须癣毛癣菌是皮肤癣菌病中常见的致病真菌之一,其分型复杂,具有多个变种和有性型,引起脓癣的常为亲动物性分离菌。从形态学角度难以明确区分不同种型,需要从分子生物学角度加以鉴定。rDNA序列测定是目前最常用于真菌鉴定的一种方法。我们从一脓癣患儿头部分离出一株须癣毛癣菌,对其形态学、rDNA序列和体外对抗真菌药物的敏感性进行了研究。  相似文献

目的 报道2例误诊为头皮脓肿经长期抗生素及植皮治疗失败的须癣毛癣菌所致的脓癣病患者,分析脓肿和脓癣的鉴别要点。方法 例1为9岁男童,头皮外伤后脓肿、溃疡28d,经抗生素治疗无效,行植皮术后5d再发生脓肿溃疡。取皮损处断发行10%KOH涂片镜检、培养,发现并分离出致病真菌,沙堡弱琼脂培养基上呈白色粉状菌落,可使含尿素培养基变红,即尿素酶试验阳性,小培养见螺旋菌丝及分隔棒状大分生孢子,鉴定为须癣毛癣菌。例2为8岁女童,头顶脓肿、溃疡24d,抗感染治疗不愈而接受植皮,术后7d再发脓性丘疹。从皮损处标本中发现、分离出致病真菌,经上述方法鉴定为须癣毛癣菌。结果 2例患者结合真菌学检查和临床表现确诊为脓癣,予伊曲康唑100mg/d内服近2个月皮损均痊愈,但供皮区遗留瘢痕和色素改变。结论 真菌病原学检查是避免脓癣误诊的关键,伊曲康唑内服治疗脓癣有效、安全。  相似文献

报道由须癣毛癣菌引起的儿童头部脓癣1例.患儿为4岁幼女,因头皮部丘疹1个月,脓肿4d就诊.内服青霉素V钾无效.取断发镜检查见发外真菌孢子,培养鉴定为须癣毛癣菌,细菌培养为棒状杆菌.经内服和外用特比萘芬抗真菌,静脉输入头孢噻肟钠联合克林霉素及万古霉素抗细菌治疗,12d后脓肿减轻,细菌培养阴转,但真菌培养仍阳性,继续抗真菌治疗2个月后皮损消退,真菌检查阴性.  相似文献

目的对临床上分离自人(36株)及狐狸(5株)的须癣毛癣菌菌株进行新的分类系统鉴定,并检测传统的分类方法是否能满足临床鉴定需要。方法①观察原鉴定为须癣毛癣菌菌株在沙氏培养基、1%蛋白胨培养基、溴甲酚紫乳固体葡萄糖琼脂培养基(BCP-MSG)和显微镜下形态学及尿素酶、毛发穿孔等生理学试验表现。②通过ITS区段和LSU区段分子生物学序列分析进行新的分类系统的菌种分型,并对传统形态学和生理学鉴定方法进行检测。结果①41株须癣毛癣菌形态学及生理学试验符合须癣毛癣菌(38株)和红色毛癣菌(3株)菌落的特点。②ITS区段序列分析发现ITS区段能将须癣毛癣菌和红色毛癣菌准确的鉴定到种,但无法明确其种内分型;而LSU区段序列分析可对36株(36/38)须癣毛癣菌有性型做出明确的鉴定。结论传统实验室鉴定方法仍具有其有效性及可靠性。通过分子生物学鉴定,临床分离的须癣毛癣菌皆为指(趾)间毛癣菌(38/38),而LSU区段的序列分析鉴定狐狸源性菌株皆属于本海姆节皮菌,有别于大多数人源性菌株有性型为万博节皮菌,对于须癣毛癣菌的菌种鉴定更优于ITS区段,但分子生物学试验还需结合形态学的观察,才能够对菌种做出正确的鉴定。  相似文献

目的了解近19年间上海地区脓癣的发病情况、病原菌及其变迁。方法选取头皮屑及头发真菌直接镜检阳性和(或)真菌培养为皮肤癣菌者,记录其临床表现、感染方式及动物接触史等。结果头癣患者1 009例,其中男性437例,女性572例;年龄20 d~93岁,平均10.44岁;脓癣114例(11.30%),其中男性44例,女性70例。年龄20 d~68岁,平均10.60岁,5~10岁最多见(45.61%),其次为5岁以下儿童(31.58%)。感染方式:发内型48例,发外型66例。真菌培养阳性的脓癣95例,其中犬小孢子菌29例(30.53%)、紫色毛癣菌21例(22.11%)、须癣毛癣菌19例(20.00%)、红色毛癣菌15例(15.79%)、断发毛癣菌7例(7.37%)、石膏样小孢子菌4例(4.21%)。脓癣的发病呈上升趋势,须癣毛癣菌脓癣尤为显著。结论上海地区脓癣主要发生于10岁以内儿童,犬小孢子菌是最常见的致病菌,发外型感染较发内型多见。  相似文献

患者男,57岁。面部红斑、丘疹、唇部脓疱灼痛2个月余。曾口服和外用多种中药治疗无效且呈加重趋势。取皮屑、胡须镜检见大量真菌菌丝,培养见棒状分隔大分生孢子、葡萄串状小分生孢子。尿素酶实验阳性、毛发穿孔实验阳性,鉴定为须癣毛癣菌。经内服伊曲康唑胶囊和外用盐酸布替萘芬乳膏治疗10 d后病情好转,2个月后皮损消退。复查真菌阴性。  相似文献

目的建立一种快速的红色毛癣菌分子生物学鉴定方法。方法根据红色毛癣菌保守区域-真菌核糖体DNA(rDNA)的转录间隔区(ITS)设计特异性引物,采用上游:ITS19865'GAC ACC AAG AAA AAA TTC TCT GAA GA3',下游:ITS24415'GTC CTG AGG GCG CTG AA3'为引物对45株红色毛癣菌、5株须癣毛癣菌和1株紫色毛癣菌菌株的DNA进行PCR扩增,观察产物电泳带型的差异。结果 45株红色毛癣菌均能扩增出目的片段,5株须癣毛癣菌和1株紫色毛癣菌均无目的片段扩增出。结论红色毛癣菌可用特异引物PCR方法快速鉴定。  相似文献

目的探讨面癣的致病菌种、临床特点及发病相关因素。方法对126例面癣患者进行真菌分离培养鉴定及流行病学分析。结果面癣见于各年龄段,以11~30岁最多见。患病动物接触史、合并其他部位浅部真菌病史及糖皮质类固醇激素类药物外用史是面癣发病的重要危险因素。分离出皮肤癣菌108株,包括红色毛癣菌63株(58.3%)、犬小孢子菌25株(23.1%)、须癣毛癣菌18株(16.7%)和石膏样小孢子菌2株(1.9%)。结论面癣的发病没有年龄差异,患者可见于各个年龄阶段;面癣的常见致病真菌绝大多数为红色毛癣菌和犬小孢子菌,其发病可能与患者自体接种或接触患病动物相关。面癣容易被误诊,及时进行真菌镜检和培养是降低面癣误诊率的关键。  相似文献

目的报道病程30余年、由紫色毛癣菌感染所致成人黑点癣1例。方法对头发及鳞屑标本进行多次真菌直接镜检和培养,对病原菌进行形态学鉴定和药物敏感性试验。结果鉴定为紫色毛癣菌。体外药敏试验该菌对特比萘芬、伊曲康唑敏感。结论本病例证实为紫色毛癣菌感染导致黑点癣,患者经口服伊曲康唑和局部治疗痊愈。  相似文献

From 180 gray squirrels (Sciurus c. carolinesis), 942 isolates of fungi representing 19 genera were recovered upon culture of hair-skin scrapings and toenails. Of the isolates, 170 represented known human pathogens and 142, squirrel pathogens. A human infection of Trichophyton mentagrophytes was derived from handling the squirrels. Skin lesions of seven squirrels were attributable to T. mentagrophytes and Mucor sp.  相似文献

Soil samples from twenty salt pans and their vicinity around Mumbai and Thane were screened for the occurrence of keratinophilic fungi and related dermatophytes. Ten species classified in six genera were recovered using horse hair as bait. The isolated species were reported in the following order of dominance: Chrysosporium indicum (12.0%), Microsporum gypseum complex (7.2%), C. tropicum (5.6%), C. state of Ctenomyces serratus (4.0%), Trichophyton terrestre (3.2%), Malbranchea aurantiaca (2.4%), C. fluviale (1.6%), Uncinocarpus reesii (1.6%), Malbranchea sp. Of 10 057 specimens of scrapings from skin, nails and scalp examined for dermatophytes, yeasts, pityriasis versicolor and systemic mycoses between 1963 and 1973, 30.4 percent were positive for fungi. Skin produced the highest proportion (68.6 percent) of positive scrapings, scalp the lowest (4.2 percent). Trichophyton rubrum was the predominant species (23.6 percent); of lesser prevalence were Microsporum canis (9.3 percent), T. mentagrophytes (8.4 percent) and Epidermophyton floccosum (4.8 percent). Double infections were encountered on 102 occasions; T. rubrum and Candida parapsilosis were the most frequent cohabiting species. The introduction in 1966 of periodic acid-Schiff staining for routine examination of scrapings resulted in better diagnostic results, particularly in the case of culturally nonproductive specimens and cases of pityriasis versicolor. Blastomyces dermatitidis and Cryptococcus neoformans were isolated from two patients in the course of routine investigation for dermatophytes.  相似文献

目的 探讨皮肤浅部真菌病致病真菌菌种的构成.方法 对297例真菌涂片阳性和培养阳性的浅部真菌病患者,取标本进行分离培养及菌种鉴定,培养阳性标本在形态学上不能准确鉴定的,进行梅里埃API 20C AUX酵母菌鉴定试剂盒或核糖体DNA (rDNA) ITS区序列测定,确切鉴定菌种.使用SPSS 17.0统计软件对于结果进行统计分析.结果 共分离培养出致病菌13种,其中红色毛癣菌86株(29.0%),须癣毛癣菌68株(22.9%),念珠菌属59株(19.9%),暗色真菌属13株(4.4%),曲霉菌属13株(4.4%),红酵母菌12株(4.0%),青霉菌属9株(3.0%),毛霉菌9株(3.0%),犬小孢子菌5株(1.7%),浅白隐球菌3株(1.0%),毛孢子菌属2株(0.7%),絮状表皮癣菌1株(0.3%),混合感染17株(5.7%).结论 本地区浅部真菌病以甲癣为主,主要致病真菌是红色毛癣菌,但其他种类真菌感染尤其是念珠菌属有明显上升趋势.  相似文献

A survey on keratinophilic fungi from poultry-farm soils at Namakkal and from feather dumping soils at Chennai, India, revealed the existence of 34 species of fungi. Most of the fungi exhibited variable efficiency in producing extracellular keratinase when grown in plates with chicken feathers as the sole carbon and nitrogen source. The fungi Aspergillus flavus, Aspergillus niger, Aspergillus versicolor, Chrysosporium state of Arthroderma tuberculatum, Paecilomyces carneus, Scopulariopsis brevicaulis, Trichoderma viride, and Trichophyton mentagrophytes were efficient candidates to degrade the feathers. However, when cultivating the strains in submerged conditions in a medium containing chicken feathers as the sole nutrients source, Aspergillus glaucus, Chrysosporium keratinophilum, Curvularia lunata, Fusarium solani, and Penicillium citrinum also proved to be potent. Among all species, S. brevicaulis and Trichophyton mentagrophytes produced higher amounts of keratinase in both methods. Conditions for keratinase production were optimized by statistical design and surface plots. The highest keratinase activity was estimated by S. brevicaulis (3.2 KU/mL) and Trichophyton mentagrophytes (2.7 KU/mL) in the culture medium with chicken feathers and shows (79% and 72.2% of degrading ability, respectively).  相似文献

目的 观察姜堰地区儿童头癣流行病学、传播方式、病原菌及治疗结果.方法 对我科2006年8月~ 2013年5月诊治的33例儿童头癣病例进行总结分析.结果 33例儿童头癣中白癣24例、黑癣5例、脓癣4例;男19例、女14例;犬小孢子菌22例、紫色毛癣菌2例、须癣毛癣菌3例、断发毛癣菌3例、红色毛癣菌1例、石膏样小孢子菌2例.患者中有动物接触史的占42.42% (14/33).经口服特比萘芬或伊曲康唑6~8周,每日用硫磺皂洗头,局部外用萘替芬酮康唑乳膏均取得满意疗效.8周时治愈率分别为87.50%和88.24%,真菌清除率分别为100%和94.12%.结论 近年来本病增多的原因与喂养猫、犬等宠物有关,主要病原菌是亲动物的犬小孢子菌,经综合治疗,疗效满意.  相似文献
