目的:探讨实时三维超声心动图对心脏多发性粘液瘤诊断和术前风险评估及手术方式选择的临床意义。方法:回顾性分析 我院2007 年1 月至2012 年12 月收治的经手术病理确诊为心脏多发性粘液瘤5 例患者的二维超声心动图(two-dimensionale echocardiography,2DE)、实时三维超声心动图(real-time three-dimensional echocardiography,RT-3DE)声像图特征,并结合相关文献 进行分析。结果:5 例患者的超声心动图均在心腔内探及多个活动性团块,各团块均与一蒂相连接,RT-3DE 较2DE 能准确显示粘 液瘤整体形态、位置及与周围组织清晰的界限,并定量测定其大小。结论:RT-3DE 弥补了2DE 不能显示粘液瘤立体形态图像的 缺点,可更为准确、简便、快捷地对心脏多发性粘液瘤做出诊断,并为术前风险评估及手术方式的选择提供了更可靠的依据。  相似文献

目的:研究彩色多普勒超声(CDDS)在血管性勃起功能障碍(ED)中的诊断价值,从而为患者临床诊断方式的选择提供参考。方法:选择本院内2012年4月至2015年3月期间因ED入院接受治疗的男性患者248例,在患者接受检查前,需将酚妥拉明、罂粟碱以及前列腺素-E1等药物混合液0.2 m L注入阴茎海绵体内,使得诱导阴茎勃起,随后使用彩色多普勒超声系统进行诊断,对阴茎海绵体动脉的收缩期最大血流率(PSV)、舒张末期血流率(EDV)、阻力指数(RI)等指标进行记录,对各指标诊断ED的应用价值进行评估。结果:在248例患者中,存在血流动力学异常病例172例,其中96例患者为动脉性ED,72例患者为静脉性ED;72例血流动力学正常病例,为非血管性ED。合并糖尿病25例,占10.08%;高血压17例,占6.85%;高血脂116例,占46.77%;阴茎硬结症7例,占2.82%;阴茎海绵体纤维化8例,占3.23;经腹前列腺切除术后者9例,占3.2%,经尿道切除前列腺术后者8例,占3.23;吸烟者196例,占79.03%。血管性ED患者的FPSV、PSV、EDV明显低于非血管性ED患者的,差异均有统计学意义(P0.05);且在血管性ED患者中,动脉性FPSV、ED、EDV患者的PSV明显低于非血管性ED患者的,差异均有统计学意义(P0.05)。在指标对比中,血管性ED患者的RI与非血管性ED患者的RI无明显差异(P0.05),但静脉性ED患者的RI明显低于非血管性ED患者的,差异均有统计学意义(P0.05)。在96例动脉性ED患者中,行选择性阴部内动脉造影术有11例,动脉性病变8例,彩色超声多普勒检查与选择性阴部内动脉造影术符合率为72.73%。结论:在诱导阴茎勃起后,对患者进行彩色多普勒超声系统诊断,能够有效排除阴茎在疲软状态下存在的可变性因素,能够准确反映阴茎血流动力学状态,从而能够准确筛查血管性ED疾病,其检测结果的准确性优于动脉造影,临床应用价值较高,值得进一步推广使用。  相似文献

A 15-year-old male Sumatran orangutan (Pongo pymaeus abeli) with a history of an interventricular septal defect was evaluated with cardiac catheterization and two-dimensional echocardiography. Results demonstrated that by Homo sapiens standards the right heart pressures were normal. The oxygen saturations were consistent with a small ventricular septal defect. Echocardiography demonstrated a slight enlargement of the right ventricle.  相似文献

摘要 目的:基于三维超声心动图对比分析扩张型心肌病(DCM)与二尖瓣关闭不全(MI)左室构型和收缩功能的研究。方法:收集我院2018年1月至2021年7月就诊患有左心室(LV)扩张的患者100例,其中DCM患者57例,MI患者43例。LV大小大致相仿,DCM组(43±5)mm/m2,MI组(42±5)mm/m2。另选取同时期50例健康受试者作为对照组。所有患者均进行常规超声心动图及三维超声心动图检查,测量指标主要包括左室大小(LVID)、左室后壁厚度(PWT)、左室舒张末期内径(LVEDD)、左室舒张末期室间间隔厚度(IVS)、左室舒张末期容积(LVEDV)、收缩末期容积(LVESV)、相对室壁厚度(RWT)、LV质量指数(LVMI)、三维左室射血分数(3D-LVEF)、三维舒张末期血流速度(3D-EDV)、二维或三维超声心动图球形指数(2D-SI/3D-SI)。结果:DCM组和MI组LVEDD均大于对照组,差异有统计学意义(P<0.05)。DCM组比MI组患者心功能分级III/IV和心力衰竭的发生率更高,差异有统计学意义(P<0.05)。DCM组和MI组患者的LVEDD、LVEDD指数、LVEDV、LVEDV指数、3D-EDV、3D-EDV指数均高于对照组,差异有统计学意义(P<0.05);但DCM组和MI组对比差异无统计学意义(P>0.05)。DCM组和MI组患者的LV长度、LV长度指数、LVMI均高于对照组,差异有统计学意义(P<0.05);且MI组高于DCM组,差异有统计学意义(P<0.05)。DCM组和MI组患者的LVESV、LVESV指数、2D-SI、3D-SI均高于对照组,差异有统计学意义(P<0.05);且DCM组高于MI组,差异有统计学意义(P<0.05)。DCM组3D-LVEF、RWT均低于对照组和MI组,差异有统计学意义(P<0.05)。ROC分析显示,3D-SI在评估左室扩大患者的左室重构方面优于其他变量,3D-SI的ROC曲线下面积为0.875,95%CI为0.816-0.920,3D-SI>0.62对于DCM和MI区分左室构型的特异性(81.66%)和敏感性(92.09%)较高。DCM和MI患者的3D-LVEF和3D-SI均呈线性负相关(r=-0.719,P=0.000;r=-0.682,P=0.000)。DCM和MI患者3D-SI检测心力衰竭的ROC曲线下面积均大于3D-LVEF的ROC曲线下面积,差异有统计学意义(P=0.000)。结论:与MI患者相比,尽管LV大小大致相仿,但DCM患者的左室几何形状更接近球形,且收缩功能更差。收缩功能与3D-SI显著相关,3D-SI较好地描述了左室重构,可能是LV扩张患者心力衰竭的较强指标。  相似文献

Objective. To determine the influence of transoesophageal echocardiography (TEE) on therapy and prognosis in patients with cryptogenic transient ischaemic attack (TIA) or ischaemic stroke under the age of 50 years. Methods and results. We evaluated all patients aged 50 and under who were referred to our university hospital for cryptogenic TIA or ischaemic stroke during the period 1 January 1996 to 31 December 2004. All patients underwent both transthoracic echocardiography (TTE) and TEE. Patients with known pre-existent heart disease, such as atrial fibrillation, were excluded. Eighty-three patients with TIA (22) and ischaemic stroke (61) were enrolled. Mean age was 39±8 years (range 18 to 50). In 30% of the patients TEE detected one or more potential cardioembolic source, compared with 10% for TTE (p=0.003). Standard treatment (aspirin 38 mg daily) was changed in 7% of the patients due to the TEE findings. Complete followup was obtained in 93% with an average of 5±3 years. Twelve recurrences occurred; two out of six patients (33%) with therapy change and ten out of 71 (14%) of the patients without therapy change had a recurrent TIA or ischaemic Stroke. Conclusion. In patients with cryptogenic TIA or ischaemic stroke, TEE is superior to TTE in the detection of a potential cardiac source of embolism. However, findings obtained by TEE only influence the already initiated treatment in a small percentage of patients. The recurrence rate both in the group with and without therapy change is high. Background. Duchenne muscular dystrophy (DMD) patients used to die mainly from pulmonary problems. However, as advances in respiratory care increase life expectancy, mortality due to cardiomyopathy rises. Echocardiography remains the standard diagnostic modality for cardiomyopathy in DMD patients, but is hampered by scoliosis and poor echocardiographic acoustic windows in adult DMD patients. Multigated cardiac radionuclide ventriculography (MUGA) does not suffer from these limitations. N-terminal proBNP (NTproBNP) has shown to be a diagnostic factor for heart failure. We present our initial experience with plasma NT-proBNP measurement in the routine screening and diagnosis of cardiomyopathy in adult mechanically ventilated DMD patients. Methods. Retrospective study, 13 patients. Echocardiography classified left ventricular (LV) function as preserved or depressed. NT-proBNP was determined using immunoassay. LV ejection fraction (LVEF) was determined using MUGA. Results. Median (range) NT-proBNP was 73 (25 to 463) ng/l. Six patients had an NT-proBNP >125 ng/l. Seven patients showed an LVEF <45% on MUGA. DMD patients with depressed LV function (n=4) as assessed by echocardiography had significantly higher median NT-proBNP than those (n=9) with preserved LV function: 346 (266 to 463) ng/l versus 69 (25 to 257) ng/l (p=0.003). NT-proBNP significantly correlated with depressed LV function on echocardiogram and with LVEF determined by MUGA. Conclusion. Although image quality of MUGA is superior to echocardiography, the combination of echocardiography and NT-proBNP achieves similar results in the evaluation of left ventricular function and is less time consuming and burdensome for our patients. A 55-year-old man was admitted to our hospital because of chest distress, associated with activity. Two-dimensional echocardiography (2DE) demonstrated a suspected trabeculation versus false tendon of the left ventricular apex cordis but not meeting the diagnostic criteria of noncompaction of the ventricular myocardium (NVM). Threedimensional echocardiography (3DE) revealed more prominent trabeculations and deeper intertrabecular recesses of the left ventricular apex, which were consistent with the diagnostic criteria of NVM. In contrast to 2DE, 3DE provides wide, pyramid-shaped datasets that encompass the entire left ventricle. (Neth Heart J 2009;17:208–10.)  相似文献

Significant advances in three-dimensional echocardiography have made this modality a powerful diagnostic tool in the cardiology clinic. It can provide accurate and reliable measurements of chamber size and function, including the quantification of left ventricular mechanical dyssynchrony to guide patient selection for cardiac resynchron-isation therapy. Furthermore, three-dimensional echocardiography offers novel views and comprehensive anatomic definition of valvular and congenital abnormalities, improving diagnosis and preoperative planning. In addition, it is extremely useful in monitoring the effectiveness of surgical or percutaneous transcatheter interventions. As its efficacy for more and more clinical applications is demonstrated, it is clear that three-dimensional echocardiography has become part of the routine clinical diagnostic armamentarium. In this article, we describe the development of three-dimensional echocardiography over the last decades, review the scientific evidence for its current clinical use and discuss potential future applications. Background Cardiovascular disease, especially cardiomyopathy, was the major cause of death among owl monkeys (Aotus sp.) at a major colony and threatened colony sustainability. For this study, echocardiography (echo) and electrocardiography (ECG) normal values were established, and cardiomyopathy animals identified. Methods Forty‐eight owl monkeys were studied, 30 older than 10 years of age (‘aged’) and 8 of age 5 years (‘young’). Eight aged owl monkeys had cardiomyopathy. Results and Conclusions Aged Aotus had increased left ventricular posterior wall thickness over young animals. Left ventricular diameter and ejection fraction appeared to be the best identifying measurements for cardiomyopathy. There were no differences in the ECG.  相似文献

KARASON, KRISTJAN, INGEMAR WALLENTIN, BO LARSSON, LARS SJOSTROM. Effects of obesity and weight loss on cardiac function and valvular performance. Obes Res. 1998;6:422–429. Objective : To study the consequences of long-standing obesity on myocardial function and valvular performance and to determine the effects of weight loss on these cardiovascular features. Research Methods and Procedures : We included 41 patients with obesity referred for weight-reducing gastroplasty, 31 patients with obesity who received dietary recommendations, and 43 lean subjects. Body weight and blood pressure were measured, and cardiac function and valvular performance were estimated echocardiographically. Left ventricular ejection fraction was used to assess systolic heart function, and the ratio of transmitral early to atrial (E/A) peak flow velocity was used as an estimate of diastolic filling. All three study groups were investigated at baseline, and the two groups with obesity were re-examined at 1-year follow-up. Results : Patients with obesity had higher blood pressure, greater cardiac output, lower ejection fraction, and reduced E/A ratio, compared with lean subjects (p<0.01). Surgical treatment of obesity led to significant decreases in body weight, whereas body weight remained unchanged in the group treated with dietary recommendations (p<0.001). In the weight loss group, blood pressure and cardiac output decreased and the E/A ratio increased (p<0.001). Left ventricular ejection fraction tended to increase in the weight loss group and decrease in the obese control group p<0.01). No significant valvular disease was observed in any of the subjects with obesity at baseline or after weight loss. Discussion : We conclude that weight reduction in subjects with obesity is associated with improvements in left ventricular diastolic filling and has favorable effects on left ventricular ejection fraction. Neither obesity nor weight loss seem to promote valvular heart disease.  相似文献
