AIM: To evaluate the effect of Helicobacter pylori eradication on ulcer bleeding recurrence in a prospective, long-term study including more than 400 patients. METHODS: Patients with peptic ulcer bleeding were prospectively included. H. pylori infection was confirmed by rapid urease test, histology or (13)C-urea breath test. Several eradication regimens were used. Ranitidine 150 mg was administered daily until eradication was confirmed by breath test 8 weeks after completing eradication therapy. Patients with therapy failure received a second or third course of therapy. Patients with eradication success did not receive maintenance anti-ulcer therapy, and were controlled yearly with a repeated breath test. RESULTS: Four hundred and twenty-two patients were followed up for at least 12 months, with a total of 906 patient-years of follow up. Mean age was 59 years, and 35% were previous nonsteroidal anti-inflammatory drug (NSAID) users. Sixty-nine percent had duodenal, 24% gastric, and 7% pyloric ulcer. Recurrence of bleeding was demonstrated in two patients at 1 year (incidence: 0.22% per patient-year of follow up), which occurred after NSAID use in both cases. CONCLUSION: Peptic ulcer rebleeding does not occur in patients with complicated ulcers after H. pylori eradication. Maintenance anti-ulcer (antisecretory) therapy is not necessary if eradication is achieved. 相似文献
Patch dynamics of the Mediterranean slow-growing seagrass Posidonia oceanica was studied in two shallow sites (3–10 m) of the Balearic Archipelago (Spain) through repeated censuses (1–2 year−1). In the sheltered site of Es Port Bay (Cabrera Island), initial patch density (October 2001) was low: 0.05 patches m−2, and the patch size (number of shoots) distribution was bimodal: most of the patches had less than 6 shoots or between 20 and 50 shoots. Mean patch recruitment in Es Port Bay (0.006 ± 0.002 patches m−2 year−1) exceeded mean patch loss (0.001 ± 0.001 patches m−2 year−1), yielding positive net patch recruitment (0.004 ± 0.003 patches m−2 year−1) and a slightly increased patch density 3 years later (July 2004, 0.06 patches m−2). In the exposed site of S’Estanyol, the initial patch density was higher (1.38 patches m−2, August 2003), and patch size frequency decreased exponentially with size. Patch recruitment (0.26 patches m−2 year−1) and loss (0.24 patches m−2 year−1) were high, yielding a slightly increased patch density in the area 1 year later (October 2004, 1.40 patches m−2). Most recruited patches consisted of rooting vegetative fragments of 1–2 shoots. Seedling recruitment was observed in Summer 2004 at both sites. Episodic, seedling recruitment comprised 30% and 25% of total patch recruitment in Es Port Bay and S’Estanyol, respectively. Patch survival increased with patch size and no direct removal was observed among patches of 5 shoots or more. Most patches grew along the study, shifting patch distribution towards larger sizes. Within the size range studied (1–150 shoots), absolute shoot recruitment (shoots year−1) increased linearly with patch size (R2 = 0.64, p < 4 × 10−5, N = 125), while specific shoot recruitment was constant (about 0.25 ± 0.05 year−1), although its variance was large for small patches. Given the slow growth rate and the high survival of patches with 5 or more shoots, even the low patch recruitment rates reported here could play a significant role in the colonisation process of P. oceanica. 相似文献
Laser ablation of tooth enamel was used to analyze stable carbon isotope compositions of teeth of hominins, red deer, and bears from middle Pleistocene sites in the Sierra de Atapuerca in northern Spain, to investigate the possibility that this technique could be used as an additional tool to identify periods of physiological change that are not detectable as changes in tooth morphology. Most of the specimens were found to have minimal intra-tooth variation in carbon isotopes (< 2.3‰), suggesting isotopically uniform diets through time and revealing no obvious periods of physiological change. However, one of the two sampled hominin teeth displayed a temporal carbon isotope shift (3.2‰) that was significantly greater than observed for co-occurring specimens. The δ13C value of this individual averaged about -16‰ early in life, and -13‰ later in life. This isotopic change occurred on the canine crown about 4.2 mm from the root, which corresponds to an approximate age of two to four years old in modern humans. Our dataset is perforce small owing to the precious nature of hominid teeth, but it demonstrates the potential utility of the intra-tooth isotope profile method for extracting ontogenetic histories of human ancestors. 相似文献
Evaluating habitat suitability is often complex, as habitat effects may be scale‐dependent, critical resources may be spatially segregated, and resource availability may also depend on intra‐ and inter‐specific interactions. Using analyses that spanned multiple years and spatial scales, we investigated habitat requirements of a territorial generalist, the common raven Corvus corax, in a relatively pristine woodland, Bia?owie?a Forest (E Poland). We compiled data from multiple raven surveys conducted between 1985 and 2001. Ravens were regularly distributed over the entire study area but declined in density over 50% within the 16 yr interval. In the same period game and forest management significantly reduced ungulate densities and likely diminished the habitat quality with regard to food supply, especially carrion. To better understand habitat requirements of ravens we studied breeding performance in relation to three different habitat types across multiple scales: open areas, coniferous‐dominated forest, and deciduous‐dominated forest. We found a prominent dissimilarity between raven nesting and foraging habitats highlighting the importance of resource complementation for ravens. On a fine scale, large old pines were exclusively selected as nesting trees and nesting areas were generally coniferous‐dominated. However, at increasingly broader scales, coniferous habitats were negatively associated with raven reproductive success as those habitats likely provide a lower food supply. Only where the coniferous nesting areas at smaller scales were complemented with high percentages of deciduous forests and open areas at broader scales did the breeding performance increase. In addition to habitat composition, intra‐specific interactions were important determinants of reproductive performance and very successful neighbors decreased reproductive performance of a focal pair. Most of previous studies have investigated resource complementation in terms of habitat edges or proximity of complementing resource patches. Our study demonstrates that the concept of landscape complementation also applies to gradients in landscape composition and emphasizes the importance of scales and intraspecific interactions in habitat analyses. 相似文献
This study evaluated the effect of application of the semisynthetic triterpenes 3β-acetoxy-norlup-20-one (F4) and 3-chloro-4α,14α-dimethyl-5α-cholest-8-ene (F6) triterpene derivatives from Euphorbia officinarum on the growth of tomato seedlings under normal conditions and when challenged with the pathogens Verticillium dahliae and Agrobacterium tumefaciens. Foliar spray of F4 and F6 significantly improved growth rate, fresh weight, dry weight, and leaf area. In addition, they enhanced several physiological parameters including photosynthetic pigments, proline content, and nitrate reductase activity. Moreover, they induced H2O2 accumulation and increased the activity of several antioxidant enzymes such as catalase, ascorbate peroxidase, and guaiacol peroxidase. They also enhanced disease resistance against V. dahliae and A. tumefaciens. These results suggest that the two semisynthetic triterpenes represent new plant growth regulators and inducers of plant disease resistance.
Households from vulnerable groups experiencing epidemiological transitions are known to be affected concomitantly by under-nutrition and obesity. Yet, it is unknown to what extent this double burden affects refugee populations dependent on food assistance. We assessed the double burden of malnutrition among Western Sahara refugees living in a protracted emergency.
Methods and Findings
We implemented a stratified nutrition survey in October–November 2010 in the four Western Sahara refugee camps in Algeria. We sampled 2,005 households, collecting anthropometric measurements (weight, height, and waist circumference) in 1,608 children (6–59 mo) and 1,781 women (15–49 y). We estimated the prevalence of global acute malnutrition (GAM), stunting, underweight, and overweight in children; and stunting, underweight, overweight, and central obesity in women. To assess the burden of malnutrition within households, households were first classified according to the presence of each type of malnutrition. Households were then classified as undernourished, overweight, or affected by the double burden if they presented members with under-nutrition, overweight, or both, respectively.The prevalence of GAM in children was 9.1%, 29.1% were stunted, 18.6% were underweight, and 2.4% were overweight; among the women, 14.8% were stunted, 53.7% were overweight or obese, and 71.4% had central obesity. Central obesity (47.2%) and overweight (38.8%) in women affected a higher proportion of households than did GAM (7.0%), stunting (19.5%), or underweight (13.3%) in children. Overall, households classified as overweight (31.5%) were most common, followed by undernourished (25.8%), and then double burden–affected (24.7%).
Conclusions
The double burden of obesity and under-nutrition is highly prevalent in households among Western Sahara refugees. The results highlight the need to focus more attention on non-communicable diseases in this population and balance obesity prevention and management with interventions to tackle under-nutrition.

Bacterial population changes were investigated in the monomictic Lake Estanya by combining microscopic analysis and two molecular
methods involving the amplification of 16S rDNA genes using primers for the domain Bacteria and subsequent restriction fragment length polymorphism (PCR–RFLP) and denaturing gradient gel electrophoresis (PCR–DGGE).
Both approaches revealed the vertical distribution of predominant microbial morphotypes and phylotypes in both holomictic
and stratified periods, respectively, and showed that variations in structure and composition of bacterial populations are
occurring in this lake as a function of depth and time. Through principal component analysis (PCA), these shifts could be
related to different physicochemical parameters with temperature, oxygen concentration, and the incident light being of paramount
importance as structuring variables. Comparison of RFLP and DGGE profiles by scoring similarities using the Jaccard coefficient
and then building a multidimensional scaling map (MDS) showed equivalent results. Both techniques revealed that bacterial
populations, present in the whole water column in the holomictic period, showed a high similarity with those located in the
deeper part of the lake in the stratified period, evidencing that other factors, both biotic and abiotic, should also be considered
as a force driving change in the composition of the bacterial community. Furthermore, DGGE analysis showed that sequences
from prominent bands were affiliated to members of four major phyla of the domain Bacteria: Cyanobacteria, Bacteroidetes, Proteobacteria, and Actinobacteria, most of which corresponded to heterotrophic bacterial populations involved in carbon, sulfide, and nitrogen biogeochemical
cycles, which were indistinguishable under the light microscope. 相似文献
This work analyzed possible dopamine-mediated cadmium effects on plasma prolactin, GH and ACTH levels and if these changes were related to metal accumulation. Male rats were treated from day 30 to 60 of life with 50 mg/L of CdCl2 in the drinking water. Cadmium exposure decreased the dopamine (DA) metabolism (DOPAC/DA ratio) in all brain areas studied, and plasma levels of prolactin, GH and ACTH were diminished. The cadmium concentration did not increase nor in hypothalamus nor in the pituitary after the metal exposure. These results suggest that cadmium inhibits the secretion of these pituitary hormones and this inhibitory effect is not mediated by dopamine or the degree of metal accumulation. 相似文献
