In view of the increasing complexity of both cardiovascular implantable electronic devices (CIEDs) and patients in the current era, practice guidelines, by necessity, have become increasingly specific. This document is an expert consensus statement that has been developed to update and further delineate indications and management of CIEDs in pediatric patients, defined as ≤21 years of age, and is intended to focus primarily on the indications for CIEDs in the setting of specific disease categories. The document also highlights variations between previously published adult and pediatric CIED recommendations and provides rationale for underlying important differences. The document addresses some of the deterrents to CIED access in low- and middle-income countries and strategies to circumvent them. The document sections were divided up and drafted by the writing committee members according to their expertise. The recommendations represent the consensus opinion of the entire writing committee, graded by class of recommendation and level of evidence. Several questions addressed in this document either do not lend themselves to clinical trials or are rare disease entities, and in these instances recommendations are based on consensus expert opinion. Furthermore, specific recommendations, even when supported by substantial data, do not replace the need for clinical judgment and patient-specific decision-making. The recommendations were opened for public comment to Pediatric and Congenital Electrophysiology Society (PACES) members and underwent external review by the scientific and clinical document committee of the Heart Rhythm Society (HRS), the science advisory and coordinating committee of the American Heart Association (AHA), the American College of Cardiology (ACC), and the Association for European Paediatric and Congenital Cardiology (AEPC). The document received endorsement by all the collaborators and the Asia Pacific Heart Rhythm Society (APHRS), the Indian Heart Rhythm Society (IHRS), and the Latin American Heart Rhythm Society (LAHRS). This document is expected to provide support for clinicians and patients to allow for appropriate CIED use, appropriate CIED management, and appropriate CIED follow-up in pediatric patients.  相似文献

Development of vegetation communities in areas of Antarctica without permanent ice cover emphasizes the need for effective remote sensing techniques for proper monitoring of local environmental changes. Detection and mapping of vegetation by image classification remains limited in the Antarctic environment due to the complexity of its surface cover, and the spatial heterogeneity and spectral homogeneity of cryptogamic vegetation. As ultra-high resolution aerial images allow a comprehensive analysis of vegetation, this study aims to identify different types of vegetation cover (i.e., algae, mosses, and lichens) in an ice-free area of  Hope Bay, on the northern tip of the Antarctic Peninsula. Using the geographic object-based image analysis (GEOBIA) approach, remote sensing data sets are tested in the random forest classifier in order to distinguish vegetation classes within vegetated areas. Because species of algae, mosses, and lichens may have similar spectral characteristics, subclasses are established. The results show that when only the mean values of green, red, and NIR bands are considered, the subclasses have low separability. Variations in accuracy and visual changes are identified according to the set of features used in the classification. Accuracy improves when multilayer information is used. A combination of spectral and morphometric products and by-products provides the best result for the detection and delineation of different types of vegetation, with an overall accuracy of 0.966 and a Kappa coefficient of 0.946. The method allowed for the identification of units primarily composed of algae, mosses, and lichens as well as differences in communities. This study demonstrates that ultra-high spatial resolution data can provide the necessary properties for the classification of vegetation in Maritime Antarctica, even in images obtained by sensors with low spectral resolution.  相似文献

In studies of primary production of the open ocean, the measurement of new production is often considered a measure of the degree of eutrophication. Because of new research this is questionable, which in our opinion, calls for a refinement to the original concept. We believe that the measure of variable fluorescence is pivotal to a new understanding. Our research enforces the growing conviction that the measurement of Fv/Fm can be interpreted as an analogue for nutrient stress. We measured variable fluorescence in axenic cultures at the CCMP at the Bigelow Laboratory. The Fv/Fm of cultures, upon transfer to new media reached a maximum followed by a decline after approximately 30 days. The rate of decline does not appear to be species specific. Most of the clones remained relatively high after 30 days. The high Fv/Fm values observed in nutrient-replete cultures are not characteristic of the oligotrophic surface waters off Florida and the Bahamas, but are approached in the eutrophic waters of the Gulf of Maine. In contrast, Fv/Fm measurements of attached macroalgae and coral zooxanthellae are characteristically much higher than microalgae of either oligotrophic or eutrophic regions. Accordingly, we advance the case for interpretation of Fv/Fm in terms of nutrient stress in ecological studies and advocate that the old concept of new production should be modified.  相似文献

Microorganisms are but a few micrometers in diameter and are not visible to the naked eye. Yet, the large numbers of microorganisms present in the oceans and the global impact of their activities make it possible to observe them from space. Here a few examples of how microorganisms can be studied from satellites are presented. The first case is the best known: the main pigment used in photosynthesis (chlorophyll a) can be determined from satellites. These kinds of studies have contributed a tremendous amount of understanding about the distribution and dynamics of primary production in the oceans. Two other examples will concern analysis of heterotrophic prokaryotic production and estimates of dimethyl sulfide (DMS) concentration and flux to the atmosphere. These three processes are of fundamental importance for the functioning of the biosphere. Marine microbes carry out about half of the total primary production in the planet. A substantial fraction of the respiration in the oceans is due to the activity of heterotrophic prokaryotes. Finally, the flux of DMS to the atmosphere is believed to constitute one of the mechanisms by which the biota can regulate climate. The global implications of microbial processes in the oceans can only be addressed with the help of satellites. Electronic Publication  相似文献
