The presence of a parathyroid gland in ectopic position constitutes one of the leading causes of surgery failure. For this purpose, scintigraphy takes a major place in the anatomical localization of pathological parathyroid and so facilitates its treatment. The advent of hybrid cameras combining single photon emission tomography to CT (SPECT/CT) has further strengthened the contribution of this imaging modality in the precise localization of ectopic parathyroid and its anatomical relationships. The objective of our work is to illustrate the interest of hybrid imaging SPECT/CT, compared to planar scintigraphy and ultrasounds within the framework of primary hyperparathyroidism, particularly ectopic situation through three cases.  相似文献

摘要 目的:观察西那卡塞联合碳酸司维拉姆对血液透析并发继发性甲状旁腺功能亢进(SHPT)患者钙磷代谢、成纤维细胞生长因子23(FGF23)/Klotho轴和心血管事件的影响。方法:选取2018年3月-2020年5月新疆医科大学第一附属医院收治的200例血液透析并发SHPT患者,根据随机数字表法分成观察组(n=100)与对照组(n=100)。对照组患者接受碳酸司维拉姆治疗,观察组患者接受西那卡塞联合碳酸司维拉姆治疗,两组患者均连续治疗3个月。观察两组疗效以及钙磷代谢指标、全段甲状旁腺激素(iPTH)、甲状旁腺体积、FGF23、Klotho水平变化,记录治疗期间发生的心血管事件。结果:观察组的临床总有效率较对照组高(P<0.05)。治疗后两组血钙升高,血磷、钙磷乘积下降(P<0.05),且观察组治疗后血钙较对照组高,血磷、钙磷乘积较对照组低(P<0.05)。两组治疗后血清iPTH水平下降,甲状旁腺体积缩小,且观察组的变化程度较对照组大(P<0.05)。两组治疗后Klotho水平升高,FGF23水平下降,且观察组的变化程度大于对照组(P<0.05)。观察组的非致死性心血管事件发生率明显低于对照组(P<0.05)。结论:西那卡塞联合碳酸司维拉姆治疗血液透析并发SHPT患者疗效显著,可调节钙磷代谢,降低血清iPTH水平,缩小甲状旁腺体积,调节Klotho、FGF23水平,降低非致死性心血管事件发生率。  相似文献

Cases of pathologically changed bone might constitute a diagnostic pitfall and frequently need histological methods to be etiologically properly evaluated. With micro‐computed tomography (μCT), a new epoch of 2D and 3D imaging has been launched. We evaluated the diagnostic investigation of this analytical method versus well established histological investigations of historical human bone. Pathological changes due to various etiologies (infectious, traumatic, endocrinological, neoplasia) observed in autopsy‐based macerated human skulls (Galler Collection, Natural History Museum Basel, Switzerland) were investigated by μCT and compared with histological thin ground sections using polarized light. Micro‐CT images visualize the architecture of the bone with high spatial resolution without preparation or destruction of the sample in the area to be sectioned. Changes in the bone surfaces as well as alterations of the diploë can be assessed. However, morphological patterns caused by reactive response, such as typical arrangements of collagen fibers, can only be visualized by the microscopic investigation of thin ground sections using polarized light. A great advantage of μCT is the high number of slices obtained so that spatial differences within the areas of the specimen become visible. Micro‐CT is a valuable tool for the diagnosis of vestiges of skull bone diseases. Its advantages over histology are the fast, automated image acquisition and the fact that the specimen is not completely destroyed. Only excision of the area to be scanned is necessary, if the specimen is too large to be scanned as a whole. Further, the 3D visualization of the micro‐architecture allows an easy orientation within the sample, for example, for the choice of the location of the histological slices. 目的:比较不同血磷水平的维持性血液透析的尿毒症患者的临床表现和实验室指标,探讨其临床意义。方法:选择上海交通大学医学院附属新华医院(崇明)肾内科28例高血磷(SP>1.6mmol/L)的维持性血液透析患者为病例组,30例血磷正常(SP≤1.6mmol/L)维持性血液透析患者为对照组,比较两组患者的原发病组成,年龄,性别,透析龄,皮肤瘙痒发生率,腰背痛发生率,血钙,血碳酸氢根,血红蛋白,红细胞压积,血碱性磷酸酶,肾功能,血浆白蛋白水平及左心室肥厚发生率。结果:病例组与对照组在原发病组成,年龄(43.2±9.8岁vs 40.5±12.2岁),男女性别比例(16/12 vs.17/13),透析龄(32.56±6.71月vs.35.43±5.82月)等方面无显著性差异(P>0.05),有可比性,在血红蛋白(83.22±6.71g/L vs 103.36±5.84g/L),红细胞压积(24.83±1.92%vs.30.76±1.52%),血钙(1.71±0.16mmol/L vs.2.23±0.21 mmol/L),血碳酸氢根(14.2±3.1mmol/L vs 20.6±4.9 mmol/L),血碱性磷酸酶(124.26±16.33U/L vs.61.47±14.91 U/L),皮肤瘙痒发生率(22/28 vs.7/30),腰背痛发生率(19/28 vs.6/30),左心室肥厚发生率(20/28 vs 12/30),有显著性差异(P<0.05),肌酐(956±142mmol/L vs.923±156 mmol/L),尿素氮(23.1±6.3mmol/L vs.24.8±8.9mmol/L),血浆白蛋白(30.5±3.8g/L vs.31.2±2.9g/L),无显著性差异(P>0.05)。结论:伴有高磷血症的维持性血液透析患者与血磷正常的患者相比,血碱性磷酸酶,皮肤瘙痒发生率,腰背痛发生率及左心室肥厚发生率较高,而血钙,血碳酸氢根,血红蛋白及红细胞压积较低,有一定差异。  相似文献

ObjectiveWe aimed to find and compare the efficacy of ultrasonography (US), technetium-99m methoxyisobutylisonitrile parathyroid scintigraphy (MIBI-S), and single-photon emission computed tomography–computed tomography (SPECT-CT) in detecting the localization of parathyroid adenomas in patients with primary hyperparathyroidism.MethodsIn total, 348 patients were included in this study. Preoperative parathyroid imaging with US, MIBI-S, and SPECT-CT was evaluated and compared with operative findings. The results of the imaging methods were compared with pathology and operation reports.ResultsIn 318 patients (91.3%), one of the imaging methods was able to localize the lesion correctly. US detected the localization of the parathyroid lesions correctly in 268 patients (77%), whereas SPECT-CT and MIBI-S were correct in 254 (73%) and 209 (60%) patients, respectively. There was a statistically significant relationship between the parathyroid hormone (PTH) level and 3 imaging methods’ success rates (P < .05). The PTH cut-off value, which best determined the correct localization, was 152.5 pg/mL for US, 143 pg/mL for MIBI-S, and 143 pg/mL for SPECT-CT. It was observed that the correct localization rate for parathyroid lesions increased with higher PTH levels.ConclusionIn our study population, US was more successful, in most cases, than other imaging methods in localizing parathyroid lesions but SPECT-CT was more accurate in localizing mediastinal lesions. In addition, it was found that preoperative PTH levels affect the accuracy of imaging methods.  相似文献

继发性甲状旁腺功能亢进是慢性肾脏病的常见并发症,严重影响患者的生活质量和生存率。其发病机制主要包括低钙血症、高磷血症、活性维生素D缺乏、维生素D受体及钙敏感受体表达下调等。近年来,成纤维细胞生长因子23在继发性甲状旁腺功能亢进发病机制中的作用,也越来越受到重视。上述机制均可导致慢性肾脏病患者的甲状旁腺细胞增生,由多克隆增生逐渐发展为单克隆增生、腺瘤等,甲状旁腺功能亢进也逐渐加重。现就慢性肾脏病继发性甲状旁腺功能亢进的发病机制的进展做一综述。  相似文献

ObjectivesThis study aims to assess performances of preoperative conventional imaging (99mTc-MIBI scintigraphy, cervical Ultrasonography) and 18F-fluorocholine PET/CT (FCH PET/CT) exams in the detection of hyperfunctioning parathyroid gland(s) in patients operated from primary hyperparathyroidism (pHPT).MethodsBefore surgery, 51 consecutive patients with biochemically confirmed pHPT had successively cervical ultrasonography (cUS), 99mTc-MIBI scintigraphy and FCH PET/CT, all performed in the same Nuclear Medicine Department. 99mTc-MIBI scintigraphy were performed immediatly after cUS and interpreted by same operators. FCH PET/CT exam were interpreted independently by two nuclear medicine physicians. An additional interpretation session integrating the three imaging modalities read in consensus as a combined imaging set was performed.ResultsAt surgery, 69 lesions were removed: 32 parathyroid adenoma and 37 parathyroid hyperplasia. 70% of patients had single-gland disease and 30% had multiglandular disease at histopathology analysis. In the patient-based analysis, sensitivity and accuracy in the detection of single gland disease) for FCH PET/CT, cUS and for 99mTc-MIBI scintigraphy were 86% [0.71–0.94] and 86% [0.70–0.95], 69% [0.52–0.81] and 69% [0.51–0.83], 40% [0.26–0.56] and 40% [0.24–0.58], respectively. Sensitivity and accuracy of the combined imaging set for the detection of single gland disease were 94% [0.81–0.99] and 94% [0.81–0.99].ConclusionOur results suggest that cUS/99mTc-MIBI scintigraphy and FCH PET/CT interpreted during a comprehensive session could be the ideal practice to precisely localize parathyroid lesion(s) in patients with pHPT before surgery.  相似文献
