎的临床疗效。方法:回顾性分析2014年1月至2016年5月我院91例行LA患者(LA组)及113例行OA患者(OA组),于治疗前及治疗后3天检测血清肿瘤坏死因子(TNF-a)、血清C反应蛋白(CRP)水平,对比两组临床相关指标、并发症发生情况。结果:LA组较OA组出血量少、术后住院时间短、胃肠功能恢复及下床时间短、术后疼痛评分低、术后镇痛药需求少、并发症发生率低,但手术时间长,差异有统计学意义(P0.05);两组患者治疗后血清TNF-a、CRP水平均均明显下降,且LA组下降更明显,差异有统计学意义(P0.05)。结论:相比OA,LA治疗阑尾炎具有创伤小、住院时间短、恢复快、并发症少等优点,可有效降低血清炎症因子,值得在临床上推广应用。

Comparison of the Effect of Laparoscopic Appendectomy and Open Appendectomy in the Treatment of Appendicitis

Objective:To compare the clinical efficacy of the treatment of appendicitis between Laparoscope appendectomy group (LA) and open appendectomy group (OA).Methods:The clinical data was collected in general surgery department from January 2014 to May 2016, which had 91 cases of LA (LA group), and 113 cases of OA (OA group). Detected serum tumor necrosis factor (TNF-a) and serum C reactive protein (CRP) levels before treatment and 3 d after treatment, the clinical status and complications were compared between two groups. The tumor necrosis factor (TNF-a) and C reactive protein (CRP) levels were detected by t test before and after treatment in two groups.Results:Compared with OA, the LA has less bleeding,shorter postoperative hospital stay, recovery time of gastrointestinal function and bed time, lower postoperative pain score, ess postoperative analgesic requirement, lower complication rate, the difference was statistically significant (P<0.05). The TNF-a and CRP levels in all patients were lower compared with the preoperation, and LA was lower than OA after surgery, the difference was statistically significant(P<0.05).Conclusion:Compared with OA, LA has the advantages of less trauma, shorter hospital stay, faster recovery, less complications and so on. It can effectively reduce the serum inflammatory factors, which is worthy of clinical application.
