目的 对公立医院卫生人才流失现状进行实证研究,分析其原因并提出相应对策。方法 采用分层抽样方法对潍坊市29家公立医院卫生人才进行问卷调查。结果 公立医院卫生人才流失现象较为普遍,流失率为30.9%,流失原因多为个人发展、薪酬、执业环境。结论 应采取相应对策提高公立医院卫生人才的稳定性,完善相应的管理机制,规范人才的合理流动。  相似文献

目的 了解京郊乡镇卫生院卫生服务可及性现状,分析基层卫生人力资源对服务可及性的影响,并提出相应的建议。方法 采用分层抽样与目的抽样结合的方法,对北京市5个远郊区县15个乡镇的21名医务人员以及11名患者进行关键知情人访谈。结果 乡镇卫生院人员队伍不稳定、专业人员缺乏、人员编制有限以及梯队不合理对卫生服务可及性产生直接的影响。结论 从满足人员基本需求、配备专业技术人员、科学制定人员编制以及合理配置人员梯队四方面改善基层卫生人才队伍建设,从而提高卫生服务可及性。  相似文献

目的 研究改良早期预警(MEWS)评分对转院转运工作的影响.方法 在转院转运工作中应用MEWS评分系统对患者进行病情评估,对比其应用前(对照组)、应用后(研究组)转院患者转运途中病情恶化的发生率、患者或家属满意度及医生、护士对实施MEWS评分转运管理模式前(对照组)、后(研究组)满意度的变化.结果 转院患者转运途中病情恶化发生率由应用前(对照组)的10.63%降至应用后(研究组)的3.74%,应用前、后比较差异有显著统计学意义(P<0.01);患方满意度则由96%提高至99%,医护人员对实施MEWS评分转运管理模式前、后的满意度由应用前的83.3%提高至应用后的95%,应用前、后比较均有显著性意义(P<0.05).结论 MEWS评分能对转院患者的病情进行比较客观、准确的评估,提高了医务人员的质量和医疗干预意识,从而有效降低转院患者转运途中的病情恶化发生率和提高院前医患双方的满意度,是转院转运工作中简便、快捷、客观、实用的病情评估方法和模式,值得推广应用.  相似文献

The purpose of this study was to survey antibacterial resistance in outpatients of Chinese county hospitals. A total of 31 county hospitals were selected and samples continuously collected from August 2010 to August 2011. Drug sensitivity testing was conducted in a central laboratory. A total of 2946 unique isolates were collected, including 634 strains of Escherichia coli, 606 Klebsiella pneumoniae, 476 Staphylococcus aureus, 308 Streptococcus pneumoniae, and 160 Haemophilus influenzae. Extended-spectrum β-lactamases were detected in E. coli (42.3% strains), K. pneumoniae (31.7%), and Proteus mirabilis (39.0%). Ciprofloxacin-resistance was detected in 51.0% of E. coli strains. Salmonella spp. and Shigella spp. were sensitive to most antibacterial agents. Less than 8.0% of Pseudomonas aeruginosa isolates were resistant to carbapenem. For S. aureus strains, 15.3% were resistant to methicillin, and some strains of S. pneumoniae showed resistance to penicillin (1.6%), ceftriaxone (13.0%), and erythromycin (96.4%). β-lactamase was produced by 96.5% of Moraxella catarrhalis strains, and 36.2% of H. influenzae isolates were resistant to ampicillin. Azithromycin-resistant H. influenzae, imipenem-resistant but meropenem-sensitive Proteus, and ceftriaxone- and carbapenem non-sensitive M. catarrhalis were recorded. In conclusion, cephalosporin- and quinolone-resistant strains of E. coli and Klebsiella pneumonia and macrolide-resistant Gram-positive cocci were relatively prominent in county hospitals. The antibacterial resistance profiles of isolates from different geographical locations varied significantly, with proportions in county hospitals lower than those in their tertiary counterparts in the central cities, although the difference is diminishing.  相似文献

Clinical ethics committees have recently been established in nearly all Norwegian hospital trusts. One important task for these committees is clinical ethics consultations. This qualitative study explores significant barriers confronting the ethics committees in providing such consultation services. The interviews with the committees indicate that there is a substantial need for clinical ethics support services and, in general, the committee members expressed a great deal of enthusiasm for the committee work. They also reported, however, that tendencies to evade moral disagreement, conflict, and 'outsiders' are common in the hospitals. Sometimes even the committees comply with some of these tendencies. The committees agree that there is a need to improve their routines and procedures, clarify the committees' profile and field of responsibility, to make the committees well-known, to secure adequate operating conditions, and to develop organizational integration and support. Various strategies to meet these challenges on a local, regional or national level are also explored in this paper.  相似文献

目的:调查军队医院非现役编制医务人员的军事素质和卫勤保障意识,探讨提升该群体卫勤保障能力的重要性和必要性,为军队医院卫勤管理提供参考。方法:采用问卷调查的方式对我院154名非现役编制医务人员进行调查,主要包括军事理论、卫生勤务、突发事件应急管理及战伤急救等,分析非现役编制医务人员缺乏的基本军事素质,并提出针对性的加强军队医院文职人员卫生勤务保障能力的可行性培训计划,在培训结束时对医护人员的军事素质进行测评。结果:问卷调查结果:非现役文职人员的军事素质评分为(3.70±0.46)、卫生勤务能力评分为(2.48±0.91)、突发事件应急管理评分为(2.61±0.53)、战伤急救技能评分为(2.42±1.02)。测评结果:军事理论知识评分为(4.11±1.04)、卫生勤务能力评分为(4.78±1.37)、突发事件应急意识评分为(4.56±0.56)、战伤急救技能评分为(4.68±0.91)。培训后,非现役医务人员的卫勤保障能力显著提高,差异具有统计学意义(P0.05)。结论:非现役文职作为军队医院的聘用制人员已成为医务工作的主要力量,因此军队医院应加强非现役医务人员的卫生勤务保障能力,提高我军后勤医疗保障的坚固力量。  相似文献

In the writings of Alois Alzheimer and many of his contemporaries, complaints abounded about psychiatric hospitals not only failing to appreciate the importance of senile dementia, but also inhibiting scientific research into the nature and causes of the disorder. This article exploits these discontents in order to examine what Alzheimer and others thought to be optimal conditions for psychiatric research on dementia. It first analyzes the various institutional contexts in which Alzheimer worked during his career (especially in Frankfurt and Munich). It then traces some of the administrative and diagnostic practices that were deployed to enhance the conditions for his clinical and pathoanatomic research. Finally, it reflects on the implications of these practices for psychiatric care and patient experience.  相似文献
