目的分析对肥胖急性阑尾炎病人实施腹腔镜阑尾切除与开腹阑尾切除的临床治疗效果。方法回顾性分析我院2007年1月至2011年12月期间腹腔镜阑尾切除与开腹阑尾切除相关资料。结果实施腹腔镜阑尾切除手术病人的总体并发症发生率与致死率均较低,住院时间较短,住院费用较少,两种方法相比差异显著,具有统计学意义。结论对于肥胖病人实施腹腔镜阑尾切除手术比实施开腹阑尾切除手术更为安全可靠,疗效更好,不论阑尾炎是否穿孔,腹腔镜阑尾切除手术应作为首选方法。  相似文献

In 1995, an unusually perforated femur of a juvenile cave bear was found in the Divje babe I Palaeolithic cave site in Slovenia. The supposition that it could be a flute led to heated debates. According to its archaeological context and chronostratigraphic position, if made by humans, it could only be attributed to Neanderthals. The crucial question was related to the origin of the holes. These could only have been made either by a carnivore or by human intervention. Results of experimental testing of both hypotheses do not support a carnivore origin of the holes. Furthermore, the method of artificial creation of the holes, which left no conventional traces of manufacture, was defined. Computed tomography revealed traces, which could be the result of human agency and called into serious question the origin of some features previously declared to be solely of carnivore origin. Recent musical experiments performed on a replica of the reconstructed musical instrument revealed its great musical capability. Together with some other findings from Divje babe I, the Mousterian musical instrument offers a unique insight into the Neanderthals’ symbolic behaviour and their cognitive abilities. The multidisciplinary results of comprehensive analyses of this exceptional find are first presented here together with its chronostratigraphic, palaeo-environmental, and archaeological contexts.  相似文献

Purinergic signalling in rat GFSHR-17 granulosa cells was characterised by Ca2+-imaging and perforated patch-clamp. We observed a resting intracellular Ca2+-concentration ([Ca2+]i) of 100 nM and a membrane potential of −40 mV. This was consistent with high K+− and Cl permeability and a high intracellular Cl concentration of 40 mM. Application of ATP for 5–15 s every 3 min induced repeated [Ca2+]i increases and a 30 mV hyperpolarization. The phospholipase C inhibitor U73122 or the IP3-receptor antagonist 2-aminoethoethyl diphenyl borate suppressed ATP responses. Further biochemical and pharmacological experiments revealed that ATP responses were related to stimulation of P2Y2 and P2Y4 receptors and that the [Ca2+]i increase was a prerequisite for hyperpolarization. Inhibitors of Ca2+-activated channels or K+ channels did not affect the ATP-evoked responses. Conversely, inhibitors of Cl channels hyperpolarized cells to −70 mV and suppressed further ATP-evoked hyperpolarization. We propose that P2Y2 and P2Y4 receptors in granulosa cells modulate Cl permeability by regulating Ca2+-release.  相似文献

The present study was designed to characterize pharmacological, biophysical and electrophysiological properties of the recombinant human cardiac I Ks (KCNQ1/KCNE1) channels at physiological temperature. Human cardiac KCNQ1 and KCNE1 genes were cotransfected into HEK 293 cells, and a cell clone stably expressing both genes was selected. Membrane currents were recorded using a perforated patch-clamp technique. The typical I Ks was slowly activated upon depolarization voltages in HEK 293 cells stably expressing human cardiac KCNQ1 and KCNE1 genes, and the current was inhibited by I Ks blockers HMR 1556 and chromanol 293B, with 50% inhibitory concentrations (IC50s) of 83.8 nM and 9.2 μM, respectively. I Ks showed a significant temperature-dependent increase in its magnitude upon elevating bath temperature to 36°C from room temperature (21°C). The current was upregulated by the β-adrenoceptor agonist isoproterenol, and the effect was reversed by H89. In addition, I Ks was inhibited by Ba2+ in a concentration-dependent manner (IC50 = 1.4 mM). Action potential clamp revealed a “bell-shaped” time course of I Ks during the action potential, and maximal peak current was seen at the plateau of the action potential. A significant use- and frequency-dependent increase of I Ks was observed during a train of action potential clamp. These results indicate that the recombinant human cardiac I Ks stably expressed in HEK 293 cells is similar to native I Ks in drug sensitivity and regulated by Ba2+ and β-adrenoceptor via the cyclic adenosine monophosphate/protein kinase A pathway. Importantly, the current exhibits significant temperature dependence, a bell-shaped time course during action potential and prominent use- or frequency-dependent accumulation during a train of action potentials.  相似文献

Voltage-gated ionic channels are known to be involved in oncogenesis. However, only a few studies describe the functional characteristics of these channels or the mechanisms by which they are involved in the proliferation and invasive processes. Breast cancer cells proliferate and migrate under the constant activation of growth factors, hormones, extracellular matrix interactions, etc. It would thus not be surprising if the activity of the ionic channels was modulated by intracellular regulation pathways such as kinases or phosphatases, which in turn can affect oncogenic properties. In the present study, we investigated some of the electrophysiological properties of the fast inward sodium current found in the breast cancer cell line MDA-MB-231 with two configurations of the patch-clamp technique. With perforated patch, a configuration which allows to keep the cytoplam intact, the mean current amplitude was lower, the relative conductance–voltage relationship was shifted to more positive potentials and the recovery from inactivation was accelerated when compared to ruptured patch, where the cytoplasm is dialysed by the intrapipette solution. There was no difference in availability–voltage (pseudo-steady-state inactivation) relationship and in time to peak of the current. These results suggest that regulation mechanisms, possibly involving kinases or phosphatases, are switched off when the cytoplasm is diluted. We propose that such a regulation can modulate the functioning of the channels even in the absence of membrane voltage changes, which in turn can affect oncogenic properties. 目的 分析上海市浦东新区区属公立综合医院急性阑尾炎住院诊疗的过程质量。方法 根据原卫生部推出的急性阑尾炎临床路径标准住院流程要求,对浦东新区7所区属公立综合医院随机抽取的152份病案进行评价。结果 浦东新区区属公立综合性医院急性阑尾炎住院诊疗基本符合原卫生部对急性阑尾炎临床路径的要求,但部分术前评估与病历记录不完整,术后细菌培养及药敏试验率仅为3.88%。结论 浦东新区卫生行政部门和医院应进一步加强急性阑尾炎临床路径管理,临床医师应改善术前评估与病历记录,并意识到细菌培养及药敏试验在抗菌药物选择中的重要性。  相似文献

目的:研究CO_2气腹对老年急性阑尾炎患者术后炎症反应及免疫应激反应的影响,为临床治疗提供依据。方法:选取2015年1月到2016年3月我院收治的老年急性阑尾炎患者130例,按照随机数字表法将患者分为研究组和对照组,每组65例,对照组给予常规开腹阑尾炎切除术,研究组应用CO_2气腹下腹腔镜下阑尾炎切除术,比较两组术前、术后1天、3天和5天白细胞计数、中性粒细胞计数、白细胞介素-10(IL-10)、肿瘤坏死因子-α(TNF-α)水平。结果:术后1天两组白细胞计数、中性粒细胞计数、IL-10和TNF-α水平均明显升高,比较差异具有统计学意义(P0.05),研究组术后1天白细胞计数和中性粒细胞计数与对照组比较,差异无统计学意义(P0.05),研究组术后1天TNF-α和IL-10水平显著低于对照组,比较差异具有统计学意义(P0.05);术后3天、5天与术后1天相比,两组白细胞计数、中性粒细胞计数、IL-10和TNF-α水平均明显降低,术后3天与术前比较差异无统计学意义(P0.05),术后5天与术前比较差异具有统计学意义(P0.05),术后3天和5天研究组白细胞计数、中性粒细胞计数、IL-10和TNF-α均显著低于对照组,比较差异具有统计学意义(P0.05)。结论:CO_2气腹能有效降低急性阑尾炎患者术后炎症反应和免疫应激反应,改善患者预后。  相似文献

BACKGROUND: A 7-year old, female chimpanzee (Pan troglodytes) developed acute abdominal pain and anorexia. An irregular, mineral opacity was identified in the caudal right quadrant of the abdomen on radiographs and computed tomography scan, which appeared to be in the region of the cecal appendage. RESULTS AND CONCLUSIONS: A diagnosis of acute appendicitis was made based on clinical signs, abnormal haematology findings, and consultation with a human radiologist. Exploratory laparotomy was performed and the cecal appendage was removed. On histologic examination, the mucosal epithelium contained eosinophilic and neutrophilic inflammation. The inflammation extended through the tunica muscularis to the serosal surface and adjacent mesentery. The histologic findings were consistent with acute appendicitis in humans. The chimpanzee recovered well from surgery with immediate improvement in clinical signs and no post-operative complications.  相似文献

Abstract: The effects of the proadrenomedullin N-terminal 20-amino acid peptide (PAMP) on the nicotinic acetylcholine (ACh) receptor (nAChR)-mediated inward current were investigated in neurons acutely dissociated from the rat locus coeruleus using whole-cell recording under voltage clamp. Nicotine and cytidine mimicked the ACh response, whereas the maximal response to dimethyl-phenylpiperazinium was lower in amplitude compared with that to ACh. Nicotine-induced current ( I nic) was suppressed more effectively by mecamylamine than by hexamethonium. In addition, neither atropine nor α-bungarotoxin affected the I nic. PAMP reversibly and noncompetitively suppressed the peak amplitude of 10−4 M I nic. PAMP concentrations for the threshold, half-maximal inhibition, and maximal inhibition of 10−4 M I nic were 10−8, 2.6 × 10−7, and 10−5 M , respectively. The peak amplitudes of 10−4 M I nic elicited at 2-min intervals showed a gradual decline in the presence of 10−7 M PAMP. This decline in the I nic was independent of the period of PAMP pretreatment. The suppression of I nic by PAMP did not show any voltage dependency at a holding potential ( V H) of <0 mV, although the inhibitory effect was masked by the marked inward rectification of I nic at a V H of 0 mV. These results suggest that PAMP could thus be a unique endogenous peptide that antagonizes the nAChR in the CNS.  相似文献

目的:探讨并分析腹腔镜手术应用于普外科治疗不同类型急性阑尾炎的临床效果。方法:选取2011年至2012年在我院接受急性阑尾炎手术的患者150例,并随机分为观察组和对照组。观察组行腹腔镜手术治疗急性阑尾炎;对照组行阑尾切除术。观察并比较两组患者围术期相关指标。结果:与对照组相比,观察组中患者的各项指标均优于对照组。采用腹腔镜手术治疗急性阑尾炎的患者术中出血量少,手术时间短,术后恢复快,并发症的发生率低,差异有统计学意义(P〈0.05)。结论:腹腔镜手术具有手术持续时间短、创伤面积小以及术后恢复快等优势。对不同类型急性阑尾炎的治疗效果显著,值得推广。  相似文献
