Knee ligamentous injuries persist in the sport of Alpine skiing. To better understand the load mechanisms which lead to injury, pure varus/valgus and pure axial moments were applied both singly and in combination to the right knees of six human test subjects. The corresponding relative knee rotations in three degrees of freedom were measured. Knee flexion angles for each test subject were 15 and 60 degrees for the individual moments and 60 degrees for the combination moments. For both knee flexion angles the hip flexion angle was 0 degrees. Leg muscles were quiescent and axial force was minimal during all tests. Tables of data include sample statistics for each of four flexibility parameters in each loading direction. Data were analyzed statistically to test for significant differences in flexibility parameters between the test conditions. In flexing the knee from 15 to 60 degrees, the resulting knee rotations under single moments depended upon flexion angle with varus, valgus, and internal rotations increasing significantly. Also, rotations were different depending on load direction; varus rotation was significantly different and greater than valgus rotation at both flexion angles. Also external rotation was significantly different and greater than internal at 15 degrees flexion, but not at 60 degrees flexion. Coupled rotations under single moments were also observed. Applying pure varus/valgus moments resulted in coupled external/internal rotations which were inconsistent and hence not significant. Applying pure axial moments resulted in consistent and hence significant varus/valgus rotations; an external axial moment induced varus rotation and an internal axial moment induced valgus rotation. For combination moments, varus/valgus rotations decreased significantly from those rotations at similar load levels in the single moment studies. Also, a varus moment significantly increased external rotation and a valgus moment significantly decreased internal rotation. These differences indicate significant interaction between corresponding load combinations. These results suggest that load interaction is a potentially important phenomenon in knee injury mechanics.  相似文献

The complete force and moment of each cross section of the leg between the ski boot top and the knee during normal skiing were predicted from measurements of the force and moment under the toe and heel of the boot and the flexion of the ankle. The force and moment components predicted at the base of the boot were significantly different from those predicted at sites of potential injury at the boot top and the knee. The maximum torsional and maximum varus-valgus moments predicted at the knee over all subjects tested were 70 Nm and 149 Nm, which are within the estimated range of the ultimate strength of the knee without support from contracted muscles crossing the knee. Regression analyses were used to find the force components at the base of the boot that best predict the bending and torsional moments at the boot top and knee. The torsional moments at the boot top and knee are best predicted by the medial-lateral force at the toe. The varus-valgus moment at the boot top and knee are best predicted by the resultant medial-lateral force component at the base of the boot. The set of best predictors of the anterior-posterior bending moments at the boot top and knee includes the vertical force at the toe, the vertical force at the heel and the component of the total vertical force directed perpendicular to the leg.  相似文献

Ski boots are designed to transfer high forces from the skier to the ski. For this purpose they are made of stiff materials and constrain the leg of the skier to an unnatural position. To overcome the problem of unnatural knee posture, the ski boots can be adjusted in the frontal plane as well as in the horizontal plane by the canting mechanism and the "v-position", respectively. Canting enables lateral and medial orientation of the shaft with respect to the base of the boot. The "v-position" is a pronounced outward rotation of the boot's base with respect to the ski's long axis. The purpose of this study is to investigate the effect of different foot rotations and ski boot canting settings on knee kinematics during standing and simulated skiing. Knee kinematics was measured by means of motion analysis and with the help of skin-mounted markers on 20 subjects. The ski boots in their standard settings significantly constrained the skier to an unnatural valgus position. Ski boot base rotation had a significant effect on internal external knee rotation, whereas canting had an effect on varus-valgus angles during standing. However, for the simulated skiing position no effects were observed. The study suggests that the constraints of the ski boots result in a clinically relevant valgus misalignment. Canting settings reduced the misalignment but only by about 10%. Increased ski boot canting settings would therefore be desirable. Knee kinematics showed that rotational misalignment could not be linked to any significant increase in injury risk.  相似文献

The in vivo torsional laxity and stiffness of the knee joint are usually determined by rotating the foot and measuring the torque generated at the knee. However, when rotation is applied to the foot, significant three-dimensional forces and moments are produced at the knee. These forces and moments depend upon the external constraint of the ankle complex, and as a result, the observed laxity of the knee also depends on the ankle constraint. Tests are conducted with the foot of a subject in a shoe, with and without the ankle taped, and in a buckled and unbuckled (ski) boot that can effectively constrain ankle rotation. The average laxity of the primary (linear) region of the axial moment vs internal-external rotation is 30% greater when the ankle is constrained by the buckled boot than it is in three other cases of lesser ankle constraint.  相似文献

Joint moments are of interest because they bear some relation to muscular effort and hence rider performance. The general objective of this study is to explore the relation between joint moments and pedalling rate (i.e. cadence). Joint moments are computed by modelling the leg-bicycle system as a five-bar linkage constrained to plane motion. Using dynamometer pedal force data and potentiometer crank and pedal position data, system equations are solved on a computer to produce moments at the ankle, knee and hip joints. Cadence and pedal forces are varied inversely to maintain constant power. Results indicate that average joint moments vary considerably with changes in cadence. Both hip and knee joints show an average moment which is minimum near 105 rotations min-1 for cruising cycling. It appears that an optimum rotations min-1 can be determined from a mechanical approach for any given power level and bicycle-rider geometry.  相似文献

An acoustic transducer design to measure the relative translations and rotations across the knee with no mechanical coupling between the tibia and femur is presented. Platforms attached to femoral and tibial tracking fixtures hold acoustic sources and receivers, respectively. The distance from each source to each receiver is measured by the acoustic transit time and the translations and rotations across the knee joint are computed. For rotations less than 30 deg around the expected operating position, the resolution of the transducer is 0.3 deg; for translations less than 1.5 cm around the expected operating position, the resolution is 0.03 cm. Theoretical error analysis using a Monte Carlo method shows that the uncertainty in the measurement depends on the relative position of the sources and receivers. The analysis predicts the worst case resolution of the transducer as 0.09 cm in translation and 0.6 deg in rotation when the receiver platform is translated 8.0 cm parallel to the source platform. The transducer and fixturing system are demonstrated on a cadaver specimen for applied anterior force and applied internal-external rotation. Errors due to (soft tissue) motion of the transducer relative to the bone during in vivo measurements are assessed on the cadaver specimen. For internal-external rotation the error due to soft tissue motion is a maximum of 0.5 cm in translation and 1.8 deg in rotation. For applied anterior force the error due to soft tissue motion is a maximum of 0.16 cm in translation and 2.7 deg in rotation.  相似文献

This paper derives a simple mathematical model relating changes in the orientations of the two Cartesian coordinate systems involved in recording knee movement and the varus-valgus and the internal-external rotation angles for describing the knee's motion. Rotation matrices are given for changing the orientations of the two Cartesian coordinate systems in such a way that the quadratic variations in the varus-valgus and in the external-internal angles are minimal. These estimated rotation matrices are used to correct for axis misalignment. The correction is calibrated by considering the impact of the new orientation of the thigh Cartesian coordinate system on the hip joint angles. The procedure is applied to kinematic data collected on normal subjects. The uncertainty about the specification of the thigh Cartesian coordinate system is shown to explain some of the between subject variability in the varus-valgus and in the internal-external rotation angles curves.  相似文献

When comparing previous studies that have measured the three-dimensional moments acting about the lower limb joints (either external moments or opposing internal joint moments) during able-bodied adult gait, significant variation is apparent in the profiles of the reported transverse plane moments. This variation cannot be explained on the basis of adopted convention (i.e. external versus internal joint moment) or inherent variability in gait strategies. The aim of the current study was to determine whether in fact the frame in which moments are expressed has a dominant effect upon transverse plane moments and thus provides a valid explanation for the observed inconsistency in the literature. Kinematic and ground reaction force data were acquired from nine able-bodied adult subjects walking at a self-selected speed. Three-dimensional hip, knee and ankle joint moments during gait were calculated using a standard inverse dynamics approach. In addition to calculating internal joint moments, the components of the external moment occurring in the transverse plane at each of the lower limb joints were calculated to determine their independent effects. All moments were expressed in both the laboratory frame (LF) as well as the anatomical frame (AF) of the distal segment. With the exception of the ankle rotation moment in the foot AF, lower limb transverse plane joint moments during gait were found to display characteristic profiles that were consistent across subjects. Furthermore, lower limb transverse plane joint moments during gait differed when expressed in the distal segment AF compared to the LF. At the hip, the two alternative reference frames produced near reciprocal joint moment profiles. The components of the external moment revealed that the external ground reaction force moment was primarily responsible for this result. Lower limb transverse plane joint moments during gait were therefore found to be highly sensitive to a change in reference frame. These findings indicate that the different transverse plane joint moment profiles during able-bodied adult gait reported in the literature are likely to be explained on this basis.  相似文献

The relationships between extrinsic forces acting at the knee and knee kinematics were examined with the purpose of identifying specific phases of the walking cycle that could cause abnormal kinematics in the anterior cruciate ligament (ACL) deficient knee. Intersegmental forces and moments in directions that would produce anterior-posterior (AP) translation, internal-external (IE) rotation and flexion-extension (FE) at the knee were compared with the respective translation and rotations of the tibia relative to the femur during four selected phases (heel strike, weight acceptance, terminal extension and swing) of the walking cycle. The kinematic changes associated with loss of the ACL occurred primarily during the terminal portion of swing phase of the walking cycle where, for the ACL deficient knee, the tibia had reduced external rotation and anterior translation as the knee extended prior to heel strike. The kinematic changes during swing phase were associated with a rotational offset relative to the contralateral knee in the average position of the tibia towards internal rotation. The offset was maintained through the entire gait cycle. The abnormal offsets in the rotational position were correlated with the magnitude of the flexion moment (balanced by a net quadriceps moment) during weight acceptance. These results suggest that adaptations to the patterns of muscle firing during walking can compensate for kinematic changes associated with the loss of the ACL. The altered rotational position would cause changes in tibiofemoral contact during walking that could cause the type of degenerative changes reported in the meniscus and the articular cartilage following ACL injury.  相似文献

The hypothesis which motivated the work reported in this article was that neglecting pure moments developed between the foot and pedal during cycling leads to a substantial error in computing axial and varus/valgus moments at the knee. To test this hypothesis, a mathematical procedure was developed for computing the three-dimensional knee loads using three-dimensional pedal forces and moments. In addition to data from a six-load-component pedal dynamometer, the model used pedal position and orientation and knee position in the frontal plane to determine the knee joint loads. Experimental data were collected from the right leg of 11 male subjects during steady-state cycling at 90 rpm and 225 W. The mean peak varus knee moment calculated was 15.3 N m and the mean peak valgus knee moment was 11.2 N m. Neglecting the pedal moment about the anterior/posterior axis resulted in an average absolute error of 2.6 N m and a maximum absolute error of 4.0 N m in the varus/valgus knee moment. The mean peak internal and external axial knee moments were 2.8 N m and 2.3 N m, respectively. The average and maximum absolute errors in the axial knee moment for not including the moment about an axis normal to the pedal were found to be 2.6 N m and 5.0 N m, respectively. The results strongly support the use of three-dimensional pedal loads in the computation of knee joint moments out of the sagittal plane.  相似文献

Moments measured by a dynamometer in biomechanics testing often include the gravitational moment and the passive elastic moment in addition to the moment caused by muscle contraction. Gravitational moments result from the weight of body segments and dynamometer attachment, whereas passive elastic moments are caused by the passive elastic deformation of tissues crossing the joint being assessed. Gravitational moments are a major potential source of error in dynamometer measurements and must be corrected for, a procedure often called gravity correction. While several approaches to gravity correction have been presented in the literature, they generally assume that the gravitational moment can be adequately modeled as a simple sine or cosine function. With this approach, a single passive data point may be used to specify the model, assuming that passive elastic moments are negligible at that point. A new method is presented here for the gravity correction of dynamometer data. Gravitational moment is represented using a generalized sinusoid, which is fit to passive data obtained over the entire joint range of motion. The model also explicitly accounts for the presence of passive elastic moments. The model was tested for cases of hip flexion-extension, knee flexion-extension, and ankle plantar flexion-dorsiflexion, and provided good fits in all cases.  相似文献

The purpose of this study was to determine the effect of dynamometer and joint axis misalignment on measured isometric knee-extension moments using inverse dynamics based on the actual joint kinematic information derived from the real-time X-ray video and to compare the errors when the moments were calculated using measurements from external anatomical surface markers or obtained from the isokinetic dynamometer. Six healthy males participated in this study. They performed isometric contractions at 90° and 20° of knee flexion, gradually increasing to maximum effort. For the calculation of the actual knee-joint moment and the joint moment relative to the knee-joint center, determined using the external marker, two free body diagrams were used of the Cybex arm and the lower leg segment system. In the first free body diagram, the mean center of the circular profiles of the femoral epicondyles was used as the knee-joint center, whereas in the second diagram, the joint center was assumed to coincide with the external marker. Then, the calculated knee-joint moments were compared with those measured by the dynamometer. The results indicate that 1) the actual knee-joint moment was different from the dynamometer recorded moment (difference ranged between 1.9% and 4.3%) and the moment calculated using the skin marker (difference ranged between 2.5% and 3%), and 2) during isometric knee extension, the internal knee angle changed significantly from rest to the maximum contraction state by about 19°. Therefore, these differences cannot be neglected if the moment-knee-joint angle relationship or the muscle mechanical properties, such as length-tension relationship, need to be determined.  相似文献

Knowledge of the coupled motions, which develop under compressive loading of the knee, is useful to determine which degrees of freedom should be included in the study of tibiofemoral contact and also to understand the role of the anterior cruciate ligament (ACL) in coupled motions. The objectives of this study were to measure the coupled motions of the intact knee and ACL-deficient knee under compression and to compare the coupled motions of the ACL-deficient knee with those of the intact knee. Ten intact cadaveric knees were tested by applying a 1600 N compressive load and measuring coupled internal-external and varus-valgus rotations and anterior-posterior and medial-lateral translations at 0 deg, 15 deg, and 30 deg of flexion. Compressive loads were applied along the functional axis of axial rotation, which coincides approximately with the mechanical axis of the tibia. The ACL was excised and the knees were tested again. In the intact knee, the peak coupled motions were 3.8 deg internal rotation at 0 deg flexion changing to -4.9 deg external rotation at 30 deg of flexion, 1.4 deg of varus rotation at 0 deg flexion changing to -1.9 deg valgus rotation at 30 deg of flexion, 1.4 mm of medial translation at 0 deg flexion increasing to 2.3 mm at 30 deg of flexion, and 5.3 mm of anterior translation at 0 deg flexion increasing to 10.2 mm at 30 deg of flexion. All changes in the peak coupled motions from 0 deg to 30 deg flexion were statistically significant (p<0.05). In ACL-deficient knees, there was a strong trend (marginally not significant, p=0.07) toward greater anterior translation (12.7 mm) than that in intact knees (8.0 mm), whereas coupled motions in the other degrees of freedom were comparable. Because the coupled motions in all four degrees of freedom in the intact knee and ACL-deficient knee are sufficiently large to substantially affect the tibiofemoral contact area, all degrees of freedom should be included when either developing mathematical models or designing mechanical testing equipment for study of tibiofemoral contact. The increase in coupled anterior translation in ACL-deficient knees indicates the important role played by the ACL in constraining anterior translation during compressive loading.  相似文献

This paper presents the design and field evaluation of the first microcomputer controlled ski binding system. This system incorporates an Intel 8086 microcomputer controller and an integral binding/dynamometer. This instrumentation system not only undertakes real time control, but also it records dynamometer data via a miniature digital cassette tape recorder. The integral binding/dynamometer offers the same operational and mounting convenience of commercially available mechanical bindings. The binding may be released either manually or electrically via the controller. Comprised of four octagonal half strain rings, the strain gage dynamometer measures the three moment load components at the boot. To enable the user to conveniently operate the computer, extensive operating software was developed. The operating software is discussed in relation to both the acquisition and storage of data from the dynamometer and the control of the electro-mechanical snow ski binding. The binding system has been used successfully to both record boot moment components and control ski binding release during actual skiing maneuvers. Moment histories typical of three common recreational skiing maneuvers are presented.  相似文献

The study focusses on the prevention of knee injuries during snow skiing. In order to develop a technology of knee injury prevention, both the strength and loading on the knee during skiing activity must be known. This paper reports measurements of variables influencing both knee strength and loading of the joint. The strength variables measured included the degree of activity in six muscles crossing the knee, the knee flexion angle, and the axial load (i.e. weight bearing) transmitted to the knee. Transducers included surface electrodes to monitor electromyogram signals indicating the degree of muscle activity and a goniometer to measure both hip and knee flexion angles. The complete loading on the knee was derived from a dynamometer which measured the six load components at the boot-dynamometer interface. The transducer data were acquired and stored by a compact, battery powered digital data acquisition-controller system. Three male subjects of similar physical size (nominal was 1.8 m and 75 kg) and skiing ability (advanced intermediate to expert) were tested under similar conditions. Each subject skied a total of four slalom runs--one snowplow and three parallel. The total time of each test was 21 s. Example data plots from different types of runs are presented and discussed. Based on observations from the data, necessary performance features for ski bindings offering improved protection from knee ligamentous injuries are defined.  相似文献

This paper examined if an electromyography (EMG) driven musculoskeletal model of the human knee could be used to predict knee moments, calculated using inverse dynamics, across a varied range of dynamic contractile conditions. Muscle-tendon lengths and moment arms of 13 muscles crossing the knee joint were determined from joint kinematics using a three-dimensional anatomical model of the lower limb. Muscle activation was determined using a second-order discrete non-linear model using rectified and low-pass filtered EMG as input. A modified Hill-type muscle model was used to calculate individual muscle forces using activation and muscle tendon lengths as inputs. The model was calibrated to six individuals by altering a set of physiologically based parameters using mathematical optimisation to match the net flexion/extension (FE) muscle moment with those measured by inverse dynamics. The model was calibrated for each subject using 5 different tasks, including passive and active FE in an isokinetic dynamometer, running, and cutting manoeuvres recorded using three-dimensional motion analysis. Once calibrated, the model was used to predict the FE moments, estimated via inverse dynamics, from over 200 isokinetic dynamometer, running and sidestepping tasks. The inverse dynamics joint moments were predicted with an average R(2) of 0.91 and mean residual error of approximately 12 Nm. A re-calibration of only the EMG-to-activation parameters revealed FE moments prediction across weeks of similar accuracy. Changing the muscle model to one that is more physiologically correct produced better predictions. The modelling method presented represents a good way to estimate in vivo muscle forces during movement tasks.  相似文献

The contributions of this paper are twofold. One is the design and performance evaluation of new equipment to determine the rotational flexibility of the human knee in vivo. Since determining knee flexibility requires the application of external loads and the measurement of knee rotations, the new equipment consists of a load application stand and a triaxial goniometer. The triaxial goniometer noninvasively mounts to the leg and directly measures the relative three degrees-of-freedom rotations of the knee sequentially and independently. The goniometer incorporates several unique design features which enhance measurement accuracy. The load stand applies pure varus/valgus and external/internal axial moments either individually or in combination through the use of motors controlled by the test subject. Unique to this design are features which enable the application of moments to the knee which minimise shear forces. Other unique design features permit the stand to control hip and knee flexion angles, muscle contraction, and axial loading. To assess the accuracy with which rotations are measured during experiments, three tests were conducted with the equipment. One test evaluated the inherent accuracy of the goniometer, a second test assessed the potential for goniometer slippage during loading, and a third explored the effect of goniometer mounting on the repeatability of results. A special verification apparatus facilitated evaluation of goniometer inherent accuracy. A second contribution of the paper is an investigation of the effect of foot constraints (i.e. boundary conditions) on flexibility results. To make this investigation, three subjects were tested with the knee at 15 degrees of flexion. Results revealed large differences in flexibility between constraining the foot in both external/internal and varus/valgus rotations and permitting the foot to rotate freely in the direction not being loaded. Further, constraint moments as high as 23 Nm were also recorded. These results emphasise that in order to obtain accurate flexibility results for isolated loads, the foot must be unconstrained by the loading apparatus.  相似文献

Assessing the importance of non-driving intersegmental knee moments (i.e. varus/valgus and internal/external axial moments) on over-use knee injuries in cycling requires the use of a three-dimensional (3-D) model to compute these loads. The objectives of this study were: (1) to develop a complete, 3-D model of the lower limb to calculate the 3-D knee loads during pedaling for a sample of the competitive cycling population, and (2) to examine the effects of simplifying assumptions on the calculations of the non-driving knee moments. The non-driving knee moments were computed using a complete 3-D model that allowed three rotational degrees of freedom at the knee joint, included the 3-D inertial loads of the shank/foot, and computed knee loads in a shank-fixed coordinate system. All input data, which included the 3-D segment kinematics and the six pedal load components, were collected from the right limb of 15 competitive cyclists while pedaling at 225 W and 90 rpm. On average, the peak varus and internal axial moments of 7.8 and 1.5 N m respectively occurred during the power stroke whereas the peak valgus and external axial moments of 8.1 and 2.5 N m respectively occurred during the recovery stroke. However, the non-driving knee moments were highly variable between subjects; the coefficients of variability in the peak values ranged from 38.7% to 72.6%. When it was assumed that the inertial loads of the shank/foot for motion out of the sagittal plane were zero, the root-mean-squared difference (RMSD) in the non-driving knee moments relative to those for the complete model was 12% of the peak varus/valgus moment and 25% of the peak axial moment. When it was also assumed that the knee joint was revolute with the flexion/extension axis perpendicular to the sagittal plane, the RMSD increased to 24% of the peak varus/valgus moment and 204% of the peak axial moment. Thus, the 3-D orientation of the shank segment has a major affect on the computation of the non-driving knee moments, while the inertial contributions to these loads for motions out of the sagittal plane are less important.  相似文献

Nondriving intersegmental knee moment components (i.e., varus/valgus and internal/external axial moments) are thought to be primarily responsible for the etiology of overuse knee injuries such as patellofermoral pain syndrome in cycling because of their relationship to muscular imbalances. However the relationship between these moments and muscle activity has not been studied. Thus the four primary objectives of this study were to test whether manipulating the inversion/eversion foot angle alters the varus/valgus knee moment (Objective 1) and axial knee moment (Objective 2) and to determine whether activation patterns of the vastus medialis oblique (VMO), vastus lateralis (VL), and tensor fascia latae (TFL) were affected by changes in the varus/valgus (Objective 3) and axial knee moments (Objective 4). To fulfill these objectives, pedal loads and lower limb kinematic data were collected from 15 subjects who pedaled with five randomly assigned inversion/eversion angles: 10 deg and 5 deg everted and inverted and 0 deg (neutral). A previously described mathematical model was used to compute the nondriving intersegmental knee moments throughout the crank cycle. The excitations of the VMO, VL, and TFL muscles were measured with surface electromyography and the muscle activations were computed. On average, the 10-deg everted position decreased the peak varus moment by 55% and decreased the peak internal axial moment by 53% during the power stroke (crank cycle region where the knee moment is extensor). A correlation analysis revealed that the VMO/VL activation ratio increased significantly and the TFL activation decreased significantly as the varus moment decreased. For both the VMO/VL activation ratio and the TFL activation, a path analysis indicated that the varus/valgus moment was highly correlated to the axial moment but that the correlation between muscle activation and the varus moment was due primarily to the varus/valgus knee moment rather than the axial knee moment. The conclusion from these results is that everting the foot may be beneficial towards either preventing or ameliorating patellofemoral pain syndrome in cycling.  相似文献

Non-contact anterior cruciate ligament (ACL) injuries account for approximately 70% of ACL ruptures and often occur during a sudden change in direction or pivot. Decreased neuromuscular control of the trunk in a controlled perturbation task has previously been associated with ACL injury incidence, while knee abduction moments and tibial internal rotation moments have been associated with ACL strain and ACL injury incidence. In this study, the association between movement of the trunk during a run-to-cut maneuver and loading of the knee during the same activity was investigated. External knee moments and trunk angles were quantified during a run-to-cut maneuver for 29 individuals. The trunk angles examined were outside tilt (frontal plane angle of the torso from vertical), angle between the ground reaction force (GRF) and the torso in the plane containing the GRF and shoulders (torso-GRF_shoulders); and angle between GRF and torso in the plane containing the GRF and pelvis (torso-GRF_pelvis). Significant positive associations were found between torso angles and peak knee abduction moments (outside tilt, p=0.002; and torso-GRF_shoulders, p=0.036) while a significant negative association was found between peak tibial internal rotation moment and outside tilt (p=0.021). Because the peaks of these moments occur at different times and minimal axial rotation moment is observed at peak knee abduction moment (-0.29±0.46%BW*ht), the positive association between peak knee abduction moment and torso lean suggests that increasing torso lean may increase ACL load and risk of injury.  相似文献
