Abstract

Increasing complications in incisional hernia surgery call for novel treatments. A gene expression analysis of injured tissues displays important parameters for tissue regeneration. Until today, no reliable method has been described for a quantitative gene expression analysis of hernia tissues. In this work, a protocol is described for the isolation of DNA‐free total RNA of incisional hernias for the first time. Moreover, real‐time RT PCR assays for collagen type I and III and TGF‐β1 are demonstrated for relative gene expression analyses. Both methods enable relative gene expression analyses of hernia tissues for the first time.  相似文献

目的:探讨影响腹腔镜腹股沟疝修补术患者术后出血及二次手术相关因素。方法:回顾性分析行腹腔镜腹股沟疝修补术的6632例患者的临床资料,将凝血病、抗凝治疗或抗血小板治疗的829例患者归入风险组(n=829),其他患者归为对照组(n=5803),收集并比较二组患者的手术方式、年龄、性别、美国麻醉师协会(ASA)分级、疝缺损面积(I-III级)、一期手术与二次手术等患者资料,进行一年随访;采用多变量分析影响患者继发性出血及并发症所致二次手术的相关因素。结果:风险组的术后出血发生率显著高于对照组(4.22%vs 1.26%,P0.001),所有患者术后出血发生率为1.63%。影响术后继发性出血的其它负面因素有:开放式腹股沟疝术式、年龄增加、较高ASA分级、二次手术、男性和较大的疝缺损。风险组与并发症相关的二次手术发生率显著高于对照组(2.65%vs 1.14%,P0.001),所有患者与并发症相关的二次手术发生率为1.32%。影响患者并发症所致二次手术负面因素有:双侧手术、较高ASA分级、凝血病与抗凝治疗和抗血小板治疗、高龄,保护因素包括:较小的疝缺损面积与腹腔镜手术式。结论:行腹腔镜腹股沟疝手术患者术后出血性及并发症相关的二次手术的风险小于行开放术式患者。  相似文献

目的对发生嵌闭性疝的猕猴,采用疝手术治疗法挽救其生命。方法各种猕猴疝经确诊,实施手术治疗。结果共实施各种猕猴疝手术7例,手术成功率100%,愈后恢复良好。结论根据疝的位置和临床症状,确诊疝的种类,采取相应的疝根治手术疗法,7例猕猴疝病例手术后经过治疗护理,没有手术并发症及手术死亡,手术治疗法是治疗猕猴疝最有效的方法,对相关的灵长类动物外科手术及保护珍贵猕猴动物资源,减少猕猴因各种疝引起的痛苦或死亡均有一定的参考价值。  相似文献

Inguinal herniation represents a common condition requiring surgical intervention. Despite being regarded as a connective tissue disorder of uncertain cause, research has focused predominantly on biochemical changes in the key tissue layer, the transversalis fascia (TF) with little direct analysis of functional tissue mechanics. Connective tissue tensile properties are dominated by collagen fibril density and architecture. This study has correlated mechanical properties of herniated TF (HTF) and non-herniated TF (NHTF) with fibrillar properties at the ultrastructural level by quasi-static tensile mechanical analysis and image analysis of collagen electron micrographs. No significant difference was found between any of the key mechanical properties (break stress, strain or modulus) for HTF and NHTF. In addition, no significant differences were found in average collagen fibril diameter, density or fibre bundle spacing. However, both groups displayed anisotropy with greater break stress (p=0.001) on average in the transverse anatomical plane compared to the longitudinal plane in a mean ratio of 2:1 (anisotropy ratio), though there was no evidence of a difference in this ratio for HTF and NHTF for both break stress and modulus. It was noted that this anisotropy ratio corresponds closely with the expected force distribution on a model cylindrical structure loaded axially. The absence of other functional differences does not support the idea of a failing (injured) tissue but is consistent with it being a tissue undergoing chronic growth/expansion under multi-vectored mechanical loading. These findings provide new clues to collagen tissue herniation for mathematical modelling and model tissue engineering.  相似文献

目的:采用生物材料补片进行修补,加强腹壁薄弱区域实现功能重建,分析该方法治疗腹股沟疝的临床疗效。方法:选择我院收治的无嵌顿性择期腹股沟疝手术患者30例,随机分为传统修补组与生物补片修补组。比较患者的围手术期指标(手术时间、出血量、住院时间、术后下地活动时间、镇痛药物应用例数)、临床疗效,通过随访比较术后1个月、3个月、6个月患者的VAS评分及主观满意度,记录切口感染及其他并发症的发生情况。结果:所有患者均顺利完成手术,并完成半年以上随访。研究组的手术时间、术中失血量、住院时间、术后下地活动时间、镇痛药物应用均明显短于或低于对照组(P0.05)。两组患者术后1个月、3个月、6个月的VAS评分、主观满意度均较术前显著降低(P0.05),但两组之间相比差异无统计学意义(P0.05)。结论:采用生物补片材料修补腹股沟疝的手术创伤较小且患者恢复期短,近期疗效及安全性与传统手术相当。  相似文献
