Kneeling is required during daily living for many patients after total knee replacement (TKR), yet many patients have reported that they cannot kneel due to pain, or avoid kneeling due to discomfort, which critically impacts quality of life and perceived success of the TKR procedure. The objective of this study was to evaluate the effect of component design on patellofemoral (PF) mechanics during a kneeling activity. A computational model to predict natural and implanted PF kinematics and bone strains after kneeling was developed and kinematics were validated with experimental cadaveric studies. PF joint kinematics and patellar bone strains were compared for implants with dome, medialized dome, and anatomic components. Due to the less conforming nature of the designs, change in sagittal plane tilt as a result of kneeling at 90° knee flexion was approximately twice as large for the medialized-dome and dome implants as the natural case or anatomic implant, which may result in additional stretching of the quadriceps. All implanted cases resulted in substantial increases in bone strains compared with the natural knee, but increased strains in different regions. The anatomic patella demonstrated increased strains inferiorly, while the dome and medialized dome showed increases centrally. An understanding of the effect of implant design on patellar mechanics during kneeling may ultimately provide guidance to component designs that reduces the likelihood of knee pain and patellar fracture during kneeling. 相似文献

Patella vulgata is a boreal cold temperate species and is the dominant limpet in northern Europe. Few works have focussed on the population
genetics of this species. Therefore, the aim of this work was to assess the degree of genetic and morphological differentiation
of P. vulgata on a macroscale by using 20 allozyme loci and 6 morphological variables. Samples were taken from the following locations:
Dingle Peninsula (Southwest Ireland), Port Erin (Southwest Isle of Man), St. Bees Head (north Cumbria, England), St. Agnes
Head (north Cornwall, England), Cellar Beach (south Devon, England), Whitley Bay (north Newcastle-Upon-Tyne, England), Sines
(Portugal), and Pointe de Chanchardon, La Rochelle (Bay of Biscay, France). Morphological variables were analysed by the multivariate
Canonical discriminant analysis. Genetic variation was assessed by diversity measures such as polymorphism and heterozygosity;
genetic subdivision of P. vulgata population was determined by the estimator θ of F
ST, and the genetic similarity between populations was measured by Nei’s genetic identity. No significant morphological differentiation
was observed among samples. Moderate genetic population subdivision was observed (θ = 0.137±0.074) despite great geographic
distances. The minimum genetic identity observed was between Ireland and France (I = 0.942) and maximum was observed between Portugal and north-east England (I=0.998). Two main groups were shown by UPGMA cluster analysis (I = 0.965). One formed by Irish, Manx, north Cumbria, and curiously, south Devon samples, while the second includes Portuguese,
French, north-Newcastle-upon-thyne, and north Cornwall samples. No association (g = 0.956; p>0.050) was found between pair-wise genetic divergence and geographic distance separating subpopulations, mainly due to an

Influence of patella alta on knee extensor mechanics 总被引:2,自引:0,他引:2
The purpose of this study was to compare the knee extensor mechanics in persons with and without patella alta. Thirteen subjects with patella alta and 14 subjects with normal patellar position participated in the study. Sagittal and axial MR images of the knee were acquired at 0°, 20°, 40°, and 60° of knee flexion. Measurements of actual moment arm, patellar ligament/quadriceps tendon force ratio, quadriceps effective moment arm, and joint reaction force/quadriceps force ratio were obtained. There were no differences between groups in terms of actual moment arm. However, subjects with patella alta had significantly larger patellar ligament/quadriceps tendon force ratios (1.04±0.02 vs. 0.92±0.02) and quadriceps effective moment arms (4.40±0.09 vs. 4.00±0.09 cm) when averaged across the range of knee flexion angles tested. There was no difference in the joint reaction force/quadriceps force ratio between groups. The observed differences in knee extensor mechanics suggest that individuals with patella alta have a more efficient knee extensor mechanism and would be expected to generate similar joint reaction forces per unit quadriceps force compared to subjects with normal patellar position. Therefore, persons with patella alta may experience less patellofemoral joint reaction force to overcome the same knee flexion moment in the range of 0°–60° of knee flexion. 相似文献

Aim Recent colonization of northern Portuguese shores by Patella rustica Linnaeus, 1758, led to the bridging of a historical gap in the distribution known since the 1900s. Long‐term oceanographic data collected over the last half‐century were examined in order to detect possible mechanisms for the observed change in its distribution. Location This study was carried out along the entire Portuguese coastline, from 41°50′ to 37°06′ N. Time‐series of hydrographical variables (sea surface temperature and salinity) were derived for the Atlantic coast of the Iberian Peninsula. Methods Abundance and size‐frequency distributions of the newly observed limpet populations were compared with those from well‐established populations in southern Portugal. Anomalies were computed for sea surface temperature (1950–2000) and sea surface salinity (1958–2001) data, covering the whole Atlantic coast of the Iberian Peninsula. An upwelling index (1967–2005) was derived for a single location within the distributional gap of P. rustica. Split moving window analysis was performed to detect significant discontinuities in hydrographical data sets. Results Patella rustica has gradually been expanding in northern Iberia, and in the late 1990s the historical gap in distribution in northern Portugal was bridged. Size‐frequency distribution differed between historical and recent populations, the latter lacking small‐sized individuals. At the same time, several anomalous oceanographic events occurred off the Portuguese coast and were probably related to this expansion. Main conclusions Although sea surface temperature might be a major determinant of the reproductive success of P. rustica and hence its dispersal potential, it is more likely that a coincidence of several factors occurring in the late 1990s provided exceptional conditions that allowed the geographical expansion of this species. 相似文献

Quantification of the wear rate in vitro is now considered an essential step in the development of a new joint replacement prior to clinical trials. However, little research exists around in vitro simulation of wear in the patellofemoral joint (PFJ) despite over 200,000 being implanted annually within the European Union. A method to simulate wear in the laboratory using four input degrees of freedom within the PFJ of total knee replacement (TKR) has been developed. Wear simulation was validated through comparison of functional kinematics and patellar surface damage modes produced in vitro to clinical outcomes. The technique has been shown to replicate the prescribed in vivo kinematics in a reproducible and repeatable manner. The wear scar areas were similar to those found in vivo. However, geometrical measurements of wear were not reliable due to creep and geometry changes. As has been found previously with tibial inserts, geometrical determination of wear volume was not found to be an effective method of comparing wear from simulators and retrievals. Change in volume calculated gravimetrically was seen to be the most repeatable measure of patellar wear in vitro. 相似文献

Biodiversity is defined by the scales at which organisms coexist. Coexistence at larger spatial scales may be underpinned
by segregation at smaller spatial scales or temporal variability in behaviour and/or distribution. Limpets of the genus Patella are keystone grazers controlling the growth of macro-algae on many rocky shores. This study examined the distribution and
abundance of P. vulgata and P. ulyssiponensis in relation to habitat and to each other at a range of spatial scales. Limpets were sampled on four shores on each of the
east and west coasts of Ireland, in two different transects (10 m apart) at each of three shore heights in winter 2003 and
summer 2004. P. vulgata and P. ulyssiponensis coexist around the coast of Ireland, but on shores where they co-occur, the pattern of coexistence begins to break down at
the level of shore height: P. ulyssiponensis were more abundant on the low-shore than mid-shore, and P. vulgata were distributed evenly throughout the low and the mid-shore. At the level of habitat (pools vs. open rock), the two species
exhibited a high degree of segregation: P. vulgata of all sizes were more common on open rock and P. ulyssiponensis of all sizes were more common in pools. The presence or absence of P. ulyssiponensis had no effect on the distribution of juvenile P. vulgata with respect to pools. Unexpectedly, however, a greater proportion of adult P. vulgata were found in pools at sites where P. ulyssiponensis were present in the pools than at sites where they were absent. The results of this study emphasize the need for multiple-scale
analyses of the distribution and abundance of organisms to understand the organisation of natural assemblages.

As knee flexion increases, so do tendofemoral and patellofemoral compression forces. Three cases of long-standing uncorrected flexion contractures of the knees are presented that resulted in marked erosion of the femoral trochlea, patella baja, and remodeled patellar articular surfaces likely reflecting increased angles of flexion. It is probable that the deep femoral grooves are the result of adaptation under high-level persistent contact loads from the quadriceps tendon. These examples expand our knowledge of bone remodeling and offer information of potential use in studying ancient skeletal samples. 相似文献
