全子宫切除术对阴道菌群的影响

目的研究全子宫切除术后阴道菌群及pH的变化,寻求适当的治疗措施,以提高术后患者的生活质量。方法分别观察术后未发生阴道炎、发生老年性阴道炎及正常近龄妇女3组共90例,进行菌群分析、pH测定,并使用乳杆菌DM9811代谢物脂肪酸组分对发生老年性阴道炎者进行治疗。结果术后妇女检出各种菌的数量少,乳杆菌的数量更少,阴道内没有占绝对优势的菌种;发生阴道炎组乳杆菌数量下降更明显,葡萄球菌及大肠埃希菌数量相对增多。术后阴道pH在5.9~7.3,发生阴道炎组较未发生阴道炎组高;以乳杆菌DM9811代谢物脂肪酸组分阴道用药治疗老年性阴道炎,治愈率为82.14%。结论全子宫切除术后阴道乳杆菌数量下降,pH升高,易发生老年性阴道炎。治疗以乳杆菌DM9811代谢物脂肪酸组分阴道用药效果佳。

The effect on the vagina flora in patients with hysterectomy

Objective To study the alterations of vagina flora and pH of vaginal secretion in women after hysterectomy,and to find appropriate treatment for vaginitis in these patients.Methods Investigation was carried out in 30 patients without vaginitis after the operation,30 patients with senile vaginitis after the operation,and 30 healthy age-matched women respectively.pH of the vagina secretion was assayed,and the flora was analyzed.Patients were treated by the fatty acid fraction of lactobacillus DM9811 metabolite.Results The data showed that the number of many bacterium were decreased,particularly lactobacilli after the operation.There was not an absolute dominant bacterium.The number of lactobacillus was decreased,and the number of staphylococcus aureus,bacteroides and coli.bacillus were risen obviously in patients with vaginitis.Vagina pH was 5.9~7.3 in 60 cases of the patients with the operations.The pH of the patients with vaginitis was higher than that of the patients without vaginitis.The patients were treated by the fatty acid fraction of lactobacillus DM9811 metabolite.The cure rate was 82.14%,and the total effective rate was 85.71%.Conclusions The number of lactobacillus in vagina were decreased obviously after the operation,pH of the vagina secretion was risen,senile vaginitis were happened easily.The therapeutic effect of the fatty acid fraction on senile vaginitis was certain.
