An accurate knowledge of the relationship between the neck and the epiphyseal plate at the end of growth is important for biomechanical investigations of femoral neck remodelling during childhood. Statistical data about the position of the epiphyseal femoral cartilage in relation to the neck axis at the end of the growth, are rare in the literature. As the trace of the epiphysis can be observed on a CT scan view of an adult hip, cadaver femurs were investigated to study this relationship and to avoid irradiation of children. The mean anteversion angle of the epiphyseal line towards the patient's coronal plane is 2.3 degrees. The plate is retroverted in an average of 8.2 degrees in relation to the neck axis and is related to neck anteversion. The more the neck is anteverted, the more the plate is retroverted.  相似文献

Quantifying soft-tissue tension around the hip joint during total hip arthroplasty remains difficult. In this study, a three-dimensional computer-aided design model was developed to clarify how component position in total hip arthroplasty contributes to the primary cause of posterior dislocation in cases of flexion, adduction and internal rotation. To better understand the influences of anteversion angle of the acetabular component, its effects on the primary causes of dislocations and the range of motion were investigated. Three different primary dislocation mechanisms were noted: impingement of the prosthetic femoral neck on the cup liner; impingement of the osseous femur on the osseous pelvis; and spontaneous dislocation caused by soft-tissue traction without impingement. Spontaneous dislocation could be detected by calculating hip forces at any thigh position using the computer-aided design model developed. In computer analysis, a transition from prosthetic impingement rate to osseous impingement rate occurred with increasing anteversion angle of the acetabular component. Spontaneous dislocation was detected at angles > 10° of anteversion of the acetabular component when flexion occurred with extreme adduction and internal rotation. This study demonstrated the possibility of spontaneous dislocation that results not from prosthetic or bony impingement but from muscle traction with increased range of motion.  相似文献

The reduced range of motion (ROM) resulting from total hip replacement (THR) leads to frequent prosthetic impingement, which may restrict activities of daily living and cause subluxation and dislocation. Therefore, to know the ROM of THR is very important in clinical situations and in the design of prostheses. THR involves a pure ball and socket joint. We created a mathematical formula to calculate the theoretical ROM of THR limited by the prosthetic impingement. The ROM of THR is governed by the following five factors, (1) The prosthetic ROM (oscillation angle: obtained from company data), (2) cup abduction (3) cup anterior opening, (4) the angle of the femoral neck component from the horizontal plane, and (5) the femoral neck anteversion. The last 4 factors are able to be obtained from anterior-posterior, axial X-rays and CT of the patient's THR. The objective was to create mathematical formulas that could accurately and quickly calculate the ROM of THR. By entering the five values into a computer programmed with the formulas, one could obtain the ROM for the THR. This reveals the effect on ROM of the oscillation angle and the interaction of ROM with cup abduction, anterior opening and neck anteversion. Furthermore this readily would enable a clinical evaluation of the possibility of postoperative dislocation and help in postoperative rehabilitation. The calculated numerical values of ROM by these mathematical formulas were successfully compared with the ROMs obtained from 3-dimensional computer graphics (3D-CG).  相似文献

Results of recent imaging studies and theoretical models suggest that the superior femoral neck is a location of local weakness due to an age-related thinning of the cortex, and thus the site of hip fracture initiation. The purpose of this study was to experimentally determine the spatial and temporal characteristics of the macroscopic failure process during a simulated hip fracture that would occur as a result of a sideways fall. Twelve fresh frozen human cadaveric femora were used in this study. The femora were fractured in an apparatus designed to simulate a fall on the greater trochanter. Image sequences of the surface events related to the fractures were captured using two high-speed video cameras at 9111 Hz. The videos were analyzed with respect to time and load to determine the location and sequence of these events occurring in the proximal femur. The mean failure load was 4032 N (SD 370 N). The first surface events were identified in the superior femoral neck in eleven of the twelve specimens. Nine of these specimens fractured in a clear two-step process that initiated with a failure in the superior femoral neck, followed by a failure in the inferior femoral neck. This cadaveric model of hip fracture empirically confirms hypotheses that suggested that hip fractures initiate with a failure in the superior femoral neck where stresses are primarily compressive during a sideways fall impact, followed by a failure in the inferior neck where stresses are primarily tensile. Our results confirm the superolateral neck of the femur as an important region of interest for future hip fracture screening, prevention and treatment research.  相似文献

Herniation pits (HPs) of the femoral neck were first described in a radiological publication in 1982 as round to oval radiolucencies in the proximal superior quadrant of the femoral neck on anteroposterior radiographs of adults. In following early clinical publications, HPs were generally recognized as an incidental finding. In contrast, in current clinical literature they are mentioned in the context of femoroacetabular impingement (FAI) of the hip joint, which is known to cause osteoarthritis (OA). The significance of HPs in chronic skeletal disorders such as OA is still unclear, but they are discussed as a possible radiological indicator for FAI in a large part of clinical studies.In this paleoradiological study we examined a sample of mummies from the Capuchin Catacombs of Palermo, Sicily, by a mobile computed tomography (CT) scanner. Evaluation of the CT examinations revealed HPs in six out of 16 (37.5%) adult male mummies.The first aim of this study was to compare the characteristics of HPs shown in our mummy collection to the findings described in clinical literature. Thereby CT evaluation revealed that their osseous imaging characteristics are in accordance, consisting of round to oval subcortical lesions at the anterior femoral neck, clearly demarcated by a sclerotic margin.The second aim was to introduce HPs to the paleoradiological and paleopathological methodology as an entity that underwent a renaissance from an incidental finding to a possible radiological indicator of FAI in the clinical situation. As FAI plays an important role in the development of OA of the hip, which is a very common finding in human skeletal remains, HPs should always be considered in paleoradiological evaluation of hip joint diseases.  相似文献

This prospective study evaluated differences in vastus medialis (VM) and gluteus medius (GM) EMG amplitude:composite hip abductor (gluteus maximus, gluteus medius, tensor fascia lata) EMG amplitude ratios among subjects with low or high relative femoral anteversion. Data were collected during the performance of a non-weight bearing, non-sagittal plane maximal volitional effort isometric combined hip abduction-external rotation maneuver. Eighteen nonimpaired athletically active females participated in this surface EMG study. Medial hip rotation (relative femoral anteversion estimate) was measured with a handheld goniometer. Subjects were grouped by medial hip rotation displacement (group 1 < or = 42 degrees =36.1+/-7 degrees and group 2 > 42 degrees =52.7+/-7 degrees ) for statistical analysis (Mann Whitney U-tests, p < 0.05). Group 2 had decreased VM (42+/-23% vs. 69+/-30%, U=19, p=0.034) and GM (62+/-25% vs. 96+/-39%, U=19, p=0.034) normalized mean peak EMG amplitude:composite mean peak hip abductor EMG amplitude ratios compared to group 1. Decreased normalized VM (-27%) and GM (-34%) EMG amplitudes among subjects with increased relative femoral anteversion suggest reduced dynamic frontal and transverse plane femoral control from these muscles, possibly contributing to the increased incidence of non-contact knee injury observed among athletic females.  相似文献

Hip contact stress is considered to be an important biomechanical factor related to development of coxarthrosis. The effect of the lateral coverage of the acetabulum on the hip contact stress has been demonstrated in several studies of hip dysplasia, whereas the effect of the anterior anteversion remains unclear. Therefore, the joint hip contact stress during normal level walking and staircase walking, in normal and dysplastic hips, for small and large acetabular anteversion angle was computed. For small acetabular anteversion angle, the hip contact stress is slightly increased (less than 15%) in staircase walking when compared with normal walking. In hips with large angle of acetabular anteversion, walking downstairs significantly increases the maximal peak contact stress (70% in normal hips and 115% in dysplastic hips) whereas walking upstairs decreases the peak contact stress (4% in normal hips and 34% in dysplastic hips) in comparison to normal walking. Based on the presented results, we suggest that the acetabular anteversion should be considered in biomechanical evaluation of the hips, especially when the lateral coverage of the acetabulum is small.  相似文献

Femoral neck angles were measured radiographically in 41 dogs examined for hip dysplasia. Steep femoral neck inclination was found to be a phenomenon of hip dysplasia and coxofemoral joint laxity. The altered biomechanics of a steep femoral neck inclination may be a factor in the pathogenesis of hip dysplasia and secondary osteoarthritis.  相似文献

Introduction

Treatment of femoral neck fractures in young adults may require total hip arthroplasty or hip hemiarthroplasty using a bipolar cup. The latter can, however, result in migration of the femoral head and poor long-term results.

Case presentation

We report a case of femoral head migration after hemiarthroplasty performed for femoral neck fracture that had occurred 22 years earlier, when the patient (a Japanese man) was 20 years old. He experienced peri-prosthetic fracture of the femur, subsequent migration of the prosthesis, and a massive bone defect of the pelvic side acetabular roof. After bone union of the femoral shaft fracture, the patient was referred to our hospital for reconstruction of the acetabular roof. Intra-operatively, we placed two alloimplants of bone from around the transplanted femoral head into the weight-bearing region of the acetabular roof using an impaction bone graft method. We then implanted an acetabular roof reinforcement plate and a cemented polyethylene cup in the position of the original acetabular cup. Eighteen months post-operatively, X-rays showed union of the transplanted bone.

Conclusions

Treatment of femoral neck fractures in young adults is usually accomplished by osteosynthesis, but it may be complicated by femoral head avascular necrosis or by infection or osteomyelitis. In such cases, once an infection has subsided, either hip hemiarthroplasty using a bipolar cup or total hip arthroplasty may be required. However, if the acetabular side articular cartilage is damaged, a bipolar cup should not be used. Total hip arthroplasty should be performed to prevent migration of the implant.  相似文献

The presence of the obturator groove in the atelines and the hominoids is thought to be caused by the pressure of the active muscle obturator externus against the femoral neck during extension of the hip. However, the presence of an obturator groove is common in some Cercopithecoids and Platyrrhines who rarely extend the hip during their usual posture and locomotion. The functional significance of this groove is called into question, especially in some Cercopithecoids, with reference to a hip dissection of Macaca. It is suggested that the presence of the groove can also indicate a hip frequently flexed.  相似文献

Saurischian dinosaurs evolved seven orders of magnitude in body mass, as well as a wide diversity of hip joint morphology and locomotor postures. The very largest saurischians possess incongruent bony hip joints, suggesting that large volumes of soft tissues mediated hip articulation. To understand the evolutionary trends and functional relationships between body size and hip anatomy of saurischians, we tested the relationships among discrete and continuous morphological characters using phylogenetically corrected regression. Giant theropods and sauropods convergently evolved highly cartilaginous hip joints by reducing supraacetabular ossifications, a condition unlike that in early dinosauromorphs. However, transitions in femoral and acetabular soft tissues indicate that large sauropods and theropods built their hip joints in fundamentally different ways. In sauropods, the femoral head possesses irregularly rugose subchondral surfaces for thick hyaline cartilage. Hip articulation was achieved primarily using the highly cartilaginous femoral head and the supraacetabular labrum on the acetabular ceiling. In contrast, theropods covered their femoral head and neck with thinner hyaline cartilage and maintained extensive articulation between the fibrocartilaginous femoral neck and the antitrochanter. These findings suggest that the hip joints of giant sauropods were built to sustain large compressive loads, whereas those of giant theropods experienced compression and shear forces.  相似文献

A reduced bone mineral density (BMD) is observed in several rheumatic autoimmune diseases, including Systemic Sclerosis (SSc); nevertheless, data concerning the possible determinants of bone loss in this disease are not fully investigated. The aim of this study is to evaluate the relationship between BMD, body mass composition, skin sclerosis and serum Vitamin D levels in two subsets of SSc patients. 64 post-menopausal SSc patients, classified as limited cutaneous (lcSSc) or diffuse cutaneous (dcSSc) SSc, were studied. As control, 35 healthy post-menopausal women were recruited. Clinical parameters were evaluated, including the extent of skin involvement. BMD at lumbar spine, hip, femoral neck and body mass composition were determined by dual-energy X-ray absorptiometry. Serum calcium, phosphorus, alkaline phosphatase, urine pyridinium cross-links, intact parathyroid hormone and 25-hydroxyvitamin D (25OHD) were measured. BMD at spine, femoral neck and total hip was significantly lower in SSc patients compared to controls. In dcSSc subset, BMD at spine, femoral neck and total hip was significantly lower compared to lcSSc. No differences in both fat and lean mass were found in the three study groups even if patients with dcSSc showed a slightly lower total body mass compared to healthy controls. Total mineral content was significantly reduced in dSSc compared to both healthy subjects and lcSSc group. Hypovitaminosis D was observed both in healthy post-menopausal women and in SSc patients, but 25OHD levels were significantly lower in dcSSc compared to lcSSc and inversely correlated with the extent of skin thickness. These results support the hypothesis that the extent of skin involvement in SSc patients could be an important factor in determining low circulating levels of 25OHD, which in turn could play a significant role in the reduction of BMD and total mineral content.  相似文献

Muscle and joint contact force influence stresses at the proximal growth plate of the femur and thus bone growth, affecting the neck shaft angle (NSA) and femoral anteversion (FA). This study aims to illustrate how different muscle groups’ activation during gait affects NSA and FA development in able-bodied children. Subject-specific femur models were developed for three able-bodied children (ages 6, 7, and 11 years) using magnetic resonance images. Contributions of different muscle groups—hip flexors, hip extensors, hip adductors, hip abductors, and knee extensors—to overall hip contact force were computed. Specific growth rate for the growth plate was computed, and the growth was simulated in the principal stress direction at each element in the growth front. The predicted growth indicated decreased NSA and FA (of about \(0.1 {^{\circ }}\) over a four-month period) for able-bodied children. Hip abductors contributed the most, and hip adductors, the least, to growth rate. All muscles groups contributed to a decrease in predicted NSA (\(\sim \)0.01\({^{\circ }}\)–0.04\({^{\circ }})\) and FA (\(\sim \)0.004\({^{\circ }}\)\(0.2{^{\circ }}\)), except hip extensors and hip adductors, which showed a tendency to increase the FA (\(\sim \)0.004\({^{\circ }}\)\(0.02{^{\circ }}\)). Understanding influences of different muscle groups on long bone growth tendency can help in treatment planning for growing children with affected gait.  相似文献

ABSTRACT: We present a case of a 62-year-old man who underwent total hip arthroplasty for treatment of pathologic femoral neck fracture associated with adefovir dipivoxil-induced osteomalacia. He had a 13-month history of bone pain involving his shoulders, hips, and knee. He received adefovir dipivoxil for treatment of lamivudine-resistant hepatitis B virus infection for 5 years before the occurrence of femoral neck fracture. The implantation of a bipolar partial hip endoprosthesis is a treatment of choice for displaced medial femoral neck fracture. We present an experimental study which asses and compare biomechanical and clinical status through period before and after hip fracture and implantation of bipolar partial hip endoprosthesis. This study encompassed 75 patients who suffered from an acute medial femoral neck fracture and were treated with the implantation of a bipolar partial hip endoprosthesis. Their biomechanical status (stress distribution on the hip joint weight bearing area) and clinical status (Harris Hip Score) were estimated for the time prior to the injury and assessed at the follow-up examination that was, on average, carried out 40 months after the operation. Despite ageing, the observed Harris Hip Score at the follow-up examination was higher than that estimated prior to the injury (77.9 > 69.6; p = 0.006). Similarly, the hip stress distribution was reduced (2.7 MPa < 2.3 MPa; p = 0.001). While this reduction can be attributed to a loss of weight due to late ageing, the principal improvement came from the operative treatment and corresponding restoration of the biomechanical properties of the hip joint. The implantation of a bipolar partial hip endoprosthesis for patients with displaced medial femoral neck fractures improves the biomechanical and clinical features of the hip, what should have on mind during making decision about treatment.  相似文献

Children with cerebral palsy (CP) walk with altered gait and frequently exhibit proximal femoral deformities, such as anteversion and coxa valga. The objective of this research was to investigate the effect of specific gait patterns on the femoral morphology in CP.

In this study, the mechanobiological principles were implemented on a 3D finite element (FE) model of the proximal femur in order to predict changes in morphology over time in healthy and CP children. This model relies on the assumption that cyclic octahedral shear stress promotes growth and cyclic hydrostatic compressive stress inhibits growth. Growth was simulated over 16 iterations, representing approximately 5 months of growth.

The FE model predicts an increase in the femoral anteversion and coxa valga for CP loading conditions when compared with healthy ones. Understanding the role of loading in skeletal morphogenesis may help prevent bone deformities and improve function in children with gait abnormalities.  相似文献

Persons with cerebral palsy frequently walk with a crouched, internally rotated gait. Spastic medial hamstrings or adductors are presumed to contribute to excessive hip internal rotation in some patients; however, the capacity of these muscles to produce internal rotation has not been adequately investigated. The purpose of this study was to determine the hip rotation moment arms of the medial hamstrings and adductors in persons with femoral anteversion deformities who walk with a crouched, internally rotated gait. A musculoskeletal model with a "deformable" femur was developed. This model was used, in conjunction with kinematic data obtained from gait analysis, to calculate the muscle moment arms for combinations of joint angles and anteversion deformities exhibited by 21 subjects with cerebral palsy and excessive hip internal rotation. We found that the semimembranosus, semitendinosus, and gracilis muscles in our model had negligible or external rotation moment arms when the hip was internally rotated or the knee was flexed -- the body positions assumed by the subjects during walking. When the femur was excessively anteverted, the rotational moment arms of the adductor brevis, adductor longus, pectineus, and proximal compartments of the adductor magnus in our model shifted toward external rotation. These results suggest that neither the medial hamstrings nor the adductors are likely to contribute substantially to excessive internal rotation of the hip and that other causes of internal rotation should be considered when planning treatments for these patients.  相似文献

Reduction of the range of motion (ROM) until prosthetic impingement of a total hip replacement may lead to frequent impingement, subluxation and dislocation especially for patients with good hip movement. The ROM until prosthetic impingement can be calculated using the technical ROM (theta) and the cup and neck positions by a previously created mathematical formula. A larger (theta) with proper cup and neck positions results in a larger ROM. However there was only one paper written in English, which revealed the optimum theoretical combination of cup and neck anteversions. ROM of more than 110 degrees flexion, 30 degrees internal-rotation at 90 degrees flexion, 30 degrees extension and 40 degrees external-rotation were defined as the criteria for essential ROM for ADL. The safe-zones for combined cup anteversion (betaanat) and neck anteversion (b) were defined as the areas that fulfill all the criteria of ROM without prosthetic impingement. The safe-zones were created for 35 degrees , 45 degrees and 55 degrees cup abductions (alpha) and for 120 degrees and 135 degrees (theta). The safe-zones for combined (betaanat) and (b) were much larger for a 135 degrees (theta) than a 120 degrees (theta). Their safe-zones showed that (b) should be reduced if (betaanat) is increased and choosing a lower (alpha) requires that the sum of (betaanat) and (b) should be higher and vice versa. A (theta) of more than 135 degrees is recommended as it further increases the size of the safe-zone and provides a larger ROM, and the optimum values of combined cup and neck anteversions can be estimated by the formula: (alpha) + (betaanat) + 0.77(b) = 84.3.  相似文献

目的:分析股骨颈短缩的发生率、对骨折愈合造成的影响以及造成颈短缩的影响因素。方法:选择在我院行闭合复位空心钉内固定术治疗并成功获得随访信息的老年股骨颈骨折患者106例作为研究对象。于术后1、3、6、12个月对患者进行门诊随访,记录患者的髋关节Harris评分、骨折愈合情况以及颈短缩的发生率,并将患者分为短缩组和无短缩组,对两组患者的骨折愈合率与髋关节Harris评分进行对比,并分析空心钉内固定术后颈短缩发生的危险因素。结果:短缩组43例,未短缩组63例。短缩组中骨折愈合率93.02%;未短缩组中骨折愈合率93.65%,两组骨折愈合率的差异无统计学意义(P>0.05)。短缩组中术后髋关节Harris评分为(74.58±7.85)分;未短缩组中术后髋关节Harris评分为(85.69±11.34)分,短缩组Harris评分低于未短缩组,差异有统计学意义(P<0.05)。颈短缩的发生与年龄、性别、骨折类型、骨密度值及骨折复位质量密切相关(P<0.05),与受伤至手术时间、置入方式、负重时间、住院时间无明显的相关性(P>0.05)。颈短缩的发生与骨密度值和骨折复位质量相关性最大,骨折类型与年龄次之,性别的相关性相对较小。结论:空心钉内固定治疗股骨颈骨折并发颈短缩的发生率较高,颈短缩会影响髋关节的功能,但不影响骨折的愈合。骨密度、骨折复位质量、骨折类型、年龄以及性别是颈短缩的独立影响因素。  相似文献

A complex of traits in the femur and pelvis of Homo ereclus and early “erectus-like” specimens has been described, but never satisfactorily explained. Here the functional relationships between pelvic and femoral structure in humans are explored using both theoretical biomechanical models and empirical tests within modern samples of diverse body form (Pecos Amerindians, East Africans). Results indicate that a long femoral neck increases mediolateral bending of the femoral diaphysis and decreases gluteal abductor and hip joint reaction forces. Increasing biacetabular breadth along with femoral neck length further increases M-L bending of the femoral shaft and maintains abductor and joint reaction forces at near “normal” levels. When compared to modern humans, Homo erectus and early “erectus-like” specimens are characterized by a long femoral neck and greatly increased M-L relative to A-P bending strength of the femoral shaft, coupled with no decrease in hip joint size and a probable increase in abductor force relative to body size. All of this strongly suggests that biacetabular breadth as well as femoral neck length was relatively large in early Homo. Several features preserved in early Homo partial hip bones also indicate that the true (lower) pelvis was very M-L broad, as well as A-P narrow. This is similar to the lower pelvic shape of australopithecines and suggests that nonrotational birth, in which the newborn's head is oriented transversely through the pelvic outlet, characterized early Homo as well as Australopithecus. Because M-L breadth of the pelvis is constrained by other factors, this may have limited increases in cranial capacity within Homo until rotational birth was established during the late Middle Pleistocene. During or after the transition to rotational birth biacetabular breadth decreased, reducing the body weight moment arm about the hip and allowing femoral neck length (abductor moment arm) to also decrease, both of which reduced M-L bending of the proximal femoral shaft. Variation in femoral structural properties within early Homo and other East African Early Pleistocene specimens has several taxonomic and phylogenetic implications. © 1995 Wiley-Liss, Inc.  相似文献
