Stroke is the culmination of a heterogeneous group of cerebrovascular diseases that is manifested as ischemia or hemorrhage of one or more blood vessels of the brain. The occurrence of many acute cardiovascular events—such as myocardial infarction, sudden cardiac death, pulmonary embolism, critical limb ischemia, and aortic aneurysm rupture—exhibits prominent 24 h patterning, with a major morning peak and secondary early evening peak. The incidence of stroke exhibits the same 24 h pattern. Although ischemic and hemorrhagic strokes are different entities and are characterized by different pathophysiological mechanisms, they share an identical double-peak 24 h pattern. A constellation of endogenous circadian rhythms and exogenous cyclic factors are involved. The staging of the circadian rhythms in vascular tone, coagulative balance, and blood pressure plus temporal patterns in posture, physical activity, emotional stress, and medication effects play central and/or triggering roles. Features of the circadian rhythm of blood pressure, in terms of their chronic and acute effects on cerebral vessels, and of coagulation are especially important. Clinical medicine has been most concerned with the prevention of stroke in the morning, when population-based studies show it is of greatest risk during the 24 h; however, improved protection of at-risk patients against stroke in the early evening, the second most vulnerable time of cerebrovascular accidents, has received relatively little attention thus far.  相似文献

The olive tree (Olea europaea L.) is a very important evergreen fruit tree because of the high interest of olive oil and table olives in the human diet. Differentiation of olive floral buds during winter is strictly related to flowering during spring and finally to fruit production during autumn–winter. In order to determine which are the developmental events in differentiating olive floral buds we studied in four olive cultivars “Amfissis”, “Kalamon”, “Manzanillo” and “Chalkidikis” the anatomical progress of the ontogeny using median longitudinal sections of floral buds (cutting odd nodes), in weekly intervals from early February (initiation of differentiation) to mid-April (one week after floral bud burst) keeping the cultivars under the same environmental conditions. At the same time, we determined the changes in their water extractable proteins, as an index of the meristematic activity. At the beginning of differentiation, floral buds consisted of the apical and one pair of developed (odd) axillary meristems which were covered with the corresponding bracts. Floral buds showed a slow gradual enlargement in all cultivars. The sequence of the developmental events (initiation of intense mitotic activity in the apical and the first axillary meristems, initiation of mitotic activity in the second axillary meristem, development of the third pair of bracts (except from cv. Chalkidikis), appearance of mitotic activity in the third axillary meristem, swelling of apical and axillary meristems and appearance of primordial whorls) are presented comparatively. All cultivars showed similar ontogenetic trends but there were temporal differences in the initiation, the progress and the completion of the process. Cv. Amfissis initiated differentiation one week later compared to the other cultivars, also delaying the development of the second and the third axillary meristems. On the other hand, cvs. Kalamon and Chalkidikis delayed one week to complete the process when compared to cvs. Amfissis and Manzanillo.  相似文献

目的应用选择性冠状动脉前降支(LAD)球囊闭塞结合微血栓微球混悬液灌注方法造成心肌缺血坏死,探索建立稳定存活的小型猪急性心肌梗死(AMI)后心力衰竭(HF)动物模型。方法选择中国五指山小型猪18头,行冠脉造影后沿血管送球囊至LAD中段,依次扩张球囊阻断前向血流1、2、5 min,每次间隔60 s,然后扩张球囊堵闭血流120 min。再以4F导管超选LAD,行微血栓微球混悬液分次注入,间隔10 min重复注射,TIMI心肌灌注分级(TMPG)2级和左室舒张末压(LVEDP)15 mm Hg时停止注射,同时监测心电图及应用漂浮导管监测有创血流动力学参数。并行pigtail导管测量(LVEDP)的变化,待LVEDP稳定在15~18 mm Hg之间后结扎血管,并加压包扎。监测心肌坏死标志物(cTnI和CK-MB)变化。分别于制模前,制模后第1天、7天、14天行心脏超声检查,制模第14天复查有创血流动力学检查,并行心脏病理检查,认定和评价模型的成功率、稳定性和可重复性。结果制模14 d后共有15头小型猪成活,心电图、心肌坏死标记物、病理检查均符合AMI病理生理过程。其中14头小型猪达到动物模型标准【肺毛细血管楔压(PCWP)18 mmHg和心输出量(CO)下降30%以上】,模型成功率为77.78%。制模后第14天PCWP明显升高(P0.01),CO平均下降50.76%;左室射血分数(LVEF)明显降低(P0.01)。病理检查显示心肌梗死面积占左心室面积的25.4%~34.9%。结论球囊闭塞结合微血栓微球混悬液灌注构建小型猪急性心肌梗死后心力衰竭模型具有闭胸、高成功率、稳定和重复性好等优点,较药物、冠状动脉结扎和起搏诱导的心力衰竭模型更接近临床病理生理学特点。  相似文献

采用描述性流行病学方法对江苏省2008年疑似预防接种异常反应(Adverse event following immunization,AEFI)监测数据及网络直报系统运转情况进行分析和评价。共收集到AEFI报告个案6197例,其中疫苗引起的反应占98.97%,偶合症占0.95%,不明原因占0.02%,待定占0.06%。临床诊断分布是以发热、局部红肿和局部硬结等一般反应为多见,其次为异常反应中的过敏性皮疹,无菌性脓肿。全省AEFI网络直报系统运行正常,各项考核指标均完成得较好。采用统一的AEFI分类与临床诊断病例定义,通过网络直报系统的三级审核,报告的数据质量有了很大的提高。监测结果显示,全省免疫规划疫苗安全性与预防接种服务质量良好。  相似文献

Effect of Ischemic Neuronal Insults on Amyloid Precursor Protein Processing   总被引:3,自引:0,他引:3  
The nature of the association between ischemic stroke and Alzheimer’s disease (AD) at the cellular and molecular level is still unknown. We evaluated the effect of ischemic neuronal insults on the regulation of amyloid precursor protein (APP) processing. We used an in vitro model of cerebral ischemia (oxygen-glucose deprivation) to evaluate the effect of ischemic neuronal insults on the amyloidogenic and non-amyloidogenic pathways using human neuroblastoma cell line and primary cultured cells of transgenic mice which expressed human APP (Tg2576). Ischemic neuronal insults increased the production of Aβ in Tg2576 primary culture cells compared to controls. A disintegrin and metalloprotease 10 (ADAM 10) was markedly increased in early stage of ischemic insults, which was followed by decreased level of ADAM 10 expression in later stage. The protein and mRNA expression of β-site cleavage enzyme (BACE) and BACE activity was not significantly different between the group of ischemic insults and control. By contrast, the activity of γ-secretase was significantly increased after 4 h of ischemic insults, as compared to controls. The present study showed that the ischemic neuronal insults increased the production of Aβ by influencing APP metabolism, which may link the role of ischemic insults to the pathogenesis of AD.  相似文献

The activation of the [Ca2+]-dependent cysteine protease calpain plays an important role in ischemic injury. Here, the levels of two calpain-specific substrates, p35 protein and eukaryotic initiation factor 4G (eIF4G), as well as its physiological regulator calpastatin, were investigated in a rat model of transient global cerebral ischemia with or without ischemic tolerance (IT). Extracts of the cerebral cortex, whole hippocampus and hippocampal subregions after 30 min of ischemia and different reperfusion times (30 min and 4 h) were used. In rats without IT, the p35 levels slightly decreased after ischemia or reperfusion, whereas the levels of p25 (the truncated form of p35) were much higher than those in sham control rats after ischemia and remained elevated during reperfusion. The eIF4G levels deeply diminished after reperfusion and the decrease was significantly greater in CA1 and the rest of the hippocampus than in the cortex. By contrast, the calpastatin levels did not significantly decrease during ischemia or early reperfusion, but were upregulated after 4 h of reperfusion in the cortex. Although IT did not promote significant changes in p35 and p25 levels, it induced a slight increase in calpastatin and eIF4G levels in the hippocampal subregions after 4 h of reperfusion.  相似文献

Ischemic preconditioning (IP) conferred by brief ischemia-reperfusion induces resistance to cell injury due to the following lethal ischemia. This study aimed to elucidate whether 78-kDa glucose-regulated protein (GRP78), a main ER molecular chaperone, contributes to IP-mediated protection against ischemic myocardial injury. In a rat coronary artery occlusion model, the GRP78 protein level increased to 210% of the sham level by early IP with three cycles of 4-min ischemia and 4-min reperfusion. The IP reduced infarct size in subsequent lethal ischemia. In primary cardiomyocytes, the simulated IP procedure, incubation in oxygen-glucose deprivation (OGD) medium, also increased the GRP78 expression and suppressed the cell death caused by lethal ischemia. Transfection of grp78 antisense oligonucleotide attenuated the IP-mediated resistance to ischemia. This study showed for the first time that early IP up-regulates myocardial GRP78. It was suggested that GRP78 induced by early IP contributes to protect cardiomyocytes against ischemic injury.  相似文献
